1.Construction of a family-centered care program for children with tuberculosis based on the double ABC-X model and intervention effects evaluation

ObjectiveTo construct a family-centered care model for children with tuberculosis based on the double ABC-X model, and to evaluate its clinical effects. MethodsFrom December 2022 to October 2023, 64 newly admitted children with tuberculosis who met the criteria and their caregivers were recruited from the tuberculosis department of Shanghai Public Health Clinical Center were randomly divided into an experimental group (32 cases) and a control group (32 cases).The control group was given a conventional health care, while the experimental group was given a family-centered health care intervention based on the double ABC-X model, in which a multidisciplinary care team provided personalized information and emotional support for the caregivers and their children. Medication adherence of the children, caregiver’s teading burden, and disease management competence were compared between the 2 groups. ResultsA total of 29 cases in the experimental group and 27 cases in the control group completed the intervention. At 12 weeks of intervention, the medication adherence score (7.72±0.45 vs 7.41±0.50, P<0.05) and disease management competence score (36.97±7.85 vs 31.56±7.30, P<0.05) were higher in the experimental group than that in the control group while the caregiving burden score (31.79±13.40 vs 40.04±9.01, P<0.05) and difficulty of disease management score (30.41±12.41 vs 38.56±9.48, P<0.05) were lower than that in the control group. At 24 weeks of intervention, the medication adherence score (7.34±0.97 vs 6.70±1.14, P<0.05) and disease management competence score (42.07±6.93 vs 35.63±7.32, P<0.05) were higher in the experimental group than that in the control group as well, but the caregiving burden score (31.62±11.72 vs 39.63±10.17, P<0.05) and difficulty of disease management score (30.59±10.87 vs 37.81±9.32, P<0.05) were lower than that in the control group. ConclusionFamily-centered care based on the double ABC-X model can effectively promote medication adherence among children with tuberculosis, reduce caregivers’ care burden and disease management difficulties, and improve caregiver’s disease management competence.

Objective: To compare the efficacy of heat and acid elution methods for IgG anti-M and anti-Ku. Methods: Ten samples with IgG anti-M and two samples with IgG anti-Ku were selected and standardized to a titer of 64. These antibodies underwent overnight absorption at 4℃ with O-type MM and kk-type erythrocytes, and then heat and acid elution methods were used on the absorbed sensitized erythrocytes respectively by detecting the titer of anti-M and anti-Ku in the eluate to compare the differences in the elution efficiency of IgG anti-M and anti-Ku between the two elution methods. Results: In heat elution tests, all 10 anti-M samples showed positive results with titers ranging from 8 to 64, while 2 anti-Ku samples yielded negative results. In acid elution tests, all 10 anti-M samples demonstrated negative results, whereas both anti-Ku (n=2) samples exhibited positive reactions with consistent titers of 32. Following acid elution with subsequent heat elution, 8 of 10 anti-M samples showed positive results with titers ranging from 8 to 32, while 2 remained negative. Both anti-Ku samples demonstrated positive with titers of 4. Conclusion: Heat elution demonstrated superior efficiency for IgG anti-M compared to acid elution, whereas acid elution showed greater efficacy for IgG anti-Ku than heat elution.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.

Severe fever with thrombocytopenia syndrome(SFTS)is an acute infectious disease caused by the Dabie Banda-virus(DBV),with a high mortality rate.The annually reported number of cases is steadily increasing.DBV is transmitted pri-marily by tick bites,and its epidemic range is gradually expanding with the spread of its vector,thus posing a serious threat to human health.No licensed preventive vaccine is currently available.The World Health Organization has listed SFTS as an infec-tious disease requiring further investigation of vaccines and treatments with high priority.This article reviews current research progress in SFTS vaccines,to provide a reference for the development of DBV vaccines.

