Despite of the widespread use of hydrogen and methane breath test, the variability in testing protocols, gas measurement techniques, and interpretation criteria continues to challenge the reproducibility and comparability across centers, especially in the Asia-Pacific region. The Asian Neurogastroenterology and Motility Association hence presents the first Asian monograph guiding application and interpretation of breath test. The monograph was formulated according to the framework of indications, preparatory process, performance, and interpretation of results, as well as future direction for research.
2.Visual analysis of domestic literature on traditional Chinese medicine herbal compresses by CiteSpace

Objective To visually analyze the research literature of traditional Chinese medicine (TCM) herbal compresses in China by CiteSpace software, sort out the development process and direction changes of research, reveal cutting-edge hotspots, predict research trends, and provide reference for in-depth research and application of TCM herbal compresses. Methods Literature related to TCM herbal compresses were retrieved from databases including China National Knowledge Infrastructure (CNKI), Wanfang Data Knowledge Service Platform, VIP Journal Integration Platform (VJIP), covering the period from September 1, 2002 to November 15, 2024. CiteSpace software was used for visualization analysis. Results A total of 508 documents were included, and the annual publication volume showed a fluctuating growth. The number of publications was higher in northern regions than in southern regions, with Jilin and Henan provinces being the majority. The institution with the highest number of publications was Changchun University of TCM. The authors with the highest number of publications were Xing Linbo and Hou GuiHong. Collaboration among authors and institutions was generally suboptimal, with limited cross-regional cooperation. The top three high-frequency keywords were nursing, (clinical) efficacy, and pain. The emerging keywords indicated that adverse reactions, clinical efficacy, and traditional Chinese medicine rubbing were hot topics in the study of Chinese medicine herbal compresses. Conclusion The research on TCM herbal compresses in China was in a steady development stage, with research hotspots focused on clinical efficacy observation and adverse reactions of multiple therapies combined. In the future, cross institutional cooperation and in-depth research need to be strengthened to promote further development in this field.

4.Mechanism of Liuwei dihuang pill promoting tendon-bone healing by regulating cell crosstalk through Wnt/β-catenin pathway

OBJECTIVE To explore the mecha nism by which Liuwei dihuang pill promotes tendon-bone healing by regulating the interaction between bone marrow mesenchymal stem cells (BMSCs) and macrophages through Wnt/ β -catenin pathway. METHODS BMSCs were co-cultured with macrophages at a ratio of 1∶4 and divided into control group, 10% drug-containing serum group, 20% drug-containing serum group, Wnt/ β -catenin pathway inhibitor Dickkopf-related protein 1 (DKK-1) group, and 20% drug-containing serum+DKK-1 group. After 48 hours of cell co-culture and 14 days of osteogenic induction, the CD86 and CD206 positive cell rates in macrophages, the proportion of calcium nodule positive staining area in BMSCs, the levels of osteocalcin (OCN) and oncostatin M (OSM) and alkaline phosphatase (ALP) activity in cell supernatant, as well as the mRNA and protein expression levels of OCN, Runt-related transcription factor 2 (RUNX2), OSM, low-density lipoprotein receptor-related protein 5 (LRP5), β -catenin in cells were detected. Pearson correlation analysis was performed to assess the correlation between LRP5 and β -catenin protein expression with OCN, RUNX2, OSM protein expression, and CD206 positive cell rate, respectively. RESULTS Compared with the control group, the CD86 positive cell rates in the 10% drug-containing serum group and the 20% drug-containing serum group were significantly reduced, while the CD206 positive cell rate, proportion of calcium nodule positive staining area, OCN level, OSM level, ALP activity, and mRNA and protein expression levels of OCN, RUNX2, OSM, LRP5, β -catenin were significantly increased ( P <0.05). Compared with the 20% drug-containing serum group, the changes in the above indicators in the 20% drug-containing serum+DKK-1 group were significantly reversed ( P <0.05). The protein expression of LRP5 and β -catenin was positively correlated with the pro tein expression of OCN, RUNX2, OSM, and CD206 positive cell rate. CONCLUSIONS Liuwei dihuang pill can activate the Wnt/ β -catenin pathway, induce M2 polarization of macrophages and secretion of OSM, promote osteogenic differentiation and matrix mineralization of BMSCs, and synergistically promote tendon-bone healing.

6.A Study on Optimization Strategies for Bidirectional Referral of Diseases Based on DRG in Graded Diagnosis and Treatment

Objective To analyze the current status of medical services in secondary and tertiary hospitals in Shandong Province based on DRG,and to provide effective support for the implementation strategies of bidirectional referral.Methods The DRG group was used to analyze 42.348 3 million cases from 75 secondary hospitals and 69 tertiary hospitals in Shandong Province from 2019 to 2023.Experts were organized to establish standards for bidirectional referral of diseases.Results(1)35 high-frequency DRG disease groups with diagnosis,treatment ability and medical resources in secondary hospitals were selected,(2)The medical expenses and medical quality in the high-frequency DRG disease groups within secondary hospitals were lower than those in tertiary hospitals,(3)To develop standardized referral standards and programs with esophageal cancer as an example.Conclusion It is urgent to triage patients gradually and accurately through disease classification management,and formulate disease diagnosis and treatment plans and bidirectional referral standards to improve the medical quality of secondary hospitals.

