Object To examine the preliminary clinical efficacy of custom-made three-dimensional(3D) printed talus prosthesis in the treatment of collapse talus necrosis. Methods:The clinical data of 8 patients who received 3D printed custom-made talus prostheses replacement for severe collapsed necrosis of the talus at the Orthopaedic Sports Medical Center, the First Affiliated Hospital to Army Medical University were analyzed retrospectively.All patients were male,with an average age of 38.0 years (range:22 to 65 years).There were 5 cases of left talus collapse and 3 cases of right talus collapse,with the course of disease of 29.7 weeks (range:6 to 96 weeks).The CT data of contralateral healthy talus were used for mirror image design references for the prosthesis,and the electron-beam 3D printing technology was used to prepare the prosthesis.Titanium alloy (Ti6Al4V) was taken as the material for the preparation of the talus body prosthesis,and Co-Cr-Mo material was used as the material for the preparation of the tibialis talus lateral joint surface prosthesis,and the subtalar joint surface of the prosthesis was made from a microporous casting technique.The prosthesis was analyzed preoperatively by digital three-dimensional finite element analysis and solid comparison techniques to measure anatomic match of the prosthesis.A longitudinal incision on medial ankle was made.The necrotic talus was completely removed and the prosthesis was then implanted.The patient was reexamined in the outpatient department 3, 6, and 12 months after surgery.Primary outcome measures were the American Orthopaedic Foot and Ankle Society(AOFAS) ankle-hind foot score,visual analogue scale(VAS) and ankle range of motion.Changes in imaging data and plantar pressure were also assessed.Repeated measures analysis of variance and paired- t test were used to compare the data. Results:The talus prosthesis measure preoperatively was completely consistent with that contralateral healthy talus and there was no operation-related complication. All the wounds healed primarily. The patients were followed up effectively for 23.17 months (range:12 to 48 months).The preoperative dorsiflexion of patients was (7.6±5.7)°,it increased to(14.2±6.6)° at 12 month after surgery ( t=-2.67, P=0.03).The plantar flexion increased from (22.0±9.9)°preoperatively to (29.2±8.7)° at 12 month after surgery ( t=-8.95, P<0.01).Preoperative AOFAS ankle-hind foot score was 26.3±6.6,and it increased to 70.1±2.2,76.0±3.4 and 79.3±4.2 at 3 month,6 month and 12 month after surgery( F=56.81, P<0.01);Pre-operative VAS was[ M( Q R)]3.0(0.8),and it increased to 2.5(1.0),1.5(1.0),1.0(1.0)at 3 month,6 month and 12 month after surgery( F=20.00, P<0.01).At the last follow-up,imaging reexamination showed that the prosthesis of all patients were in stable position with no sign of subsidence.No secondary ankle fusion or revision was required.The talus height increased from (27.6±6.0)mm preoperatively to (34.6±3.5)mm ( t=-2.94, P<0.01).The plantar pressure showed that the maximum pressure on the healthy ankle was(629.9±26.1)N,and that on the affected side was(521.4±14.4)N.The pressure on the healthy ankle was(350.6±29.6)N,and that on the necrotic side was (212.3±9.7)N.The load on the contralateral forefoot was(38.1±2.8)% and that on the necrotic side was(11.5±2.0)%.The load on the contralateral hindfoot was (24.6±2.5)% and that on the necrotic side was (21.1±1.8)%. Conclusions:The custom-made 3D printed talus prosthesis could restore the talus anatomy,recover the ankle joint function,relieve the pain of patients and improve the life quality of patients.The effect on plantar pressure is mainly achieved by adjusting the center of gravity of plantar pressure backwards and the increase of weight bearing of the healthy foot.

ObjectiveThe purpose of this study was to analyze the correlation between SUVmax on primary prostate lesion of two-phase ⁶⁸Ga-PSMA-11 PET/CT and the clinicopathological features of patients with preliminary diagnosis of prostate cancer. MethodsPET/CT images of 19 patients with prostate cancer first diagnosed in our hospital from November 2017 to September 2020， aged 69.0±7.4 years， were collected. All patients underwent ⁶⁸Ga-PSMA-11 PET/CT dual-phase examination， and radical resection of prostate cancer was performed within two weeks. Semi-quantitative parameters of ⁶⁸Ga-PSMA-11 PET/CT on primary prostate cancer， SUVmax1 in the standard phase imaging， SUVmax2 in the delayed phase imaging and SUVmax3 （difference between SUVmax2 and SUVmax1）， were measured by automatic segmentation algorithm method. Clinical and pathological data of each patient were collected. Mann-Whitney U test and ROC curve analysis were used to compare SUVmax and the clinicopathological characteristics of patients in two independent samples. P<0.05 was considered as statistically significant difference. ResultsThe differences of SUVmax1， SUVmax2 and SUVmax3 between GS≤7 group and >7 group were statistically significant（P＜0.05）， and the areas under the ROC curves were 0.845， 0.845 and 0.893， respectively. SUVmax1， SUVmax2 and SUVmax3 were significantly different between ISUP grade ≤2 group and >2 group （P<0.05）， and the areas under the ROC curve were 0.773， 0.784 and 0.852， respectively. The difference of SUVmax3 between T stage ≤T2 group and >T2 group was statistically significant （P<0.05）， while the difference of SUVmax1 and SUVmax2 between T stage ≤T2 group and >T2 group was not statistically significant （P>0.05）. The difference of SUVmax3 between D’ Amico low-medium-risk group and high-risk group was statistically significant （P<0.05）， while the difference of SUVmax1 and SUVmax2 between D’ Amico low-medium-risk group and high-risk group was not statistically significant （P>0.05）. There was no significant difference in SUVmax1， SUVmax2 and SUVmax3 between total PSA ≤20 ng/mL group and >20 ng/mL group （P>0.05）. SUVmax1， SUVmax2 and SUVmax3 were significantly different between positive nerve invasion group and negative group （P<0.05）， and the areas under the ROC curves were 0.872， 0.923 and 0.974， respectively. ConclusionsSUVmax2 and SUVmax3 were positively correlated with multiple clinical and pathological features of prostate cancer. Compared with standard phase imaging， dual-phase imaging of ⁶⁸Ga-PSMA-11 PET/CT may provide more favorable value in predicting the clinical and pathological features of prostate cancer patients before surgery.

