1.Mechanism of Wendan Ningxin Granules in Modulating Diastolic Calcium Leakage-related Proteins to Improve Inflammation-associated Atrial Fibrillation Susceptibility

ObjectiveTo investigate the protective effect and mechanism of Wendan Ningxin granules (WNG) on susceptibility to atrial fibrillation (AF) in mice with inflammatory injury. Methods100 C57BL/6 mice were divided into a blank control group, a model group, a low-dose WNG group (2.34 g·kg-1·d-1), a high-dose WNG group (4.68 g·kg-1·d-1), and an amiodarone positive control group (0.091 g·kg-1·d-1), with 20 mice in each group. Except for the blank control group, mice in other groups received intraperitoneal injections of lipopolysaccharide (LPS) to establish an inflammatory injury model. Treatment groups received continuous intragastric administration of their respective interventions for four weeks. During the fourth week, the treatment groups received LPS injections concurrently with their treatments. The blank control and model groups received distilled water (10 mL·kg-1·d-1) by gavage, with a gavage volume of 10 mL·kg-1 for all groups, once daily. Hematoxylin-eosin (HE) staining and Sirius red staining were used to observe atrial tissue morphology and fibrosis degree. Immunohistochemistry was used to assess the expression of α-smooth muscle actin (α-SMA) in mouse atrial tissue. Electrophysiological detection was performed using a multi-channel electrophysiology mapping system to measure AF inducibility, AF duration, and atrial effective refractory period (AERP). High-resolution optical mapping was used to measure action potential duration (APD) dispersion, conduction heterogeneity index, and calcium transient (CaT) dispersion. Real-time quantitative polymerase chain reaction (Real-time PCR) was used to detect mRNA expression of proteins related to diastolic calcium leakage in mouse atria: Ca2+/calmodulin-dependent protein kinase Ⅱ(CaMKⅡ), ryanodine receptor 2(RyR2), sarco/endoplasmic reticulum Ca²⁺-ATPase (SERCA), and sodium-calcium exchanger (NCX). Western blot analysis was performed to detect the expression of CaMKII, RyR2, SERCA, and NCX proteins in myocardial tissue from each group. Enzyme-linked immunosorbent assay (ELISA) was used to measure serum levels of inflammatory factors interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α). ResultsPathological staining results showed that compared with the blank control group, the model group exhibited disrupted atrial tissue structure, inflammatory cell infiltration, atrial fibrosis, and diffuse infiltration of numerous brown α-SMA positive cells in the atrial interstitium (P<0.01). AF could be induced by electrical stimulation with a longer duration. AERP was shortened, while APD dispersion, conduction heterogeneity index, and CaT dispersion were increased (P<0.01). The expression of proteins associated with diastolic calcium leakage, including CaMKⅡ, RyR2, and NCX1, showed elevated mRNA and protein levels, whereas SERCA2a mRNA and protein expression decreased (P<0.05). Serum levels of inflammatory factors IL-1β and TNF-α were elevated (P<0.01). Compared with the model group, intervention with WNG alleviated cardiac structural damage, reduced inflammatory cell infiltration, improved atrial fibrosis, and reduced the diffuse infiltration of α-SMA positive cells (P<0.01). AF inducibility and AF duration upon electrical stimulation were significantly reduced (P<0.05), AERP was prolonged (P<0.05), mRNA and protein expression of CaMKⅡ, RyR2, and NCX1-proteins associated with diastolic calcium leakage-were reduced, whilst mRNA and protein expression of SERCA2a increased (P<0.05), and serum levels of IL-1β and TNF-α were decreased (P<0.01). ConclusionBoth low‑ and high‑dose WNG can effectively reduce susceptibility to inflammation-related AF. The mechanism by which WNG reduce AF susceptibility may be related to regulating proteins involved in sarcoplasmic reticulum diastolic calcium leak, thereby improving cardiac electrical remodeling, and alleviating inflammation-induced myocardial fibrosis, thus improving cardiac structural remodeling.