Sustained inflammatory responses are closely related to various severe diseases,and inhibiting the excessive activation of inflammasomes and pyroptosis has significant implications for clinical treatment.Natural products have garnered considerable concern for the treatment of inflammation.Huanglian-Wumei decoction(HLWMD)is a classic prescription used for treating inflammatory diseases,but the necessity of their combination and the exact underlying anti-inflammatory mechanism have not yet been elucidated.Inspired by the supramolecular self-assembly strategy and natural drug compatibility theory,we successfully obtained berberine(BBR)-chlorogenic acid(CGA)supramolecular(BCS),which is an herbal pair from HLWMD.Using a series of characterization methods,we confirmed the self-assembly mechanism of BCS.BBR and CGA were self-assembled and stacked into amphiphilic spherical supra-molecules in a 2:1 molar ratio,driven by electrostatic interactions,hydrophobic interactions,and π-πstacking;the hydrophilic fragments of CGA were outside,and the hydrophobic fragments of BBR were inside.This stacking pattern significantly improved the anti-inflammatory performance of BCS compared with that of single free molecules.Compared with free molecules,BCS significantly attenuated the release of multiple inflammatory mediators and lipopolysaccharide(LPS)-induced pyroptosis.Its anti-inflammatory mechanism is closely related to the inhibition of intracellular nuclear factor-kappaB(NF-κB)p65 phosphorylation and the noncanonical pyroptosis signalling pathway mediated by caspase-11.
7.Detection and Transfusion Strategy of Mimicking Antibodies

Objective:To explore serological detection and blood transfusion strategies of mimicking antibodies,so as to provide appropriate transfusion strategies.Methods:Detailed serological tests,including ABO blood group,Rh typing,antibody specificity,etc,were performed on two patients with autoimmune hemolytic anemia(AIHA).Meanwhile,the references about blood transfusion from mimicking antibody patients published from 1977 to 2024 in China and abroad were retrospectively summarized and analyzed.Results:The patient 1 blood type was AB,CCDee and the antibody is mimicking anti-e,transfusion the e-negative red blood cells(RBCs)was effective.After two transfusions of e-RBCs,hemoglobin levels significantly increased from 48 g/L to 91 g/L,with complete resolution of hemolytic symptoms.The patient 2 blood type was O,CcDee,and the antibody was mimicking anti-c,the patient was diagnosed with AIHA and treated with hormone.No blood products were transfused during hospitalization,and his hemolysis was relieved.Conclusion:Strictly grasping the indication of blood transfusion,blood transfusion should not be performed in the unnecessary conditions,and the corresponding antigen-negative RBC should be screened for transfusion in the necessay conditions.
8.Analysis of the influencing factors of nausea and vomiting caused by moderate emetic chemotherapy drugs in advanced colorectal cancer

Objective To explore the factors for chemotherapy-induced nausea and vomiting(CINV)in patients with advanced colorectal cancer receiving moderately emetic chemotherapy regimens.Methods 296 patients with colorectal cancer treated with oxaliplatin/irinotecan combination therapy and standard antiemetic prophylaxis regimen at the First Hospital of Jiaxing from July 1,2022 to June 30,2024 were selected,including 164 patients in oxaliplatin combination therapy group and 132 patients in irinotecan combination therapy group.Evaluated the nausea and vomiting status of patients within 5 days after the start of chemotherapy.Analyzed demographic data,medical history data,follow-up data.Results The incidence of CINV in oxaliplatin combination therapy group was 61.59%,and that in irinotecan combination therapy group was 53.79%.The incidence of delayed CINV and severe acute CINV in oxaliplatin combination therapy group were significantly higher than those in irinotecan combination therapy group(P<0.05).Multivariate analysis showed that the risk factors for acute CINV in the oxaliplatin-based combination regimen were smoking history and CINV history.The risk factors for delayed CINV were age,no history of drinking,history of pregnancy-induced vomiting,motion sickness history,CINV history,and first use of the regimen.For the irinotecan-based combination regimen,the risk factors for acute CINV were motion sickness history and CINV history.The risk factors for delayed CINV were CINV history and anxious emotions.Conclusion When useing standard anti-emetic prophylaxis that still have a high incidence of CINV by moderate emetic chemotherapy drugs in advanced colorectal cancer patients.Patients with high risk factors should actively take preventive measures to control CINV.
9.Predictive Value of MIC Typing for IDH1/2 Mutations in Patients with Acute Myeloid Leukemia

