1.Perioperative immune dynamics and clinical outcomes in patients undergoing on-pump cardiac surgery

Objective: To characterize perioperative dynamic changes in immune-cell phenotypes and inflammatory cytokines in patients undergoing CPB (cardiopulmonary bypass) cardiac surgery, and to explore their associations with postoperative outcomes. Methods: In this prospective cohort study, 120 adult patients who underwent elective cardiac surgery under CPB at West China Hospital from May 2022 to March 2023 were enrolled. Perioperative immune-cell phenotypes and concentrations of 40 inflammation-related cytokines were measured. The primary outcomes were the sequential organ failure assessment (SOFA) score at 24 h after surgery and ΔSOFA (the peak SOFA score within 48 h after surgery minus the preoperative SOFA score). Secondary outcomes included major adverse cardiovascular events (MACE), acute kidney injury (AKI), respiratory failure, severe liver injury, and infection. Results: The mean age of enrolled patients was 57±10 years. Of these, 52% (62/120) were male and 90% (108/120) underwent valve surgery. Plasma analysis showed that multiple pro-inflammatory cytokines increased during CPB and remained elevated up to 24 h after surgery; five chemokines and the anti-inflammatory cytokine IL-10 peaked at the end of CPB. The SOFA score increased from 1 (1, 2) preoperatively to 7 (5, 10) at 24 h after surgery, with a ΔSOFA of 6 (4, 8). Within 30 days after surgery, 48 patients (40.0%) developed AKI, 17 (14.2%) developed infection, 4 (3.3%) developed severe liver injury, 3 (2.5%) developed respiratory failure, and 3 (2.5%) experienced MACE. During the 2-year follow-up, 8 patients (6.7%) experienced MACE and 5 (4.2%) died. Conclusion: Multi-organ dysfunction is common after cardiac surgery under CPB (median ΔSOFA, 6), accompanied by perioperative activation of multiple immune-cell subsets and upregulation of pro-inflammatory, anti-inflammatory, and chemotactic mediators. This study provides data-driven evidence and research clues for further investigation of the associations between CPB-related immune perturbations and postoperative organ dysfunction and clinical outcomes.
2.Research Progress of Traditional Chinese Medicine in Improving Diabetic Retinopathy Based on Nrf2 Signaling Pathway

Diabetic retinopathy (DR) is a microvascular complication of diabetes and one of its most common complications. Prolonged hyperglycemia induces oxidative stress, inflammatory responses, apoptosis, and pathological angiogenesis, ultimately disrupting the blood-retinal barrier(BRB) and leading to visual impairment or even blindness. Recent studies show that the nuclear factor erythroid 2-related factor 2 (Nrf2) signaling pathway plays an important role in the development of DR's pathological changes. Meanwhile, Chinese herbal monomers have been shown to modulate the Nrf2 signaling pathway, thereby intervening in the development of DR. In terms of inhibiting oxidative stress, saponin compounds such as platycodin-D and ginsenoside Rb1 downregulate the expression of malondialdehyde (MDA), thereby ameliorating retinal oxidative stress. Flavonoids such as total flavonoids from Pueraria lobata flower and puerarin upregulate the expression of superoxide dismutase (SOD) and glutathione peroxidase (GPx), effectively clearing lipid peroxides. Regarding the suppression of inflammation, phenolic compounds like resveratrol and chlorogenic acid inhibit the nuclear factor kappa B (NF-κB) pathway, reducing the release of tumor necrosis factor-alpha (TNF-α) and mitigating inflammatory responses. In the context of inhibiting apoptosis, polysaccharides such as Polygonatum sibiricum polysaccharide and Angelica sinensis polysaccharide downregulate the expression of the pro-apoptotic protein Bcl-2-associated X protein (Bax) and suppress the activity of the executioner Caspase-3, thereby reducing the apoptosis rate. As for the inhibition of neovascularization, compounds including bilobalide and physcion significantly decrease the protein expression of vascular endothelial growth factor (VEGF), leading to a reduction in retinal pathological angiogenesis. Furthermore, Chinese herbal compound prescriptions such as Tongluo Zhujing pills, Yiqi Huoxue Yangyin decoction, Qiming granules, and Danlou tablets can also intervene in the onset and progression of DR through the mechanisms described above. In summary, both Chinese herbal monomers and Chinese herbal compound prescriptions can modulate the Nrf2 signaling pathway to inhibit oxidative stress, alleviate inflammation, and participate in maintaining BRB integrity, suppressing retinal neovascularization, and preventing neurodegeneration, thereby delaying the progression of DR. Therefore, this paper reviews and summarizes recent studies at home and abroad on how traditional Chinese medicine (TCM) works to treat DR, and the relationship between the Nrf2 pathway and DR. It aims to provide research ideas for preventing and treating DR.
3.The predictive value of serum signal lymphocyte activation molecule family member 8 for predicting post-stroke cognitive impairment

