1.Pathological characteristics of cytologically diagnosed metastatic clear cell renal cell carcinomas

Objective:To investigate the clinical, cytopathological characteristics, and differential diagnosis of metastatic clear cell renal cell carcinomas (CCRCC).Methods:Nine cases of metastatic CCRCC cytologically diagnosed in the Department of Pathology, the First Affiliated Hospital of Air Force Medical University from July 2021 to December 2024 were collected. The HE staining, May-Grunewald-Giemsa staining, liquid-based slides, cell block preparation, and immunocytochemistry of EnVision two-step staining were performed. The clinical and cytopathological features, treatments and follow-up data were analyzed in combination with literature review.Results:Among the 9 cases of metastatic CCRCC, there were 7 males and 2 females. The age range was 43-78 years, and the average age was 63.6 (57.5, 72.5) years. The metastatic sites were lymph node in 3 cases (2 cases of mediastinal lymph nodes and 1 case of left cervical lymph node), bone in 3 cases (pubis, thoracic vertebrae and femur, respectively), thyroid in 2 cases, and adrenal gland, lung and pancreas in 1 case, respectively. Two of the 9 cases had two metastatic sites (case 8 had metastases of lung and mediastinal lymph nodes; case 9 had metastases of thyroid and cervical lymph nodes). The median time from the diagnosis to metastasis was 9.4 years (range 1.1 to 13.8 years). The tumor cells were arranged in papillary, acinar, sheet, cluster or single scattered pattern. Most cases had uniform nuclei with mild atypia and inconspicuous nucleoli, while some cases had variable nuclei with prominent nucleoli. The cytoplasm of the tumor cells was abundant. Some cases showed clear cytoplasm with small vacuoles, while some of them showed eosinophilic and granular cytoplasm. Immunocytochemically, the tumor cells were positive for CKpan(AE1/AE3,6/6), PAX8 (9/9), CAⅨ (9/9), CD10 (9/9), and vimiten (8/8). Patients were treated primarily with targeted therapy and/or immunotherapy and curettage and radiation therapy for bone lesions. The follow-up time ranged from 1.0 month to 41.5 months (median, 20 months), and all patients survived at the end of follow-up.Conclusions:The cytology of metastatic CCRCC often shows uniform cell size, abundant and clear cytoplasm, low nuclear/cytoplasmic ratio, and mild nuclear atypia. Its cytological diagnosis is challenging because it occurs in various sites and needs to be differentiated from primary tumors of these sites. Emphasis should be placed on the morphological recognition of CCRCC, and immunocytochemical staining should be used to improve diagnosis. When necessary, molecular testing can be employed for diagnosis. Meanwhile, the medical history should be carefully inquired by pathologists to avoid missed diagnosis and misdiagnosis.
2.Expression of TLDC2 in colorectal adenocarcinoma and its clinical significance

Purpose This study aimed to investigate the expression and clinical significance of TLDC2 in colorec-tal adenocarcinoma.Methods Data from the human protein atlas(HPA)and the cancer genome atlas(TCGA)indi-cated that TLDC2 was highly expressed in colorectal adenocarcinoma.We further analyzed the expression of TLDC2 in 400 colorectal adenocarcinomas and 447 other solid tumors using tissue microarrays and immunohistochemical(IHC)staining.Result The positive expression rate of TLDC2 was significantly higher than that of SATB2 in colorectal ade-nocarcinomas(96.5%vs 87.0%,P<0.000 1).TLDC2 positivity exceeded that of SATB2 in both low-or high-grade colorectal adenocarcinoma(99.4%vs 88.7%,P<0.000 1;83.3%vs 79.2%,P=0.669 9).In addition,the expression of TLDC2 and SATB2 was evaluated in 447 cases of other types of solid tumors.TLDC2 was expressed in neuroendocrine tumors as well as in gastric and appendiceal adenocarcinomas,whereas SATB2 was detected in a small number of melanomas,ovarian cancers,breast cancers and gallbladder cancers.The positive and specificity of TLDC2 for colorectal adenocarcinoma were 97%(95%CI=0.94-0.98)and 85%(95%CI=0.81-0.88),respectively.Combined detection of TLDC2 and SATB2 yielded a sensitivity of 96%(95%CI=0.93-0.97)and a specificity of 93%(95%CI=0.90-0.95).Conclusion Analysis of large-scale datasets and IHC staining demonstrated that TLDC2 is a highly sensitive and specific biomarker for colorectal adenocarcinoma.
3.Research progress and therapeutic prospects of visfatin in tumor immunity

