1.Distribution characteristics of pathogens and influencing factors analysis of infections within 90 days after liver transplantation

Objective To investigate the distribution characteristics of pathogens causing infections within 90 days after liver transplantation and the influencing factors of infection. Methods Clinical data of 176 recipients who underwent liver transplantation at the Liver Transplant Center of Beijing Friendship Hospital Affiliated to Capital Medical University from September 2021 to August 2024 were retrospectively analyzed. Patients were divided into the infection group (n=124) and the non-infection group (n=52) based on whether they developed infection within 90 days after transplantation. The distribution characteristics of pathogens in infected patients were analyzed. Univariate and multivariate logistic regression analyses were used to explore the influencing factors of infection. Results Among the 176 liver transplant recipients, 124 cases developed 243 episodes of 518 bacterial, fungal, viral or mycoplasma infections within 90 days after transplantation, with an overall infection rate of 70.5% (124/176). The composition of pathogens was mainly Gram-negative bacteria (38.6%, 200/518), followed by Gram-positive bacteria (32.2%, 167/518) and viruses (15.4%, 80/518), and fungi accounted for 13.1% (68/518). Among Gram-negative bacteria, the main pathogen was Klebsiella pneumoniae (6.8%, 35/518), and among Gram-positive bacteria, the main pathogen was Enterococcus faecalis (8.5%, 44/518). Viruses included Epstein-Barr virus (3.7%, 19/518) and cytomegalovirus (3.7%, 19/518), and fungi were mainly Candida albicans (6.8%, 35/518). The most common infection site among the 243 episodes was pulmonary infection (42.0%, 102/243), followed by abdominal infection (22.6%, 55/243) and bloodstream infection (18.1%, 44/243). The infections mainly occurred within 2 weeks after transplantation (60.9%, 148/243). Multivariate logistic regression analysis indicated that preoperative infection within 2 weeks, a high preoperative model for end-stage liver disease (MELD) score, and preoperative sarcopenia were independent risk factors for infection within 90 days after liver transplantation (all odds ratio>1, P<0.05). After multivariate correction, the levels of CD4+T cells and CD8+T cells within 90 days after surgery were independently associated with the occurrence of infection. Low levels of CD4+T cells and CD8+T cells might be related to an increased risk of infection. Conclusions The infection rate after liver transplantation is high, and the pathogens are mainly Gram-negative bacteria. The lungs are the most common infection site. Preoperative MELD score, preoperative sarcopenia and preoperative infection within 2 weeks are independent risk factors for infection within 90 days after liver transplantation. Regular monitoring of immune indicators CD4+T cells and CD8+T cells levels after transplantation is helpful to reduce the occurrence of post-transplantation infection.
2.Theoretical Exploration of "Treat Heat with Cold" and "Disperse Fire Constraint" and Clinical Application in Acupuncture

The connotation of "treat heat with cold" and "disperse fire constraint" differs, and under the guidance of these two principles, there are variations in the selection of acupoints, choice of needles, and manipulation techniques in clinical acupuncture. "Treat heat with cold" is used to clear and drain heat pathogens, while "disperse fire constraint" focuses on diffusing and dissipating fire from constraint. When both fire constraint and internal heat coexist, treatment should combine both "dispersing the fire" and "cooling the heat". In clinical acupuncture, for clearing heat and draining heat pathogens, acupoints on Governor vessel (督脉) and Yangming (阳明经) channel are commonly selected, with Jing-well points, Ying-spring points and He-sea points. To diffuse and dissipate fire constraint, acupoints on the Taiyang (太阳) and Shaoyang (少阳) channels are usually chosen, with Jing-river points and Ashi points as the primary options. In terms of needle choice, for heat syndrome, filiform needles, three-edged needles, and cupping are often used for clearing heat and resolving toxins; for fire constraint syndrome, moxibustion and fire needles are employed to expel fire from the body. Regarding manipulation techniques, draining method can be used in both conditions, with heat syndrome requiring shallow needling and quick removal, while fire constraint syndrome requiring deeper and longer retention of needles, having an emphasis on hand pressure coordination and smooth qi flow. In clinical practice, it is essential to distinguish the presence of "constraint" and the depth of the disease location to appropriately combine "dispersing fire" and "cooling heat".
3.Study on quality markers of Hyssopus cuspidatus against airway remodeling in bronchial asthma