Chronic thromboembolic pulmonary hypertension(CTEPH)is a complication of pulmonary embolism. It is emphasized that the need for a multidisciplinary team involving diagnosis and treatment. The understanding and management of CTEPH have been updated on the 6 th World Symposium on Pulmonary Hypertension(WSPH)in 2018. Key diagnostic steps in early identification and accurate diagnosis are emphasised. An updated treatment algorithm is proposed. Combined with clinical practice, the article will give an interpretation of diagnosis and treatment of CTEPH based on this consensus opinions.

Objective:To explore the effect of early cognitive rehabilitation combined with music therapy on cognitive function, activities of daily living (ADL) and psychology in stroke patients. Methods:From March, 2016 to September, 2017, 56 stroke patients were were randomly divided into control group (n = 28) and observation group (n = 28). All the patients accepted early motor and cognitive rehabilitation, the observation group accepted music therapy in addition, for four weeks. They were assessed with Mini-Mental State Examination (MMSE), Loewenstein Occupational Therapy Congnitive Assessment (LOTCA), modified Barthel Index (MBI), Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS). Results:The scores of MMSE, scores of subscales and total scale of LOTCA, and MBI increased in both group after treatment (t > 5.480, P < 0.05), while the scores of SAS and SDS decreased (t > 10.227, P < 0.001). The total scores of MMSE, LOTCA and MBI increased more (t > 2.648, P < 0.05), and the scores of SDS decreased more (t = 2.818, P < 0.05) in the observation group than in the control group. Conclusion:Combination of music therapy may promote the recovery of cognitive function, ADL and mental health for stroke patients.

OBJECTIVETo observe the effect of Schistosoma japonicum cysteine protease inhibitor (rSjCystatin) for treatment of lipopolysaccharide (LPS)-induced sepsis in mice.

METHODSAfter a week of adaptive feeding, 54 BALB/c mice were randomly divided into normal control group (group A), sepsis group (group B), and rSjCystatin intervention group (group C). The mice in group A received an intraperitoneal injection of PBS (100 µL), and those in groups B and C were injected with PBS (100 µL) containing LPS (10 mg/kg); the mice in group C were also intraperitoneally injected with 25 µg sjCystatin in 100 µL PBS 30 min after LPS injection. From each group, 10 mice were randomly selected 24 h after PBS or LPS injection for detecting serum levels of TNF-α, IL-6, and IL-10 using ELISA and the levels of ALT, AST, BUN, and Cr using automatic biochemical analyzer; the pathological changes in the liver, lung and kidney were observed with HE staining. The remaining 8 mice in each group were used for observing the changes in the general condition and the 72-h survival.

RESULTSThe 72-h survival rates of the mice was 100% in group A, 0 in group B, and 36% in group C, showing a significant difference among the 3 groups (P<0.05). Compared with those in group A, the mice in group B exhibited obvious liver, lung, and renal pathologies with increased levels of ALT, AST, BUN, Cr, IL-6, and TNF-α (P<0.05). Treatment with sjCystatin significantly lessened LPS-induced organ pathologies, lowered the levels of liver and renal functional indexes and the pro-inflammatory cytokines, and increased the serum level of IL-10 in the mice (P<0.05).

CONCLUSIONSjCystatin can produce a significant therapeutic effect on sepsis induced by LPS in mice.

Menopausal metabolic syndrome (MMS) is a series of syndrome caused by ovarian function decline and hormone insufficiency, and is a high risk factor for cardiovascular diseases (CVD) and type II diabetes mellitus (T2DM). Erzhiwan (EZW), composed of Herba Ecliptae and Fructus Ligustri Lucidi, is a traditional Chinese herbal formula that has been used to treat menopausal syndrome for many years. We added Herba Epimedii, Radix Rehmanniae, and Fructus Corni into EZW, to prepare a new formula, termed Jiawei Erzhiwan (JE). The present study was designed to determine the anti-MMS effects of JE using ovariectomized (OVX) adult female rats that were treated with JE for 4 weeks, and β-tc-6 cells and INS cells were used to detected the protect effectiveness of JE. Our results showed JE could increase insulin sensitivity and ameliorated hyperlipidemia. Metabolomics analysis showed that the serum levels of branched and aromatic amino acids were down-regulated in serum by JE administration. Moreover, JE enhanced the function of islet β cells INS-1 and β-tc-6, through increasing the glucose stimulated insulin secretion (GSIS), which was abolished by estrogen receptor (ER) antagonist, indicating that JE functions were mediated by ER signaling. Additionally, JE did not induce tumorigenesis in rat mammary tissue or promoted proliferation of MCF-7 and Hela cells. In conclusion, our work demonstrated that JE ameliorated OVX-induced glucose and lipid metabolism disorder through activating estrogen receptor pathway and promoting GSIS in islet β cells, thus indicating that JE could be a safe and effective medication for MMS therapy.
