1.Atopic Dermatitis Associated Retinal Detachment and Retinal Vasculitis:A Case Report

Atopic dermatitis is usually associated with various ocular complications.We report a 21-year-old Chinese male who presented to our ophthalmology clinic with bilateral retinal detachment and cataracts.The patient had a clear medical history of atopic dermatitis,which had been diagnosed eight years earlier and had been treated with loratadine and pimecrolimus.Cataract surgery was performed for both eyes,combined with scleral buckling for the right eye and pars plana vitrectomy for the left eye.During postoperative follow-up,fundus fluorescein angiography showed retinal vasculitis in both eyes and macular edema in the left eye,which coincided with an exacerbation of atopic dermatitis.Macular edema improved after four months of regular dupilumab treatment in the dermatology department.The ocular condition remained stable three years postoperatively.
2.Serological Survey of Zika Virus in Humans and Animals in Dejiang Prefecture, Guizhou Province, China.

OBJECTIVE:
The current outbreak of Zika virus (ZIKV) poses a severe threat to human health. Two ZIKV strains were isolated from mosquitoes collected from the Dejiang prefecture in China in 2016, which was the first isolation of ZIKV in nature in China.
METHODS:
In this study, serum samples were collected from 366 healthy individuals and 104 animals from Dejiang prefecture in 2017, and the plaque reduction neutralization test (PRNT) was used to evaluate the seroprevalence of ZIKV.
RESULTS:
None of the 366 residents from whom the samples were collected were seropositive for ZIKV. None of the 11 pigs from whom the samples were collected were seropositive for ZIKV, while 1 of 63 (1.59%) chickens and 2 of 30 (6.67%) sheep were seropositive for ZIKV.
CONCLUSION
The extremely low seropositivity rate of ZIKV antibodies in animals in the Dejiang prefecture, Guizhou province in this study indicates that ZIKV can infect animals; however, there is a low risk of ZIKV circulating in the local population.
3.Identification of Y-chromosomal Genetic Types for the Soldier's Remains from Huaihai Campaign

Objective To identify the Y-chromosomal genetic types for the soldier's remains from Huaihai Campaign,and to offer a clue for search of their paternal relatives.Methods DNA of the remains were extracted by the ancient DNA extraction method.Yfiler kit was used for the multiplex amplification of 17 Y-STR loci.The haplogroups of the samples were speculated.Detailed genotyping of the selected Y-SNP was performed based on the latest Y-chromosome phylogenetic tree.Haplotype-sharing analysis was done based on the data of Y-SNP and Y-STR,the closest modern individual information to the genetic relationship of remains was gained.Results A total of 8 Y-STR haplotypes were observed on 17 Y-STR loci of 8 male individuals.Furthermore,6 Y-SNP haplogroups were identified,which were O2a1-M95+,O1a1-P203+,O3*-M122+/M234-,D1-M15+,C3*-ST and R1a1-M17+.Conclusion Identification of Y-chromosomal genetic types for the soldier's remains from Huaihai Campaign shows a reference value on inferring the geographical origins of old materials.
4.Body temperature control in patients with refractory septic shock: too much may be harmful.

BACKGROUNDThe lowering of body temperature is a common, almost reflexive step in the daily care of septic shock patient. However, the effect of different magnitudes of fever control on the outcome of refractory septic patients with a very poor outcome is controversial and has yet to be explored.
METHODSThis prospective trial examined sixty-five refractory septic shock patients with a core temperature higher than 38.5°C. Patients were randomly assigned to a group achieving a "low temperature" range (LT group: 36.0 - 37.5°C) or to a group achieving a "high temperature" range (HT group: 37.5 - 38.3°C) by physical methods including a water-flow cooling blanket and ice packs. A target core temperature was achieved in 1 - 2 hours post-treatment, and maintained for 72 hours. Averaged values of core temperature as well as hemodynamic, respiratory, and laboratory variables were analyzed at baseline and during the first 72 hours after fever control.
RESULTSThirty-four (52.31%) patients were assigned to the LT group and thirty-one (47.69%) patients were assigned to the HT group. The mean core temperature was significantly lower in the LT group than in the HT group (36.61 vs. 37.85°C, respectively; P < 0.0001). The average heart rate (HR) (75.5 vs. 91.9 beats/min, respectively; P < 0.0001) and the mean cardiac output (CO) (5.35 vs. 6.45 L/min, respectively; P = 0.002) were also statistically significant lower in the LT group than in the HT group. The averaged serum lactate level was significantly higher in the LT group compared to the HT group (5.59 vs. 2.82 mmol/L, respectively; P = 0.008). Fibrinogen and activated partial thromboplatin time were also different between the two groups. The 28 days mortality was significantly higher in the LT group than in the HT group (61.8 vs. 25.8%, respectively; P = 0.003). A Cox-regression model analysis showed that mean core temperature during the 72 h period was an independent predictor of 28 days mortality (odds ratio (OR) = 0.42, 95%CI 0.25, 0.6; P = 0.001).
CONCLUSIONControlling fever to a lower range (36.0 - 37.5°C) may be harmful to patients with refractory septic shock by worsening tissue perfusion, compared to controlling it within a higher range (37.5 - 38.3°C). An understanding of the mechanisms responsible for these observations requires further investigation.

