1.Clinical application of modified edge resection technique for composite labia minora and clitoral hood reduction with preserved microstructures.

OBJECTIVE:
To investigate effectiveness of the modified edge resection technique for composite labia minora and clitoral hood reduction with preserved microstructures.
METHODS:
A retrospective analysis was conducted on 36 female patients, who were diagnosed with composite hypertrophy of the labia minora and clitoral hood and admitted between September 2022 and December 2024. The patients' ages ranged from 18 to 45 years (mean, 27.4 years). The primary surgical motivations included poor appearance alone (8 cases), functional impairment alone (14 cases), both poor appearance and functional impairment (12 cases), and psychological factors (2 cases). All patients were treated with the modified edge resection technique for composite labiaminora and clitoral hood reduction with preserved microstructures. The surgical technique emphasized precise adjustments to the clitoral-labial junction and optimization of the anterior labial structure to ensure a natural postoperative appearance and functional integrity. Postoperative follow-up assessed improvements in appearance and function of clitoral hood and labia minora, complications, and overall patient satisfaction.
RESULTS:
One patient exhibited suboptimal wound healing, while the remaining patients experienced no complications such as postoperative bleeding, hematoma, wound dehiscence, suture cutting, or labial edema and enlargement. Thirty patients were followed up with a duration of 1-6 months (mean, 2.4 months). In the early postoperative period, 2 patients perceived asymmetry of the bilateral labia minora; 1 underwent labial revision surgery, while the other achieved near-symmetry without intervention. At last follow-up, 25 patients experienced varying degrees of relief from preoperative functional impairments, while the remaining patients showed no improvement; 27 patients reported varying degrees of improvement in appearance, 2 reported no change, and 1 reported a worse appearance compared to preoperatively. Sixteen patients were very satisfied with the surgical results, 8 were satisfied, 5 were moderate satisfied, and 1 was dissatisfied, with a satisfaction rate of 80% (24/30).
CONCLUSION
The modified edge resection technique for composite labia minora and clitoral hood reduction with preserved microstructures which preserves and optimizes fine anatomical structures through precise adjustments at the clitoral-labial junction, achieves high patient satisfaction with both aesthetic and functional outcomes while minimizing postoperative complications.

2.Lipomatous angiomyofibroblastoma in the vulva: A case report with review of literature.

This is a case of a 55-year-old female, surgical menopause for nine years, not on hormone therapy with one year history of a progressively enlarging left vulvar mass, who sought consult and subsequently underwent excision of mass. Microscopic findings showed alternating hypo- and hypercellular areas composed of plump spindle to epithelioid cells distributed in an edematous stroma with varying amount of collagen. Tumor cells have bland chromatin, inconspicuous nucleoli, and absent mitoses. The cells surround small to medium-sized thin-walled, hyalinized vessels found in hypercellular areas. An adipocytic differentiation of more than fifty percent of tumor is observed further classifying this neoplasm as lipomatous angiomyofibroblastoma, making this an even rarer type of benign mesenchymal tumor of the vulva. Immunohistochemistry stains performed showed positive staining for vimentin, smooth muscle actin, BCL-2, ER, PR, and negative staining for desmin which supports the diagnosis. The clinical presentation, operative findings, histopathologic features and the various considerations are discussed. Literature review of vulvar angiomyofibroblastoma is also presented.

3.Urogenital health and intimate hygiene practices among Filipino women of all ages: Key issues and insights.

