1.Construction and application of the criteria for drug utilization evaluation of low-dose rivaroxaban in atherosclerotic cardiovascular disease

OBJECTIVE To construct and apply drug utilization evaluation (DUE) criteria for low-dose rivaroxaban in atherosclerotic cardiovascular disease (ASCVD) based on the dual pathway inhibition (DPI) antithrombotic therapy scheme, to promote clinical rational drug use. METHODS Based on the instructions and relevant guidelines of low-dose rivaroxaban (2.5 mg, bid), the Delphi method was used to establish the DUE criteria for low-dose rivaroxaban used in ASCVD. Weighted technique for order preference by similarity to an ideal solution method was used to determine the relative weights of each evaluation index, and the rationality of the filing medical records of discharged patients using low-dose rivaroxaban for ASCVD at Anqing Municipal Hospital from February 2024 to January 2025 was evaluated. RESULTS The established DUE criteria included 3 primary indicators (medication indications, medication process, medication results) and 11 secondary indicators (such as indications, contraindications, etc.). The higher weighted secondary indicators being contraindications (0.117 9) and indications (0.112 1). A total of 265 medical records were included for evaluation. The evaluation results showed that 192 cases (72.45%) had reasonable medical records, 69 cases (26.04%) had basic reasonable medical records, and 4 cases (1.51%) had unreasonable medical records; unreasonable types mainly included inappropriate combination therapy, inappropriate usage and dosage, inappropriate post- medication monitoring, and inappropriate drug switching, etc. CONCLUSIONS This study establishes a DUE criteria for low-dose rivaroxaban in ASCVD based on the DPI antithrombotic treatment regimen, and the evaluation results are intuitive, reliable, and quantifiable. The use of low-dose rivaroxaban in ASCVD patients in our hospital is relatively reasonable, but further management needs to be strengthened.
2.Non-contrast CT radiomics extreme gradient boosting(XGBoost)model for predicting acute necrotic collection around acute pancreatitis

Objective To observe the value of non-contrast CT radiomics extreme gradient boosting(XGBoost)model based on SHAP method for predicting acute necrotic collection(ANC)around acute pancreatitis(AP).Methods A total of 307 patients with initially clinically diagnosed AP were retrospectively enrolled.The optimal radiomics features of peripheral pancreatic tissue volume of interest(VOI)were extracted and screened based on automatic segmentation on the first non-contrast CT,and the evaluation results of modified CT severity index(MCTSI)score of AP severity based on first enhanced CT were recorded.The patients were divided into peripancreatic ANC group(ANC group)and acute peripancreatic fluid collection(APFC)group according to follow-up abdominal CT.XGBoost method was used to construct radiomics model,MCTSI model and combined model for predicting AP ANC based on the optimal radiomics features,MCTSI and their combination,respectively.The diagnostic efficacy of each model was evaluated using 5-fold cross-validation method,and the contribution of each variable to combined model was analyzed with SHAP method.Results Among 307 cases,there were 134 cases in ANC group and 173 in APFC group.Totally 6 optimal radiomics features were screened based on the first non-contrast CT.The area under the receiver operating characteristic curve(AUC)of radiomics model,MCTSI model and combined model was 0.936,0.693 and 0.917,respectively.The AUC of MCTSI model was lower than that of radiomics model and combined model(Z=-3.485,-2.824,both P<0.01),while no significant difference of AUC was found between radiomics model and combined model(Z=-0.817,P=0.415).The contribution of optimal radiomics features to combined model were all higher than that of MCTSI score.Conclusion Non-contrast CT radiomics XGBoost model could effectively predict AP ANC.
3.Predictive value of machine learning models based on CT imaging features for papillary thyroid carcinoma

Objective:To establish three machine learning prediction models based on CT imaging characteristics of papillary thyroid carcinoma (PTC) , and use SHAP (shapley additive explanations) analysis to investigate the contribution of each CT image features in the best model.Methods:CT imaging features in 426 cases of 440 PTCs confirmed pathologically from Jan. 2016 to Jan. 2021 at the affiliated Hangzhou First People’s Hospital of Westlake University Medical School were retrospectively analyzed. compared with 467 cases of 528 nodular goiter (NG) , evaluating the distribution of four CT characteristics: cookie bite sign, enhanced range of narrowing/blur (ERNB) , microcalcifications, and irregular shape. We split the data into 8∶2 ratio for training and testing sets, then constructed three machine learning models using XGBoost, RF, and SVM. Based on AUC, accuracy, F1 score, and other metrics, we selected the best model. Lastly, we used SHAP values to assess each CT feature’s contribution and positive/negative effects on the model.Results:Among 440 PTC and 528 NG nodules, CT features like cookie bite sign, ERNB, microcalcifications, and irregular shape occurred in 326 and 30 ( χ 2=483.05, P<0.001) , 363 and 106 ( χ 2=374.45, P<0.001) , 158 and 53 ( χ 2=94.24, P<0.001) , and 354 and 52 ( χ 2=491.34, P<0.001) nodules, respectively. The machine learning models built using XGBoost, RF, and SVM had AUC, accuracy, and F1 scores ranging from 0.884~0.925, 0.867~0.873, and 0.844~0.854 respectively on the training set. On the test set, the scores ranged from 0.869~0.923, 0.845~0.871, and 0.803~0.845. Among them, the XGBoost model demonstrated the highest diagnostic performance on the test set. Among the four CT features, irregular shape had the highest absolute SHAP value, positively contributing to PTC diagnosis. Conclusion:XGBoost model showed the highest PTC diagnostic performance. Irregular shape had the greatest positive impact on PTC diagnosis.
4.Prevalence and influencing factors of chronic somatic comorbidities in patients with schizophrenia in Shanghai communities

