1.Efficacy and safety of ropivacaine combined with oxycodone for iliac fascia nerve block analgesia in patients undergoing hip replacement

OBJECTIVE To investigate the efficacy and safety of ropivacaine combined with oxycodone for the analgesia of iliac fascia nerve block in patients undergoing hip replacement. METHODS Sixty-six patients who underwent hip replacement at the Central Hospital of Enshi Tujia and Miao Autonomous Prefecture from October 2023 to April 2024 were selected and randomly divided into observation group and control group, with 33 cases in each group. Before induction of anesthesia, ultrasound-guided iliac fascial nerve block was performed. Patients in the observation group were treated with 0.33% ropivacaine+0.1 mg/kg oxycodone injection mixture 30 mL, and patients in the control group were treated with 0.33% ropivacaine injection 30 mL. The time of first postoperative rescue analgesia, 24 h postoperative analgesic drug consumption, sensory block and motor block effective and maintenance time, satisfaction degree, numerical rating scale (NRS) pain score, Ramsay sedation score, muscle strength score, heart rate (HR), mean arterial pressure (MAP), oxygen saturation(SpO2), sleep score, anxiety score, and the occurrence of adverse reactions in the two groups were all recorded. RESULTS Compared with the control group, the first rescue analgesia time after operation was significantly prolonged in the observation group, and 24 h postoperative analgesic drug consumption after operation decreased; the effective time of sensory block was significantly shortened, and the maintenance time of sensory block was significantly prolonged, and the satisfaction score was higher; the NRS pain score after iliac fascia nerve block was lower, HR and MAP were lower, and the anxiety score and sleep score 24 and 48 h after operation were lower (P<0.05). In terms of safety, patients in both groups had adverse reactions after operation, such as hypertension, nausea, vomiting, and dizziness, but there was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). CONCLUSIONS Oxycodone combined with ropivacaine shows good efficacy and safety for iliac fascial nerve block analgesia in patients undergoing hip replacement, can significantly prolong the analgesic time of ropivacaine, reduce postoperative analgesic drug consumption, improve the sleep quality of patients, and promote the rapid recovery of patients.
2.Clinical practice guidelines for intraoperative cell salvage in patients with malignant tumors

Intraoperative cell salvage (IOCS) has been widely applied as an important blood conservation measure in surgical operations. However, there is currently a lack of clinical practice guidelines for the implementation of IOCS in patients with malignant tumors. This report aims to provide clinicians with recommendations on the use of IOCS in patients with malignant tumors based on the review and assessment of the existed evidence. Data were derived from databases such as PubMed, Embase, the Cochrane Library and Wanfang. The guideline development team formulated recommendations based on the quality of evidence, balance of benefits and harms, patient preferences, and health economic assessments. This study constructed seven major clinical questions. The main conclusions of this guideline are as follows: 1) Compared with no perioperative allogeneic blood transfusion (NPABT), perioperative allogeneic blood transfusion (PABT) leads to a more unfavorable prognosis in cancer patients (Recommended); 2) Compared with the transfusion of allogeneic blood or no transfusion, IOCS does not lead to a more unfavorable prognosis in cancer patients (Recommended); 3) The implementation of IOCS in cancer patients is economically feasible (Recommended); 4) Leukocyte depletion filters (LDF) should be used when implementing IOCS in cancer patients (Strongly Recommended); 5) Irradiation treatment of autologous blood to be reinfused can be used when implementing IOCS in cancer patients (Recommended); 6) A careful assessment of the condition of cancer patients (meeting indications and excluding contraindications) should be conducted before implementing IOCS (Strongly Recommended); 7) Informed consent from cancer patients should be obtained when implementing IOCS, with a thorough pre-assessment of the patient's condition and the likelihood of blood loss, adherence to standardized internally audited management procedures, meeting corresponding conditions, and obtaining corresponding qualifications (Recommended). In brief, current evidence indicates that IOCS can be implemented for some malignant tumor patients who need allogeneic blood transfusion after physician full evaluation, and LDF or irradiation should be used during the implementation process.
3.Multi-index quantitative detection and quality difference evaluation of Gleditsia sinensis from different producing areas

