1.Quantitative research on China's disability rehabilitation policy using policy modeling consistency index model

Objective To quantitatively analyze and evaluate the content of rehabilitation policy for people with disabilities in China. Methods This study focused on ten national policies of disability rehabilitation issued from 2021 to 2023.It employed text mining techniques to process policy texts and constructed a policy modeling consistency index model for dis-ability rehabilitation policies in China.The relevant policies were evaluated and analyzed quantitatively. Results The disability rehabilitation policies in China were relatively comprehensive in terms of policy transparency,op-erational mechanisms and policy nature.However,there was still a need for optimization in terms of policy per-spectives,target groups,incentive mechanisms,and other aspects. Conclusion The overall quality of disability rehabilitation policy texts at the national level in China is relatively good.There is a need to further enhance the predictability of policy objectives,clarify the responsibilities and division of labor among various departments,and improve policy incentive mechanisms in future policy formulation,which will further refine China's disability rehabilitation policy system and contribute to high-quality develop-ment of the disability cause.
2.Analysis of HIV-1 Subtypes and Transmitted Drug Resistance in Hospitalized Treatment-Native Patients With AIDS

Objective To investigate the distribution characteristics of HIV-1 subtypes,the status of transmitted drug resistance(TDR),and the influencing factors of TDR in treatment-naive patients with AIDS who are hospitalized.Methods Treatment-naive patients with AIDS who were admitted to the Infectious Disease Department,Public Health Clinical Center of Chengdu between January 2020 and December 2022 were enrolled in the study.The diagnosis and confirmation diagnosis of all the subjects were made at the same hospital.Blood samples were collected from the subjects before antiretroviral therapy(ART).The in-house method was used for HIV gene amplification and sequencing.A phylogenetic tree was constructed to analyze the HIV-1 subtypes.The Stanford HIV Drug Resistance Database was used to conduct an online comparative analysis of the drug resistance mutation sites and to determine the types and levels of drug resistance.The distribution characteristics of HIV-1 subtypes,the occurrence of TDR,and the influencing factors of TDR were analyzed.Results A total of 213 patients were included in the study and their blood samples were collected.HIV-1 subtypes were successfully amplified in 83.10%(177/213)of the subjects.Ten HIV subtypes were identified,with CRF07_BC being the most common subtypes,accounting for 43.50%(77/177),which was followed by CRF01_AE at 37.85%.Unique recombinant forms(URFs)were relatively uncommon,accounting for 8.47%.The other subtypes accounted for 10.17%.These 4 categories of HIV-1 subtypes were distributed with statistically significant differences in different age groups(P=0.024).Further analysis revealed significant differences in the distribution of the HIV-1 subtypes of CRF01_AE and URFs between the groups of patients aged 30-50 years and those over 50.In addition,URFs accounted for a higher proportion in patients aged 30 to 50 years(P=0.008).The incidences of TDR were 6.49%,8.96%,13.33%,and 5.56%for CRF07_BC,CRF01_AE,URFs,and other subtypes,respectively,showing no significant difference(P>0.05).The overall TDR was 6.57%.The TDR for non-nucleoside reverse transcriptase inhibitors(NNRTIs)was 5.16%,and the main mutation sites were V179D/E,E138A/G,V106M/I,and Y181C.The TDR for nucleoside reverse transcriptase inhibitors(NRTIs)was 1.88%,and the main mutation site was M184V.One patient was found to be resistant to both NNRTIs and NRTIs.The highly resistant rate was 4.23%,moderate resistance was 0.47%,and low resistance was 1.88%.No significant effects of the specific years,demographic characteristics,transmission route,baseline condition,and opportunistic infections on TDR were found in this study(P>0.05).Conclusions The HIV-1 subtypes are diverse and complex in treatment-naive patients with AIDS who were hospitalized.The overall prevalence of TDR is relatively high.It is necessary to strengthen HIV drug resistance testing to optimize ART treatment and reduce the risk of drug resistance transmission.
3.A case report of tuberculous meningoencephalitis with anti-neurexin-3α antibody-associated encephalitis

