1.Analysis of factors influencing the effectiveness of pelvic floor muscle repair in postpartum women and their predictive value

Objective:To investigate the factors that affect pelvic floor muscle repair in postpartum women.Methods:The clinical data of 100 patients who gave birth at the Third Affiliated Hospital of Xi'an Medical University between January 2021 and January 2024 and underwent pelvic floor muscle repair were analyzed retrospectively. Based on whether they received the repair, the patients were divided into a repair group ( n = 70) and a non-repair group ( n = 30). The clinical data of both groups were compared. Multivariate logistic regression was performed to analyze the main factors that influence postpartum pelvic floor muscle repair. A predictive model was constructed, and its predictive value for the effectiveness of postpartum pelvic floor muscle repair was evaluated using the receiver operating characteristic curve. Results:After pelvic floor muscle repair, total effective rate was 70% (70/100). There were significant differences between the repair group and the non-repair group in terms of age, parity, number of pregnancies, prenatal body mass index (BMI), neonatal body weight, mode of pelvic floor muscle repair, delivery mode, relaxin level, elastin level, pelvic floor muscle strength classification, degree of stress urinary incontinence, and training compliance (all P < 0.05). Multivariate analysis results indicated that age, prenatal BMI, parity, number of pregnancies, neonatal body weight, pelvic floor muscle strength grading, training compliance, relaxin level, and elastin level were independent influential factors for the effectiveness of postpartum pelvic floor muscle repair. The model can be represented as: Y = 1.253 × age + 0.131 × prenatal BMI + 1.313 × parity + 2.603 × number of pregnancies + 0.559 × neonatal body weight -1.512 × pelvic floor muscle strength grading + 1.271 × training compliance + 0.404 × relaxin level -9.945 × elastin level -4.015. The receiver operating characteristic curve results showed that the area under the curve values for age, prenatal BMI, parity, number of pregnancies, neonatal body weight, pelvic floor muscle strength grading, training compliance, relaxin level, and elastin level in predicting the effectiveness of postpartum pelvic floor muscle repair were 0.650, 0.634, 0.657, 0.786, 0.610, 0.783, 0.645, 0.949, and 0.991, respectively. The area under the curve values for relaxin and elastin levels exceeded 0.9, with the corresponding sensitivity and specificity for relaxin level being 90.00% and 92.90%, respectively, and for elastin level being 100.00% and 95.70%, respectively. Conclusions:Age, prenatal BMI, parity, number of pregnancies, neonatal body weight, pelvic floor muscle strength grading, training compliance, relaxin level, and elastin level are factors that affect the effectiveness of postpartum pelvic floor muscle repair. These factors have certain predictive value for the effectiveness of pelvic floor muscle repair, with relaxin and elastin levels showing particularly high predictive value. Clinically, this information can be used to develop a reasonable pelvic floor muscle repair plan for postpartum women.

Objective:To evaluate the value of ultrasound radiomics combined with machine learning for early diagnosis of seronegative Hashimoto’s thyroiditis (SN-HT) .Methods:This retrospective study included 164 patients from Liaoning Provincial People’s Hospital , Lixin County People’s Hospital, Linghai Dalinghe Hospital, Fengcheng Phoenix Hospital, who underwent thyroidectomy for solitary nodules with normal thyroid function between Nov. 2016 and Jan. 2024. Postoperative pathology confirmed Hashimoto’s thyroiditis (HT) in some cases, who were further categorized into antibody-positive and antibody-negative groups based on serum antibody status. Patients without Hashimoto’s thyroiditis served as the control group. A total of 298 ultrasound images were analyzed. Radiomics features were extracted from hypoechoic non-nodular areas within 0.5 cm surrounding the tumor. Two senior pathologists and two senior ultrasound physicians independently assessed lymphocytic infiltration, eosinophilic changes of follicular epithelium, and the proportion of hypoechoic areas in pathology and ultrasound images, respectively. A machine learning model, CCH-NET, was developed using linear regression and t-distributed stochastic neighbor embedding (t-SNE) techniques. The dataset was divided into a training set (80%) and a validation set (20%) to compare the diagnostic accuracy of CCH-NET with that of senior ultrasound physicians. Results:In internal validation, CCH-NET achieved a diagnostic accuracy of 88.89% for both antibody-positive and antibody-negative groups, significantly higher than the 66.67% accuracy of senior ultrasound physicians ( P<0.01). In external validation, CCH-NET achieved 75.00% and 66.67% accuracy for the two groups, compared to 50.00% by senior ultrasound physicians. For the control group, both methods achieved 93.33% accuracy. The AUC of CCH-NET was 0.848, outperforming senior ultrasound physicians (0.681) ,demonstrating superior diagnostic performance. Conclusion:The radiomics-based CCH-NET model, using non-nodular hypoechoic areas as a specific indicator, can accurately identify early SN-HT in euthyroid patients. It significantly outperforms senior ultrasound physicians, improving diagnostic accuracy and reducing missed diagnoses.
3.Fructus Akebiae induces apoptosis via regulating ROS-mediated PI3K-Akt signaling pathway in non-small cell lung cancer cells

