Objective To propose a lead attention network-based ST-T change recognition algorithm to detect ECG ST-T changes accurately. Methods Firstly, heartbeat signals were extracted through R-wave localization, and a 12-lead heartbeat matrix was generated by correlation-based screening and merging to realize data augmentation.Secondly,a lead attention module was constructed by combining depthwise convolution(DWConv)with the channel attention squeeze-and-excitation block(SE-block)structure to perceive the differences in ST-T status among electrocardiogram leads.Thirdly, the mapping output by two independent attention modules was fused and splicing with the original signal residual was carried out,so that attention information extraction and original information transfer were enhanced effectively.Finally,SE-ResNet was used as the backbone network to extract signal features to complete the classification and identification of ST-T changes. To validate the recognition performance of the proposed algorithm for ST-T changes in ECG,the 12-lead ECG data of 97 472 patients containing different ECG rhythms were collected for ablation and comparison experiments at the First Affiliated Hospital of Army Medical University. Results The proposed algorithm achieved an AUC of 0.965 with a sensitivity of 90.51%,specificity of 90.23%,positive predictive value of 89.24%and overall accuracy of 90.36%on an independent test set.Comparative analysis demonstrated superior performance to four benchmark architectures, including VGG16,ResNet18,MobileNetV3-Small and ShuffleNet,in terms of both classification accuracy and computational efficiency. Conclusion The algorithm designed can accurately detect ST-T changes and can be used for wearable ECG automatic analysis to assist in the early warning of cardiovascular diseases in both acute and chronic patients and highland residents.[Chinese Medical Equipment Journal,2025,46(7):1-11]

Objective:To explore the therapeutic methods and drug-symptom patterns of Traditional Chinese Medicine (TCM) for idiopathic pulmonary fibrosis (IPF) at different stages under the guidance of the " three-stage differentiated management" principle. Methods:Under the three-stage principle, 133 IPF prescriptions treated by prof. Yan Jun from 2017.2-2023.12 were analyzed. Disease and category information of each eligible case was entered into the " FangNet" platform. Association and cluster analyses were performed for core herbs, key herb pairs, and symptom-drug relationships, aiming to uncover the clinical medication characteristics and rules in the three stages.Results:(1) Early inflammatory stage: phlegm-stasis harassment, incipient but unformed masses, superficial disease location; priority was dispelling wind, dispersing pathogen, clearing lung and resolving phlegm-commonly used: ephedra, stemona, processed pinellia, peucedanum, armeniaca. (2) Mid injury stage: formed phlegm-stasis, struggle of pathogen and healthy Qi, impaired lung collaterals, obstructive phlegm-stasis, excess pathogen with intact healthy Qi; emphasis on expelling pathogen, resolving phlegm, activating blood and dredging collaterals-e.g., angelica, pheretima, bombyx, scorpio. (3) Late fibrotic stage: healthy Qi deficiency with excess pathogen, phlegm-stasis masses entrenched in collaterals; treatment focuses on tonifying lung and dissolving masses, combining dredging and tonifying to regulate Qi and blood-e.g., pinellia, rhodiola, ginseng, gecko.Conclusions:Phlegm-stasis as excess pathogen and dual deficiency of lung-kidney run through all three stages of IPF. The rise and fall of healthy Qi vs pathogen and the degree of mass formation determine stage-specific therapeutic strategies and drug-pattern adjustments.

Extremely unbalanced data refers to datasets with independent or dependent variables showing severe imbalances in proportions, which might lead to deviation of classical test statistics from theoretical distribution and difficulties in controlling type Ⅰ error. The increased availability of genome-wide resources from large population cohorts has highlighted the growing demand for efficient and accurate statistical methods for the process of extremely unbalanced data to improve the development of genetic statistical methods. This paper introduces two widely used correction methods in current genome-wide association study for extremely unbalanced data, i.e. Firth correction and saddle point approximation, describes their effectiveness in controlling type Ⅰ errors confirmed by simulation experiments, finally, and summarizes the commonly used software for extremely unbalanced genomic data to provide theoretical reference and suggestion for its application for the statistical analysis on extremely unbalanced data in future.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