2.New advances in the treatment of neonatal diabetes mellitus with sulfonylureas

Neonatal diabetes mellitus (NDM) is a rare monogenic disorder primarily caused by insufficient insulin secretion resulting from mutations in the KCNJ11 and ABCC8 genes. Sulfonylureas, represented by glibenclamide, have become the standard therapy for this type of NDM by precisely closing the mutated ATP-sensitive potassium channels in pancreatic β cells, thereby restoring insulin secretion. Clinical studies confirm that sulfonylureas enable over 90% of patients to successfully transition from insulin to oral treatment, achieving long-term stable glycemic control and improving neurological outcomes to a certain extent. In terms of safety, severe hypoglycemia induced by sulfonylureas is relatively rare and gastrointestinal reactions are mild; moreover, sulfonylureas show good long-term tolerability, and have no adverse effects on child growth and development. In the future, by further refining the full-chain management pathway of “rapid genetic diagnosis-early intervention-specialized dosage forms-long-term follow-up”, the clinical application of sulfonylureas is expected to provide NDM patients with an optimized treatment regimen and maximize their health benefits.
3.Discussion on Pathogenesis and Treatment of Esophageal Precancerous Lesions from the Perspective of "Turbidity Damaging the Clear and Harmonious"

It is considered that "turbid pathogen damaging the clear, and the loss of clarity and harmony regulation" is the core pathogenesis of esophageal precancerous lesions, with turbid pathogen persisting throughout the disease course. Turbid pathogen encompasses qi turbidity, phlegm turbidity, stasis turbidity, and toxin turbidity, closely related to dietary indiscretion, emotional disharmony, spleen-stomach deficiency, and kidney deficiency failing to consolidate. Turbid pathogen undergoes a dynamic evolution process of qi constraint generating turbid→ phlegm-stasis transforming into toxin→turbidity toxin harming the healthy qi, corresponding to the pathological alterations of hypoxia, inflammation and immunosuppression within the tumor microenvironment. The overall therapeutic principle is to remove turbidity and restore clarity. In the early stage, targeting the hypoxic tumor microenvironment, the suggested treatment method is opening constraint and dispersing the block, regulating qi and remove turbidity, with emphasis on soothing liver and regulating stomach, raising the clear and directing the turbid downward. In the middle stage, targeting inflammatory microenvironment, the treatment aims to clear toxin and dissipate nodules, resolve stasis and eliminate turbid, emphasizing phlegm-stasis elimination, turbid-toxin resolution, and concurrent stomach qi protection. In the late stage, corresponding to immunosuppressive microenvironment, the method of boosting qi, nourishing yin, reinforcing healthy qi, and eliminating turbidity can be used, with focus on spleen-stomach tonification, kidney essence replenishment, and healthy qi reinforcement to expel pathogenic factors.
4.Obstructive sleep apnea-hypopnea syndrome: risk of complicated coronary heart disease and construction of its prediction model

Objective To explore the independent risk factors for complicated coronary heart disease (CHD) in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS) and to construct a risk prediction model. Methods A retrospective study was performed to analyze the clinical data of 308 OSAHS patients who visited Jiangsu Provincial People's Hospital from January 2021 to June 2025. The patients were divided into the CHD group (n=80) and non-CHD group (n=228) according to the presence or absence of complicated CHD. Independent risk factors were identified using univariate analysis and multivariate logistic regression, and a nomogram prediction model was constructed. The model was evaluated using the calibration curve, Hosmer-Lemeshow test, Decision Curve Analysis (DCA) curve, and Receiver Operating Characteristic (ROC) curve. Results The incidence of CHD in OSAHS patients was 25.97%. Age, history of hypertension, oxygen desaturation index (ODI), severe polysomnography (PSG) grade, decreased high-density lipoprotein cholesterol, elevated homocysteine, and elevated troponin I were independent risk factors for OSAHS complicated with CHD (P<0.05). The nomogram model demonstrated good calibration (Hosmer-Lemeshow test, P=0.995), and the DCA curve showed high clinical net benefit. The area under the ROC curve was 0.940 (95% CI: 0.908~0.972), with a sensitivity of 0.842 and a specificity of 0.900. Conclusion Age, history of hypertension, ODI, PSG grade, high-density lipoprotein cholesterol, homocysteine, and troponin I are independent risk factors for CHD in OSAHS patients. The prediction model constructed based on these factors can provide a reliable tool for clinical nurses to early identify high-risk individuals and formulate targeted intervention strategies.
