1.Clinical Questions Construction in Clinical Practice Guidelines:Based on Case-guided Approach

Currently,there are practical and technical difficulties in the construction of clinical ques-tions in the development of clinical practice guidelines.Clinicians or guideline developers seldom construct clin-ical questions based the actual case scenario,leading to some information loss between structured and actual clinical connotation.To overcome this challenge,we proposed a case-guided questions construction approach,and carried out case research and verification in the formulation of the guideline.We found that this method could more efficiently and scientifically assist the formulation of clinical questions,and provide reference for clinicians or guideline developers.
2.The reduction of reproductive function of male zebrafish caused by high-fat diet:A molecular basis study

Objective To investigate the effects and mechanisms of high-fat diet(HFD)-induced obesity on male ze-brafish reproductive function.Methods Adult male zebrafish were divided into normal diet(ND)group and HFD group.Growth and metabolic conditions were evaluated by measuring body weight,body length,BMI,organ index,and glucose/lipid lev-els.Reproductive capacity was assessed via sperm concentration,motility,fertilization rate,and testosterone levels.Testicular tissues from both of groups were subjected to transeriptomic sequencing(RNA-seq).And qRT-PCR was used to validate the expression of genes.Results Compared to male zebrafish in ND group,the ones in HFD group exhibited hepatic steatosis and glucose/lipid meta-bolic disorders(P<0.05).Testicular structural disorganization,along with reduced testosterone levels,decreased gonadosomatic in-dex,and impaired sperm concentration and motility occurred in HFD group(P<0.05).GO analysis revealed that differentially ex-pressed genes were enriched in spermatogenesis and ciliary system,while KEGG analysis highlighted metabolic related pathways(pu-rine metabolism,thyroid hormone synthesis,mTOR signaling)and cell adhesion molecules.Twenty key differentially expressed genes were validated by qRT-PCR,which confirmed the reliability of RNA-Seq results.Conclusion Impairment of reproductive function induced by HFD in zebrafish may be associated with three regulatory mechanisms including ciliary system,metabolic dysregulation,and aberrant cell adhesion molecule signaling.This study provides mechanistic insights and identifies potential therapeutic targets for clinical management of diet-associated infertility.
3.68 Ga-DOTA-NOC PET/CT for diagnosing pheochromocytoma and paraganglioma

Objective To observe the value of 68Ga-DOTA-NOC PET/CT for diagnosing pheochromocytoma(PCC)and paraganglioma(PGL).Methods Thirty-eight patients with suspected or confirmed PCC/PGL who underwent 68 Ga-DOTA-NOC PET/CT were retrospectively enrolled,among them 20 cases underwent 131I-metaiodobenzylguanidine(MIBG)SPECT/CT during the same period.The value of 68Ga-DOTA-NOC PET/CT for diagnosing PCC/PGL at individual and lesion levels were analyzed and compared to the results of 131I-MIBG SPECT/CT.Results Among 38 cases,there were 20 cases of PCC,14 cases of PGL,1 case of adrenocortical carcinoma and 3 cases of benign adrenal hyperplasia.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of 68 Ga-DOTA-NOC PET/CT for diagnosing PCC/PGL in all 38 cases was 87.88%(29/33),60.00%(3/5),93.55%(29/31),42.86%(3/7)and 84.21%(32/38),respectively.Totally 188 lesions were detected in 34 cases,with detection rate of 89.95%(188/209).For 20 patients who underwent both 2 kinds examinations,the detection rate of bone,lymph node,liver,lung metastases and the overall lesions of 68Ga-DOTA-NOC PET/CT were all higher than those of 131I-MIBG SPECT/CT(all P<0.05).No significant difference of diagnostic accuracy of PCC/PGL was found between 68 Ga-DOTA-NOC PET/CT and 131I-MIBG SPECT/CT(P>0.05).Conclusion The value of 68 Ga-DOTA-NOC PET/CT for diagnosing PCC/PGL was comparable to that of 131I-MIBG SPECT/CT,but the former showed higher detection rate of metastases,hence being helpful to staging and risk stratification of PCC/PGL.
4.Clinical effects of Jinfukang Oral Liquid combined with thymosin α1 on patients with non-small cell lung cancer due to Dual Deficiency of Qi and Yin