AIM:To explore the way and mecha-nism of cell death induced by Fructus Akebiae in non-small cell lung cancer(NSCLC)cells.METH-ODS:CCK-8,Hochest33342/PI staining,and colony formation experiments were used to detect cell via-bility and proliferation.Western blot was used to detect the expression of cell death-related proteins ATG5,HMGB1,GPX4,apoptosis-related proteins,and PI3K-Akt signaling pathway-related proteins.Flow cytometry was used to detect the apoptosis rate.DCFH-DA fluorescent probes were used to de-tect the levels of total intracellular ROS under a flu-orescence microscope.RESULTS:Fructus Akebiae significantly inhibited the cell viability and prolifera-tion of human NSCLC cells(P<0.05).The expression of apoptosis-related proteins BAX,Cleaved cas-pase3,and Cleaved caspase9 in NSCLC cells was sig-nificantly increased(P<0.05),while p-PI3K,p-Akt,and BCL2 were markedly decreased(P<0.05).Fruc-tus Akebiae was found to clearly increase the apop-tosis rate and the levels of total intracellular ROS(P<0.05).The antioxidant NAC significantly reversed apoptosis,ROS production and regulation of PI3K-Akt pathway related proteins induced by Fructus Akebiae(P<0.05).CONCLUSION:Fructus Akebiae in-duces apoptosis via regulating ROS-mediated PI3K-Akt signaling pathway in non-small cell lung cancer cells.
4.Selection of fecal antigen diagnostic markers for Echinococcus granulosus

This study aims to screen the diagnostic biomarkers for fecal antigen of Echinococcus granulosus(E.granulosus)in dogs with high specificity and sensitivity.The sheep-derived EgPSC artificially infected dogs were collected,and the negative and positive fecal samples of dogs with E.granulosus were prepared by arecoline hydrobromide leakage method.Polyclonal antibody,negative fecal antigen-polyclonal antibody conjugates and positive fecal antigen-polyclonal antibody conju-gates were purified by ammonium sulfate precipitation and affinity chromatography,three groups of samples were detected by ELISA and Western blot,LC-MS/MS and bioinformatics analysis were performed on the three groups of samples.The positive fecal antigen-polyclonal antibody con-jugate was used as the treatment group,the polyclonal antibody and the negative fecal antigen-polyclonal antibody conjugates were used as the control groups to screen the unique peptides of the treatment group.ELISA and Western blot showed that only the positive fecal antigen-polyclonal antibody conjugates were positive.According to LC-MS/MS and bioinformatics analysis,11 unique peptides were screened out only in the treatment group.Among them,3 proteins were related to E.granulosus,namely dysferlin,integrator complex 9 and diagnostic antigen gp50,which were mem-brane-associated proteins,INT complex components and diagnostic antigens.This study has pre-liminarily screened out three candidate canine E.granulosus fecal antigen diagnostic markers,pro-viding a reference for further exploration of diagnostic standards for E.granulosus,screening of echinococcosis target genes,and vaccine development.
5.Application of autofluorescence examination technology in the early warning of potential malignant disea-ses of the oral cavity

