1.Analysis of CVAI, TyG index and risk factors of hypertension among elderly people in Zhengxiangbai Banner community of Inner Mongolia

Objective:To evaluate the effects of the Chinese visceral adiposity index(CVAI)and the Triglyceride-Glucose(TyG)index on the risk of hypertension among the elderly in the Zhengxiangbai Banner community of Inner Mongolia.Methods:This study selected elderly individuals who underwent physical examinations at the community health service center in Zhengxiangbai Banner, Inner Mongolia, from January 2024 to July 2024 as the study population.Based on the diagnostic criteria for hypertension, participants were divided into hypertensive and non-hypertensive groups.Data on demographics, body mass index(BMI), waist circumference(WC), fasting blood glucose(FSG), and blood lipids were collected.The CVAI, TyG, Triglyceride Glucose-Body Waist Circumference(TyG-WC), and Triglyceride Glucose-Body Mass Index(TyG-BMI)were calculated.Subjects were categorized into quartiles(1st to 4th)based on CVAI, TyG, TyG-WC, and TyG-BMI.Multivariate logistic regression analysis was employed to explore the relationship between these indicators and the risk of hypertension in the elderly.Results:This study included a total of 3, 319 elderly subjects, comprising 1, 743 individuals in the hypertensive group and 1, 576 in the non-hypertensive group.Multivariate logistic regression analysis indicated that the fourth quartile of the CVAI was a risk factor for hypertension when compared to the first quartile.Additionally, the second, third, and fourth quartiles of the TyG-BMI were identified as risk factors for hypertension relative to the first quartile.The analysis of the correlation strength between various obesity indexes and hypertension revealed that CVAI, TyG-WC, and TyG-BMI were independently correlated with the risk of hypertension.Notably, TyG-BMI exhibited a stronger association with hypertension( OR=1.006, 95% CI: 1.004-1.008).ROC curve analysis demonstrated that TyG-BMI possessed superior predictive ability for hypertension, with an AUC of 0.587(95% CI: 0.568-0.607).Furthermore, Decision Curve Analysis(DCA)also indicated that TyG-BMI had enhanced predictive capability.Further stratification by gender revealed that TyG-BMI in elderly men was independently correlated with the risk of hypertension( P<0.001), while CVAI in elderly women was independently correlated with the risk of hypertension( P<0.05).ROC curve analysis indicated that TyG-BMI in elderly men had better predictive ability for hypertension(AUC=0.582, 95% CI: 0.554~0.610).The difference in AUC compared to CVAI was statistically significant( P<0.01).DCA analysis further confirmed that TyG-BMI exhibited superior predictive ability. Conclusions:Our findings suggest that both CVAI and TyG-BMI exhibit superior predictive value for hypertension, with TyG-BMI demonstrating a particularly strong predictive capability in elderly men.

Debates persist regarding the efficacy and safety of azvudine, particularly its real-world outcomes. This study involved patients aged ≥60 years who were admitted to 25 hospitals in mainland China with confirmed SARS-CoV-2 infection between December 1, 2022, and February 28, 2023. Efficacy outcomes were all-cause mortality during hospitalization, the proportion of patients discharged with recovery, time to nucleic acid-negative conversion (T NANC), time to symptom improvement (T SI), and time of hospital stay (T HS). Safety was also assessed. Among the 5884 participants identified, 1999 received azvudine, and 1999 matched controls were included after exclusion and propensity score matching. Azvudine recipients exhibited lower all-cause mortality compared with controls in the overall population (13.3% vs. 17.1%, RR, 0.78; 95% CI, 0.67-0.90; P = 0.001) and in the severe subgroup (25.7% vs. 33.7%; RR, 0.76; 95% CI, 0.66-0.88; P < 0.001). A higher proportion of patients discharged with recovery, and a shorter T NANC were associated with azvudine recipients, especially in the severe subgroup. The incidence of adverse events in azvudine recipients was comparable to that in the control group (2.3% vs. 1.7%, P = 0.170). In conclusion, azvudine showed efficacy and safety in older patients hospitalized with COVID-19 during the SARS-CoV-2 omicron wave in China.