Objective To investigate the prevalence of chronic somatic comorbidities in schizophrenic patients in Shanghai communities,and to explore the factors influencing comorbidities.Methods Based on Shanghai community-based severe mental disorders cohort,5 422 patients with schizophrenia(SCZ)were included in the study.12 common chronic somatic diseases,defined by patients'self-report,were selected to analyze the prevalence of comorbidity,and Logistic regression model was used to analyze the factors influencing the number of somatic comorbidities.Results The total prevalence of somatic comorbidity was 37.0%in 5 422 patients with SCZ,with the highest prevalence of hypertension(22.6%)and diabetes mellitus(13.1%)among 12 somatic diseases selected.Older age was the main factor associated with chronic somatic comorbidities in community schizophrenic patients.The risk of 1-2 comorbidities in patients aged≥60 years was 3.34(95%CI:2.74-4.07)times higher than those aged<45 years,while the risk of≥3 comorbidities was 3.27(95%CI:2.11-5.09)times higher,correspondingly.Female gender,marriage,smoking,and longer duration of illness were also risk factors for comorbidity.Women after menopause had higher risk of comorbidity than perimenopausal women.Conclusion Cardiovascular and metabolic diseases were common somatic comorbidities among schizophrenic patients in Shanghai communities.Older age,female gender,marriage,smoking,and longer duration of illness were risk factors for increasing number of comorbidities.
5.3D C-arm Combined With Orthopedic Robot-assisted Iliosacral Screw Fixation Through S3 Vertebra for Pelvic Fracture:Report of 4 Cases

Objective To verify the feasibility and safety of three-dimensional(3D)image acquisition by C-arm combined with orthopedic robot-assisted iliosacral screw fixation through S3 vertebra.Methods From July 2023 to June 2024,a total of 4 patients with closed pelvic fracture were surgically treated.Preoperative pelvic CT 3D reconstruction showed the presence of an ideal iliosacral screw bone channel through S3 vertebra which required screw fixation.There was no such channel through the S1 vertebra,or the stability of fixation through the S3 channel was better than that through the S1 channel.The 3D image acquisition by C-arm combined with orthopedic surgical robot was used to insert screws.Follow-ups were conducted for at least 6 months after surgery to assess fracture healing and limb function.Results All the 4 patients were successfully implanted with a S2 and a S3 iliosacral screw,and all the 8 screws did not penetrate the cortical bone.Three cases had excellent Majeed scores and 1 case had good scores at 3 months after surgery.Follow-ups were conducted for 18,12,7,and 6 months after surgery,respectively,and all fractures healed smoothly without any loosening or breakage of screws.Conclusion 3D image acquisition by C-arm combined with orthopedic robot-assisted iliosacral screw fixation through S3 vertebra is feasible,safe and effective.

Extensive epigenetic reprogramming involves in memory CD8+ T-cell differentiation. The elaborate epigenetic rewiring underlying the heterogeneous functional states of CD8+ T cells remains hidden. Here, we profile single-cell chromatin accessibility and map enhancer-promoter interactomes to characterize the differentiation trajectory of memory CD8+ T cells. We reveal that under distinct epigenetic regulations, the early activated CD8+ T cells divergently originated for short-lived effector and memory precursor effector cells. We also uncover a defined epigenetic rewiring leading to the conversion from effector memory to central memory cells during memory formation. Additionally, we illustrate chromatin regulatory mechanisms underlying long-lasting versus transient transcription regulation during memory differentiation. Finally, we confirm the essential roles of Sox4 and Nrf2 in developing memory precursor effector and effector memory cells, respectively, and validate cell state-specific enhancers in regulating Il7r using CRISPR-Cas9. Our data pave the way for understanding the mechanism underlying epigenetic memory formation in CD8+ T-cell differentiation.

10.Analysis of transurethral water vapor thermal therapy for the treatment of benign prostatic hyperplasia

Objective:To investigate the efficacy and safety of transurethral water vapor thermal therapy(WVTT)using the Rezūm system for benign prostatic hyperplasia(BPH)in the real world.Methods:A total of 181 patients with BPH were recruited from the Second Affiliated Hospital of Nanjing Medical University from August 2022 to December 2023,of whom 173 patients were treated with WVTT using the Rezūm system,while 8 patients were treated with WVTT combined with TURP.They were followed up at 1,3,and 6 months postoperatively to assess changes in the IPSS,QoL,Qmax,IIEF-5,and the occurrence of any complications.Results:All 181 surgeries in this group were successfully completed.The operation time of the Rezūm system was(4.6±1.4)mi-nutes.The postoperative indwelling catheterization time was(8.0±2.1)days.With a follow-up of at least 6 months,there was a significant decrease in PV,IPSS and QoL,and a remarkable increase had been found in Qmax as well(P<0.05).There was no sig-nificant difference in IIEF-5 before and after the operation(P>0.05).In this groups of patients,postoperative complications mainly included 95 cases(52.5%)of gross hematuria,6 cases(3.3%)of retrograde ejaculation,5 cases(2.8%)of urethral stricture,4 cases(2.2%)of prostatitis,and 10 cases(5.5%)of urinary tract infection.Four cases(2.2%)underwent surgical retreatment for BPH after surgery.Conclusion:In the real world,the use of Rezūm thermal steam ablation system for the treatment of BPH has sat-isfactory short-term effect,short surgical time,and significant improvement in IPSS,QoL,Qmax,which does not adversely affect sexu-al function.