Visfatin,also known as nicotinamide phosphoribosyl transferase(NAMPT)or pre-B-cell colony-enhancing factor,is a proinflam-matory adipokine.Its immunomodulatory effects are closely associated with its pro-inflammatory activity,influencing the secretion of vari-ous cytokines and the function of immune cells.Visfatin exerts pro-tumorigenic effects in most cancers.The tumor immune microenviron-ment(TIME)comprises immune cells within the tumor tissue and an array of secreted cytokines.Growing evidence suggests that visfatin plays a regulatory role in the TIME by promoting inflammatory responses and modulating the polarization of immune cells,their secretory functions,surface protein expression,and energy metabolism,thereby influencing cancer progression.These newly discovered mechanisms provide a critical foundation for the application of NAMPT inhibitors(NAMPTi)in cancer immunotherapy.This review summarizes recent ad-vances in understanding the role of visfatin in tumor immunology and explores the therapeutic potential of NAMPTi in oncology.
4.Optimization strategy for patient experience on the new campus of a cancer-specialized hospital from the perspective of peak experience

Tumor diseases have emerged as a major threat to human health.In this case,cancer hospitals must optimize patient experience while developing their facilities and medical resources.This paper takes the Huangpu Campus of Sun Yat-Sen University Cancer Center as an example to explore the strategies and practical effects of optimizing patient experience on the cam-pus from the perspective of peak experience.This hospital has successfully created high-quality peak experiences across multiple dimensions such as visual perception,trust,emotion,operations,efficiency,and lifestyle.The results indicated that Huangpu Hospital significantly improved patient satisfaction,medical resource expansion,operational efficiency,and social recognition.These achievements have validated the effectiveness of peak experience theory in the field of medical services,providing valuable insights for other hospitals.
5.Exploring Mechanism of Polygoni Cuspidati Rhizoma et Radix in Treating Respiratory Syncytial Virus Infection Based on Pulmonary Surfactant Lipid Homeostasis

ObjectiveTo investigate the efficacy and mechanism of Polygoni Cuspidati Rhizoma et Radix (Huzhang) in treating respiratory syncytial virus(RSV) infection by regulating pulmonary surfactant lipid homeostasis through lipidomics. MethodsSixty BALB/c mice were randomly divided into the blank group, model group, positive group(ribavirin group, 46 mg·kg-1), and low- and high-dose Huzhang groups(0.75, 2.25 g·kg-1), with 12 mice in each group. Except for the blank group, all other groups were infected with RSV via intranasal instillation. The drug intervention groups were given corresponding doses of drug by gavage for 3 consecutive days, while normal saline was used in the blank and model groups. Hematoxylin-eosin(HE) staining was used to observe pathological changes in mouse lung tissue. Real-time fluorescence quantitative polymerase chain reaction(Real-time PCR) was employed to detect viral loads[RSV-nucleoprotein(N) and RSV-glycoprotein(G) mRNA] and inflammatory factor levels[interleukin(IL)-1β and tumor necrosis factor(TNF)-α mRNA] in the lung tissue. Mouse bronchoalveolar lavage fluid was collected to detect the levels of pulmonary surfactant lipids through ultra-high performance liquid chromatography-quadrupole-electrostatic field orbitrap high-resolution mass spectrometry(UPLC-Q-Exactive Orbitrap-MS), followed by principal component analysis and differential lipid identification. ResultsCompared with the blank group, the model group exhibited extensive inflammatory cell infiltration, congestion, and tissue damage in the lungs, and the pathological score and lung index of lung tissue significantly increased(P<0.01), along with significantly elevated mRNA expressions of RSV-N, RSV-G, IL-1β, and TNF-α(P<0.01). Compared with the model group, different doses of Huzhang and ribavirin significantly reduced the pathological scores of the lung tissue and lung index(P<0.01). In addition, the mRNA levels of RSV-N, RSV-G and TNF-α in the lungs significantly decreased in the Huzhang high dose group(P<0.01). Lipidomics analysis identified multiple significantly changed differential metabolites. Compared with the blank group, the model group showed obvious abnormal lipid metabolism, which was manifested by the elevated levels of prostaglandin(PG), ceramide(Cer), phosphatidylcholine(PC), phosphatidylethanolamine(PE), phosphatidylinositol(PI), sphingomyelin(SM), and the decreased levels of diglycerides(DG) and acylethanolamine(NAE). After the intervention of low dose of Huzhang, the above lipid metabolites showed a significant reversal trend, while the intervention of high dose of Huzhang could regulate levels of PI lipids, PG lipids and PC lipids. ConclusionHuzhang can significantly reduce the viral load of lung tissue and improve lung inflammation in RSV-infected mice. The underlying mechanism may be related to the maintenance of homeostasis in pulmonary surfactant lipids such as PI and PG.