Routine intimate hygiene care has a major contribution in maintaining overall urogenital and perineal health in women. However, Filipino women continue to experience a major surge in vulvar and vaginal symptoms across all age groups, in a context of major changes in lifestyles and risk factors impacting their genital health. Personal beliefs, preferences, apprehensions to discuss intimate topics with health care practitioners (HCPs), availability of cleansing products in the market, and their affordability prevent many women from discussing the role of intimate hygiene care with their HCPs. Communication difficulties and lack of robust evidence, supporting optimal hygiene recommendations are some of the challenges experienced by HCPs. Through this review, the authors discuss the following factors: (i) Differing physiological needs and pathological effects that result from changing dynamics of microflora in the vulvar, perineal, and vaginal region across all age groups of women, (ii) Importance of focusing on perianal and perineal hygiene, and bowel habits, to improve the quality of vulvar hygiene and genital health, (iii) Designing approaches for HCPs to maintain genital health in the light of intimate hygiene, (iv) Recommending improvements in HCP-patient communications to help HCPs dispel the misconceptions pertaining to intimate hygiene practices, and (v) Highlighting the antimicrobial efficacy of feminine hygiene cleansers that preserve the natural microbiome and help maintain the vaginal pH within the normal range. These strategies can fill the knowledge gaps among HCPs, women, and their caregivers’ perspectives and help achieve optimal intimate hygiene.

5.Soboliphyme baturini (Nematoda: Soboliphymatidae) Recovered from Stomach of Asian Badger, Meles leucurus, in Geochang-gun, Gyeongsangnam-do, Korea

We are going to describe the female soboliphymid nematodes, which were recovered from the stomach of a Asian badger, Meles leucurus (Mammalia: Mustelidae), in Geochang-gun, Gyeongsangnam-do, Korea. In February 1998, we found 2 peculiar nematodes with a cup-like organ in the anterior end from the stomach of badger. Recovered worms were fixed with 10% formalin, cleared in glycerin-alcohol solution and observed under a light microscope with a micrometer. They were 34.46 (33.43–35.50) mm long by 2.13 mm at maximum width. Cephalic sucker cup-like, 3.34 (3.13–3.55) mm wide, 2.40 (2.25–2.55) mm long, with the oral aperture and meridionally striated on the buccal capsule. Oral aperture 2.38 mm in diameter. Circumoral membrane 0.41 (0.38–0.45) mm wide. Esophagus muscular, 4.81 (4.50–5.00) mm long by 0.80 (0.78–0.83) mm at maximum width. Vulva situated at 3.13 mm ventro-anterior level from the esophago-intestinal junction. Vagina anteriad, 3.38 mm long, making a canal from the uterus to the vulva opening. Uterus single, large. Tail 0.35 (0.33–0.38) mm long. Intrauterine eggs long elliptical, 0.058–0.065 (0.062) mm long and 0.030–0.033 (0.031) mm wide. Based on the some morphological characters and host-specificity, our specimens are nearly identical with S. baturini. Therefore, the present report describes S. baturini for the first time in Korea.

6.A case report on primary Cutaneous Mucoepidermoid Carcinoma of the vulva and its clinico-pathologic identity

Mucoepidermoid Carcinoma (MEC) is an epithelial malignant tumor that was first described as a salivary gland malignancy. Though common in salivary gland, it is extremely rare in the vulva with only 2 cases reported in the English language literature and none yet in the Philippines. Due to its low incidence, prognosis and definitive management is still unclear. This is a case of a 68-year-old woman with a history of vulvar pruritus and vulvar mass at the left labia majora. Punch biopsy and review of slides revealed Invasive Squamous Cell Carcinoma, Non-Keratinizing type. She underwent Radical Vulvectomy and Bilateral Lymph Groin Dissection; Wide Excision of Perineal Area; Protective Transverse Loop Colostomy; Gracilis Myocutaneous Flap with Identification of Right and Left Median Circumflex Artery with a final histopathology report of Primary Cutaneous MEC of the vulva with lymph node metastasis.

7.A Case of Lymphoepithelioma-Like Carcinoma in the Thyroid Gland.

Lymphoepithelioma-like carcinoma exhibits immunohistochemically similar features to lymphoepithelioma and commonly occurs in the skin, salivary gland, breast, lung, gastrointestinal tract, liver, urinary tract, prostate, vulva and vagina. Lymphoepithelioma-like carcinoma from the thyroid gland is extremely rare. We recently experienced a case of lymphoepithelioma-like carcinoma of thyroid gland in a 28-year-old female, who presented a thyroid nodule that was suspicious of papillary carcinoma. We report this unusual case of lymphoepithelioma-like carcinoma of thyroid gland with a brief review of literature.
