1.Pathogenic Mechanisms of Spleen Deficiency-Phlegm Dampness in Obesity and Traditional Chinese Medicine Prevention and Treatment Strategies:from the Perspective of Immune Inflammation

Based on spleen deficiency-phlegm dampness as the core pathogenesis of obesity, and integrating recent advances in modern medicine regarding the key role of immune inflammation in obesity, this paper proposes a multidimensional pathogenic network of "obesity-spleen deficiency-phlegm dampness-immune imbalance". Various traditional Chinese medicine (TCM) herbs that strengthen the spleen, regulate qi, and resolve phlegm and dampness can treat obesity by improving spleen-stomach transport and transformation, promoting water-damp metabolism, and regulating immune homeostasis. This highlights immune inflammation as an important entry point to elucidate the TCM concepts of "spleen deficiency-phlegm dampness" and the therapeutic principle of "strengthening the spleen and eliminating dampness to treat obesity". By systematically analyzing the intrinsic connection between "spleen deficiency generating dampness, internal accumulation of phlegm dampness" and immune dysregulation in obesity, this paper aims to provide theoretical support for TCM treatment of obesity based on dampness.
2.Epidemiological characteristics analysis of tuberculosis among college students in Yangzhou during 2020-2024

Objective:
To analyze the epidemiological characteristics of pulmonary tuberculosis (PTB) among college students in Yangzhou from 2020 to 2024, so as to provide a scientific basis for developing prevention and control strategies.
Methods:
An epidemiological investigation was conducted among 162 college students with PTB, and 7 134 of their contacts were screened. Data were obtained from the tuberculosis information management system and on campus screening records. Using descriptive epidemiological methods, trends in incidence, seasonal distribution, and bacteriological characteristics were analyzed.
Results:
From 2020 to 2024, the annual average incidence of pulmonary tuberculosis among college students in Yangzhou was 29.42 per 100 000, showing an overall fluctuating downward trend ( χ 2=12.36, P <0.01). Cases were mainly concentrated in summer and autumn, with the highest proportion in autumn (41.36%, 67/162), followed by summer (23.46%, 38/162). The proportion of etiologically positive cases increased from 37.21% in 2020 to 71.43% in 2024; among positive cases, the proportion of latent tuberculosis infection (LTBI) decreased from 66.67% (10/15) to 26.67% (4/15). The etiological positive rate was higher in females than in males ( χ 2= 11.76 , P <0.01). Comparison of screening methods showed that among index cases, the LTBI detection rate of the recombinant Mycobacterium tuberculosis fusion protein skin test (C-TST) was higher than that of the tuberculin skin test (TST), but the difference was not statistically significant ( χ 2=0.65, P =0.42); among close contacts, the detection rate of TST was higher than that of C-TST (15.1%,10.1%; χ 2=5.23, P <0.05).
Conclusion
From 2020 to 2024, the annual average incidence of pulmonary tuberculosis among college students in Yangzhou showed an overall fluctuating downward trend, with differences in TB infection screening methods and gender.
3.Application of HTS2 Technology in Traditional Chinese Medicine and Formulas: A Review

High-throughput sequencing-based high-throughput screening (HTS2) technology, as a new advancement in the field of high-throughput biotechnology, is the world's first technology to integrate high-throughput sequencing into large-scale drug screening and target discovery. The artificially designed DNA probes were bound to the undetermined mRNAs of thousands of genes in cell lysates, and then the probes were ligated with ligases. The large-scale simultaneous detection of gene expression changes in thousands of drug-treated cell samples was performed using barcoding, automated operating platforms, and high-throughput sequencers. This technology enables high-throughput identification of drugs that significantly perturb the gene expression profiles characteristic of diseases. It can also take gene expression signature as the readout and exert great high-throughput advantages in the screening of multi-drug, multi-component, and multi-target drugs, as well as the research on complex mechanisms. Therefore, it is particularly suitable for elucidating the multi-target mechanisms of traditional Chinese medicine and identifying its multi-effective components. Its main technical advantages include high throughput, automation, and low cost. In recent years, HTS2 technology has yielded important achievements in the elucidation of the mechanism of action of traditional Chinese medicine formulas, the scientific connotation analysis of the regional characteristics of traditional Chinese medicine, the targeted isolation of active compounds of traditional Chinese medicine, and the discovery of novel pharmacological functions of monomeric compounds of traditional Chinese medicine. In the era of artificial intelligence, HTS2 technology will serve as a powerful tool for generating high-quality, original big data of traditional Chinese medicine, providing core data support and promoting AI-driven traditional Chinese medicine research. Ultimately, HTS2 technology offers new strategies and critical data support for deeply analyzing the scientific connotation of traditional Chinese medicine and discovering novel traditional Chinese medicine-based drugs, thereby accelerating the modernization and internationalization of traditional Chinese medicine in China.
4.Qijia Rougan Decoction Ameliorates Liver Fibrosis Through miRNA-mRNA Network

