1.A technical modification in cavo-portal hemi-transposition in adult living donor liver transplantation

Extensive porto-mesenteric thrombosis presents a significant challenge in liver transplantation and was previously considered a contraindication. However, advancements in surgical techniques have made liver transplantation feasible. For optimal allograft function, adequate portal flow is crucial, as it generates shear stress that stimulates regeneration. In such cases, portal inflow options include the left renal vein (reno-portal anastomosis; RPA), the inferior vena cava (cavo-portal hemi-transposition; CPHT), any patent splanchnic territory, portal vein arterialization, or multi-visceral transplantation. Among these, CPHT and RPA are the most commonly performed. Generally, CPHT is used in pediatric liver transplantation; however, it is rarely reported in adult living donor liver transplantation (LDLT) due to technical challenges. In this report, we describe our technical modifications to CPHT and present the results in two patients with extensive porto-mesenteric thrombosis who underwent LDLT.
2.Estimating the ideal pretransplant waiting time for living donor liver transplantation in acute-on-chronic liver failure:a retrospective study

Background:
Acute-on-chronic liver failure (ACLF) is associated with high mortality, but transplantation improves survival. Optimizing organ function is time-consuming and can increase infection risk. This study investigated the optimal pretransplant waiting period.
Methods:
In this retrospective study of patients with ACLF who underwent transplantation between January 2021 and August 2024, perioperative details and morbidity were compared between survival and mortality groups. Receiver operating characteristic (ROC) analysis was used to determine the cutoff for the pretransplant waiting period.
Results:
Among 112 patients with ACLF under the European Association for the Study of the Liver criteria, 61 (54.5%) underwent living donor liver transplantation (mean age, 41 years; 51 males [83.6%]). The most common etiology was viral infection (44.2%). The median Chronic Liver Failure Consortium (CLIF-C) score was 44 (respiratory failure, 14.7%; renal failure, 16.4%). There were 14 (22.9%) posttransplant deaths. The median waiting period between admission and surgery was longer in the mortality group (8 vs.4 days, P=0.2). The area under the ROC curve for the optimal pretransplant waiting period was 0.723 (P=0.01). A cutoff of 5 days predicted mortality with 71.4% sensitivity and 61.9% specificity. On univariate analysis, survivors and nonsurvivors differed significantly in age, hemoglobin level, warm ischemia time, and postoperative gastrointestinal (GI) bleeding; on multivariate analysis, postoperative GI bleeding independently predicted mortality. During a median follow-up of 17 months, no deaths occurred.
Conclusions
A pretransplant waiting period of 5 days predicted mortality; furthermore, postoperative GI bleeding was an independent predictor of mortality. Further studies are required to confirm these findings.
3.A dual-lobe strategy in living-donor liver transplantation: overcoming small-for-size graft limitations in an obese patient:a case report

The primary goal of living donor liver transplantation (LDLT) is to optimize outcomes for the recipient while ensuring donor safety. However, there is a risk of small-for-size syndrome, particularly in obese patients, where graft weight might be inadequate. In such scenarios, dual-lobe LDLT represents a more viable alternative. Although ethical concerns exist regarding potential risks posed to two healthy donors, this method may be most appropriate when deceased donor grafts are unavailable. Here, we describe a case involving an obese patient classified as Child-Pugh class C with chronic liver disease who underwent dual-lobe LDLT. The criteria for selecting the right lobe graft differed slightly from standard protocols, as did the implantation technique. The patient experienced low-output bile leakage, which resolved spontaneously. Priority in graft selection was given to the number of bile ducts, as the right lobes of both donors had nearly equal volumes. Despite simultaneous implantation, cold and warm ischemia times remained manageable due to effective coordination. Although the literature suggests a higher incidence of vascular and biliary complications in dual-lobe LDLT than in single-lobe procedures, these risks can be mitigated by careful planning and coordination.
4.Hepatic basidiobolomycosis masquerading as cholangiocarcinoma: a case report and literature review

Basidiobolus ranarum is known to cause subcutaneous mycoses; however, rare cases of hepatic and gastrointestinal involvement by basidiobolomycosis have been reported. Hepatic basidiobolomycosis may be confused with a carcinoma on imaging, and histological examination and fungal culture can help distinguish between these two. We report a rare case of basidiobolomycosis in a 16-year-old male with liver and gastrointestinal involvement.
5.Small Cell Type Undifferentiated Carcinoma of Gall Bladder with Pas Positive Hyaline Globule Masquerading as Liver Mass: A Case Report and Literature Review

An undifferentiated carcinoma (UC) of the gall bladder behaves aggressively and has a grave prognosis. Small cell type undifferentiated carcinoma of the gall bladder is a rare variant. This paper reports a case of UC of gall bladder with PAS-positive diastase-resistant eosinophilic hyaline globules present as liver mass (on imaging) in a male patient. The microscopic findings of the liver and gall bladder after a right tri-segmentectomy showed an un-differentiated malignant neoplasm composed of cells with round to oval nuclei, prominent nucleoli, and scanty neoplasm. No definite cell pattern was identified with these neoplastic cells. A section from the gall bladder revealed a tumor arising from the lining epithelium and infiltrating through the muscularis. This tumor was infiltrating the adherent liver tissue directly and forming a mass of undifferentiated malignant cells. The focal area within the tumor mass showed the presence of PAS-positive, diastase-resistant, eosinophilic hyaline globules within the neoplastic cells. The immunohistochemistry test was diffusely positive for perinuclear anti-neutrophil cytoplasmic antibodies and negative for chromogranin, vimentin, Desmin, alpha-fetoprotein, leukocyte common antigen, CD34, and bcl2. When the clinical and radiological data are inconclusive, careful analysis of the histological and immunophenotypic features is needed to make the final diagnosis of UC of the gall bladder. The biological behavior and prognosis of this tumor remain unclear because of its rarity. Further studies will be needed to understand the characteristics of this deadly tumor and to establish an effective therapy for it.