OBJECTIVE To identify the quality markers (Q-Markers) of Hyssopus cuspidatus against airway remodeling in bronchial asthma (referred to as “asthma”), an d to provide a reference for the quality control research of H. cuspidatus based on pharmacodynamic activity. METHODS Potential active components and action targets of H. cuspidatus were screened by network pharmacology method. Using human airway smooth muscle cells (HASMCs) as objects, airway remodeling cell model was induced by platelet-derived growth factor-BB (PDGF-BB); validation test was then performed for anti-asthmatic effects of H. cuspidatus and the potential active components. HPLC method was employed to establish the fingerprints of 14 batches of H. cuspidatus samples, and chemometric analysis was also conducted. Combined with the results of pharmacodynamic experiments and fingerprint analysis, the Q-Markers of H. cuspidatus against airway remodeling in asthma were determined. RESULTS&CONCLUSIONS Network pharmacology analysis showed that the potential active components of H. cuspidatus against asthma might be flavonoids and phenolic acids such as luteolin, quercetin and rosmarinic acid, and the core anti-asthmatic targets were interleukin-6, mitogen-activated protein kinase, etc. In vitro experimental results confirmed that 25, 50, 100 μg/mL of H. cuspidatus , as well as neochlorogenic acid (80 μmol/L), acacetin (80 μmol/L), salvianolic acid B (40 μmol/L) and quercetin-3- O - β -D-glucuronide (80 μmol/L), significantly reduced the cell viability induced by PDGF-BB, inhibited cell proliferation, migration, and the phosphorylation level of extracellular signal-regulated protein kinase 1/2, decreased the levels of interleukin-6, tumor necrosis factor-α and reactive oxygen species, and generally arrested cells in the G 0 /G 1 phase ( P <0.05). Fingerprint analysis showed that there were 27 common peaks in the fingerprints of the 14 batches of H. cuspidatus samples, with 15 compounds (including luteolin) identified, and the similarities of fingerprints were all greater than 0.8. The 14 batches of samples could be divided into three categories: S1-S7 as one category, S8-S13 as one category, and S14 as one category. The variable importance in the projection values of rosmarinic acid, chlorogenic acid, caffeic acid, salvigenin, luteolin, ferulic acid, quercetin-3- O - β -D-glucuronide, and the components corresponding to peaks 5 and 8 were greater than 1, indicating they were potential differential markers affecting quality. Integrating network pharmacology, in vitro experimental validation, and chemometric analysis, rosmarinic acid, neochlorogenic acid, caffeic acid, salvigenin, luteolin, ferulic acid, quercetin-3- O - β -D-glucuronide, acacetin, salvianolic acid B and chlorogenic acid may be the Q-Markers of H. cuspidatus against asthma.
4.Analyses of risk factors for mortality in patients with Klebsiella pneumoniae bloodstream infection at a tertiary hospital in Hangzhou from 2018 to 2024

ObjectiveTo explore the 28-day mortality risk of Klebsiella pneumoniae (KP) bloodstream infections (BSI) and its related influencing factors, thereby providing a scientific basis for the effective control of KP-BSI and improvement of patient prognosis. MethodsFrom January 2018 to December 2024, a retrospective review was conducted on hospitalized patients aged >18 years old treated for KP-BSI at a tertiary hospital in Hangzhou. Logistic and Cox regression analyses were performed to identify the epidemiological characteristics and the risk factors for the 28-day mortality associated with KP-BSI. ResultsA total of 123 patients with KP-BSI were included in this study, comprising 64 cases infected with carbapenem-resistant Klebsiella pneumoniae (CRKP) and 59 cases infected with carbapenem-susceptible Klebsiella pneumoniae (CSKP). Compared with CSKP-BSI, patients with CRKP-BSI more frequently presented with chronic pulmonary disease (χ²=4.29, P=0.038), concomitant infections at other sites (χ²=10.90, P=0.001), and a higher frequency of invasive procedures prior to infection (central venous catheterization, mechanical ventilation, and indwelling urinary catheter), as well as glucocorticoid use, hemodialysis, and blood transfusion (all P<0.05). The 28-day mortality was significantly higher in BSI cases caused by CRKP compared to that caused by CSKP (37.50% vs 5.08%, P<0.001). Cox regression analyses revealed that carbapenem resistance (HR=6.67, 95%CI: 1.48‒30.08, P=0.014) and blood transfusion (HR=3.58, 95%CI: 1.15‒11.19, P=0.028) were risk factors for the 28-day mortality in KP-BSI, while removal of central venous catheters after infection (HR=0.24, 95%CI: 0.08‒0.67, P=0.006) was associated with a reduced risk for the 28-day mortality. ConclusionCarbapenem resistance is associated with mortality outcomes in patients with KP-BSI. Strengthening infection control measures targeting the identified risk factors for CRKP-BSI may improve patient prognosis.
5.Study on quality control of Inula salsoloides from different producing areas in Xinjiang

