1.Effect of bipolar radio frequency in combination with small incision on lower facial lifting and shaping

Objective:To discuss the effect of bipolar radio frequency (RF) in combination with small incision on lower facial (cheek, chin and neck) shaping and rejuvenation.Methods:From August 2019 to June 2020, 26 patients (aged 35-60 years, with mean 45 years) required for facial rejuvenation and received the fine liposuction in the lower cheek, mandibular margin and submental neck area in Beijing Huangsi Plastic Surgery Hospital. With the help of bipolar RF device - EMBRACE RF (diamond carving), the facetite face and hand radiofrequency were adopted for the subcutaneous tissue to tighten the skin. A concealed small incision was made from the front of the ear to the posterior ear. The superficial layer of the superficial muscular aponeuritic system (SMAS) was stripped, folded, tightened and suspended. The excessive skin was removed while reducing multilayer tension. The incision was cosmetically sutured.Results:All 26 patients had their wounds primary healing, with no complications such as skin necrosis and sensory disorder. All the 24 patients were followed up for 6-12 months. Their scars were concealed and tiny, the skin was tight, the lifting effect was obvious, and the pouch of mouth corners disappeared or alleviated. The contour of the mandibular margin was defined and the mental cervical angle was clear. The shape of the neck was slender and the fat chin was significantly improved. The effect of facial rejuvenation was distinct. Two patients felt it worked but did not achieve the expected results. They were satisfactory after giving the second surgery 6 months later. The satisfactory rate of the first surgery was 92.3% in 24 patients.Conclusions:Bipolar RF in combination with small incision lifting surgery is effective in lower facial (cheek, chin and neck) shaping and rejuvenation. The trauma is little and the wound is recovered fast. The scar is concealed and tiny. It is worthy of clinical application.
2.Initial experience of ileal ureter replacement combined with augmentation cystoplasty in the treatment of ureteral stenosis with contracted bladder

Objective To explore the availability and safety of ileal ureter replacement combined with ileal augmentation cystoplasty in treating ureteral stenosis with contracted bladder.Methods From August 2015 to November 2018,three patients who underwent ileal ureter replacement combined with augmentation cystoplasty were treated with ileal ureter replacement combined with augmentation cystoplasty.There were 1 male and 2 females with the age ranging from 34 to 55 years (mean 39 years).Two patients suffered from left ureter stenosis,and one patient had stenosis on the both sides.The length of the ureter stenosis ranged from 6 to 18 cm (mean 9.8 cm).The preoperative bladder capacity ranged from 60 to 150 ml (mean 103.3 ml).In the surgery,part of ileum was used to replace the ureter,and the distal intestine was made into U-shape to enlarge the bladder.Results All operation were completed successfully.The operation time ranged from 220 to 400 min (mean 303.0 min),and the blood loss ranged from 150 to 500 ml (mean 283.3 ml).Laparoscopic surgery was performed in 1 case and open surgery in 2 cases.Three weeks after the surgery,the bladder volume underwent cystography ranged from 300 to 400 ml (mean 360.0 ml).Three months after the surgery,the postvoid residual urine volume ranged from 20 to 50 ml (mean 33.3 ml).Postoperative frequency and urgency of urine were completely relieved in 1 case,alleviated in 2 cases.Flank pain was completely relieved in 1 case,and alleviated in 2 cases postoperatively.The serum creatinine of 2 patients mildly increased after the surgery,while 1 patient remained stable.For complications,1 patient had urinary infection,and 1 patient suffered from metabolic acidosis.Conclusions Ileal ureteral replacement combined with augmentation cystoplasty can be the choice of treatment for long segment ureteral stenosis and enlarge the bladder simultaneously.The postoperative complications and the kidney functions should be regularly followed up.
3.Research and prospect of hospital scientific research performance management

