1.Clinical value of endoscopic thyroidectomy by gasless unilateral axillary approach in the treatment of elderly papillary thyroid carcinoma based on COX survival analysis

Objective:To explore the clinical value of endoscopic thyroidectomy by gasless unilateral axillary approach (ETGUA) on papillary thyroid carcinoma (PTC) in the elderly based on COX survival analysis.Methods:Clinical data of 51 elderly PTC patients who received traditional open surgery in Yantaishan Hospital, Yantai, Shandong Province from Mar. 2022 to Mar. 2025 were retrospectively analyzed and the patients included in control group. Clinical data of 54 elderly PTC patients who were treated with ETGUA during the same period were retrospectively analyzed and the patients were enrolled as observation group. The incidence rates of postoperative complications in the two groups were counted. The clinical related indexes (intraoperative blood loss, number of lymph node dissection, hospitalization time, surgical time) were compared. The survival rate was analyzed at 1 year, 2 years and 3 years after surgery, and the influencing factors of survival time of papillary thyroid carcinoma were analyzed by COX regression model.Results:The intraoperative blood loss in observation group was significantly less than that in control group ( t=12.56, P=0.000) while the number of lymph node dissection and surgical time were significantly more or longer than those in control group ( t=14.71, 13.90, P<0.05). There was no significant difference in hospitalization time between the two groups ( t=1.85, P=0.068). The total incidence rate of postoperative complications in observation group was significantly lower than that in control group ( χ 2= 8.57, P=0.000). Follow-up after 3 years revealed that the survival rate in observation group was significantly higher than that in control group ( χ 2= 4.95, P=0.026). COX survival analysis showed that age, TNM staging and tumor count were the influencing factors of survival time in PTC patients ( χ 2= 9.54, 7.30, 5.41, P=0.002, 0.007, 0.020) . Conclusion:ETGUA has good clinical value on elderly PTC patients, and it is helpful to reduce the degree of intraoperative trauma and the probability of postoperative complications, and improve the prognosis.

3.Study of Extraction Process of Polysaccharide from Seeds of Toona sinensis and Antioxidant Activity in Vitro

Objective To optimize the conditions of extraction process of polysaccharides from seeds of Toona sinensis; To preliminary determinate the antioxidant activity in vitro. Methods The content of polysaccharides from Toona sinensis was chosen as evaluation index; the extraction time, material liquid ratio, and extraction times were chosen as factors; L9(34) orthogonal design was used to optimize extraction process; antioxidant activity of polysaccharides from Toona sinensis was investigated by the total reducing power and 1,1-diphenyl-2-picryhydrazyl (DPPH) method. Results From the statistical data of processing, the optimal extracting conditions were as follows:liquid to material was 1:12, extraction time was 120 min, and extracted for 3 times. Polysaccharides from seeds of Toona sinensis had certain reducing capacity and had better scavenging ability on DPPH? and the scavenging rate significantly increased with the concentration. The effect was simulated by curve equation. Conclusion The process is simple, feasible, stable and reliable and can be used for the small-scale study on extraction of polysaccharides from seeds of Toona sinensis. Polysaccharides from seeds of Toona sinensis have antioxidant activity.
4.Clinical and pathological features of patients with anti-GBM disease lacking linear IgG deposition along GBM in kidney

ObjectiveTo investigate the clinical and pathological features of patients with anti-glomerular basement membrane(GBM) disease lacking linear IgG deposition along GBM on renal biopsy.Method Ninety-three patients with anti-GBM disease were collected in our hospital from 1991 to 2008,with 40 patients presenting negative linear IgG deposition along GBM on renal biopsy by direct immunofluorescence(group A) and 53 patients presenting classical linear IgG deposition along GBM(group B).The clinical manifestation,pathological presentation and prognosis were compared between two groups.Results Between two groups,there were no significant differences in gender,age,hemoptysis,oliguria or anuria,gross hematuria,proteinuria,anemia,ANCA positivity,level of circulating anti-GBM antibodies,the percentage of crescent formation in glomeruli and patient outcomes(P>0.05).Patients in group A were diagnosed significantly later than patients in group B(68 d vs 36 d,P=0.013) and serum creatinine was significantly lower at diagnosis(716.0 μmol/L vs 896.8 μmol/L,P=0.027).Direct immunofluorescence was performed on the paraffin-embedded renal sections from four patients in group A,and all of them revealed positive linear IgG deposition along GBM.Conclusions Patients with circulating anti-GBM antibodies but withont IgG deposition along GBM present slower progress of renal injury,but same clinical,pathological and prognostic features as those with classical anti-GBM disease.Serum anti-GBM antibodies should be prescribed earlier to the suspected patients,and the diagnosed patients should be treated with plasmapheresis and extensive immunosuppression to improve prognosis.
5.Determination of Paeonol in Yuxianling Granules by HPLC