OBJECTIVE To control the quality of Inula salsoloides from different producing areas in Xinjiang. METHODS The macroscopic and microscopic identification of medicinal materials was performed using conventional morphological and microscopic observation methods. Thin-layer chromatography(TLC)was applied for the qualitative identification of chlorogenic acid in the medicinal materials. In ac cordance with the General Principles in Volume Ⅳ of the 2025 edition of the Chinese Pharmacopoeia , the contents of water, total ash, acid-insoluble ash, and extractives of the samples were determined. High-performance liquid chromatography was used to measure the contents of chlorogenic acid, isoquercitrin, and quercetin. Taking water, total ash, acid-insoluble ash, extractives, and the contents of chlorogenic acid, isoquercitrin, and quercetin as evaluation indices, the entropy weight-TOPSIS method was adopted to comprehensively evaluate the quality of 18 batches of samples. RESULTS I. salsoloides from Xinjiang exhibited distinct macroscopic and microscopic characteristics, and the qualitative TLC identification results for chlorogenic acid were clear and reliable. The water content ranged from 6.20% to 7.23%, the total ash content from 12.27% to 15.80%, the acid-insoluble ash content from 1.89% to 2.57%, and the extractive contents from 21.24% to 28.09%. The contents of chlorogenic acid, isoquercitrin, and quercetin were 0.198 5 to 2.335 0 mg/g, 0.014 5 to 0.402 8 mg/g, and 0.016 5 to 0.578 7 mg/g, respectively. The comprehensive closeness degrees of 18 batches of samples were 0.106 to 0.673. CONCLUSIONS The established quality control method is stable and reliable, which can be applied to the quality control of I.salsoloides , there are significant differences in the quality of medicinal materials from different producing areas.

Effective symptom management can alleviate the physical and psychological distress experienced by patients with cancer, improve quality of life, and contribute to treatment adherence and improve clinical outcomes. However, most existing guidelines are developed for healthcare professionals, and patients and the public have limited access to standardized and comprehensible guidance on symptom management. To address this gap and to facilitate effective communication and shared decision-making, this study proposes the development of a patient version of the cancer symptom management guideline. The development process will adhere to the methodological framework recommended by the Guidelines International Network and the World Health Organization. The GRADE approach will be employed to assess the certainty of evidence and to formulate recommendations. In addition, the process will be informed by the Appraisal of Guidelines for Research and Evaluation Ⅱ (AGREE Ⅱ) instrument and the Reporting Items for Practice Guidelines in Healthcare-Public or Patient Versions of Guidelines (RIGHT-PVG). This protocol outlines the establishment of the guideline working group, the identification and prioritization of key questions, evidence retrieval and appraisal, and the formulation of recommendations, with the aim of ensuring methodological rigor and transparency in the development of the patient guideline and providing methodological reference for similar guideline initiatives.
7.Application effect of dual-track nursing intervention in children with lobar pneumonia