7.Impacts of radiotherapy on anti-tumor immunity: a comprehensive review from the fundamental to the clinical

Radiotherapy is a crucial component of the treatment of tumors. In addition to directly causing DNA damage in tumor cells and inducing cell apoptosis, radiotherapy is also involved in the regulation of systemic immune status. Increasing evidence indicates that radiotherapy can remodel the tumor immune microenvironment (TIME) and reverse the immunosuppressive state, thereby enhancing anti-tumor effects and demonstrating stronger immune responses and therapeutic benefits when combined with immune checkpoint inhibitors. Taking lung cancer as an example to explore the impacts and potential mechanisms of radiotherapy on the TIME and anti-tumor immunity, which can provide a scientific basis for optimizing the treatment strategies of lung cancer.
8.Clinical implications of morular metaplasia in fertility-preserving treatment for endometrial hyperplasia and grade 1 endometrial endometrioid carcinoma patients

Objective:To evaluate the clinical significance of morular metaplasia (MM) in fertility-preserving treatment for young patients with endometrial hyperplasia and grade 1 endometrial endometrioid carcinoma.Methods:Clinical data was retrospectively collected from patients diagnosed with endometrial hyperplasia or grade 1 endometrial endometrioid carcinoma under 40 years old who underwent progestin-based fertility-sparing treatmentat in Tianjin Medical University General Hospital between January 2018 and November 2022.Patients were divided into the MM group (37 cases) and the non-MM group (63 cases) based on pathological findings. Clinical characteristics, hysteroscopic features, treatment efficacy and fertility outcomes were compared between the two groups. The MM group was further stratified into three subgroups based on the timing of MM occurrence:(1) MM-Bef group ( n=10): MM was present in the initial endometrial curettage or hysteroscopic biopsy pathology before fertility-sparing treatment and disappeared after treatment; (2) MM-Sus group ( n=14): MM persisted consistently before and after therapy;(3) MM-Aft group ( n=13): MM was absent before therapy but appeared after treatment. The risk factors which had impact on the treatment outcomes of the patients were analyzed using univariate and multivariate Cox regression analysis. Results:The rate of polycystic ovary syndrome were higher in the MM group than the non-MM group [51% (19/37) vs 27% (17/63), P=0.014]. The complete response (CR) rate was significantly lower in the MM group than in the non-MM group [73% (27/37) vs 95% (60/63), P=0.006], and the median time to CR was significantly longer in the MM group (6.0 vs 5.0 months, P=0.005).Multivariate analysis identified that MM-Sus ( HR=0.355, 95% CI:0.174-0.723; P=0.004) and MM-Aft ( HR=0.314, 95% CI:0.145-0.681; P=0.003) were independent risk factors for delayed CR in fertility-sparing treatment. The patients in the MM group and non-MM group underwent hysteroscopic biopsy for 76 and 131 times. "Gravel-like change" was a more frequent hysteroscopic manifestation in the MM group than that in the non-MM group [18% (14/76) vs 2% (2/131), P<0.001]. Conclusions:Patients in the MM group have poorer treatment outcomes than patients in the non-MM group. MM-Sus and MM-Aft are risk factors for fertility-preserving treatment in young patients with endometrial hyperplasia or grade 1 endometrial endometrioid carcinoma. "Gravel-like change" is the characteristic hysteroscopic manifestations of MM.
9.Innovative exploration of multi-campus cultural integration through healing space development