5.Mechanism of Qishen Yiqi Dropping Pills in regulating gut microbiota and ROS/TXNIP/NLRP3 signaling pathway to improve chronic heart failure in rats

Objective:
This study explored the regulatory effects of QiShen Yiqi Dropping Pills (QSYQ) on chronic heart failure (CHF) in rats and their related mechanisms based on the gut microbiota and reactive oxygen species (ROS)/thioredoxin interacting protein (TXNIP)/NOD-like receptor protein 3 (NLRP3) signaling pathway.
Methods:
Sixty-five SPF-grade male SD rats were used to establish a CHF model through subcutaneous multiple injections of isoproterenol (ISO) combined with exhaustion and food control methods. The modeled rats were randomly divided into model, captopril (5.30 mg/kg), and QSYQ low-, medium-, and high-dose groups (0.08, 0.16, and 0.32 g/kg, respectively), with 11 rats per group, plus a blank group of seven rats. The medication groups were given corresponding drugs by gavage, whereas the blank and model groups were administered an equivalent volume of purified water continuously for four weeks. Rat heart function was assessed via transthoracic echocardiography, and myocardial tissue pathology changes were observed through hematoxylin and eosin staining. Serum levels of brain natriuretic peptide (BNP), lipopolysaccharide (LPS), interleukin-18 (IL-18), and interleukin-1β (IL-1β) were measured using an enzyme-linked immunosorbent assay. Automated biochemical analyzers were used to determine creatine kinase (CK), lactate dehydrogenase (LDH), and MB isoenzyme of creatine kinase (CK-MB) content. Myocardial ROS levels were examined using flow cytometry; myocardial TXNIP and NLRP3 expression were detected using immunohistochemistry. Real-time qPCR and Western blotting were used to examine myocardial mRNA and protein expression of TXNIP, NLRP3, apoptosis-related spot-like protein (ASC), caspase-1, and IL-1β, as well as myocardial thioredoxin (Trx) and colonic tight junction proteins (zonula occludens-1, ZO-1), occludin, and claudin-5. Differences in the gut microbiota of the blank, model, and QSYQ high-dose groups were determined using high-throughput 16S rDNA sequencing.
Results:
Compared to the blank group, the model group exhibited significantly reduced left ventricular ejection fraction (LVEF) and left ventricular fraction shortening (LVFS) (P<0.01); increased serum BNP, LPS, IL-18, and IL-1β (P<0.01) levels; increased CK, LDH, and CK-MB (P<0.01) contents; visible myocardial tissue fibrous edema, wavy appearance, cytoplasmic loosening, round vacuolar degeneration, local tissue fibrous dissolution replaced by proliferative connective tissue, accompanied by inflammatory cell infiltration; significantly increased myocardial ROS levels (P<0.01); and significantly increased myocardial TXNIP and NLRP3 expression (P<0.01). TXNIP, NLRP3, ASC, caspase-1, and IL-1β mRNA and protein expression were significantly increased (P<0.05, P<0.01, respectively), whereas Trx, ZO-1, occludin, and claudin-5 expression was significantly decreased (P<0.01). Compared to the model group, the QSYQ high-dose group showed the most significant changes (P<0.05, P<0.01), with significant increases in LVEF and LVFS (P<0.01); significant decreases in serum BNP, LPS, IL-18, and IL-1β levels (P<0.01); significant reductions in CK, LDH, and CK-MB content (P<0.01); improved myocardial tissue damage; significantly decreased myocardial ROS levels (P<0.01); and significantly reduced myocardial TXNIP and NLRP3 expression (P<0.01). TXNIP, NLRP3, ASC, caspase-1, and IL-1β mRNA and protein expression were significantly decreased (P<0.05, P<0.01), whereas Trx, ZO-1, occludin, and claudin-5 expression was significantly increased (P<0.01). 16S rDNA sequencing results confirmed that the gut microbiota of rats changed after modeling and drug intervention, with significant differences in both α- and β-diversity. Compared to the blank group, at the family level, the abundance of Oscillospiraceae decreased (P<0.05), whereas the abundance of Lactobacillaceae increased. At the species level, the abundance of Segatella copri and Treponema succinifaciens increased, whereas the abundance of Kineothrix alysoides (P<0.05), Ruminococcus callidus, and Prevotellamassilia timonensis decreased. Compared to the model group, at the family level, the abundance of Oscillospiraceae increased (P<0.05) in the QSYQ high-dose group, whereas the abundance of Lactobacillaceae decreased. At the species level, the abundance of Segatella copri and Treponema succinifaciens decreased, whereas the abundance of Kineothrix alysoides increased (P<0.05).