Objective:To verify the efficacy of VELscope autofluorescence examination technology in the early warning of potential oral malignant diseases.Methods:80 patients with suspected dysplasia were included and underwent oral examination,visually en-hanced lesion scope(VELscope)autofluorescence examination and histopathology examination respectively,the fluorescence imaging results were compared with the histopathological results.Results:Pathological reports showed 64 cases with mild or no dysplasia,9 cases with moderate or severe dysplasia,1 case with moderate dysplasia without malignant transformation and 6 cases with cancer.The results of VELscope showed that there were 73 positive cases and negative 7 cases of fluorescence deletion.The sensitivity and specificity of VELscope were 93.75%and 9.37%respectively,and the sensitivity could reach 100%in the cancerous tissue.Con-clusion:As a non-invasive examination method,VELscope is highly sensitive but lowly specific.
6.Transcriptome sequencing analysis of the mechanism by which cold water swimming regulates inflammatory response in rats

BACKGROUND:When exercising in a cold environment,the body's inflammatory response is affected by both low temperature and exercise intervention,and its impact and mechanism remain to be explored.OBJECTIVE:To explore the effects and mechanisms of cold water swimming on inflammatory response of rats based on transcriptome sequencing technology.METHODS:40 male SD rats were randomly divided into room temperature control group,room temperature swimming group,cold water control group,and cold water swimming group,with 10 rats in each group.The room temperature control group had no intervention and was free to eat.The room temperature swimming group received swimming at 30 min/time,6 times/week,for 5 weeks;the water temperature was(28±2)℃,and the water depth was 35 cm.In the cold water control group,the rats were placed in a water tank with a depth of 3 cm;the water temperature was(18±2)℃,and they were free to move.The cold water swimming group received swimming at 30 min/time,6 times/week,for 5 weeks;the water temperature was(18±2)℃,and the water depth was 35 cm.Enzyme-linked immunosorbent assay was used to detect the levels of serum interleukin-6,tumor necrosis factor-α,and high-sensitivity C-reactive protein.Based on the transcriptome sequencing results,differentially expressed genes were screened to draw Venn diagrams and heat maps,and Gene Ontology and Kyoto Encyclopedia of Genes and Genomes enrichment analysis were performed.The protein-protein interaction network was used to screen core genes.RT-qPCR was used to detect the mRNA expression of IRF7,OAS2,and OASL in rat spleen tissue.RESULTS AND CONCLUSION:(1)The ELISA results showed that compared with the room temperature control group,the levels of various inflammatory indicators in the room temperature swimming group and the cold water swimming group were significantly increased(P<0.05),and there was no significant difference in the cold water control group.Compared with the room temperature swimming group,there was no significant difference in the expression of inflammatory indicators in the cold water swimming group.Compared with the cold water control group,the expressions of interleukin-6 and tumor necrosis factor-α in the cold water swimming group showed an upward trend,and high-sensitivity C-reactive protein increased significantly(P<0.05).(2)Transcriptome analysis:Venn diagram showed that there were 39 differentially expressed genes affected by the dual factors of temperature and exercise intervention.Cluster heat map analysis results showed that the overall gene expression trends of the room temperature swimming group and the cold water swimming group were similar,and the cold water control group showed an opposite trend.Gene Ontology and Kyoto Encyclopedia of Genes and Genomes enrichment analysis results showed that differentially expressed genes were enriched in the immune system,locomotion,nucleic acid-binding transcription factor activity,NOD-like receptor signaling pathways and other pathways.The number of genes enriched in the NOD-like receptor signaling pathway was relatively large,and the q value was small,which may be a key pathway.The protein-protein interaction network screened out IRF7,OAS2,OASL,IFIT2,IFIT3 and other core genes.(3)RT-qPCR verification results showed that compared with the room temperature control group,the expressions of IRF7,OAS2 and OASL were significantly increased in the room temperature swimming group and the cold water swimming group(P<0.01),and there was no significant difference in the cold water control group.Compared with the cold water control group,the expression of each gene was significantly increased in the cold water swimming group(P<0.01).(4)It is concluded that cold water swimming can promote inflammatory response,and its mechanism may be regulated through the NOD-like receptor signaling pathway.