Objective To explore the application effect of the dual-track nursing intervention model in the treatment process of children with lobar pneumonia.Methods A total of 186 children with lobar pneumonia were selected and randomly divided into control group and intervention group u-sing a double-blind method,with 93 cases in each group.The control group received conventional nursing intervention,while the intervention group implemented the dual-track nursing intervention model on the basis of conventional nursing.This model included the establishment and training of nurs-ing teams,personalized nursing plans,health education,and psychological support.Outside the hos-pital,it emphasized family support,regular follow-up guidance,and community-based collaborative ed-ucation.Both groups received a 3-week intervention.The improvement times of clinical symptoms,hos-pital stay,pulmonary function indicators before and after nursing,treatment compliance,and family members' satisfaction with nursing were compared and analyzed between the two groups.Results The fever resolution time[(3.89±0.96)d],cough relief time[(6.21±1.34)d],disappearance time of pulmonary rales[(7.89±1.56)d],and hospital stay duration[(9.45±1.89)d]in the intervention group were all shorter than those in the control group[(5.23±1.14),(7.45±1.67),(9.32±2.01),and(11.28±2.35)d,respectively],with statistically significant differences(P<0.05).After nursing,the forced expiratory volume in one second(FEV1)[(1.51±0.22)L],forced vital capacity(FVC)[(1.75±0.25)L],and FEV1/FVC[(94.12±5.65)%]in the intervention group were all higher than those in the control group[(1.42±0.15)L,(1.66±0.22)L,and(85.73±8.41)%,respectively],with statistically significant differences(P<0.05).The scores for exami-nation cooperation[(23.91±3.82)points],nursing cooperation[(24.19±4.03)points],standardized medication use[(24.26±3.94)points],and rational diet[(23.77±3.62)points]in the intervention group were higher than those in the control group[(20.16±3.53),(19.64±3.46),(23.05±3.68),and(18.85±3.41)points,respectively],with statistically significant differences(P<0.05).The satisfaction rate of family members with nursing work in the intervention group was higher than that in the control group,with a statistically significant difference(98.92%versus 89.25%,P<0.05).Conclusion The dual-track nursing intervention model has a signifi-cant application effect in children with lobar pneumonia.It can accelerate their recovery process,improve treatment compliance,promote pulmonary function improvement,and enhance family mem-bers' satisfaction.
8.Role of insulin-like growth factor binding protein 5 in cardiac remodeling after acute myocardial infarction

Objective:To investigate the role of insulin-like growth factor binding protein 5 (IGFBP5) in cardiac remodeling following acute myocardial infarction (AMI).Methods:In vivo experiments, a mouse model of AMI was established and randomly divided into three groups: sham-operation group, early AMI group (day 3), and late AMI group (day 28). Western blotting was used to detect the expression of fibrosis-related proteins and IGFBP5 in myocardial tissue. Immunofluorescence staining was performed to assess changes in IGFBP5 expression in the infarcted area at days 3 and 28. Serum IGFBP5 levels were measured using ELISA. In vivo experiments, IGFBP5 expression was silenced in mouse cardiac fibroblasts using siRNA, and the effect on transforming growth factor-β (TGF-β)-induced myofibroblast transdifferentiation was evaluated. One-way ANOVA was used for statistical comparisons among multiple groups.Results:On day 28 after AMI, IGFBP5 expression in myocardial tissue and serum was significantly elevated and positively correlated with the expression of fibrosis markers, including Fibronectin, Periostin, and α-SMA ( P<0.01). Both mouse serum analysis and immunofluorescence staining of heart tissue sections showed that the expression level of IGFBP5 was significantly increased in the late stage of AMI compared to baseline( P<0.001), indicating its potential as a prognostic marker for myocardial infarctionIn vitro, silencing IGFBP5 expression inhibited TGF β-induced fibroblast transdifferentiation and reduced the expression of fibrosis-related proteins ( P<0.05). Conclusion:IGFBP5 may play a role in the progression of AMI and serve as both a potential therapeutic target and a prognostic biomarker.
9.Sivelestat sodium alleviates paraquat-induced pulmonary fibrosis by inhibiting the Nlrp3-inflammasome pathway