Conclusion
QSYQ can regulate the relative abundance of symbiotic bacteria Kineothrix alysoides in the intestines, reduce serum LPS levels, inhibit the ROS/TXNIP/NLRP3 signaling pathway, and improve inflammatory responses, thereby exerting therapeutic effects on CHF.
6.First imported case of kala-azar from other provinces reported in Changning District of Shanghai

ObjectiveTo explore the diagnostic-therapeutic course and epidemiological characteristics of the first imported kala-azar case reported in Changning District of Shanghai, so as to provide references for early detection, timely diagnosis, and effective management of the kala-azar cases in non-epidemic areas. MethodsIn 2025, a comprehensive epidemiological investigation and a complete collection of clinical records of one kala-azar patient imported from other provinces reported by a medical institution in this city were conducted. The source of infection, process of diagnosis and treatment, and clinical outcome of the case were systematically analyzed. ResultsThe case was a male migrant worker from Shanxi Province currently residing in Shanghai. In June 2023, the patient had a history of exposure to mosquito bites during his stay in a kala-azar endemic area. The initial onset occurred in December 2023, with clinical manifestations including unexplained fever accompanied by leukopenia, and subsequently gradually developed night sweats and fatigue during the following months. In June 2024, the patient self-perceived significant weight loss, and physical examination revealed hepatomegaly and splenomegaly, leading to a diagnosis of hemophagocytic syndrome (HPS). Throughout the whole course of the illness, the patient sought treatment at hematology departments of several hospitals multiple times, where he was managed as a case of HPS. In January 2025, a large number of Leishmania donovani were detected in the patient's bone marrow smear, confirming kala-azar. After 2 weeks of standardized treatment with amphotericin B liposomal, there was significant improvement. Combined with the epidemiological investigation, it was speculated that the incubation period of this case was about 6 months, whereas the interval from initial onset to definite diagnosis was approximately 14 months. ConclusionKala-azar has atypical early clinical symptoms, which are prone to confusion with HPS. For patients with recurrent unexplained fever accompanied by peripheral cytopenias, hepatomegaly and splenomegaly, medical institutions should consider the possibility of infectious diseases. Close collaboration between medical institutions and centers for disease control and prevention is essential to conduct a detailed epidemiological investigation and laboratory screening, ensuring confirmed cases receive standardized treatment.