Under the multi-campus development model of public hospitals,cultural integration and innovation face nu-merous challenges due to spatial constraints,personnel structure differences,and functional positioning across campuses.System-atic cultural development is required to bridge these gaps and enhance the sense of belonging among both patients and staff in new campuses.This case study focuses on the Huangpu Campus of Sun Yat-sen University Cancer Center,launched in 2021,explo-ring a dual-dimensional approach to multi-campus cultural integration through"healing spaces."The practical strategies include:①Direct Space Construction:Establishing patient-centered healing environments through service hubs to optimize clinical experi-ences and humanistic care;(②Inidirect Environment Development:Leveraging staff service centers to foster the integration of the hospital's"collaborative,fulfilling,and striving culture,"thereby strengthening employee engagement.Post-implementation,significant improvements were observed in both patient and staff satisfaction metrics.The initiative has yielded a replicable cultur-al management framework for multi-campus public hospitals,offering valuable insights for cultural innovation in similar healthcare settings.
10.Clinicopathological features of 15 cases of anaplastic large cell lymphoma with small cell variant

Purpose To investigate the clinicopathological features and differential diagnosis of anaplastic large-cell lymphoma(ALCL)with the small cell variant.Methods The clinical data of 15 cases of small cell variant of ALCL were retrospectively analyzed.Hematoxylin-eosin staining,immunohistochemistry,EBER in situ hybridization,TCR and IgG rearrangement were performed.The clinicopathological features,prognosis and differential diagnosis were summarized with review of the literature.Results Among the 15 cases of small cell ALCL,5 were females and 10 were males,with a male-to-female ratio of 2:1.Among the 15 cases of small cell ALCL,14 were systemic ALCL[(12 cases of ALK+),and the age range was 5-63 years old(median age 20 years);There were 2 cases of ALK(-),aged more than 60 years],and 1 case of ALCL of primary cutaneous ALK(-).Microscopic features were dominated by small to medium-sized tumor cells,with irregular nuclear membranes,inconspicuous nucleoli,and a lack of or few iconic large cells.Immunohistochemically:3 cases were ALK-negative and 12 cases were ALK-positive.Tumor cells were CD30 positive in 14/15 cases,CD3 positive in 7/15 cases,CD2 positive in 2/6 cases,CD4 positive in 5/11 cases,CD5 positive in 2/8 cases,CD7 positive in 4/6 cases,CD8 positive in 1/8 cases,TIA-1 positive in 7/7 cases,Granzyme B positive in 4/6 cases and EMA positive in 6/7 cases.CD20 was negative in all cases.Immuno-globulin IgG and TCR gene rearrangement were negative in case 4 and case 11.Case 13 was positive for TCR rear-rangement and negative for immunoglobulin IgG.Twelve of the 15 patients were followed up for 0.7-165 months(me-dian,7 months),3 patients were lost to follow-up and 7 patients died.Conclusion Small cell variant ALCL is easily misdiagnosed as an inflammatory or infectious disease.A group of antibodies,including CD30 and ALK,should be routinely used in routine work,especially in young patients with systemic symptoms and leukemia manifestations or in small biopsies,to reduce misdiagnosis and missed diagnosis.