OBJECTIVE To establish a multi-index quantitative detection method, and to evaluate the quality difference of Gleditsia sinensis from different producing areas. METHODS The contents of protocatechuic acid, vanillic acid, isoscopoletin, scoparone, isovitexin, fustin, taxifolin, fisetin, quercetin, kaempferol, echinocystic acid, betulinic acid, β -sitosterol and stigmasterol were detected by high performance liquid chromatography-quantitative analysis of multi-components by single marker (HPLC-QAMS). The chromatographic column was Kromasil C18, the mobile phase was 0.2% phosphoric acid-acetonitrile solution (gradient elution), the detection wavelengths were 254, 360, 210 nm for different index components, the column temperature was 30 ℃ , the flow rate was 1.0 mL/min, and the sample injection volume was 10 μL. The contents of extract and total ash were detected according to the method of Chinese Pharmacopoeia. The quality differences of 30 batches of G. sinensis (No. S1-S30) from different producing areas were evaluated by chemometrics, weighted technique for order preference by similarity to an ideal solution (TOPSIS) analysis and Logistic regression model. RESULTS The linear ranges of 14 components were 1.55-77.50, 0.71- 35.50, 0.28-14.00, 0.96-48.00, 1.77-88.50, 0.09-4.50, 4.65-232.50, 1.49-74.50, 0.37-18.50, 1.18-59.00, 7.35-367.50, 3.58- 179.00, 0.49-24.50 and 0.21-10.50 μg/mL, respectively (all r>0.999). The RSDs of precision, stability (24 h) and repeatability were less than 2.00%; the average recoveries were 96.99%-100.13% (all RSDs<2.00%), and the relative correction factor had good repeatability. There was no ZY2019Q014) significant difference in the content of 14 components measured by QAMS method and external standard method (P>0.05). The results of chemometrics showed that 30 batches of samples were clustered into 3 categories: S1 to S11 form one category, S12 to S20 form another category, and S21 to S30 constitute the third category. Echinocystic acid, betulinic acid, taxifolin, kaempferol, isovitexin, scoparone and protocatechuic acid may be the differential components affecting the quality of G. sinensis from different producing areas. The analysis results of the weighted TOPSIS method revealed that relative closeness (Jb) for 30 batches of G. sinensis ranged from 0.144 5 to 0.721 8, with S27 achieving the highest value (Jb) of 0.721 8. The analysis results of the Logistic regression model showed that S21-S30 batches of samples were of superior grade, S1-S11 were of intermediate grade, and S12-S20 were of inferior grade. CONCLUSIONS The established HPLC-QAMS method is simple and accurate. The comprehensive evaluation method is objective and comprehensive, and can be used to evaluate the quality difference of G. sinensis from different producing areas.
4.Combination of Components from Tripterygii Radix et Rhizoma-Chuanxiong Rhizoma Affects RA-FLSs by Regulating NF-κB, Nrf2/HO-1 Signaling Pathways and Bcl-2/Caspase-3 Expression

ObjectiveTo investigate the effects of the combination of components from Tripterygii Radix et Rhizoma and Chuanxiong Rhizoma on rheumatoid arthritis fibroblast-like synoviocytes (RA-FLSs) and the underlying mechanism. MethodsRA-FLSs were grouped as follows: blank control, positive control (methotrexate), Tripterygii Radix et Rhizoma components, Chuanxiong Rhizoma components, and components from Tripterygii Radix et Rhizoma+Chuanxiong Rhizoma. The cell-counting kit-8 (CCK-8) assay was employed to the cell proliferation, invasion, and apoptosis. The levels of tumor necrosis factor (TNF)-α, interleukin (IL)-6, reactive oxygen species (ROS), and malondiadehyde (MDA) in cells were measured. Western blot was employed to determine the protein levels of nuclear factor erythroid 2-related factor 2 (Nrf2), heme oxygenase-1 (HO-1), nuclear factor-kappa B (NF-κB) p65, phosphorylated inhibitory subunit of NF-κBα (p-IκBα), cysteinyl aspartate-specific protease-3 (Caspase-3), and B-cell lymphoma 2 (Bcl-2). Real-time PCR was employed to determine the mRNA levels of Nrf2, HO-1, and NF-κB p65. ResultsThe cells in the groups of positive control, Tripterygii Radix et Rhizoma components, Chuanxiong Rhizoma components, and components from Tripterygii Radix et Rhizoma+Chuanxiong Rhizoma were treated with 2.50 mg·L-1 methotrexate, 0.20 mg·L-1 triptolide + 0.20 mg·L-1 celastrol, 5.00 mg·L-1 ferulic acid + 20.00 mg·L-1 ligustrazine, 0.20 mg·L-1 triptolide + 0.20 mg·L-1 celastrol + 5.00 mg·L-1 ferulic acid + 20.00 mg·L-1 ligustrazine, respectively. Compared with the blank control group, drug administration reduced the proliferation and invasion and increased the apoptosis of cells (P<0.01), lowered the levels of TNF-α, IL-6, ROS, and MDA (P<0.01), up-regulated the mRNA and protein levels of Caspase-3, Nrf2, and HO-1 (P<0.01), and down-regulated the mRNA and protein levels of Bcl-2, NF-κB p65, and p-IκBα (P<0.01). Compared with the Tripterygii Radix et Rhizoma components group, the combination of components from Tripterygii Radix et Rhizoma+Chuanxiong Rhizoma inhibited the proliferation and invasion (P<0.05) and promoted the apoptosis of RA-FLSs, up-regulated the mRNA levels of Nrf2 and HO-1 and protein levels of Nrf2 and Caspase-3 (P<0.05), and down-regulated the protein levels of NF-κB p65 and p-IκBα (P<0.05). ConclusionThe combination of components from Chuanxiong Rhizoma and Tripterygii Radix et Rhizoma can inhibit the proliferation and invasion and promote the apoptosis of RA-FLSs and alleviate oxidative stress and inflammation by inhibiting the NF-κB signaling pathway, activating the Nrf2/HO-1 pathway, and regulating the expression of Bcl-2/Caspase-3.
5.Association of mother-child relationship with sleep quality and executive function among preschool children