Addressing the enduring challenge of evaluating traditional Chinese medicines (TCMs), the integrated evidence chain-based effectiveness evaluation of TCMs (Eff-iEC) has emerged. This paper explored its capacity through a demonstration study that evaluated the effectiveness evidence of six commonly used anti-hepatic fibrosis Chinese patent medicines (CPMs), including Biejiajian Pill (BP), Dahuang Zhechong Pill (DZP), Biejia Ruangan Compound (BRC), Fuzheng Huayu Capsule (FHC), Anluo Huaxian Pill (AHP), and Heluo Shugan Capsule (HSC), using both Eff-iEC and the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) system. The recognition of these CPMs within the TCM academic community was also assessed through their inclusion in relevant medical documents. Results showed that the evidence of BRC and FHC received higher assessments in both Eff-iEC and GRADE system, while the assessments for others varied. Analysis of community recognition revealed that Eff-iEC more accurately reflects the clinical value of these CPMs, exhibiting superior evaluative capabilities. By breaking through the conventional pattern of TCMs effectiveness evaluation, Eff-iEC offers a novel epistemology that better aligns with the clinical realities and reasoning of TCMs, providing a coherent methodology for clinical decision-making, new drug evaluations, and health policy formulation.
4.Analysis of the incidence and risk factors of low anterior resection syndrome after radical sphincter-preserving surgery for locally advanced rectal cancer treated with neoadjuvant immunotherapy: a single-center retrospective study

Objective:To explore the impact of neoadjuvant immunotherapy on the occurrence of low anterior resection syndrome (LARS) in patients with locally advanced rectal cancer who underwent restorative anterior resection, and to analyze associated risk factors.Methods:This study was an observational study. Patients with adenocarcinoma, mucinous adenocarcinoma, or signet ring cell carcinoma of the rectum located 0-10 cm from the anal verge who received neoadjuvant immunotherapy followed by curative restorative anterior resection at Peking University Cancer Hospital between November 2019 and February 2024 were retrospectively examined. Exclusion criteria were as follows: (1) metastasis detected preoperatively;(2) follow-up <1 year or stoma closure <6 months; (3) local recurrence or metastasis during follow-up; and (4) stoma without closure or stoma re-creation. The Chinese version of the LARS questionnaire was used to assess bowel function by telephone interview, and patients were classified based on score into no LARS (0-20 points), minor LARS (21-29 points), and major LARS (30–42 points). The incidence of LARS, major LARS, and associated risk factors were analyzed.Results:A total of 52 patients (34 men) were included for analysis. Mean age was 58.0 ± 9.8 years and mean body mass index was 25.1 ± 2.6 kg/m 2. Median follow-up was 27.5 months (range, 12.0-63.7). Median LARS score was 21 (range, 1-41). Twenty-six patients (50.0%) developed LARS after surgery, and half of these (13 cases) were classified as major LARS. Stool clustering (repeated defecation within 1 hour) was observed in 80.8% (42/52) of patients. Distance between the tumor edge and the dentate line [odds ratio (OR), 3.597; 95% confidence interval (CI), 1.140-11.360; P=0.026], management of the left colic artery (OR, 0.133; 95% CI, 0.026-0.691; P=0.008), and interval of stoma closure (OR, 5.250; 95%CI, 1.381-19.960; P=0.011) were significantly associated with LARS. Interval of stoma closure was significantly associated with major LARS (OR, 4.200; 95%CI, 1.064–16.584; P=0.040). In multivariate logistic regression, ≤3.5 cm between the tumor edge and the dentate line (OR, 7.407; 95%CI, 1.377-40.000; P=0.020), non-preservation of the left colic artery (OR, 8.403; 95%CI, 1.183-58.823; P=0.033) and interval of stoma closure >6 months (OR, 10.865; 95% CI, 2.039-57.896; P=0.005) were independent risk factors for LARS. Interval of stoma closure >6 months (OR, 4.356; 95% CI, 1.105-17.167; P=0.035) were independent risk factors for major LARS. Conclusion:Patients with locally advanced rectal cancer treated with neoadjuvant immunotherapy experienced a high incidence of LARS after curative surgery, with stool clustering as the predominant symptom. Tumor edge–dentate line distance ≤3.5 cm, non-preservation of the left colic artery, and interval of stoma closure >6 months were risk factors for LARS.
5.Quantifying Oxaliplatin-Related Hepatic Sinusoidal Injury Based on 3D CT Image Segmentation Technology