7.Application of progressive exercise training based on mMRC grading in respiratory rehabilitation for patients with chronic obstructive pulmonary disease in a primary healthcare setting

Objective To explore the efficacy of progressive exercise training based on the modified Medical Research Council dyspnea scale (mMRC) grading in respiratory rehabilitation for patients with chronic obstructive pulmonary disease (COPD) at a primary healthcare setting. Methods A total of 106 patients with COPD admitted to Zhuanqiao Community Health Service Center in Shanghai from Aug.1, 2022 to Jul. 30, 2024 were selected as research subjects. They were randomly divided into a study group and a control group in a 1∶1 ratio, with 53 patients in each group. The control group received conventional treatment, while the study group received conventional treatment combined with progressive exercise training. After 4 weeks of continuous treatment, the changes in the 6-minute walk test (6MWT), COPD assessment test (CAT) score, mMRC grading, Global Initiative for Chronic Obstructive Lung Disease (GOLD) grading and pulmonary function were compared between the two groups. Results Patients in both groups showed improvements in 6MWT distance, CAT score, mMRC grading, GOLD grading, and pulmonary function compared to baseline (P<0.05). Moreover, the study group had better improvements in 6MWT distance, CAT score, mMRC grading, GOLD grading, and pulmonary function than the control group (P<0.05). Conclusions Conventional treatment combined with progressive exercise training based on mMRC grading can enhance the effect of respiratory rehabilitation in patients with COPD, particularly in improving pulmonary function and exercise tolerance.
8.Analysis of the timeliness of anti-retroviral therapy among newly reported HIV/AIDS cases

Objective:
To analyze the timeliness of antir-etroviral therapy (ART) and its influencing factors among newly reported HIV/AIDS cases in Wenzhou City, Zhejiang Province from 2016 to 2023, so as to provide a reference for improving the ART effect of HIV/AIDS cases.
Methods:
Newly reported HIV/AIDS cases in Wenzhou City from 2016 to 2023 were selected as the research subjects. Demographic information, the situation of the first CD4+ T lymphocyte (CD4 cell) test, baseline CD4 cell count, and ART situation were collected through the Chinese Disease Prevention and Control Information System. The timely rate of ART was analyzed, and the influencing factors for timely ART among HIV/AIDS cases were analyzed using a multivariable logistic regression model.
Results:
A total of 4 500 newly reported HIV/AIDS cases in Wenzhou City from 2016 to 2023 were included, among which 3 679 were males, accounting for 81.76%, and 821 were females, accounting for 18.24%. The median age was 46.24 (interquartile range, 26.23) years. Among these cases, 3 606 received timely ART, with a timely rate of 80.13%. The timely rate of ART increased from 57.54% in 2016 to 91.97% in 2023 (P<0.05). Multivariable logistic regression analysis showed that unmarried/divorced/widowed (OR=0.769, 95%CI: 0.641-0.922), detainees (OR=0.492, 95%CI: 0.269-0.900), untimely first CD4 cell test (OR=0.278, 95%CI: 0.234-0.330), baseline CD4 cell count ≥200 cells/µL (OR=0.709, 95%CI: 0.595-0.843) or undetected (OR=0.131, 95%CI: 0.080-0.213) were associated with a lower timeliness for ART among HIV/AIDS cases.
Conclusion
From 2016 to 2023, the timely rate of ART among newly reported HIV/AIDS cases in Wenzhou City showed an upward trend, which was mainly affected by marital status, case source, timeliness of the first CD4 cell test, and baseline CD4 cell count.
9.Epidemiological characteristics of hepatitis C in Wenzhou City from 2005 to 2023

Objective:
To analyze the epidemiological characteristics of hepatitis C in Wenzhou City, Zhejiang Province from 2005 to 2023, so as to provide the evidence for improving hepatitis C prevention and control measures.
Methods:
The data of hepatitis C cases in Wenzhou City from 2005 to 2023 were collected from the China Disease Prevention and Control Information System. Descriptive epidemiological methods were used to describe the incidence of hepatitis C by gender and age in Wenzhou City from 2005 to 2023. Average annual percent change (AAPC) was used to analyze the trend of hepatitis C incidence.