Accelerating the management of scientific research performance for hospitals can not only mobilize the enthusiasm of scientific researchers, but also effectively optimize their scientific research work. The present academia is plagued in general by such setbacks as indeterminacy of definition and scope of scientific research performance, confusion of the evaluation index system of scientific research performance, and lack of unification of the evaluation principles of scientific research performance of the hospitals. Therefore, it is imperative to define the scope of hospital scientific research performance scientifically, eliminate performance indicator deviation efficiently, accurately analyze hospital scientific research performance management cases, and encourage the application of third-party evaluation methods, which are trendy in research.
4. Effect of modified bandaging method on facial rejuvenation with small incision

Objective:
To observe the clinical effect of the modified bandaging method for facial rejuvenation with small incision and to explore its applicable scope.
Methods:
From March to October 2019, 12 people in the outpatient department underwent small incision facial rejuvenation surgery and the improved bandaging method was used.
Results:
After the small incision facial rejuvenation operation, the relaxation and drooping of the facial tissue in 12 patients were significantly improved without complications, and no cotton pad, bandage and elastic mesh cover were used for fixation.
Conclusions
This improved bandaging method does not affect daily work and life, and meets the needs of patients with this requirement, and plays a very good role in promoting the small incision facial rejuvenation surgery.
5.A study on the diagnostic reliability of classification of intertochanteric fractures

Objective To compare the diagnostic reliability of AO classification and Evans-Jensen classifications in X ray film and three-dimensional CT reconstruction images intertochanteric fractures, and explore advantage of the three-di?mensional CT. Methods A retrospective study was performed to evaluate 54 patients with intertochanteric fractures. Three orthopaedic surgeons were asked to make assessment of fracture classifications using X ray film and the three-dimen?sional CT images. Agreement test was performed to evaluate interobserver and intraobserver reliability for fracture classifica?tion. Results When X ray film was used, mean Kappa values of interobserver reliability for AO and Evans-Jensen classifi?cations between three surgeons were 0.597 and 0.571, that was medium consistency. While using three-dimensional CT to AO classification, lower Kappa value was 0.411, medium consistency. Evans-Jensen classification, Kappa value was 0.704, highly consistency. Each surgeon respectively using X ray film and three-dimensional CT, the mean Kappa values of AO and Evans-Jensen classification were 0.464, medium consistency and 0.191 (0.160-0.233), weak consistency. Conclusion Both classifications based on X ray film show moderate consistency, using three-dimensional CT does not improve the consis?tency of AO classification. The three-dimensional CT is not applicable in Evans-Jensen classification.
6.Protective Effects of Baimai Ointment on Sciatic Nerve Injury in Rabbit Model

OBJECTIVE:To study the protective effects of Baimai ointment on sciatic nerve injury in rabbit. METHODS:The rabbit sciatic nerve injury model was induced by forceps operation. 180 rabbits were randomly divided into 6 groups,with 30 rab-bits in each group (10 rabbits of 7,14,28 d),including sham operation group (blank matrix of Baimai ointment for external use),model group(same as sham operation group),positive group [ig Mecobalamine tablet solution 1.25×10-4 g/(kg·d)] and Bai-mai low-dose,medium-dose and high-dose groups [Baimai ointment 0.33,0.67 and 1.34 g/(kg·d)for external use],once a day, for consecutive 7,14 and 28 d. Gait instrument was used to test the foot-touch-land force of rabbit;pathology examination was conducted for sciatic nerve resection;immunohistochemistry was used to detect the levels of NOS and NMDA in nerve tissue. RE-SULTS:Baimai ointment can significantly relieve sciatic nerve injury,edema and inflammatory reaction;7,14 and 28 d after med-ication,compared with model group,foot-touch-land force decreased significantly in rabbits with sciatic nerve injury (P<0.01);14,28 d after medication,foot-touch-land force of rabbits increased significantly in positive group,Baimai ointment groups,while the levels of NOS and NMDA decreased in nervous tissue(P<0.05). CONCLUSIONS:Baimai ointment can protect sciatic nerve with function injury in rabbits.
7.Analysis of non-government awards on medical technology achievements in China