Objective:To investigate whether sivelestat sodium (SV) mitigates paraquat (PQ)-induced acute lung injury (ALI)/acute respiratory distress syndrome (ARDS)-associated pulmonary fibrosis in mice by suppressing the NLRP3 inflammasome signaling pathway.Methods:Male C57BL/6J mice were randomly divided into four groups ( n=8 per group): Control group, PQ group, PQ+SV group, and SV group. The PQ and PQ+SV groups received an intraperitoneal injection of PQ solution (20 mg/kg) to establish a PQ poisoning model, while the Control and SV groups received an equivalent volume of saline. One hour later, the PQ+SV and SV groups were administered SV solution (100 mg/kg) intraperitoneally, whereas the Control and PQ groups received saline. After 48 hours, the mice were euthanized, and lung tissues were collected. Pathological changes were assessed via hematoxylin-eosin (HE) and Masson staining, followed by Smith and Ashcroft scoring. Immunohistochemistry was performed to evaluate the expression of NLRP3 inflammasome, caspase-1, α-smooth muscle actin (α-SMA), and collagen I. Western blotting was used to measure NLRP3 protein levels. Intergroup comparisons were conducted using one-way ANOVA with LSD post-hoc correction. Results:The Control and SV groups exhibited normal lung morphology, whereas the PQ+SV group showed reduced hemorrhage, congestion, and edema compared to the PQ group. Both PQ and PQ+SV groups exhibited significant weight loss post-intervention compared to the Control group (both P<0.001). HE and Masson staining revealed thickened alveolar septa, congestive and edematous alveolar walls, extensive inflammatory cell infiltration, and collagen deposition in the PQ group. In contrast, the PQ+SV group demonstrated alleviated alveolar wall congestion, reduced inflammatory infiltration, and decreased collagen deposition, with significantly lower Smith and Ashcroft scores [(5.92±1.34) vs. (10.88±1.88), P<0.001; (3.42±1.35) vs. (5.75±0.79), P<0.001]. Immunohistochemistry indicated reduced expression percentages of NLRP3 and caspase-1 in the PQ+SV group compared to the PQ group [(12.79%±0.43%) vs. (16.59%±0.40%), P<0.001; (17.71%±0.92%) vs. (19.84%±0.71%), P<0.001]. Similarly, α-SMA and collagen I expression in lung interstitium was significantly lower in the PQ+SV group [(11.79%±0.58%) vs. (16.14%±0.74%), P<0.001; (16.43%±0.56%) vs. (18.86%±0.60%), P<0.001]. Western blotting confirmed decreased NLRP3 protein expression in the PQ+SV group [(0.54±0.12) vs. (0.81±0.24), P<0.05]. Conclusions:SV attenuates PQ-induced ALI/ARDS-associated pulmonary fibrosis progression by inhibiting the NLRP3 inflammasome, suggesting that NLRP3 may be a key therapeutic target for early intervention in PQ-induced pulmonary fibrosis.
10.Prediction analysis of the number of pre-hospital emergency ambulance trips in Handan based on the LPro Ensemble Model

Objective:To investigate the application of time series models in forecasting pre-hospital emergency ambulance trips in Handan City and develop the LPro ensemble model for improved prediction accuracy to support emergency resource allocation.Methods:Pre-hospital emergency data from Handan Emergency Medical Command Center (2019-2023) were retrospectively analyzed. From 324 799 original records, 289 949 valid records were included after cleaning. The training set (2019-2022: 215 918 records) included 35 527 records in 2019, 52 015 in 2020, 61 836 in 2021, and 66 540 in 2022. The validation set (2023) contained 74 031 records. ARIMA, linear trend seasonal, exponential smoothing, and Prophet models were fitted to the training set. The LPro ensemble model was constructed using MAPE-based weighting (linear trend seasonal model: 0.38, Prophet: 0.62). Performance metrics included MAPE, RMSE, MAE, and R 2. Results:Data showed annual growth (compound annual growth rate 23.27%) and seasonal patterns (October peaks, February troughs). Ambulance dispatches increased annually with monthly cyclical patterns. For 2023 validation predictions: ARIMA (MAPE 8.76%, RMSE 619, MAE 491, R 2 0.4563), linear trend seasonal (MAPE 9.83%, RMSE 671, MAE 545, R 2 0.3608), Prophet (MAPE 8.43%, RMSE 562, MAE 503, R 2 0.5513), exponential smoothing (MAPE 8.08%, RMSE 643, MAE 410, R 2 0.4124). LPro model showed superior performance (MAPE 7.05%, RMSE 491, MAE 393, R 2 0.6570), with 16.37% lower MAPE, 12.63% lower RMSE, 21.87% lower MAE, and 19.17% higher R 2 versus Prophet. Conclusion:The LPro ensemble model substantially enhances prediction accuracy and reliability, offering scientific support for emergency resource optimization and dispatch scheduling in Handan City.