AIM To explore the clinical effects of Jinfukang Oral Liquid combined with thymosin α1 on patients with non-small cell lung cancer due to Dual Deficiency of Qi and Yin.METHODS Seventy-five patients were randomly assigned into thymosin α1 group(15 cases)for 4-week administration,Jinfukang Oral Liquid group(30 cases)for 4-week administration,and combination group(30 cases)for 4-week administration.The changes in TCM clinical syndrome effects,immunity indices(CD3+T,Th,CTL,total NK,CD56dim CD16+NK,NKT,Treg,MDSC),lethality/inhibition ratios(CTL/Treg,total NK/Treg,NKT/Treg,CTL/MDSC,total NK/MDSC,NKT/MDSC)and FACT-L scores were detected.RESULTS The Jinfukang Oral Liquid group and combination group demonstrated higher total effective rates than the thymosin α1 group(P<0.05).After the treatment,the Jinfukang Oral Liquid group and combination group displayed increased NKT(P<0.05)and decreased MDSC(P<0.05),which were more obvious than those in the thymosin α1 group(P<0.05),and higher NKT was observable in the Jinfukang Oral Liquid group(P<0.05);the Jinfukang Oral Liquid group and combination group displayed increased lethality/inhibition ratios(P<0.05),among which NKT/Treg,CTL/MDSC,total NK/MDSC,NKT/MDSC were higher than those in the thymosin α1 group(P<0.05),and higher CTL/MDSC,NKT/MDSC were observable in the Jinfukang Oral Liquid group(P<0.05);the Jinfukang Oral Liquid group(except for physiological status,society and family status)and combination group(except for society and family status)displayed increased FACT-L scores(P<0.05).CONCLUSION For the patients with non-small cell lung cancer due to Dual Deficiency of Qi and Yin,Jinfukang Oral Liquid single use or combined with thymosin α1 can enhance peripheral blood immune surveillance,inhibit immune escape,restore the balanced state of tumor immune responses,and improve TCM syndromes and life quality.
5.Effect of salvianolic acid B on high glucose induced necrotic apoptosis of retinal pigment epithelial cells by regulating the receptor-interacting protein kinase 1/receptor-interacting protein kinase 3/mixed lineage kinase domain like protein signaling pathway

Objective To investigate the effect of salvianolic acid B(Sal B)on high glucose(HG)induced necrotic apoptosis of retinal pigment epithelial cells(RPE)by regulating the receptor-interacting protein kinase 1(RIP1)/RIP3/mixed lineage kinase domain like protein(MLKL)signaling pathway.Methods RPE cells ARPE-19 were used as the research object and separated into control(Con)group,HG group,L-Sal B group,M-Sal B group,H-Sal B group,pcDNA3.1-NC group,and pcDNA3.1-RIP1 group.The qRT-PCR method was applied to detect the expression of RIP1,RIP3,and MLKL in ARPE-19 cells in each group.CCK8 was applied to detect ARPE-19 cell survival rate.Flow cytometry was applied to detect cell apoptosis.DCFH-DA fluorescent probe was applied to detect reactive oxygen species(ROS)level.ELISA was applied to detect the expression of tumor necrosis factor-α(TNF-α),interleukin(IL-1β),and IL-6.Western blot was applied to detect the expression of RIP1,RIP3,and MLKL proteins.Results The cell survival rate was lower in the HG group than in the Con group(P<0.05).The cell apoptosis rate,the proportion of ROS-positive cells,TNF-α,IL-1β,IL-6,and the mRNA and protein expressions of RIP1,RIP3,and MLKL were higher in the HG group than in the Con group(P<0.05).The cell survival rates were higher,while the cell apoptosis rates,the proportion of ROS-positive cells,TNF-α,IL-1β,IL-6,and the mRNA and protein expressions of RIP1,RIP3,and MLKL were lower in the L-Sal B,M-Sal B,and H-Sal B groups than in the HG group(P<0.05).The cell survival rates increased successively(P<0.05),while the cell apoptosis rates,the proportion of ROS-positive cells,TNF-α,IL-1β,IL-6,and the mRNA and protein expressions of RIP1,RIP3,and MLKL decreased successively in the L-Sal B,M-Sal B,and H-Sal B groups(P<0.05).The cell survival rate was lower(P<0.05),while the cell apoptosis rate,the proportion of ROS-positive cells,TNF-α,IL-1β,IL-6,and the mRNA and protein expressions of RIP1,RIP3,and MLKL were higher in the pcDNA3.1-RIP1 group than in the H-Sal B and pcDNA3.1-NC groups(P<0.05).Conclusions Sal B may inhibit the necrotic apoptosis of RPE cells induced by HG by suppressing the RIP1/RIP3/MLKL signaling pathway.
6.Clinical Observation of the Effect of Kidney Tonifying and Blood Stasis Removing Formula on Nutritional Status and Quality of Life in Patients with Osteoporosis Combined with Hemodialysis