5.Establishment and preliminary application of measuring method for three-dimensional kinematics of the hindfoot joint in vivo

Objective To establish a new method for measuring the 3-D kinematics of hindfoot joints in vivo by using reverse engineering software together with the theory of rigid body kinematics.Methods CT images were gathered from 9 feet of 5 healthy volunteers in both the initial position (neutral position) and the terminal position (extremely inversion-adduction-dorsiflexion position).The 3-D digital modules of hindfoot joints in the initial position and terminal position were established with MIMICS 10.01 software.The data of reconstructed digital modules was inputted into the GEOMAGIC 10.0 software in STL format for twice registration,and then their relatively displacement and changes of angle in 3-D space between the two positions were calculated Results The rotation range of the tibiotalar joint was 3.89° ±2.77° in eversion,5.29°±4.47° in abduction,10.77°+5.70° in dorsiflexion,and the relative displacement was 0.78±0.59 mm towards lateral ankle,0.18±0.75 mm towards the hindfoot,(0.65±0.71) mm towards the proximal limbs;the range of subtalar joint was 16.46°±2.94° in inversion,12.77°±1.81° in adduction,6.33°±4.32° in plantarflexion,and the relative displacement was 5.50±1.45 mm towards medial ankle,1.96±1.77 mm towards forefoot,0.43±1.18 mm towards distal limbs; the range of talonavicular joint was 38.82°±5.98° in inversion,19.71°±6.33° in adduction,5.09°±6.89° in plantarflexion,and the relative displacement was (9.77±1.73) mm towards medial ankle,3.13±1.29 mm towards hindfoot,4.64±1.42 mm towards proximal limbs.Conclusion This method measuring 3-D kinematics of hindfoot joints in vivo is non-invasive and easy to operate.
6.CD34+ antigen expression relating to prognosis in acute myeloid leukemia.

To explore CD34(+) antigen expression in new diagnosed acute myeloid leukemia (AML) and analyze the prognosis for CD34(+) AML patients, the expression of antigen CD34 in 238 AML patients was detected by indirect immunofluorescence assay. The results showed that CD34 in 92 out of the 238 patients (38.7%) were positive, there was relationship between the CD34(+) expression and FAB subtypes (M(0), M(1)), and no CD34(+) expression was observed in M(3) subtypes. The complete remission rate of CD34(+) AML patients was 32%, which was lower than that of CD34(-) AML (61%). The lymphoid-associated antigen (CD7) was significantly increased in CD34(+) AML patients, compared with CD34(-) patients (P < 0.05). It is concluded that CD34(+) AML patients show poor prognosis and lower CR rate. The detection of CD34 expression is of some value in predicting prognosis in AML.

7.Experience of vaginal reconstruction by using a pudendal-thigh island flaps.

OBJECTIVETo evaluate a pudendal-thigh island flap for vaginal reconstruction.
METHODSForty-seven patients with congenital absence of vagina were undergoing the treatment. Based on the pedicle including the posterior labial neurovascular bundle, a pudendal-thigh island flap was designed and raised in the groin crease just lateral to the labia majora under the deep fascia. It was then transferred to the tunnel between the urethra and the anus for reconstruction of the vagina.
RESULTSFrom May of 1993 to July of 2001, 47 patients were successfully treated for the vaginal reconstruction with the flap. The results were satisfactory without complications.
CONCLUSIONThe above mentioned technique could be a safe and effect method for vaginal reconstruction with the advantages of reliable blood supply, good sensation and few complications. The areaes with the bilateral pudendal-thigh could be large enongh for the vaginal reconstruction without problem of the donor closure.

8.The value of the dynamic mechanical spinal test in the management of automated percutaneous lumbar discectomy

Objective To prospectively assess the predictive power of centralization phenomenon in the curative effect of automated PLD.Methods The survey population was consisted of 109 patients with inclusion heraiation demonstrated by CT/MRI,74 men and 35 women with average age of 43.1 years(17~75 years). All were complained of low back pain,with varying degrees of lower extremity pain and altered sensation, lasting for more than 2 months;including one symptomatic disc in 99 patients and two symptomatic discs in 10 patients.Patients were undergone dynamic mechanical spinal test and reported whether the test would aggravate their pain.The assessment included forward flexion,extension,rotation of the trunk to the right and left, rotation to the left with fight extension,rotation to the fight with left extension,and whether straight leg raising in the supine position would aggravate back pain or leg pain.Symptom resposes were categorized into three groups:centralization group(CG),partial-centralization group(PCG)and noncentralization group(NCG). Centralization of pain is the progressive retreat of the most distal extent of the referred or radicular pain toward or to the lumbar midline.Noncentralization of pain is the peripheralization of pain in one or more directions, and no change in the distal-most pain location or intensity.All patients received a single therapy with PLD. Results A follow-up of 109 patients for 3 to 6 months,including 46 cases with 24 as exellent and 22 as good reaching 100% of excellent good rate in CG by MacNab standards;43 cases with 5 as exellent,29 as good,9 as fair and poor,with total effective rate of 79.1% in PCG.Twenty cases of NCG symptoms showed no improvement and therefore surgery was considered.Conclusions Centralization phenomenon occurrence during initial mechanical evaluation is a very accurate predictor for successful PLD outcome.Nonoccurrence of centralization would accurately predict poor PLD outcome and thus helpful as early predictor of the need for surgical treatment.