Results:
A total of 15 707 hepatitis C cases were reported in Wenzhou City from 2005 to 2023, with an average annual reported incidence of 9.17/100 000. The reported incidence showed an overall upward trend (AAPC=2.401%, P<0.05). In terms of occupation, farmers/migrant workers and workers accounted for 34.26% (5 382 cases), followed by domestic workers, those engaged in housework and unemployed people, accounting for 19.39% (3 045 cases). Lucheng District, Pingyang County, and Yongjia County had relatively high average annual reported incidences of hepatitis C, which were 18.12/100 000, 13.53/100 000 and 9.88/100 000, respectively. There were 10 816 males and 4 891 females, with a male-to-female ratio of 2.21∶1. The average annual reported incidence of hepatitis C in males was higher than in females (12.06/100 000 vs. 6.00/100 000, P<0.05). The reported incidence of hepatitis C in males showed an overall upward trend (AAPC=3.011%, P<0.05), but there was no significant change in females (AAPC=1.803%, P>0.05). The case was mainly aged 25-<55 years (12 659 cases, 80.59%). There was no significant change in incidence of hepatitis C in 0-<10 years old group and 10-<20 years old group (AAPC=2.103%, 1.126%, both P>0.05), while the incidence of hepatitis C in 20-<30 years old group and 30-<40 years old group showed downward trends (AAPC=>-13.160%, -7.159%, both P<0.05). There were upward trends in the 40-<50 years old group and 50-<60 years old group (AAPC=10.457%, 11.831%, both P<0.05), but no significant change in the ≥60 years old group (AAPC=2.867%, P>0.05).
Conclusions
The reported incidence of hepatitis C in Wenzhou City showed an overall upward trend from 2005 to 2023. Lucheng District, Pingyang County, and Yongjia County were high risk areas. Farmers/migrant workers, workers, males, and people aged 40-<60 years were the key population for prevention and control of hepatitis C.
10.Trends in incidence and mortality of esophageal cancer in cancer registration areas of Anhui Province from 2014 to 2020

Objective:
To investigate the trends in incidence and mortality of esophageal cancer in cancer registration areas of Anhui Province from 2014 to 2020, so as to provide the basis for formulating prevention and control measures.
Methods:
The incidence and mortality data of esophageal cancer in Anhui Province from 2014 to 2020 was collected through the Cancer Registry in Anhui Province. The crude incidence and crude mortality were calculated. The Chinese population-standardized rate was standardized using the age structure of the standard population from the Fifth National Population Census in 2000. The trends in incidence and mortality of esophageal cancer were analyzed using the average annual percent change (APPC), stratified by genders, urban/rural areas, and ages.
Results:
In Anhui Province, the rank of esophageal cancer incidence dropped from the third in 2014 to the sixth in 2020. Concurrently, the crude incidence and Chinese population-standardized incidence declined from 28.74/100 000 and 20.74/100 000 to 19.23/100 000 and 10.59/100 000, respectively (AAPC=-5.846%, -9.658%, both P<0.05). The mortality rank remained stable at the fourth in 2014 and 2020, while the crude mortality and Chinese population-standardized mortality decreased from 19.96/100 000 and 14.09/100 000 to 16.00/100 000 and 8.41/100 000, respectively (AAPC=-3.542%, -7.784%, both P<0.05). The Chinese population-standardized incidence (AAPC=-9.682%, -9.188%, -6.175% and -12.575%, all P<0.05) and Chinese population-standardized mortality (AAPC=-7.734%. -7.447%. -5.366% and -10.209%, all P<0.05) showed declining trends in males, females, urban, and rural areas, respectively. From 2014 to 2020 in Anhui Province, the crude incidence and mortality of esophageal cancer generally increased with age. However, significant declining trends were observed in crude incidence (AAPC=-12.779%, -11.701%, -11.955% and -5.751%, all P<0.05) and crude mortality (AAPC=-12.255%, -11.120%, -10.985% and -5.751%, all P<0.05) for the age groups of 40-<50, 50-<60, 60-<70, 70-<80 years. A significant declining trend in crude incidence was also seen in the ≥80 years group (APPC=-6.334%, P<0.05), but the trend in crude mortality was no statistically significant (P>0.05).
Conclusion
In registration areas of Anhui Province, the incidence and mortality of esophageal cancer exhibited a declining trend from 2014 to 2020, calling for focused attention on the middle-aged and elderly population and enhanced health behaviors such as tobacco and alcohol control.