Objective:To investigate the predictive value of serum signal lymphocyte activation molecule family member 8(SLAMF8)levels for post-stroke cognitive impairment(PSCI).Methods:The GSE122063 dataset was selected from the Gene Expression Omnibus(GEO), and key genes associated with vascular dementia were identified using STRING network and Cytoscape software.This prospective cohort study involved 123 patients with acute cerebral infarction(ACI)who were admitted to the Department of Neurology at Jiangsu University Affiliated Hospital from January to December 2023.Patients were followed up for six months and categorized into PSCI and post-stroke non-cognitive impairment(PSNCI)groups based on the occurrence of PSCI.General data from both groups at baseline, as well as the Mini-Mental State Examination(MMSE)and Montreal Cognitive Assessment Scale(MoCA)scores at the six-month follow-up, were collected.Baseline serum levels of SLAMF8 and stabilin-1(STAB1), along with serum levels of interleukin-1β(IL-1β)and interleukin-6(IL-6)at the six-month follow-up, were measured.Pearson correlation analysis was used to assess the correlation between variables, while logistic regression analysis was employed to determine baseline factors influencing the occurrence of PSCI.Receiver Operating Characteristic(ROC)curves were plotted to analyze the predictive value of variables for PSCI occurrence.Results:The Cytoscape software identified SLAMF8 and STAB1 as key genes associated with vascular dementia, utilizing maximum neighborhood component density(DNMC)and eccentricity algorithms on the GSE122063 dataset.In the cohort study, patients in the PSCI group exhibited higher baseline NIHSS scores and serum SLAMF8 levels compared to the PSNCI group( t=3.033, 5.422; P<0.01). Additionally, they demonstrated significantly lower MMSE and MoCA scores( t=16.340, 18.634; P<0.001)and higher serum levels of IL-1β and IL-6( t=2.633, 2.632; P<0.05)at the 6-month follow-up.No significant difference was observed in baseline STAB1 levels between the two groups( t=1.280, P>0.05). In the PSCI group, there was no significant correlation between baseline serum SLAMF8 levels and admission NIHSS scores( r=0.257, P=0.082); however, SLAMF8 showed a negative correlation with both MMSE scores( r=-0.711, P<0.001)and MoCA scores( r=-0.686, P<0.001)at the 6-month follow-up.Logistic regression analysis indicated that baseline serum SLAMF8 levels( OR=1.142, P=0.001)and NIHSS scores( OR=1.094, P=0.007)were risk factors for the development of PSCI in patients with acute cerebral infarction(ACI). ROC curve analysis revealed that the area under the ROC curve(AUC)for baseline serum SLAMF8 levels in predicting PSCI occurrence in ACI patients was 0.776, while the AUC for the combined prediction using both SLAMF8 and NIHSS scores was 0.796.Furthermore, baseline serum SLAMF8 levels were positively correlated with serum IL-1β levels( r=0.652, P<0.001)and IL-6 levels( r=0.710, P<0.001)at the 6-month follow-up. Conclusions:The serum SLAMF8 level, exhibiting early high expression in ACI patients, may serve as a potential biological marker for predicting the occurrence of PSCI.
4.Effect of Xinfeng Capsules Combined with Chronic Disease Management of Traditional Chinese Medicine on Rapid Disease Control and Short-term Prognosis of Patients with Rheumatoid Arthritis

