Jaw defects may result from tumor resection, trauma, or infection-related osteomyelitis. Advances in microvascular surgery have enabled predictable restoration of form and function through vascularized free tissue transfer. Among the available options, the deep circumflex iliac artery (DCIA) flap provides a substantial segment of corticocancellous bone with a natural curvature, rendering it well suited for reconstructing the maxilla and mandible. We describe a case of delayed maxillary reconstruction using a DCIA flap with intraoral vascular anastomosis following partial maxillectomy for squamous cell carcinoma. The intraoral approach helped avoid a cervical incision, minimize morbidity, and achieve stable bone integration, allowing successful implant rehabilitation.