8.Investigation and Analysis on the Current Situation of Financial Management in Public Hospitals of Anhui Province

Objective:To understand the current status of financial management in public hospitals of Anhui Province and provide a basis for strengthening financial management and promoting high-quality development in public hospitals.Methods:Stratified and random sampling methods were used to select 100 public hospitals as sample hospitals.A self-designed questionnaire was used to conduct a questionnaire survey on the current status of financial management.Results:The overall financial management level of public hospitals in Anhui Province needs to be further improved.The main problems are that the functions of the chief accountant need to be fully exerted,the structure of financial personnel is uneven,the coverage and depth of budget management are insufficient,cost accounting is relatively rough,the operation management system is not sound,the construction of internal control emphasizes form over substance,and the informatization construction needs to be advanced urgently.Conclusion:In response to the existing problems,efforts can be made from fully exerting the functions of the chief accountant,optimizing the talent team,promoting the refinement of budget and cost management,deeply carrying out operation management,improving internal control,and making up for the shortcomings of informatization,to adapt to the new situation and policy requirements of the medical industry's development.

10.Analysis on the value of HE4 combined with 2D ultrasound for vagina in diagnosing the pathological classification of ovarian cancer and predicting its prognosis

Objective:To investigate the value of human epididymal protein 4(HE4)combined with two-dimensional(2D)ultrasound for vagina in diagnosing the pathological classification of ovarian cancer and predicting its prognosis.Methods:The clinical data of a total of 100 patients with ovarian cancer who admitted to Hengshui Maternal and Child Health Care Hospital and Hengshui People's Hospital from April 2018 to July 2023 were retrospectively analyzed,including 98 cases with epithelial ovarian cancer and 2 cases with non-epithelial ovarian cancer.The fasting venous blood pre operation of patients was extracted in morning.The serum HE4 level was detected by enzyme-linked immunosorbent assay(ELISA).The 2D ultrasound examination was performed one week before surgery to measure the resistance index(RI),pulse index(PI),peak systolic flow velocity(PSV)and end-diastolic flow velocity(EDV)of ovarian artery.All patients were followed up immediately after they completed the last chemotherapy.All of patients were divided into a death group(n=27)and a survival group(n=73)according to their survival situation.Cox regression risk model was used to analyze prognostic influence factors of patients with ovarian cancer.Results:The serum HE4 level[449.37(28.57,2 382.24)]pmol/L in patients with epithelial ovarian cancer was significantly higher than that[55.38(17.33,79.64)]pmol/L in patients with non-epithelial ovarian cancer(U=24.752,P<0.05).The RI,PSV and PI of ultrasonic parameters of patients with epithelial ovarian cancer were higher than those of patients with non-epithelial ovarian cancer(t=3.640,2.152,2.588,P<0.05),respectively.The area under curve(AUC)of the receiver operating characteristic(ROC)curve of HE4 combined with 2D ultrasound for vagina was 0.936(95%CI:0.821-1.000)in identifying epithelial and non-epithelial ovarian cancer,which was larger than that of alone each examination.The ratio of the age≥60 years old,the ratio of the III-IV staging of Federation International of Gynecology and Obstetrics(FIGO),and the ratio of existing surrounding infiltration in the death group were all higher than those in the survival group,and the serum HE4 level[528.75(34.79,1 932.43)]pmol/L was higher than that[138.23(21.49,872.59)]pmol/L of the survival group,and the difference was significant(U=25.963,P<0.05).The PSV and EDV values of the death group were all larger than those of the survival group(t=10.844,17.744,P<0.05),and the RI and PI were all less than those of the survival group(t=19.085,13.099,P<0.05).FIGO Ⅲ-Ⅳ staging,surrounding infiltration,HE4 level≥398.74 pmol/L,RI<0.31,PI<0.54,PSV≥26.12 cm/s,EDV≥16.47 cm/s were all risk factors for the prognosis of patients with ovarian cancer(HR=2.682,2.347,2.296,2.518,2.235,2.124,1.958,P<0.05).Conclusion:HE4 combined with 2D ultrasound for vagina can improve the diagnostic accuracy of pathological classification for ovarian cancer,and can be used as an important tool to predict the prognosis of patients with ovarian cancer.
