Atopic dermatitis is a common chronic, relapsing, inflammatory skin disease;however, its pathogenesis remains unclear. The theory of"clear-turbid interference"originates from the Inner Canon of Yellow Emperor and refers to a pathological condition in which clear yang and turbid yin intermingle, and qi movement becomes disordered. This paper elucidates the theoretical connotation of"clear-turbid interference"and its core mechanism in triggering atopic dermatitis,as well as the role of the"gut-skin axis"in atopic dermatitis development.Internal dampness-turbidity and disharmony between ying and wei constitute the core pathological mechanisms linking clear-turbid interference to atopic dermatitis.Treatment for different stages and syndromes should be guided by syndrome differentiation. For example, for spleen deficiency with a dampness syndrome in subacute or chronic stages,Chushi Weiling Decoction may be used to fortify the spleen, eliminate dampness, lift the clear, and downbear the turbid. For patterns with prominent heat manifestations, such as dampness-heat immersion or heart fire with spleen deficiency,bitter-cold dampness-drying herbs can be added on this basis. For wind-dampness-heat accumulation in the acute stage,Mahuang Lianyao Chixiaodou Decoction can be used to release the exterior, expel pathogens, and harmonize ying and wei.In cases of spleen deficiency with blood dryness or liver-kidney insufficiency during the chronic stage,in addition to nourishing yin to moisten the skin and tonifying the liver-kidney,wind-medicinals should also be employed to open the sweat pores and promote qi movement. These approaches restore the physiological state without clear-turbid interference while regulating intestinal flora, providing new approaches and perspectives for integrating traditional Chinese and Western medicine in treating atopic dermatitis.

Objective:To investigate diagnosis and treatment methods of nontuberculous mycobacteria (NTM) infections related to cosmetic surgery. Methods:This study was a retrospective observational study. From January 2015 to December 2023, 24 patients with NTM infections related to cosmetic surgery meeting the inclusion criteria were admitted to Shenzhen Hospital of Southern Medical University and Northern Theater Command General Hospital, all of whom were female and aged 29-51 years. Among them, 20 cases underwent lipolysis needle injection surgery, 2 cases underwent facial contouring surgery, 1 case underwent foreign body injection, and 1 case underwent wrinkle removal surgery. The clinical manifestations of infections caused by NTM included scattered dark purplish subcutaneous nodules at the infection site with unclear boundaries, thinning of the skin on the surface of the nodules and ulceration, and powdery-white fish meat like tissue and a small amount of light yellow exudation being seen at the ulceration. In order to improve the detection rate of NTM, bacterial culture of wound exudation combined with solid tissue samples, multiple bacterial cultures, and extending culture time were performed. All patients were treated with damage control debridement on the infected site and comprehensive treatment including the use of antibiotics based on the results of drug sensitivity tests. The types of NTM infections, drug sensitivity test results, wound healing time, number of bacterial cultures, and surgical debridement times were recorded. The wound healing was observed during follow-up.Results:The bacterial culture results showed that there were 22 cases of infections with Mycobacterium abscessus and 2 cases with Mycobacterium fortuitum. The drug sensitivity test results showed that the NTM was sensitive to antibiotics such as clarithromycin, amikacin, and doxycycline. The average healing time of the wound was 83.6 days with an average of 2.85 times bacterial cultures on the specimen, and an average of 2.52 times of debridement surgery. Follow up for 6 months to 5 years, all wounds of the patients were healed. Among them, 22 patients had a basically normal appearance with slight scars, while 2 patients had obvious scars and depressions. Conclusions:It is difficult to treat NTM infections related to cosmetic surgery. Damage control debridement for the infected site can ensure wound healing and reduce the damage and postoperative deformities in facial and other cosmetic areas.

Chinese Archives of Otolaryngology-Head and Neck Surgery 2016;23(6):331-334
OBJECTIVE To study the proteome of inferior colliculus and determinate the region-typical proteins which may be the candidate cause of the Central Auditory Processing Disorders. METHODS The telencephalon was taken as reference, and then identified and quantified the proteome of IC of adult rats with iTRAQ. Those with higher abundance in inferior colliculus than the other three regions were considered as IC-Region typical proteins, which may lead to functional specializations. RESULTS We identified 1937 cytomembrane proteins in total, among which there are 53 IC-Region typical proteins, which may lead to functional specializations of inferior colliculus. We used GO and KEGG pathway to analyze these proteins and then found that these proteins mainly take part in the regulation of neurons development and information integrations. CONCLUSION Our quantitative comparison of inferior colliculus has revealed two candidate proteins, including CaMKII and SV2A, which may play important roles in maintaining the balance of excitatory and inhibitory transmitters release. These proteins may be the candidate proteins for Central Auditory Processing Disorders.

Objective To investigate the therapeutic effects of rapamycin (RAPA) on concanavalin A (ConA)-induced acute autoimmune hepatitis in a mouse model and to analyze the possible mechanism. Methods Thirty eight-week-old female C57BL/6 mice were randomly divided into three groups including control group,ConA model group and ConA + RAPA treatment group. The levels of alanine transaminase (ALT) and aspartate aminotransferase (AST) in serum samples were measured after injection of mice with ConA for 24 hours for assessing the liver function.Hematoxylin-eosin (HE) staining was used to observe the hepatic pathological changes in mice.Splenocytes were harvested 24 h after ConA injection for the detection of the percentages of splenic DCs,CD4+T,CD8+T and CD4+ Foxp3+ Treg cells as well as the expression of co-stimulatory molecules (CD40,CD80 and CD86) on DCs by using flow cytometry. Results The levels of ALT and AST in mice from the RAPA treatment group were significantly lower than those of the ConA model group. Results of the HE staining assay showed that the liver damages in RAPA treated mice were less severe than those in mice from the ConA model group.Compared with mice form the ConA model group, those treated with RAPA showed decreased percentages of splenic CD4+ and CD8+ T cells, inhibited expression of CD80 and CD86 on splenic DCs,but increased percentages of splenic CD4+ Foxp3+ Treg cells. No statistically significant differences in the percentages of splenic DCs and the expression of CD40 were observed between the RAPA treatment group and the ConA model group. Conclusion The immunosuppressive effects of RAPA on mice with ConA-induced hepatitis might be achieved through the regulation of immune cells including DCs and T cells in spleen tissues. This study might pave the way for further investigation on the prevention and treatment of autoimmune hepatitis.