Objective:To investigate the predictive value of morphology,immunology,and cytogenetics for isocitrate dehydrogenase 1 and 2(IDH1/2)gene mutation in newly diagnosed acute myeloid leukemia(AML)patients.Methods:The clinical data of 186 newly diagnosed AML patients(except M3 subtype)in the First People's Hospital of Changzhou were retrospectively analyzed,and the variables associated with IDH1/2 mutation in patients were screened using LASSO regression to construct a multivariate logistic regression analysis model.The Bootstrap method was used for internal validation of the model and nomograms were used to visualize the model,and receiver operating characteristic(ROC)curve was used to evaluate the predictive performance of the model.Results:A total of 60 AML patients had IDH1/2 mutation at initial diagnosis.LASSO regression screened 9 predictive variables associated with IDH1/2 mutation,including CD7,CD56,CD11b,CD15,CD64,HLA-DR,platelet count ≥ 50 × 109/L,isolated+8 and normal karyotype.The nomogram and ROC curve were plotted based on the above 9 variables.The area under the ROC curve(AUC)of the training set and the validation set were 0.871 and 0.806,respectively.Internal validation showed that the nomogram had good predictive ability.Conclusion:The prediction model based on MIC typing constructed in this study has a good predictive ability for the presence of IDH1/2 mutations in newly diagnosed AML patients and has important clinical application value when the gene mutation detection results are unavailable.
10.Analysis of correlation between ankle instability and load-induced osteochondral lesions of the talus

Objective:To investigate the biomechanical correlation between ankle instability and osteochondral lesions of the talus (OLT) under loading conditionsMethods:A healthy 29-year-old male volunteer was selected for the study. A 64-slice spiral CT scan of the right lower limb was performed to construct a detailed finite element model of the ankle joint, including ligaments and cartilage. Three injury models were created: models of distal tibiofibular syndesmosis injury, lateral collateral ligament injury, and a combined injury of the distal tibiofibular syndesmosis and lateral collateral ligament. Differences in stress distribution on the tibiotalar joint surface, talus stress, and talus displacement were analyzed through anterior drawer test, inversion stress test, and external rotation stress test.Results:In the anterior drawer test, as the forward traction force increased (40, 60, 80, 100, 120, 140, and 150 N), all the injury models showed a progressive increase in tibiotalar joint surface stress, talus stress, and talus displacement. The combined injury model showed the highest tibiotalar joint surface stress (32.6 MPa), while the lateral collateral ligament injury model demonstrated the highest talus stress (56.5 MPa). Talus displacement increased significantly with traction, reaching the maximum (4.88 mm) in the combined injury model under 150 N. In the inversion stress test, stress on the tibiotalar joint surface in the lateral collateral ligament injury model was concentrated on the posterior-lateral and posterior-medial regions, whereas in the combined injury model, stress on the tibiotalar joint surface was predominantly concentrated in the posterior-medial region. Talus stress was localized to the talus neck and body in all the models, with the combined injury model showing the largest talus displacement (8.46 mm). In the external rotation stress test, stress on the tibiotalar joint surface was mainly distributed in the posterior-medial, posterior-lateral, and anterior-lateral regions in all the models. Talus stress was concentrated at the talus neck and body. The combined injury model exhibited the greatest talus displacement (12.50 mm).Conclusion:Ankle instability, particularly from combined injuries of the distal tibiofibular syndesmosis and lateral collateral ligament, significantly increases the stress concentration and talus displacement under loading conditions, thus elevating the risk of OLT.