ObjectiveTo explore the mechanism by which Qijia Rougan decoction ameliorates liver fibrosis through amino acid/fatty acid metabolic reprogramming and phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt) pathway, based on the miRNA-mRNA regulatory network and the interaction between metabolism and signaling pathways. MethodsSprague-Dawley (SD) rats were randomized into four groups (n=8): control, model, and low-dose and high-dose (7.0, 28.0 g·kg-1·d-1, respectively) Qijia Rougan decoction. Liver fibrosis was induced by subcutaneous injection of carbon tetrachloride (CCl4). From week 9, drug intervention was implemented for 7 weeks. After the final administration, the pathological changes in the liver were evaluated through hematoxylin-eosin (HE) and picrosirius red (PSR) staining. An automated biochemical analyzer was used to measure the serum levels of biochemical indicators, including alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP), total bile acid (TBA), albumin (ALB), and cholesterol (TC). High-throughput miRNA sequencing was performed to identify differentially expressed miRNAs (DemiRs) during liver fibrosis. A miRNA-mRNA interaction network was constructed to identify key targets, which were then subjected to GO and KEGG enrichment analyses. The expression levels of selected DemiRs were validated by Real-time PCR. ResultsCompared with the control group, the model group showed marked hepatic lobular necrosis, increased collagen deposition, significant fibrosis, elevated serum levels of ALT, AST, ALP, and TBA (P<0.01), and declined levels of ALB and TC (P<0.01). Compared with the model group, Qijia Rougan decoction treatment reduced hepatic necrosis, collagen accumulation, and fibrosis, lowered the serum levels of ALT, AST, ALP, and TBA (P<0.01), and raised the levels of ALB and TC (P<0.01). Integrated miRNA-seq and RNA-seq analysis identified 31 DemiRs (6 upregulated and 25 downregulated) and 498 targets. The expression trends of four selected DemiRs, including rno-miRNA-376b-3p, were consistent with sequencing results (R2=0.93). Functional annotation revealed that top 20 upregulated targets were enriched in amino acid and fatty acid metabolism, while top 20 downregulated targets were significantly associated with the PI3K/Akt signaling pathway and cancer progression. ConclusionQijia Rougan decoction alleviates liver fibrosis by reconstructing the miRNA-mRNA regulatory network, promoting metabolic reprogramming, and inhibiting the PI3K/Akt signaling pathway. These findings provide mechanism evidence supporting the multi-targeted antifibrotic effects of traditional Chinese medicine compound formulas.
5.Lymph nodes metastasis in pediatric patients with differentiated thyroid cancers:a prediction model based on multi-center data

OBJECTIVE To analysis the clinical and pathological characteristics of lymph nodes metastasis in pediatric patients with differentied thyroid carcinoma(pDTC),and to develop a nomogram for predicting lymph nodes metastasis in patients with pDTC.METHODS Four cohorts were integrated in this study,including internal training cohort(1419 cases)and validation cohort(609 cases)from the Surveillance,Epidemiology,and End Results(SEER)database of the National Cancer Institute,TCGA-pDTC cohort(20 cases from the Cancer Genome Atlas Program)and YHD-pDTC cohort(39 cases from true world).The least absolute shrinkage and selection operator(Lasso)and logistic regression were used to screen factors related to lymph nodes metastasis in the presence and exclusion of T stage in pDTCs based on internal training cohort and construct nomograms.Calibration curve,receiver operating characteristic(ROC)curve,area under the ROC curve(AUC)and decision curve analysis(DCA)were used to evaluate model performance in the other three cohorts.RESULTS Multivariate logistic regression results showed that pathology[OR(95%CI):0.020(0.010-0.060),P<0.001],M stage[OR(95%CI):29.550(3.190-273.490),P=0.003],and T stage[OR(95%CI):2.210(1.820-2.680),P<0.001]were independent factors affecting lymph nodes metastasis in pDTCs.Through the internal training set,internal validation set,YHD-pDTC cohort and TCGA-pDTC cohort,the AUC values were 0.734(0.709,0.758),0.752(0.716,0.788),0.969(0.920,1),and 0.600(0.342,0.858)in that order,and the calibration curves are close to the perfect reference line.T stage is a better predictor of lymph nodes metastasis than extraperitoneal intrusion and tumor size.CONCLUSION Pathology,M stage,and T stage are risk factors for predicting lymph nodes metastasis in pDTCs.The nomogram established on this basis can make individualized predictions of the probability of lymph nodes metastasis in pDTCs.
6.Associations between qi stagnation constitution, suboptimal health status, and lifestyle factors in southern China: A population-based cross-sectional study

