1.Analysis of the composition of pediatric inpatient diseases in a certain second-class public hospital from 2019 to 2023

Objective To analyze the disease composition of pediatric inpatients in a specific hospital and provide a sci-entific basis for the prevention,treatment,and rational allocation of medical resources within the jurisdiction.Methods Diagno-sis and treatment information of pediatric inpatients from January 1,2019,to May 31,2023,was collected via the hospital's medical record quality control system.A retrospective analysis was conducted on the disease composition and its changes among inpatients.Results The pediatric department of the hospital treated a total of 4 939 children,involving 16 categories and 83 types of diseases.Among them,the top three categories of diseases in terms of composition,from high to low,were respiratory system diseases,digestive system diseases,and oral diseases.From 2019 to 2022 and from January to May 2023,the top three categories of diseases in terms of composition,from high to low,were consistently:respiratory system diseases,digestive system diseases,and oral diseases.In the period from January to May 2023,the composition of respiratory system diseases decreased by 8.09%compared to 2019-2022;the composition of digestive system diseases increased by 8.00%compared to 2019-2022;and the composition of oral diseases decreased by 0.13%compared to 2019-2022.Conclusion Respiratory and digestive system diseases,as well as oral diseases,are critical areas for pediatric disease prevention and treatment within this jurisdiction.It is recommended that local health administrative departments optimize the allocation of medical resources,enhance preventive meas-ures,diagnosis,and treatment,and establish a robust health defense system for children in the region.
2.Surveillance for pneumonia of unknown etiology: current status, challenges, and implementation ways

The prevention and control of emerging and reemerging infectious diseases are crucial for national biosecurity, and surveillance and reporting of pneumonia of unknown etiology are main ways for the early detection of these diseases and mitigation of their severity. This paper summaries the surveillance methods for pneumonia of unknown etiology and emerging and reemerging infectious diseases globally, indicating that such surveillance is mainly conducted based on hospital. Western countries primarily combine active and passive surveillance while utilizing artificial intelligence technology to rapidly identify cases. China mainly use passive surveillance based on the surveillance system for pneumonia of unknown etiology, with the function of early warning in the identification of emerging infectious diseases, such as avian influenza. However, with the improvement in the surveillance system operation, the overlap with other surveillance disease systems, such as influenza, has occurred, and the improvements in case definition and operational protocol are needed. It is recommended to improve the specificity of the case definition, strenthen training in hospital staff, inclduing clinical workers and office workers, and formulate incentive mechanisms. It is necessary to emphasize the responsibility of clinicians as the main force for the detection and reporting of pneumonia of unknown etiology and emerging infectious diseases, improve the appilication of artifical intelligent technique and conduct multi-source surveillance, such as third-party testing.

4.The Bed Nucleus of the Stria Terminalis-Paraventricular Nucleus of the Hypothalamus Neural Circuit Regulates Neuropathic Pain Through the Brain-Spleen Axis.

Neuropathic pain is a chronic condition caused by damage or dysfunction in the nervous system. While the spleen may influence neuropathic pain, its role has been poorly understood. This study demonstrates that the spleen plays a crucial role in regulating neuropathic pain through the bed nucleus of the stria terminalis (BNST) - paraventricular nucleus of the hypothalamus (PVN) neural circuit in a chronic constriction injury (CCI) mouse model. Splenectomy, splenic denervation, or splenic sympathectomy significantly increased the mechanical withdrawal threshold (MWT) and reduced macrophage infiltration in the dorsal root ganglia (DRG) of CCI mice. Pseudorabies virus injections into the spleen revealed connections to the BNST and PVN in the brain. Chemogenetic inhibition of the BNST-PVN circuit increased macrophage infiltration in the DRG and decreased the MWT; these effects were reversed by splenectomy, splenic denervation, or sympathectomy. These findings underscore the critical role of the spleen, regulated by the BNST-PVN circuit, in neuropathic pain.

6.Analysis of Clinical Diagnosis and Traditional Chinese Medicine Medication Rule of Children with Nephrotic Syndrome in Single Center

