1.Current status of knowledge,attitude and practice of perioperative lung rehabilitation among thoracic surgery on nurses in 24 provinces and autonomous regions of China:a multicenter investigation

Objective In order to understand the current status and influencing factors of perioperative pulmonary rehabilitation knowledge,attitude,and practice among Chinese thoracic surgery nurses,and to provide references for managers to take targeted measures to improve their pulmonary rehabilitation knowledge,attitude,and practice level.Methods From August to October 2023,a convenience sampling method and cross-sectional descriptive survey were used to anonymously survey 1 167 thoracic surgery nurses from tertiary-level A cancer specialty hospitals and tertiary-level A general hospitals in 24 provinces through the Wenjuanxing platform.A general information questionnaire and a knowledge,attitude and practice questionnaire for lung cancer perioperative pulmonary rehabilitation nurses were used,including knowledge dimension(14 items),belief dimension(6 items),and behavior dimension(14 items),totaling 34 items.Univariate analysis and linear regression multivariate analysis were used to determine the influencing factors of the knowledge,attitude and practice level of thoracic surgery nurses in perioperative pulmonary rehabilitation.Results 1 089 nurses completed the investigation.The total standard score and the dimension scores on knowledge,attitude,and behavior of the perioperative pulmonary rehabilitation questionnaire for thoracic surgery nurses were(80.45±11.84)(73.72±14.68)(89.61±11.38)and(83.46±14.48),respectively.The nature of the hospital,whether there is an administrative position,whether the department carries out ERAS,and whether they have received pulmonary rehabilitation training are the influencing factors of the perioperative pulmonary rehabilitation knowledge,attitude,and practice level of thoracic surgery nurses(P<0.05).Conclusion The knowledge,attitude,and practice of perioperative lung rehabilitation among thoracic surgery nurses are generally at a moderate level.However,their knowledge of perioperative lung rehabilitation is relatively insufficient,and there are many influencing factors.Managers should strengthen pulmonary rehabilitation training for thoracic surgery nurses,improve their knowledge reserves and evidence conversion capabilities,and coordinate cross-integration among multidisciplinary teams to improve their knowledge,attitude and practice in perioperative pulmonary rehabilitation.
2.The experience of exercise adherence in breast cancer patients with endocrine-therapy based on the theory of stage change:a qualitative study

Objective To explore the thoughts and experiences of exercise adherence in breast cancer patients undergoing endocrine therapy,aims to provide reference for targeted intervention.Methods Employing purposive sampling,a descriptive qualitative study was conducted from Octomber 2023 to November 2024,and 24 breast cancer patients receiving endocrine therapy who visited a tertiary-level hospital in Tianjin,China,were selected for semi-structured in-depth interviews.The data was analyzed through traditional content analysis to extract themes.Results 3 primary themes and 6 sub-themes were identified,including sense of benefits from exercise(including physical benefits and psychological well-being),factors influencing exercise adherence(barriers and facilitators),and supportive needs for exercise adherence(including professional support and guidance,as well as emotional support and companionship).Conclusion Healthcare providers should consider the influencing factors,supportive needs of behavior change,and exercise experiences of breast cancer patients receiving endocrine therapy.Developing targeted exercise rehabilitation programs to address these needs could enhance exercise adherence.
3.Construction and application of a decision support education program on hospice care for family members of patients with advanced cancer

Objective To construct a decision support education program for the family members of patients with advanced cancer and to investigate its application effects,so as to improve understanding and acceptance of hospice care for family members of advanced cancer patients.Methods Using the Ottawa Decision Support Framework as a theoretical guide,the program was initially drafted based on a literature review,qualitative interview and expert consultation.From September 2023 to January 2024,a convenience sampling method was used to select patients' families in a tertiary-level hospital in Tianjin as the research subjects,and they were randomly divided into an experimental group and a control group.The experimental group received the decision support education program in addition to routine care,while the control group received routine care.Family members' knowledge about hospice,the scores on the Death Attitude Profile Scale,and their willingness to choose hospice care were compared before and after the interventions.Results The program finally included 4 first-level items,15 second-level items,and 59 third-level items.During the program implementation phase,4 cases withdrew from the study,resulting in 46 cases in the experimental group and 47 cases in the control group.After intervention,the experimental group had higher scores on hospice knowledge and positive attitude towards death than the control group,while scores on negative attitude towards death were lower(P<0.05);their willingness to choose hospice care for themselves and for the patients was higher than that of the control group(P<0.05).Conclusion The hospice care decision support education program is scientific,feasible and practical,which can improve the knowledge of hospice care of the family members,improve their attitude towards death,and ultimately improve their willingness to choose hospice care.
4.Construction of training program for nurses in shared-decision making of breast cancer surgery based on core competence

Objective:To construct a training program for nurses in shared-decision making of breast cancer surgery based on core competence, and to provide reference for clinical training.Methods:From February to May 2023, through literature review, qualitative interview, group discussion, Delphi and analytic hierarchy process, the training program were determined.Results:Among 18 experts, there were 3 males and 15 females, aged (44.67±9.24) years old. The final training program included 6 first-level indicators(admission criteria, training content (theory + practice), training format, training hours, training instructors, training evaluation), 17 second-level indicators, and 57 third-level indicators. The effective response rates of 2 rounds of expert consultations were both 18/18, and the expert authority coefficients were both 0.933. The Kendall coordination coefficients of the first and second round were 0.160, 0.213 (both P<0.05). Conclusions:The training program of decision aid nurses in breast cancer surgery is scientific, practical and clinically valuable, and can provide reference for the development of standardized training for nurses.
5.Construction of an Integrated Comprehensive Rehabilitation Management Service System for Patients with Breast Cancer Based on the Rainbow Model

Objective To construct an integrated comprehensive rehabilitation management service system for patients with breast cancer,and provide theoretical and clinical references for the development of comprehensive management of breast cancer throughout its course.Methods Guided by the rainbow model,the pool of integrated whole rehabilitation management service system for breast cancer was initially constructed by literature analysis and semi-structured interview method,and then the evaluation index system and weight were determined by Delphi method and analytic hierarchy process.Results The effective recovery rates of the two rounds of expert correspon-dence questionnaires were both 100%.The authority coefficients of the expert consultation were 0.863 and 0.861 in the first and second rounds,respectively.In the second round of expert correspondence,the coordination coeffi-cients of each index were 0.753,0.616 and 0.725(P<0.01),respectively.The final integrated comprehensive reha-bilitation management service system for breast cancer includes 6 first-level indicators,15 second-level indicators,and 63 third-level indicators.Conclusion The constructed integrated comprehensive rehabilitation management ser-vice system for patients with breast cancer based on the rainbow model has good scientificity,reliability,and practi-cability,which can provide an effective tool for the professional development of breast cancer management services.