OBJECTIVE: To establish a HPLC method for the determination of Paeonol in Yuxianling granules. METHODS: The determination was performed on Phenomenex C18-ODS with mobile phase consisted of methanol-water (60∶40) at a flow rate of 1.0mL?min-1. The detection wavelength was set at 274nm. RESULTS: The linear range of Paeonol was 8.24~41.20?g?mL-1(r=0.999 7). The recovery was 99.50% (RSD=1.12%). CONCLUSION: The established method is simple, rapid, sensitive, accurate, stable and reproducible, and suitable for the determination of the content of paeonol in Yuxianling granules.
6.Clinical and pathological charactcristics of ANCA associated systemic vasculitis with immune complex deposition in kidney

Objective To analyze the clinical and pathological characteristics of ANCA associated systemic vasculitis(AASV) with immune complex deposition in kidney and compare with that of pauci-immune AASV. Methods Patients with AASV, admitted in our hospital in last 5 years, were retrospectively studied. The clinical and pathological characteristics were compared between patients with immune complex deposition and patients with pauci-immune deposition. Results There were eitht patients with immune complex deposition (five with IgM deposition, two with IgA deposition and one with IgG deposition) and 32 patients with pauci-immune deposition. There was no significant difference in age, gender, type of ANCA, interval between onset of vasculitis and renal biopsy, clinical manifestations and short-term renal survival rate between the two groups. 7.Genetic Variation and Clustal Analysis of Trichomonas vaginalis Cysteine Proteases

Objective To clone the genes coding for cysteine proteases (CPs, TvCPs) from Trichomonas vaginalis and to analyze their genetic variations with the related sequences from NCBI database (GenBank) and T. vaginalis Genome Project database from The Institute for Genomic Research (TIGR). Method TvCP genes were amplified using PCR, and inserted into vector pET28b or pBS-T. The recombinant plasmids were then transformed to Escherichia coli BL21 or Top10 strain. The recombinant plasmids were used for sequencing. Homologous TvCP genes were blasted based on NCBI GenBank and TIGR T. vaginalis Genome Project database. The sequences of cloned TvCP genes were aligned and clustered by Clustal X (1.83 version) with retrieved sequences. Comparisons of amino acids among cathepsin L-like TvCPs, human L-like cathepsins and papaya papain were performed using DNAstar software, and their phylogenic tree was constructed based on neighbor-joining method using Clustal X. Results Two TvCP3 clones and one TvCP2 had a high identity of more than 99% with their responding TvCPs. Three clones of TvCP4 genes, GZ-CP4-clone 1-3, belonged to two members of a family showing a high percentage identity of more than 97.5% with the sequences of TvCP4 genes from databases (GenBank and TIGR) both at amino acid and nucleotide levels. Nine homologous TvCP4 pro-enzymes with 304 amino acids and other two members with deletions of N-terminal sequence existed in T. vaginalis sharing a similarity of 62.3-96.7% amino acids, which may evolve by means of gene replication and deletion. TvCP1-4, TvCP12, TvCP25 and CP65 had an identity of 61-88.2% at amino acid levels. So far, all reported sequences of C1 family from T. vaginalis belonged to capanthesin L-like subfamily with the same enzymatic active sites, conserved cysteine residues and similar structural features such as ERFNIN-like motif in pro-enzyme region, suggesting that they might result from gene duplication and mutations. Conclusion TvCPs belong to cathepsin L-like family with genetic diversity, but they have the same active amino acid residues, cysteine residues and similar structural characteristics, suggesting that they may derive from one ancestor.