Objective:To explore the effect of kidney-strengthening and blood stasis-eliminating formula on nutritional status and quality of life in patients with osteoporosis combined with hemodialysis.Methods:From January 2024 to January 2025,60 patients with liver and kidney yin deficiency and blood stasis obstruction were selected from the Pizhou City Traditional Chinese Medicine Hospital's Hemodialysis Center.60 patients were randomly divided into two groups,30 in each group.The matched group received general treatment plus paricalcitol,while the treatment group received the same treatment plus Yishen Zhuyu Fang(a traditional Chinese medicine formula for kidney and blood stasis).The treatment course lasted 12 weeks.Changes in TCM syndrome scores,the Modified Subjective Global Assessment(MQSGA),the Kidney and Life Quality(KDQOLTM-36)scale,lipid levels,and nutritional indicators were compared before and after treatment.Results:After treatment,the TCM syndrome scores in both groups decreased(P<0.05),with a more pronounced decrease in the treatment group(P<0.05).The KDQOLTM-36 score in the treatment group increased(P<0.05),while the increase was not significant in the matched group(P>0.05),indicating that the treatment group performed better than the matched group(P<0.05).Both groups experienced a decrease in the MQSGA score(P<0.05),although the difference was not statistically significant(P>0.05),the treatment group showed a more noticeable decline.The TC,TG,LDL,and TG/HDL levels in the treatment group all decreased significantly,while HDL levels increased(P<0.05).In contrast,the TC and HDL levels in the matched group did not change(P>0.05),while the TG,LDL,and TG/HDL levels increased(P<0.05).The treatment group performed significantly better than the matched group(P<0.05).The TRF,Hb,and ALB levels in the treatment group increased(P<0.05),while the increase was not significant in the matched group(P>0.05).The total effective rate of the treatment group was 90%,higher than the matched group(23.3%)(P<0.05).Conclusion:The formula of tonifying the kidney and removing stasis can improve the nutritional status and quality of life of patients with liver and kidney yin deficiency and blood stasis obstruction in hemodialysis combined with osteoporosis.
7.Visualized analysis of research hotspots and trends in shared decision-making in cardiovascular disease nursing based on CiteSpace

Objective:To explore the current status, research hotspots, and development trends of shared decision-making in the field of cardiovascular disease nursing, and to provide a reference for future research.Methods:Relevant literature on shared decision-making in cardiovascular disease nursing published up to October 31, 2024, was retrieved from the Web of Science Core Collection and China National Knowledge Infrastructure. CiteSpace 6.4.R1 software was used for visualized analysis.Results:A total of 2 748 publications were identified, including 2 446 in English and 302 in Chinese. The overall number of publications has shown an increasing trend. Research hotspots include quality of life, palliative care, machine learning, and artificial intelligence. The emerging trend involves integrating evidence-based approaches with artificial intelligence technologies to build scientific evidence frameworks that support patients in making optimal decisions.Conclusions:Research on shared decision-making in cardiovascular disease nursing has been increasing year by year but remains largely concentrated in developed countries. Future studies should draw on international research frontiers while considering China's national and cultural contexts, enhance academic exchange and collaboration, and explore effective strategies to promote shared decision-making between Medical staff and patients.
8.Clinical and genetic characteristics of 14 children with sodium taurocholate co-transporting polypeptide deficiency