Purpose To investigate the value of oxaliplatin-induced sinusoidal injury based on 3D CT image segmentation technology,and to analyze its variation.Materials and Methods A total of 24 patients with uneven hypoperfusion of liver parenchyma in the portal vein phase of enhanced CT after receiving oxaliplatin regimen chemotherapy from November 2019 to July 2021 in the Second Affiliated Hospital,Jiangxi Medical College,Nanchang University were analyzed,retrospectively.The imaging data of patients with epigastric CT enhancement before chemotherapy,early chemotherapy(1-3 cycles)and late chemotherapy(≥4 cycles)were collected.Two doctors used 3D Slicer software to segment and measure the liver volume and spleen volume at each stage on the CT image,as well as the hepatic hypoperfusion volume(HHV)of the liver in the portal vein phase after chemotherapy,and calculated the HHV ratio(HHVR),liver volume hepatomegaly rate and spleen volume splenomegaly rate,and performed statistical analysis.Results The ICC for HHV,liver volume and spleen volume measured by two doctors were 0.997(P=0.000),0.905(P=0.000)and 0.998(P=0.000),respectively.Reddish-brown could be used to represent normal liver parenchyma,and blue could be used to render the HHV area to achieve HHV visualization.There was a statistically significant increase in HHVR in the late stage of chemotherapy compared with the early stage of chemotherapy[(74.35±30.56)%vs.(57.15±30.53)%;t=3.226,P=0.004].In the early and late stages of chemotherapy,the liver volume and spleen volume increased statistically significantly compared with those before chemotherapy(all P<0.05),and the spleen volume splenomegaly rate value in the late stage of chemotherapy increased statistically compared with the early stage of chemotherapy[25.75(7.40,70.66)%vs.17.86(4.44,63.59)%;Z=2.229,P=0.026].The Pearson correlation coefficient between liver volume hepatomegaly rate and HHVR was 0.377(P=0.008),and the Spearman correlation coefficient between spleen volume splenomegaly rate and HHVR was 0.296(P=0.041).Conclusion The application of 3D CT image segmentation technology to measure the HHV caused by oxaliplatin has good consistency,and can realize the visual display of HHV.With the increase of chemotherapy cycle,the extent of oxaliplatin-induced sinusoidal injury shows an increasing trend,and is positively correlated with hepatomegaly and splenomegaly.
6.Epidemiological characteristics and risk factors for nosocomial infections in patients treated with ECMO in intensive care unit of a general hospital