To formulate an expert consensus on the principles of preoperative hair removal and provide scientific guidance for standardized removal of hair before surgical procedures so as to reduce the incidence of surgical site infections.METHODS Led by the Hospital Management Institute of National Health Commission of the People's Republic of China,this consensus was reached with the joint efforts from the expects of relevant fields such as surgeries,interventional therapies,nursing,and infection prevention and control.The consensus facilitates the classification and evaluation of literatures by following the evidence grade formulated by Oxford Evidence-based Medicine Center and focuses on the association of preoperative hair removal with surgical site infection,it reaches the evidence grade of expert consensus and recommendation intensity by integrating with discussions on meetings and clinical experience of the expects from relevant fields.RESULTS A total of 6 items of consensus were reached by summarizing the latest evidence on the aspects including the indications for preoperative hair removal,tools,range,timing and places.CONCLUSION The consensus,to some extent,make supplements to and complete the exiting regulations and standards.It provides guidance for the medical institutions to carry out the preoperative hair removal.
8.Disease burden and trends in enteric infections in China,1990-2021:a One Health perspective

This study analyzed the burden and trends in enteric infections in China from 1990 to 2021 from a One Health perspec-tive.Data on mortality associated with enteric infections were extracted from the 2021 Global Burden of Disease(GBD)database.The analysis focused on assessing the mortality rates of enteric infectious diseases attributed to various etiologies and risk factors,along with the age and sex distribution,from 1990 to 2021.Average annual percentage change(AAPC)was used to assess the total changes in disease burden.The age-standardized mortality rate of intestinal infections in China decreased from 9.642/100 000 in 1990 to 0.439/100 000 in 2021,with an AAPC of-57.103%(95%CI:-57.118%to-57.088%).In 2021,Rotavirus,Norovirus,and Crypto-sporidium were the top three etiologies contributing to disease burden,with mortality rates of 1.020/100 000,0.040/100 000 and 0.079/100 000,respectively.A significant variation in etiology distribution was observed across age groups:Rotavirus,Shigella,and Crypto-sporidium dominated among children under 5 years of age,whereas Cryptosporidium,Norovirus,and Clostridioides difficile were more prevalent in older populations.Risk factor analysis indicated that unsafe water sources and poor sanitation accounted for 73.394%of all enteric disease-related deaths.In conclusion,the burden of enteric infections in China markedly declined from 1990 to 2021,and sig-nificant variations in the etiological spectrum and disease burden were observed across age groups.The persistent effects of unsafe wa-ter sources and poor sanitation underscore the need for targeted interventions to further decrease the burden of these diseases.Our find-ings highlight the success of public health interventions in decreasing the burden of enteric infections in China,while emphasizing the need for targeted measures to address disparities in high-risk populations and improve environmental sanitation.
9.Dehydrocostus lactone ameliorates renal interstitial fibrosis in rats with unilateral ureteral obstruction by inhibiting the TGF-β1/Smad2/3 pathway

Objective To explore the action mechanism of dehydrocostus lactone(DHL)on renal interstitial fibrosis(RIF)in rats with unilateral ureteral obstruction(UUO).Methods Forty-four male SD rats were randomly divided into four groups:the sham surgery group(Sham group),the pure drug intervention group(Sham+DHL group),the experimental group(UUO+Vehicle group)and the DHL treatment group(UUO+DHL group),with 11 rats in each group.The rats underwent sham surgery,sham surgery+DHL[10 mg/(kg·d)],UUO modeling+the same volume of solvent and UUO modeling+DHL[10 mg/(kg·d)],respectively.After surgery,DHL or the same volume of solvent was administered by gavage for 14 days starting from day 2 post-surgery.Hematoxylin-eosin(HE)and Masson staining were used to observe the pathological changes in renal tissue.Immunohistochemical staining was performed to detect the expression levels of collagen I,collagen III and α-smooth muscle actin(α-SMA).Western blotting was used to detect the protein levels of the transforming growth factor(TGF)-β1/Smad2/3 pathway.Results Compared with the UUO+Vehicle group,DHL treatment alleviated renal interstitial pathological damage,reduced collagen fiber deposition,and decreased the expression of collagen I,collagen III and α-SMA.It also inhibited the expression of TGF-β1 and Smad2/3 proteins.Conclusions DHL mitigates RIF in rats by inhibiting the TGF-β1/Smad2/3 pathway,providing a new strategy for the treatment of chronic kidney disease.