ObjectiveTo study the relationship between qi stagnation constitution and suboptimal health status (SHS) or lifestyle.MethodsFrom 2012 to 2013, we conducted a cross-sectional survey of 24 159 Chinese individuals aged 12–80 years. The qi stagnation constitution was assessed using the Constitution in Chinese Medicine Questionnaire. Health status was evaluated through medical records and the Subhealth Measurement Scale V1.0 (SHMS V1.0). Health-promoting lifestyles were measured using the Health-Promoting Lifestyle Profile II (HPLP-II).ResultsOf the 24 159 participants, 16.1% and 15.2% were classified as “always” and “sometimes” having the qi stagnation constitution, respectively. Those classified as “rarely” having the qi stagnation constitution scored higher on both the HPLP-II and SHMS V1.0. The participants classified as “always” having the qi stagnation constitution showed a significant association with SHS or disease compared to other imbalanced constitutions. Those in the “always” category were approximately 21 times more likely to be classified as having SHS (odds ratio [OR]: 21.17, 95% confidence interval [CI]: 15.74–28.45), whereas those in the “sometimes” category were approximately six times more likely (OR: 5.89, 95% CI: 5.04–6.90). Accordingly, the qi stagnation constitution score was significantly associated with the diagnosis of SHS, with an area under the curve of 0.77 (P .001). A score of 18.75 yielded the highest Youden Index (0.407), with a sensitivity of 60.5% and a specificity of 80.3%. Significant associations were observed between health-promoting lifestyles and qi stagnation constitution severity in an ordinal regression analysis (P .001). Protective factors included stress management (OR: 1.59), self-actualization (OR: 1.57), and exercise (OR: 1.36). In contrast, poorer interpersonal relationships (OR: 0.79), greater health responsibilities (OR: 0.86), and poorer nutrition (OR: 0.91) were associated with increased severity.ConclusionModulating the qi stagnation constitution through lifestyle interventions may help prevent the progression of SHS to disease, which aligns with core preventive principles in traditional Chinese medicine.
7.Ethical challenges and solution paths of medical artificial intelligence technology based on intelligent algorithms

Intelligent algorithms, as the core element of artificial intelligence (AI), have played an irreplaceable and key role in fields such as medical AI-assisted diagnosis and treatment. Meanwhile, they have also presented new features such as the complexity of interdisciplinary integration, the conflict of ethical convergence in science and technology, the duality of risks and benefits coexistence, and the concealment of algorithm black box calculations. These features lead to numerous ethical challenges, such as algorithm bias, data security, and difficulty in tracing responsibility. Therefore, it is urgent to break through these challenges by establishing a micro-ethical institutional system for intelligent medical algorithms, setting up an ethical supervision mechanism for the operation of intelligent medical algorithms, improving ethical norms for data security protection of intelligent medical algorithms, and strengthening ethical standards for the identification of the main responsibility of intelligent medical algorithms.

Pulpotomy, which belongs to vital pulp therapy, has become a strategy for managing pulpitis in recent decades. This minimally invasive treatment reflects the recognition of preserving healthy dental pulp and optimizing long-term patient-centered outcomes. Pulpotomy is categorized into partial pulpotomy (PP), the removal of a partial segment of the coronal pulp tissue, and full pulpotomy (FP), the removal of whole coronal pulp, which is followed by applying the biomaterials onto the remaining pulp tissue and ultimately restoring the tooth. Procedural decisions for the amount of pulp tissue removal or retention depend on the diagnostic of pulp vitality, the overall treatment plan, the patient's general health status, and pulp inflammation reassessment during operation. This statement represents the consensus of an expert committee convened by the Society of Cariology and Endodontics, Chinese Stomatological Association. It addresses the current evidence to support the application of pulpotomy as a potential alternative to root canal treatment (RCT) on mature permanent teeth with pulpitis from a biological basis, the development of capping biomaterial, and the diagnostic considerations to evidence-based medicine. This expert statement intends to provide a clinical protocol of pulpotomy, which facilitates practitioners in choosing the optimal procedure and increasing their confidence in this rapidly evolving field.