ObjectiveTo investigate the effects of Xinfeng capsules combined with chronic disease management of traditional Chinese medicine (TCM) on rapid disease control and short-term prognosis of patients with rheumatoid arthritis (RA). MethodsA total of 80 RA patients hospitalized in the Department of Rheumatology of The First Affiliated Hospital of Anhui University of Chinese Medicine from January 2022 to March 2024 were enrolled and randomly divided into an observation group (40 cases) and a control group (40 cases). The control group was treated with conventional methotrexate combined with standard chronic disease management, while the observation group was additionally treated with Xinfeng Capsules combined with TCM chronic disease management. The treatment course lasted 24 weeks. The outcomes were compared between two groups, including disease activity [28-joint disease activity score (DAS28), clinical disease activity index (CDAI), simplified disease activity index (SDAI)], visual analogue scale (VAS) for pain, TCM syndrome score, tender joint count (TJC), swollen joint count (SJC), morning stiffness duration, Health Assessment Questionnaire (HAQ), Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS), American College of Rheumatology (ACR) 20%, 50% and 70% response rates (ACR20/50/70), erythrocyte sedimentation rate (ESR), high-sensitivity C-reactive protein (hs-CRP), rheumatoid factor (RF), anti-cyclic citrullinated peptide antibody (CCP-Ab), interleukin (IL)-6, IL-1β, tumor necrosis factor-α (TNF-α), and serum immunoglobulin G (IgG). The Chronic Disease Self-Management Scale (CDSMS) was used to evaluate patients’ self-management ability, self-care ability, and nursing satisfaction. Patients were followed up for 12 weeks to assess prognosis, and COX regression analysis was performed to determine the impact on short-term prognosis. ResultsAfter treatment, TJC, SJC, morning stiffness duration, DAS28, CDAI, SDAI, VAS, TCM syndrome score, ESR, hs-CRP, RF, CCP-Ab, IL-6, IL-1β, TNF-α, IgG, HAQ, SAS, SDS, chronic disease self-management behavior, self-efficacy, and self-care ability all improved significantly in both groups compared with baseline (P<0.05,P<0.01). Compared with the control group, the observation group showed more significant improvements in TJC, SJC, morning stiffness duration, DAS28, CDAI, SDAI, VAS, TCM syndrome score, ESR, IL-1β, IgG, HAQ, SAS, SDS, self-care ability, chronic disease self-management behavior, and self-efficacy (P<0.05 or P<0.01). The ACR70 response rate and nursing satisfaction were significantly higher in the observation group than in the control group (P<0.01). COX regression analysis showed that Xinfeng capsules combined with TCM chronic disease management reduced the risk of poor short-term prognosis in RA patients. ConclusionXinfeng capsules combined with TCM chronic disease management facilitates rapid disease control in RA patients, effectively improves short-term prognosis, and plays an important role in the treatment of the disease.
5.Facilitators and barriers to pulmonary rehabilitation in postoperative lung cancer patients:a Meta-synthesis

Objective To synthesize qualitative studies on the experiences of postoperative lung cancer patients participating in pulmonary rehabilitation,aiming to comprehensively understand the barriers and facilitators influencing their engagement and to provide evidence-based insights for improving rehabilitation adherence.Methods Qualitative studies were retrieved from databases including Web of Science,PubMed,Cochrane Library,Embase,CNKI,Wanfang Data,VIPand Sinomed,with a search period spanning from database inception to January,2025.The methodological quality of eligible studies was assessed using the Joanna Briggs Institute Critical Appraisal Checklist for Qualitative Research.Extracted findings were mapped and synthesized using the Theoretical Domains Framework(TDF)and the Capability-Opportunity-Motivation-Behavior mode(COM-B).Results Ultimately,a total of 11 studies were included,yielding 48 findings.Using the Theoretical Domains Framework(TDF)and COM-B model,facilitators and barriers to pulmonary rehabilitation in postoperative lung cancer patients were mapped and consolidated into 14 categories,culminating in 2 synthesized findings.Conclusion Participation in pulmonary rehabilitation among postoperative lung cancer patients is influenced by multiple factors.Clinicians should develop patient-centered pulmonary rehabilitation plans by considering these factors and incorporating patients' attitudes and perspectives.
6.Analysis of erectile dysfunction and its associated factors in male patients undergoing peritoneal dialysis

Objective To investigate the prevalence proportion and influencing factors of erectile dysfunction in male patients receiving peritoneal dialysis(PD),and to provide a basis for clinical intervention.Methods A total of 170 male patients undergoing maintenance PD in Jinling Hospital from January to June 2025 were selected as the study subjects.And the international erectile function index-5 was used to evaluate sexual function of the patients.The general data,dialy-sis adequacy,nutritional status and other clinical indicators were collected.The logistic regression analysis was used to identify the influence factors of erectile dysfunction in the patients with PD.Results The incidence of erectile dysfunction in male patients with PD was 45.3%.And univariate analysis showed that age,education level,smoking history,dialysis-related symptoms and depressive status were significantly correlated with erectile dysfunction in male patients(all P<0.05).Ordered logistic regression analysis identified age,sleep duration,smoking history,albumin and total cholesterol as independent risk factors for erectile dysfunction in male patients with PD.Conclusion The incidence of erectile dys-function is relatively high in male patients with PD which is influenced by various physiological and psychological factors.Clinically,attention should be paid to patients' nutritional status,sleep quality,and other factors,with comprehensive in-terventions implemented to improve sexual function and overall quality of life.
7.Construction and validation of acute pulmonary embolism diagnostic model based on clinical and laboratory indicators