OBJECTIVE To explore the current status,etiological characteristics and risk factors for nosocomial in-fections in the patients who are treated with extracorporeal membrane oxygenation(ECMO)so as to provide bases for treatment and prevention of nosocomial infections in the ICU patients treated with ECMO.METHODS The clinical data were retrospectively collected from the patients who were treated with ECMO in the ICU of Xinyang Central Hospital from Jan.2021 to Dec.2023.The patients were divided into the infection group and the non-infec-tion group according the status of nosocomial infection during the ECMO treatment period.The constituent ratios of pathogens isolated from the patients with infections were recorded,and multivariate logistic regression analysis was performed for independent risk factors for the ECMO-related nosocomial infections.RESULTS Of 86 patients who were finally enrolled in the study,33(38.37%)had nosocomial infections.Totally 54 strains of pathogens were isolated from the patients with the infections,43(79.63%)of which were gram-negative bacteria,7(12.97%)were gram-positive bacteria,and 4(7.41%)were fungi.There were 36(66.67%)strains of multi-drug-resistant organisms(MDROs)among the 54 strains of pathogens,and 27(81.82%)patients were detected with MDROs.Among the ECMO patients with postoperative nosocomial infections,21(63.64%)cases had pul-monary infections,8(24.24%)cases had bloodstream infection,and 4(12.12%)had urinary system infections.Multivariate logistic analysis showed that the high blood glucose level at the beginning of treatment with ECMO,long duration of ECMO treatment and long time of central venous catheter indwelling were the independent risk factors for the nosocomial infections in the patients treated with ECMO(P<0.05).CONCLUSION The isolation rate of gram-negative bacteria is relatively high among the pathogens isolated from the ECMO patients with post-operative nosocomial infections,and the drug resistance rates are high.The high blood glucose level,long duration of ECMO supporting treatment and long time of central venous catheter indwelling are the independent risk factors for the nosocomial infections in the patients treated with ECMO.

Objective:To evaluate the effects of the Chinese visceral adiposity index(CVAI)and the Triglyceride-Glucose(TyG)index on the risk of hypertension among the elderly in the Zhengxiangbai Banner community of Inner Mongolia.Methods:This study selected elderly individuals who underwent physical examinations at the community health service center in Zhengxiangbai Banner, Inner Mongolia, from January 2024 to July 2024 as the study population.Based on the diagnostic criteria for hypertension, participants were divided into hypertensive and non-hypertensive groups.Data on demographics, body mass index(BMI), waist circumference(WC), fasting blood glucose(FSG), and blood lipids were collected.The CVAI, TyG, Triglyceride Glucose-Body Waist Circumference(TyG-WC), and Triglyceride Glucose-Body Mass Index(TyG-BMI)were calculated.Subjects were categorized into quartiles(1st to 4th)based on CVAI, TyG, TyG-WC, and TyG-BMI.Multivariate logistic regression analysis was employed to explore the relationship between these indicators and the risk of hypertension in the elderly.Results:This study included a total of 3, 319 elderly subjects, comprising 1, 743 individuals in the hypertensive group and 1, 576 in the non-hypertensive group.Multivariate logistic regression analysis indicated that the fourth quartile of the CVAI was a risk factor for hypertension when compared to the first quartile.Additionally, the second, third, and fourth quartiles of the TyG-BMI were identified as risk factors for hypertension relative to the first quartile.The analysis of the correlation strength between various obesity indexes and hypertension revealed that CVAI, TyG-WC, and TyG-BMI were independently correlated with the risk of hypertension.Notably, TyG-BMI exhibited a stronger association with hypertension( OR=1.006, 95% CI: 1.004-1.008).ROC curve analysis demonstrated that TyG-BMI possessed superior predictive ability for hypertension, with an AUC of 0.587(95% CI: 0.568-0.607).Furthermore, Decision Curve Analysis(DCA)also indicated that TyG-BMI had enhanced predictive capability.Further stratification by gender revealed that TyG-BMI in elderly men was independently correlated with the risk of hypertension( P<0.001), while CVAI in elderly women was independently correlated with the risk of hypertension( P<0.05).ROC curve analysis indicated that TyG-BMI in elderly men had better predictive ability for hypertension(AUC=0.582, 95% CI: 0.554~0.610).The difference in AUC compared to CVAI was statistically significant( P<0.01).DCA analysis further confirmed that TyG-BMI exhibited superior predictive ability. Conclusions:Our findings suggest that both CVAI and TyG-BMI exhibit superior predictive value for hypertension, with TyG-BMI demonstrating a particularly strong predictive capability in elderly men.