Objective:
To investigate the mediating role of children s sleep quality in the association between mother-child relationship and the executive function of preschool children, providing a reference for promoting the development of the executive function of preschool children.
Methods:
A stratified cluster sampling method was used to select 842 preschoolers from 12 kindergartens in Wuhu City, Anhui Province in December 2021 as the subjects of the first follow up study with follow up every six months thereafter. Finally, 746 children were included in the study after 3 follow up. Spearman correlation analysis was used to explore the associations among mother-child relationship, sleep quality and executive function in preschool children. Bootstrap program and PROCESS software were applied to test the mediating effect of sleep quality in the association between mother-child relationship and the executive function of preschool children.
Results:
Conflictual mother-child relationship was positively correlated with the total score of executive function, as well as scores of inhibitory, shifting, emotional control, working memory, and organizational planning ( r=0.40, 0.37, 0.36, 0.41, 0.38 , 0.34, all P <0.05). Dependent mother-child relationship was positively correlated with the total score of executive function, as well as scores of inhibitory, shifting, emotional control, working memory , and organizational planning ( r=0.23, 0.20, 0.21, 0.22 , 0.22, 0.19, all P <0.05). Sleep quality was positively correlated with the total executive function score ( r=0.27, P <0.01). After adjusting for confounding factors, sleep quality played a partial mediating role in the associations between dependent and conflictual mother-child relationships and executive function, the mediating effects were 19.40% and 11.22% respectively.
Conclusions
Sleep quality plays a mediating role in the association between mother-child relationship and the executive function of preschool children. Improving sleep quality in the early stage can promote the executive function of preschool children.

7.Effect of hedysarum polybotrys polysacchcaide on inflammation of diabetic gastroparesis rats based on JAK2/STAT3 signaling pathway

Aim To investigate the effects of hedysar-um polybotrys polysacchcaide(HPS)on gastric muco-sal inflammation of diabetic gastroparesis(DGP)rats and its possible mechanism.Methods A total of 62 male Wistar rats were randomly divided into the control group(12)and the modeling group(50).Except for the control group,the remaining rats were given multi-ple intraperitoneal injections of streptozotocin(25 mg ·kg-1 for three consecutive days)and irregular feed-ing of high-sugar and high-fat diet to replicate DGP model.The model rats were randomly divided into the model group(intragastatically purified water),low,medium and high dose HPS groups(50,100,200 mg ·kg-1·d-1)and the metformin group(90 mg· kg-1·d-1),respectively,and the control group was intragastrically treated with equal volume of purified water once a day for eight weeks.The pathological morphology of gastric mucosa was observed by HE stai-ning;the contents of TNF-α,IL-6,GAS and MTL in gastric mucosa were detected by ELISA.The expres-sion of JAK2 and STAT3mRNA in gastric mucosa was detected by RT-PCR.The levels of JAK2 and STAT3 proteins and their phosphorylation in gastric mucosa were detected by Western blot.Results Compared with the control group,the gastric mucosa of the model group showed a large number of inflammatory cells in-filtrated by HE staining.The contents of TNF-α and IL-6 significantly increased(P<0.01),while the contents of GAS and MTL significantly decreased(P<0.01).The mRNA expressions of JAK2 and STAT3 significantly increased(P<0.05).p-JAK2and p-STAT3 significantly increased(P<0.01).Compared with the model group,gastric mucosal inflammation was improved in each administration group.The con-tents of TNF-α and IL-6 decreased significantly,while the contents of GAS and MTL increased significantly.The mRNA expressions of JAK2 and STAT3 were sig-nificantly reduced.The expressions of p-JAK2 and p-STAT3 significantly decreased(P<0.05).Conclu-sions HPS can improve gastric mucosal inflammation and repair gastric mucosal damage in rats,and its mechanism may be related to the regulation of JAK2/STAT3 signaling pathway.
8.Mechanism of Kechuanting granules in suppressing IL-33/ILC2s and pathogenic T cells to intervene in allergic airway inflammation