Objective:To establish and validate the diagnostic model of acute pulmonary embolism (APE) based on clinical and laboratory variables.Methods:This retrospective analysis was conducted on patients with suspected APE who underwent CT pulmonary angiography at the First Affiliated Hospital of Henan University of Chinese Medicine between February 2015 and December 2023. The patients were randomly divided into a training set and a validation set at a ratio of 7∶3. Clinical and laboratory data of the enrolled patients were collected, and patients were divided into an APE group and a non-APE group according to CT pulmonary angiography results. In the training set, univariate and multivariate logistic regression as well as Lasso regression were used to identify risk factors for APE, and a diagnostic model was developed and validated. Receiver operating characteristic curves were plotted, and calibration and decision curves were used to assess the performance of the diagnostic model. The diagnostic efficacy of the model was compared with that of the Wells score and the revised Geneva score using the DeLong test.Results:A total of 752 patients were enrolled, aged (64±15) years, including 417 (55.5%) males. The training set included 526 patients and the validation set included 226 patients. The incidence of APE in this cohort was 48.7% (366/752), with 366 cases in the APE group and 386 in the non-APE group. Multivariable logistic regression analysis showed that cyanosis ( OR=8.88, 95% CI 2.04-49.11), elevated neutrophil count ( OR=1.82, 95% CI 1.06-3.15), elevated creatine kinase isoenzyme ( OR=3.45, 95% CI 1.76-6.91), decreased partial pressure of carbon dioxide ( OR=12.88, 95% CI 7.64-22.34), elevated age-adjusted D-dimer ( OR=2.53, 95% CI 1.10-6.20), prolonged thrombin time ( OR=4.08, 95% CI 2.06-8.33), and positive lower limb venous ultrasound for thrombus ( OR=4.39, 95% CI 2.59-7.58) were risk factors associated with APE. The area under the curve ( AUC) of the diagnostic model was 0.92 (95% CI 0.90-0.94) in the training set and 0.92 (95% CI 0.89-0.95) in the validation set. The diagnostic efficacy of this model was superior to that of the Wells score ( AUC: 0.92 vs. 0.63, P<0.01) and the revised Geneva score ( AUC: 0.92 vs. 0.59, P<0.01). Conclusion:The diagnostic model for acute pulmonary embolism constructed based on clinical and laboratory parameters demonstrates excellent diagnostic performance and may facilitate rapid and accurate screening in clinical practice.
8.Intelligent Detection of Acute Pulmonary Embolism on CT Pulmonary Angiography Based on Res2Net Attention Mechanism Network

Purpose To achieve intelligent detection of acute pulmonary embolism(APE)in CT pulmonary angiography based on the Res2Net attention mechanism network.Materials and Methods Retrospectively included patients with suspected of APE who underwent CT pulmonary angiography examination and were diagnosed as APE at the First Affiliated Hospital of Henan University of Chinese Medicine from February 2015 to May 2023.The dataset was randomly divided into training,validation and test set in a ratio of 7∶2∶1.The model was trained based on the Res2Net network,combined with atrous spatial pyramid pooling and attention mechanism modules,and was performed five-fold cross internal validation.Using the area under the receiver operator characteristic curve,sensitivity and specificity to assess the diagnostic performance of the model.Dice similarity coefficient,precision and intersection over union(IOU)were used to assess the segmentation performance of thrombus on the test set and plot the corresponding curves.The performance of the Res2Net attention mechanism network was compared with the classic U-Net and CE-Net model.Results A total of 303 patients with APE were included in this study.There were 212,61 and 30 cases in the training set,validation set and test set,respectively.The model's area under the curve was 0.95,sensitivity was 0.90,specificity was 1.00,Dice similarity coefficient was 0.86,precision was 0.90,Pos-IOU was 0.78 and Neg-IOU was 1.00,respectively.The parameter curves and radar chart showed that the Res2Net attention mechanism network performed better than the U-Net and CE-Net models.The visualization results of the segmentation comparison showed that the Res2Net attention mechanism network achieved higher precision in segmenting pulmonary artery thrombus.Conclusion The Res2Net attention mechanism network has good performance for detection of APE.