The clinical manifestations of tuberculous meningoencephalitis(TBME)are varied,and diagnostic tests for Mycobacterium tuberculosis show limited sensitivity and specificity,often failing to provide timely diagnostic support,which can delay prompt treatment.Reports on anti-neurexin-3α antibody-mediated autoimmune encephalitis remain scarce,and there have been no documented cases of TBME overlapping with anti-neurexin-3α antibody-associated encephalitis either domestically or internationally.This article presents a case treated at our hospital to contribute insights for improving the diagnosis and treatment of such conditions in the future.The patient,a middle-aged woman,presented with a one-month history of headache and incoherent speech,along with one day of left-sided limb weakness.Lumbar puncture results revealed significantly elevated cerebrospinal fluid(CSF)pressure,an increased lymphocyte count,and reduced glucose and chloride levels.Enhanced MRI showed multiple intracranial lesions,basal meningeal enhancement,and marked hydrocephalus,supporting an initial diagnosis of TBME.Concurrently,both blood and CSF tests were positive for anti-neurexin-3α antibodies.After comprehensive treatment,including anti-tuberculosis therapy,high-dose immunoglobulin,and corticosteroids,the patient's clinical symptoms,CSF findings,and MRI results improved,and she was discharged.

5.Evaluation of functional tricuspid regurgitant valves and right heart structural and functional changes by four-dimensional auto quantitative techniques

Objective:To comprehensively evaluate the tricuspid valve, right heart anatomical characteristics and related dynamic parameters in patients with different degrees of functional tricuspid regurgitation (FTR) using four-dimensional auto tricuspid valve quantitative(4D Auto TVQ), four-dimensional auto right ventricle quantitative(4D Auto RVQ), and four-dimensional auto left atrium quantitative(4D Auto LAQ), and to investigate the structural and functional changes of the tricuspid valve and right heart in them.Methods:Sixty-three patients with FTR diagnosed by echocardiography at the First Affiliated Hospital of Guangxi Medical University from February to July 2022 were prospectively selected as the case group, including 30 patients with mild FTR and 33 patients with moderate or above FTR, and 30 healthy subjects were selected as the control group. Transthoracic echocardiography was used for two-dimensional and three-dimensional image acquisition of the heart. The tricuspid regurgitation volume, left ventricular ejection fraction (LVEF), right ventricular global strain (RVGS) were measured by 2D images, and pulmonary artery systolic pressure (PASP) were measured from the tricuspid regurgitation pressure difference. The 3D images were imported into EchoPAC 204 to obtain the tricuspid valve, right heart structure and related dynamic parameters. The annulus area (AA), annulus perimeter(AP), spherical index (SI), annulus area change fraction (AC), coaptation point height (CPH), and tenting volume (TV) were measured by 4D Auto TVQ. The right atrial maximum volume (RAVmax) and right atrial minimum volume (RAVmin) were measured by 4D Auto LAQ. Right ventricular end-diastolic volume (RVEDV), right ventricular end-systolic volume (RVESV), right ventricular fractional area change (RVFAC) and tricuspid annular plane systolic excursion (TAPSE) were measured by 4D Auto RVQ. After standardizing the dimension parameters with body surface area (BSA), the differences in the above parameters were compared between the three groups, the correlation between regurgitant volume and each parameter was compared by correlation analysis, and the independent factors of increased tricuspid regurgitant volume were investigated by univariate and multivariate linear regression analysis.Results:There were statistically significant differences in PASP, AA/BSA, AP/BSA, AC, TV, RAVmax/BSA, RAVmin/BSA, RVFAC, RVGS, and TAPSE between the three groups (all P<0.05). There were statistically significant differences in LVEF, CPH, RVEDV/BSA, and RVESV/BSA in the moderate and above FTR group compared with the control and mild FTR groups (all P<0.05). Correlation analysis showed that RAVmin was the most highly correlated with tricuspid regurgitant volume ( r=0.875, P<0.001) and TV and end-systolic annulus area(ESAA) were highly correlated with tricuspid regurgitant volume ( r=0.747, 0.683; both P<0.001) in patients with FTR. Multifactorial linear regression showed that RAVmin, TV and regurgitant volume were independently positively correlated (β=0.721, 0.205; both P<0.05). Conclusions:The four quantification technique can provide valid structural and functional information by quantifying the tricuspid valve as well as the right heart in patients with FTR, and RAVmin and TV are independent correlates of increased tricuspid regurgitant volume.
6.Comparison of ultrasound radiomics with conventional imaging models :diagnosis of central cervical lymph node metastasis in papillary thyroid carcinoma