ObjectiveTo analyze the clinical treatment plan and traditional Chinese medicine (TCM) medication rule of children with primary nephrotic syndrome (PNS) in the First Affiliated Hospital of Henan University of Chinese Medicine. MethodsThe gender and age of children firstly diagnosed with nephrotic syndrome in the pediatric nephrology department of the First Affiliated Hospital of Henan University of Chinese Medicine from November 2019 to December 2022 were collected, and the use of immunosuppressive agents and related frequencies were counted. According to the inclusion and exclusion criteria, an independent TCM prescription database for children with nephrotic syndrome was established. Excel was used to analyze the relevant information of the literature. The frequency counting, association rule analysis, and cluster analysis were carried out on TCM in the prescription, and the high-frequent drugs were analyzed. Results(1) General information: A total of 711 children were included, consisting of 522 males (73.42%) and 189 females (26.58%). The ratio of male to female was about 2.76∶1. The disease mainly occurred in infants and preschool age, and the average age of onset was (4.74 ± 3.48) years old. (2) Clinical treatment plan and use of immunosuppressive agents: Of the 711 children with PNS, 237 were treated with hormone alone (32.33%), and 474 (66.67%) received immunosuppressive agents combined with hormones. In the initial treatment, hormone combined with Tacrolimus (TAC) was the preferred treatment (32.91%). For children with refractory PNS who exhibited poor clinical efficacy, Rituximab (RTX) was mostly used for treatment, with a ratio of up to 23.63%. (3) TCM syndrome and medication rule: In PNS syndrome differentiation, Qi and Yin deficiency was identified as the main syndrome. This involved a total of 477 cases, accounting for 67.09%. Yang deficiency of spleen and kidney was observed in 118 cases, accounting for 16.60%. A total of 711 children were included, of which 706 children were treated with TCM. This involved a total of 706 prescriptions, 226 TCM, and 9 793 frequencies. There were 30 herbs used more than 95 times. The top five TCM were Radix et Rhizoma Glycyrrhizae (81.16%), Radix Astragali (71.81%), Poria (68.84%), Rhizoma Atractylodis Macrocephalae (63.60%), and Fructus Corni (57.37%). The drug association rules and network diagram showed that the combination of ''Radix Astragali-Rhizoma Atractylodis Macrocephalae-Poria'' was the closest, and five types of combinations were obtained by cluster analysis. ConclusionIn the diagnosis and treatment of PNS in children, TAC combined with hormones shows good clinical efficacy and high safety. For children with refractory PNS, RTX combined with hormones can be used. TCM medication for PNS should follow the basic principles of strengthening the body and vital Qi and make good use of drugs such as Radix Astragali, Poria, Rhizoma Atractylodis Macrocephalae, and cornus to regulate the Yin and Yang balance and achieve better clinical efficacy.

8.Erythrocytapheresis for the treatment of high-altitude polycythemia

Objective: To investigate the efficacy and incidence of adverse reactions of therapeutic erythrocytapheresis in high altitude polycythemia (HAPC) population. Methods: A retrospective study was conducted on 243 HAPC patients who were either native residents or long-term workers in Xizang and underwent therapeutic erythrocytapheresis in the Chengdu Office Hospital of the People's Government of Xizang Autonomous Region from 2021 to 2023. A comparative study was carried out on the changes in blood routine, vital signs, skin color, serum iron metabolism data, and the incidence of adverse reactions before and after the procedure. The total incidence of adverse reactions during erythrocytapheresis was 13.98% (34/243), including citrate toxicity 12.75% (31/243), puncture site hematoma 0.82% (2/243) and blood volume imbalance 0.41% (1/243). Conclusion: Therapeutic erythrocytapheresis could rapidly decrease HCT, Hb, serum iron, transferrin and transferrin saturation levels in HAPC patients, with a low incidence of adverse reactions. Therefore, therapeutic erythrocytapheresis has broad clinical application prospects in Xizang Autonomous Region.
9.Screening and validation of tsRNAs associated with lung adenocarcinoma

Objective To explore the roles of transfer RNA-derived small RNAs(tsRNAs)in the oncogenesis and progression of lung adenocarcinoma by analyzing the differential expression of tsRNAs in lung adenocarcinoma and the relationship between the expression levels of tsRNAs in lung adenocarcinoma and the prognosis of patients in order to further screen and validate the tsRNAs associated with lung adenocarcinoma.Methods The differential expression of tsRNAs between lung adenocarcinoma tissues and normal tissues was analyzed based on the database of the Computational Medicine Center.The effects of tsRNAs expression levels on the prognosis of lung adenocarcinoma patients were analyzed based on the Cancer Genome Atlas(TCGA)database(TCGA-LUAD).The target genes were predicted based on TRFtarget2.0 and tRFTar databases.Gene ontology(GO)enrichment analysis and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway analysis were performed based on DAVID and KOBA KEGG online websites.The expression levels of target genes in lung adenocarcinoma tissues and normal tissues were analyzed based on the University of ALabama at Birmingham CANcer data analysis Portal(UALCAN)database.In vitro cell proliferation,migration,and invasion assays were performed to investigate the biological functions of tRF-19-69M8LOJX in lung adenocarcinoma cells.Results Compared with the normal tissues,tRF-19-69M8LOJX was up-regulated in lung adenocarcinoma tissues(log2FC=4.28,FDR<0.05).High expression level of tRF-19-69M8LOJX was associated with shorter progression-free survival(HR=1.565,95%CI=1.142-2.145,P=0.005).And its overexpression promoted cell proliferation and migration(P<0.001),and invasion(P=0.009)of A549 cells,and up-regulated COL1A1(P=0.002)and VCAN(P=0.022)significantly in the tRF-19-69M8LOJX overexpression cell model.Conclusion tRF-19-69M8LOJX is up-regulated in lung adenocarcinoma tissues.And its high expression is closely associated with poor prognosis.The tsRNA may play an important role in the pathogenesis and development of lung adenocarcinoma.