Objective To summarize the clinical and genetic characteristics of children with sodium taurocholate co-transporting polypeptide(NTCP)deficiency.Methods Clinical data of children with NTCP deficiency diagnosed and treated at the First People's Hospital of Yunnan Province from July 2022 to March 2025 were retrospectively analyzed.Results A total of 14 children were included(6 males,8 females),all with normal growth and development.Reasons for initial consultation included elevated serum bile acids in 7 cases,jaundice in 4 cases,cholestatic hepatitis in 1 case,and one case each of pneumonia and cow's milk protein allergy.At the first visit,all patients had elevated serum total bile acids beyond the normal range,with a mean of 152.5 μmol/L.Elevated alanine aminotransferase was observed in 1 case,elevated aspartate aminotransferase in 2 cases,and elevated total bilirubin in 10 cases.Genetic sequencing revealed that all children carried the homozygous SLC10A1 variant c.800C>T(p.Ser267Phe),classified as likely pathogenic.Conclusions NTCP deficiency often lacks obvious clinical symptoms and signs.Some children present with transient hyperbilirubinemia,cholestasis,or other liver function abnormalities.Persistent isolated elevation of serum bile acids warrants suspicion for this disease.Biallelic pathogenic variants in SLC10A1 constitute the basis for definitive diagnosis.There is no specific treatment for this disease,and management is mainly symptomatic.
9.Experiences and implications of healthy weight management policies in Japan,South Korea,and Singapore:Based on the ICCC framework

Objective:This study aims to analyze the weight management policy practices in Japan,South Korea,and Singapore to provide international references for improving China's weight health management and metabolic disease prevention system.Methods:Based on the Innovative Care for Chronic Conditions Framework(ICCC),we conducted a literature review and policy analysis methods of domestic and international policies and publications on weight management,systematically analyzing and summarizing relevant strategies and practices to extract key insights in Japan,South Korea,and Singapore.Results:All three countries have established a multi-level collaborative governance system:at the macro level,they built institutional foundations through legislation,cross-sectoral collaboration,and diversified financing;at the meso level,they enhanced management efficiency via comprehensive medical services,multidisciplinary coordination,and smart information systems,while community-level initiatives fostered supportive environments through health education and environmental modifications;at the micro level,they emphasized personalized interventions and family involvement to strengthen self-management capabilities.Conclusion:China can learn from these neighboring countries'experiences to develop a multi-tiered prevention system,strengthen primary care capacity,improve medical security mechanisms,and facilitate a shift from disease treatment to health management,ultimately constructing a weight management solution with Chinese characteristics.
10.Development of dynamic multi-time-point clinical prediction models for bronchopulmonary dysplasia in preterm infants with gestational age<32 weeks

Objective To develop dynamic prediction models based on multiple postnatal time points to support early diagnosis and individualized intervention for bronchopulmonary dysplasia(BPD)in preterm infants with gestational age<32 weeks.Methods Clinical data of 472 preterm infants with gestational age<32 weeks admitted to the Second Xiangya Hospital of Central South University between January 2016 and November 2020 were retrospectively analyzed.Multivariable logistic regression was applied to develop five independent prediction models at postnatal days 1,7,14,21,and 28.The performance of the models was assessed using the area under the receiver operating characteristic curve(AUC)and the Hosmer-Lemeshow test.Results Baseline characteristics such as gestational age and birth weight differed significantly between the BPD group(n=147)and the non-BPD group(n=325)(P<0.05).Predictors of BPD evolved across time points:on day 1,key predictors included gestational age,birth weight,Score for Neonatal Acute Physiology II(SNAP-II),invasive mechanical ventilation,and fraction of inspired oxygen>30%;by day 7,additional variables emerged,including fasting duration>2 days,mean feeding advancement rate<8.5 mL/(kg·d),neonatal respiratory distress syndrome,apnea of prematurity,and positive sputum culture;from day 14 onward,nutrition-and treatment-related indicators were incorporated additionally.The models demonstrated good discrimination at postnatal days 1,7,14,21,and 28,with AUCs of 0.917,0.927,0.939,0.944,and 0.968,respectively,and good calibration(Hosmer-Lemeshow P>0.05).Internal validation showed AUCs ranging from 0.899 to 0.958,indicating robust performance.Conclusions Dynamic postnatal prediction models incorporating indicators spanning perinatal factors,respiratory support,nutritional management,and therapeutic interventions demonstrate high predictive performance and facilitate dynamic risk assessment for BPD in preterm infants with gestational age<32 weeks.