Digital technologies have become an integral part of complete denture restoration. With advancement in computer-aided design and computer-aided manufacturing (CAD/CAM), tools such as intraoral scanning, facial scanning, 3D printing, and numerical control machining are reshaping the workflow of complete denture restoration. Unlike conventional methods that rely heavily on clinical experience and manual techniques, digital technologies offer greater precision, predictability, and efficacy. They also streamline the process by reducing the number of patient visits and improving overall comfort. Despite these improvements, the clinical application of digital complete denture restoration still faces challenges that require further standardization. The major issues include appropriate case selection, establishing consistent digital workflows, and evaluating long-term outcomes. To address these challenges and provide clinical guidance for practitioners, this expert consensus outlines the principles, advantages, and limitations of digital complete denture technology. The aim of this review was to offer practical recommendations on indications, clinical procedures and precautions, evaluation metrics, and outcome assessment to support digital restoration of complete denture in clinical practice.

10.Hei Xiaoyaosan Modulates Oxidative Stress and Apoptosis to Exert Neuroprotective Effect in Alzheimer's Disease Rats

ObjectiveTo explore the role and mechanism of Hei Xiaoyaosan in regulating the protein kinase B (Akt)/glycogen synthase kinase-3β (GSK-3β)/nuclear factor E2-related factor 2 (Nrf2)/heme oxygenase-1 (HO-1) signaling pathway in cascade modulation of oxidative stress and apoptosis for preventing and treating Alzheimer's disease (AD). MethodsNinety male SD rats of 4 months old were randomly assigned into a control group (n=10), a sham group (with injection of 1 μL normal saline into bilateral hippocampi, n=10), and a modeling group (with injection of 1 μL beta-amyloid 1-42 solution into bilateral hippocampi to induce AD, n=70). One week after modeling, 50 successfully modeled rats were selected and randomly allocated into model, donepezil hydrochloride (0.45 mg·kg-1), and high-, medium-, and low-dose (15.30, 7.65, 3.82 g·kg-1, respectively) Hei Xiaoyaosan groups. The rats were administrated with corresponding drugs once daily for six consecutive weeks. The Morris water maze was used to assess the learning and memory abilities of rats. Hematoxylin-eosin (HE) staining was performed to reveal hippocampal morphological changes in AD rats. Apoptosis in the hippocampal CA3 region was detected by terminal-deoxynucleotidyl transferase-mediated Nick end labeling. Immunofluorescence was used to visualize the expression of neuronal nuclear antigen (NeuN) in the CA1 region. Additionally, enzyme-linked immunosorbent assay was performed to assess the activities of glutathione peroxidase (GSH-Px), glutathione-S-transferase (GST), and catalase (CAT) in the hippocampus. Real-time PCR was conducted to measure the mRNA levels of Akt, GSK-3β, Nrf2, and HO-1, while Western blot was employed to determine the protein levels of phosphorylated Akt (p-Akt)/Akt, phosphorylated GSK-3β (p-GSK-3β)/GSK-3β, Nrf2, and HO-1. ResultsCompared with the control group, the model group on day 5 showed an increase in total swimming distance (P<0.01), a reduction in the percentage of time spent in the target quadrant (P<0.01), reduced and disarranged neurons, nuclear condensation, varying degrees of cellular damage, increased apoptosis of hippocampal neurons (P<0.01), decreased NeuN content (P<0.01), weakend activities of GSH-Px, GST, and CAT (P<0.01), and down-regulated mRNA levels of Nrf2 and HO-1 (P<0.01) and protein levels of p-Akt/Akt, p-GSK-3β/GSK-3β, Nrf2, and HO-1 (P<0.01) in the hippocampus. Compared with the model group, donepezil hydrochloride and high, medium, and low doses of Hei Xiaoyaosan shortened the total swimming distance on day 5 (P<0.05, P<0.01), increased the percentage of time spent in the target quadrant (P<0.05, P<0.01), improved the arrangement and morphology of neurons, reduced nuclear condensation and the apoptosis rate of hippocampal neurons (P<0.01), increased the NeuN content (P<0.01), enhanced the activities of GSH-Px, GST, and CAT (P<0.05, P<0.01), and up-regulated the mRNA levels of Nrf2 and HO-1 (P<0.05, P<0.01) and the protein levels of p-Akt/Akt, p-GSK-3β/GSK-3β, Nrf2, and HO-1 (P<0.05, P<0.01) in the hippocampus. ConclusionHei Xiaoyaosan can regulate the Akt/GSK-3β/Nrf2/HO-1 pathway to enhance the antioxidant stress capacity and inhibit neuron apoptosis to exert the neuroprotective effect, thereby ameliorating the cognitive dysfunction and pathological damage in AD rats.