A deep analysis of the present 26 non-government awards on medical technology achievements was made to probe into setbacks in access to such awards.The findings include lack of independence due to the semi-official nature,tight budget caused by limited funding sources,variety of award sizes due to lack of levels,less corporate involvement to result in less sponsors,and lack of international awards due to unsatisfactory international awareness.The authors recommend such optimal paths as laws and regulations to regulate such awards to be hosted by legal persons,incentives for more enterprises participation to make the awards a public highlight among enterprises,control over the size and types of such awards for better branding,and greater efforts in international collaboration to make international awards.
8.Empirical study on evaluation index system of clinical science and technology achievements LI Hai-yan *,

Objective To verify the evaluation index system of clinical science and technology achievements, taking the evaluation of science and technology achievements in Guangdong province as a demonstration. Methods We specified the criteria for grading each index included in the evaluation index system that we proposed before. 5 experts in clinical medicine were invited to evaluate 20 achievements applying for the Award of Guangdong Science and Technology Achievements in 2010. The weighted sum of scores for each achievement was calculated and used for further analysis. The reliability and validation of the system were assessed. Results The Cronbach's ? coefficient of the index system was 0. 91, indicating good reliability. There were high correlation of each measure with the total score and their affiliated grade one index. The scores that the experts graded were correlated with the original results of evaluation (rs= 0. 71, P <0. 000 1), but there remained some inconsistence. Conclusion The demonstration based on investigation data indicates that the evaluation system is easy to handle and has satisfactory reliability and validation. It can be applied to the evaluation of science and technology achievements in clinical medicine.
9.Study on evaluation index system of clinical science and technology achievements

Objective To set up a scientific, objective, reasonable, qualitative and quantitative,operational evaluation index system of clinical science and technology achievements, which provides the reference to evaluating clinieal science and technology achievements scientifieally. Methods The Delphi method was used to investigate 30 experts for weight of the index via questionnaire. Results The response rate was 100%. The Kendall's concordance coefficient was up to 0. 963. The final index system includes three first-class indicators, eight second-class indicators and 18 third-class indicators. The weight coefficeient of each class was identified. Conclusion The experts showed enthusiasm for this investigation. The agreement of respondents was high after two rounds of investigation. The index system can be used in comprehensive evaluation for science and technology achievements in clinical medicine. The index system takes a guiding effect for science and technology achievements to solve clinical services and problems.
10.Influence of Sanchi gel on TIMP-1 and MMP-1 expression in epidural adhesion

Objective To explore the effects of Sanchi gel on MMP-1 and TIMP-1 expression in epidural adhesion after laminectomy. Methods Laminectomy model was set up in SD rats. 72 SD model rats were divided randomly into Saline group, Zhanlp group, Carbopol Gel group and Sanchi Gel group, with 18 rats in each group. On 7, 14 and 21 days after the laminectomy, immunohistochemistry (method of S-P) was adopted to detect the expression of TIMP-1 and MMP-1. Results Different treatments had different influence on TIMP-1 and MMP-1 expression in epidural adhesion. The expression of TIMP-1 increased gradually on 14 dand 21 d after operation, and Sanqi Gel group showed weaker expression than the other groups (P<0.05), but the number of positive cells decreased gradually. As to the expression of MMP-1, there was no difference among each group at the end of the second week (P>0.05). There was a difference between Sanchi Gel group and the other groups at the end of third week (21 d) (P<0.05) after operation. Sanchi Gel group showed higher expression of TIMP-1 than the other groups. The number of positive cells sharply decreased from 14 d to 21 d after operation. Conclusion Sanchi Gel has a significant preventive effect on fibrous scar formation after laminectomy. The possible mechanism of preventing epidural adhesion after laminectomy by Sanchi Gel could be its regulating and controlling the expression of TIMP-1 and MMP-1 in epidural tissue.