Aim To investigate the mechanisms of Ke-chuanting granules(KCT)inhibiting the IL-33/ILC2s pathway and pathogenic T cells to intervene in allergic airway inflammation.Methods Network pharmacolo-gy was utilized to analyze the potential targets and mechanisms of KCT-treated asthma.Allergic asthma models were induced in mice using OVA.Lung histo-pathology was conducted to observe injury changes.ELISA and quantitative PCR were utilized to measure key inflammatory factors and their mRNA expression levels in Th2-type asthma.Western blot was used to detect the phosphorylation levels of relevant proteins in the MAPK pathway.Flow cytometry was performed to evaluate the proportions of ILC2s,Th1,Th 17,Th2 and Treg cells.Results Network pharmacology iden-tified 227 main active components and 143 key targets of KCT in treating asthma,primarily enriched in signa-ling pathways such as MAPK and IL-17.Further vali-dation experiments demonstrated that KCT significantly alleviated lung inflammatory injury in asthmatic mice,reduced the number of B cells,production of I L-4,TNF-α and TGF-β,downregulated JNK phosphoryla-tion levels in lung tissue,as well as mRNA levels of Il-33,Bcl11b,Rorα,Tcf-7,Jun,Mapk3 and Mapk14.KCT intervention reduced the numbers of ILC2s and Th 17 cells in lungs and spleens of mice,and inhibited Th2 cell infiltration in lungs.Conclusions KCT ex-hibits therapeutic effects on allergic airway inflamma-tion in asthma,closely associated with the inhibition of the IL-33/ILC2s pathway,pathogenic T cell subsets,and JNK-MAPK signaling pathway.
9.Development of nanographene oxide as clinical drug carrier in cancer therapy

Immunotherapy is an important breakthrough in canc-er treatment.Unfortunately,low drug concentration in tumor sites almost ineffectively initiates immune responses and thereby severely limits immune therapy applications in clinics.Nanoma-terials are well-recognized drug delivery system in cancer thera-py.Nanographene oxide(NGO)have shown immense perti-nence for anti-cancer drug delivery owing to their ultra-high sur-face area,chemical stability,good biocompatibility and excel-lent photosensitivity.In addition,functionalized modifications on the surface of NGO increase tumor targeting and minimize cy-totoxicity.This study focuses on reviewing the literature and up-dates on NGO in drug delivery and discussing the possibilities and challenges of NGO in cancer synergetic therapy.
10.Effect of Tanganjianwan combined with abdominal massage on glucose and lipid metabolism and inflammatory factors in patients of metformin treatment with obesity and type 2 diabetes mellitus

Objective To explore the effects of Tanganjianwan(TGJW)combined with abdominal massage(AM)on glucose and lipid metabolism and inflammatory factors in obesity and type 2 diabetes mellitus(OT2DM)patients with metformin treatment.Methods Patients with OT2DM diagnosed in Wuhan Hospital of Traditional Chinese Medicine from June 2021 to June 2023 were included as study subjects,and were divided into the AM group(metformin combined with AM)and the TGJW group(metformin combined with AM+TGJW)according to the random number table method.The treatment efficacy and safety were assessed by continuous intervention for 3 months.Obesity-related indices[waist-hip ratio(WHR)and body mass index(BMI)],glycemic and lipid metabolic indices[glycosylated hemoglobin(HbA1c),fasting blood glucose(FBG),postprandial 2-hour glucose(P2hG),total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL-C)],insulin sensitivity indexes[homeostasis model insulin resistance index(HOMA-IR)and homeostasis model pancreaticβ-cell function index(HOMA-β)],and inflammation indexes[interleukin-6(IL-6),tumor necrosis factor α(TNF-α),and C-reactive protein(CRP)]changes were compared before and after treatment between the TGJW group and the AM group.Results A total of 100 OT2DM patients were included in the study,with 50 cases each in the TGJW group and AM group.After the intervention,the treatment efficiency of the TGJW group was significantly higher than that of the AM group(P<0.05),and no malignant adverse events occurred during treatment.Compared with the pre-treatment period,the levels of WHR,BMI,FBG,P2hG,HbA1c,TC,TG,LDL-C,HOMA-IR,IL-6,TNF-α,and CRP decreased in both groups(P<0.05),while the levels of HDL-C and HOMA-β increased significantly(P<0.05).After the intervention,the levels of WHR,BMI,FBG,P2hG,HbA1c,TC,TG,LDL-C,HOMA-IR,IL-6,TNF-α,and CRP in the TGJW group were lower than those in the AM group(P-<0.05),while the levels of HDL-C and HOMA-β were higher than those in the AM group(P<0.05).Conclusion Compared with AM treatment alone,TGJW combined with AM can improve glucose-lipid metabolism,insulin sensitivity and inflammation indexes in OT2DM patients,enhance the therapeutic effect of metformin treatment in OT2DM patients,and is safe,it has certain clinical promotion significance.