Methods A training set of 609 cases was set up . USR features were extracted and screened by USR method . A weighted formula was established to calculate the USR score of each patient by ultrasound image . T he USR score of the best diagnostic ability was obtained by statistical method and set as the diagnostic criterion . A test set of 326 cases was established to compare the diagnostic ability of USR score with ultrasound ( US ) ,computed tomography ( CT ) and US combined CT . Results T he accuracy ,sensitivity ,specificity ,area under ROC curve and Youden index of USR score in test set were 0 .804 ,0 .867 ,0 .770 ,0 .766 ,0 .533 ,respectively ,which were significantly higher than the corresponding values of US ,CT and US combined CT ( all P = 0 .000 ) . Conclusions USR score obtained with USR method can effectively predict lymph node metastasis in Ⅵ region of PTC . T he diagnostic efficiency and clinical value of USR score were significantly higher than those of conventional medical imaging models .

8.Correlation between Supportive Care Need and Quality of Life in Patients with Breast Cancer Accepting Chemo-therapy

Objective To observe the level of supportive care need and quality of life in patients postoperation for breast cancer and ac-cepting chemotherapy, and the relationship between them. Methods From September, 2015 to June, 2016, 235 patients with breast cancer af-ter surgery were conveniently sampled, and investigated with the general situation questionnaire, Supportive Care Need Survey Short-form (SCNS-SF34) and Functional Assessment of Cancer Therapy-Breast (FACT-B). Results A total of 230 patients were analyzed. Patients self-reported to need supportive care in all dimensions of SCNS-SF34, and the most frequency was in the health information (31.30%). The score of FACT-B was (93.25±23.53), that needed further improvement. The needs of supportive care in each dimension negatively correlat-ed with the scores of FACT-B in the dimensions (|r|>0.168, P<0.05) except society/family and function. Conclusion The patients with breast cancer need variety of supportive care, which may impact their quality of life.
9.A clinic study on desensitization treatment of bronchial asthma with positive specific IgE to dust mite in children

Objective To investigate the efficacy and the course of desensitization treatment in bronchial asthma with positive specific IgE to dust mite in children. Methods A total of 105 children with bronchial asthma with positive specific IgE to dermatophagoides farinae allergens were randomized into the observation group and the control group. Children in the control group were treated to continue anti-asthma according to the routine of prevention and treatment children with asthma. Chinldren in the observation group were treated by dermatophagoides farinae drops in addition to the treatment of children in the control group. The recurrence of asthma was compared between two groups at 25 weeks post-treatment. At 25 weeks post-treatment , children in the observation group was randomly divided into the observation groupⅠand group Ⅱ. Children in the observation groupⅠreceived continuous treatment except for desensitization treatment. Children in the observation group II received the sublingual immunotherapy with dermatophagoides farinae drops (No.4) for 1 year in addition to the treatment in the observation groupⅠ. The recurrence of asthma was also compared between the two sub-groups. Results The rate and times of recurrence of asthma were lower in the observation group than those in the control group(P < 0.05), with no significant differences between the observation groupⅠand groupⅡ (P > 0.05). Conclusion The recurrent rate and frequency of asthma could be reduced by the sublingual immunotherapy with dermatophagoides farinae drops in children with asthma of positive specific IgE to dust mite. The course of treatment may be half year long.
10.Clinicopathological analysis of clear cell meningioma.

We present two cases of clear cell meningioma (CCM) in the intracranial and intraspinal region with anaplastic features. On histological examination, both the tumors exhibited unusual anaplastic appearances with nuclear pleomorphism, a high mitotic activity and necrosis, which were different from classical CCMs. The tumor cells were immunoreactive to epithelial membrane antigen (EMA) and vimentin with a high MIB-1 index of 40%. Total excision of the tumors was performed in both cases. The male patient was found to have local recurrence and lateral ventricle metastasis 3 months after the total excision. We reviewed the clinicopathological features, disagnosis and prognosis of the disease. We recommend that postoperative adjuvant radiotherapy or chemotherapy be performed after total tumor excision, and MRI scan every 3-6 months is mandatory during the initial follow-up period.
