1.Therapeutic effect of external counterpulsation on cardiac rehabilitation in patients with coronary heart disease-a meta-analysis

Objective:To evaluate the clinical rehabilitation effect of external counterpulsation(ECP)on patients with coronary heart disease(CHD)by meta-analysis.Methods:We searched the databases of CNKI,WanFang,VIP,CBM,PubMed,Web of Science,Cochrane Library and Embase for randomized controlled trials(RCTs)and prospective cohort studies upon rehabilitative effect of ECP on CHD patients before May 2024.And meta-analysis was conducted to calculate the pooled MD and 95%CI using the random(P<0.5 or I2≥50%)or fixed effect models(other situations).Results:13 eligible literatures were finally included in the meta-analysis.Compared with participants in control group,those in trial group had significant higher left ventricular ejection fraction(MD=4.15,95%CI 2.55~5.76,P<0.001),stroke volume(MD=9.11,95%CI 7.59~10.64,P<0.001),peak oxygen uptake(MD=5.42,95%CI 2.53~8.32,P<0.001),6-min walking distance(MD=31.14,95%CI 24.89~37.40,P<0.001),metabolic equiv-alent(MD=0.58,95%CI 0.45~0.71,P<0.001),exercise duration time(SMD=0.77,95%CI 0.55~0.99,P<0.001),oxygen pulse(MD=0.88,95%CI 0.68~1.09,P<0.001),and significant lower left ventricular end-diastolic diameter(MD=-3.19,95%CI-5.20~-2.61,P<0.001).Conclusion:This study showed that ECP could effectively improve heart function,exercise capacity and tolerance of CHD patients.

OBJECTIVE:
To investigate the effect of different postures on direct anterior approach(DAA) total hip arthroplasty.
METHODS:
Total of 94 patients who underwent DAA total hip arthroplasty from July 2016 to June 2020 were retrospectively analyzed. They were divided into two groups according to different positions during the operation, including 45 cases in lateral position and 49 cases in supine position (with the aid of stent). The general data such as gender, affected limb, body mass index(BMI), incision length, operation time, intraoperative bleeding volume, drainage volume 24 hours after operation, hemoglobin difference before and after operation, first landing time after operation, postoperative hospitalization time, postoperative complications, visual analogue scale(VAS) at 1 day, 1, 2 weeks, 1, 3 and 6 months after operation, Harris score at 1, 2 weeks, 1, 3 and 6 months after operation were observed and compared between the two groups.
RESULTS:
Patients in both groups were followed up for 6 to 12 months with an average of (8.31±2.22) months. There was no significant difference between two groups in gender, affected limb, age, height, weight, body mass index(BMI), preoperative VAS score and preoperative Harris score(P>0.05). The incision length, operation time, intraoperative bleeding volume, 24-hour drainage volume, hemoglobin difference before and after operation, first time to the ground and postoperative hospitalization time of patients in supine position (assisted by stent) group were all better than those in lateral position group(P<0.05);There was no significant difference in the number of blood transfusions during and after operation(P=0.550). There was no significant difference in anteversion angle and abduction angle in the supine position(with the aid of stent) group during and after operation (P=0.825, P=0.066);There was significant difference in anteversion angle and abduction angle in the lateral position group during and after operation(P<0.05). VAS of patients in supine position (assisted by stent) group were lower than those in lateral position group at 1 day, 1, 2 weeks and 1 month after operation(P<0.05), and there was no statistical difference between two groups at 3 and 6 months after operation(P>0.05). Harris scores of patients in supine position(assisted by stent) group were higher than those in lateral position group at 1 week, 1 month and 3 months after operation(P<0.05), and there was no significant difference between two groups at 6 months after operation(P>0.05).
CONCLUSION
Compared with the lateral position, the supine position DAA total hip arthroplasty has the advantages of small incision, short operation time, less bleeding, early landing time, short hospitalization time, and small intraoperative acetabular cup position judgment error. It has the advantage of fast postoperative recovery, but the recovery of hip joint function is the same after 6 months.

5.Association of epidemiological characteristics and dietary patterns with risk of type 2 diabetic nephropathy

Objective To study the prevalence of diabetic nephropathy (DN) and the relationship between dietary pattern and risk. Methods From August 2018 to May 2021, 655 T2DM patients in Wuhan Puren Hospital, including 338 males and 317 females, were divided into T2DM group (n=368 cases) and DN group (n=287 cases) according to whether patients had DN. The uniformly trained staff of our hospital used the simplified version of food intake frequency questionnaire designed for diabetes to investigate the reasonable dietary intake of patients in nearly one year. Clinical data of patients in the two groups were collected and the intake of protein, fat, carbohydrate and dietary fiber in the two groups was statistically analyzed. Logistics regression was used to analyze the formula independent risk factors of DN in T2DM, and Pearson correlation analysis was performed on the incidence of diabetic nephropathy and dietary pattern. Results Among of 655 T2DM patients, there were 287 (43.82%) patients with DN, including 149 males and 138 females. The average age, duration of diabetes, smoking in DN group were significantly higher than those in T2DM group (P<0.05). There was no significant difference in sex ratio between the two groups (P>0.05). Energy, protein intake, carbohydrate and fat intake in DN group were significantly higher than those in T2DM group (P<0.05). Multivariate logistic regression analysis showed that protein, carbohydrate and fat intake were independent risk factors for DEVELOPING DN in T2DM patients (P<0.05). According to Pearson correlation analysis, the risk of developing DN in T2DM patients was positively correlated with protein and fat intake (r=0.449 , 0.517, P<0.05). Conclusion PATIENTS with T2DM have a higher risk of DEVELOPING DN, which is closely related to dietary intake. Reasonable allocation of dietary intake can reduce the risk of developing DN.

Gene mutation sequences were further identified by sequencing of the silenced sites. The replication of VZV in MRC⁃5 and
MRC⁃5IFNAR1⁃ cell lines was compared 168 h after VZV infection by using qRT⁃PCR and plaque formation unit(PFU)assay,
to evaluate the effect of MRC⁃5IFNAR1⁃cell line on VZV replication. Results The growth status of MRC⁃5IFNAR1⁃ cell line wasconsistent with that of MRC ⁃ 5 cells,and the relative expression of IFNAR1 mRNA decreased by 73%;The relative
expressions of IFNβ and OAS1 mRNA in MRC⁃5IFNAR1⁃ cell line were 61% and 90% lower than those in MRC⁃ 5 cells
respectively after VZV infection;In addition,168 h after VZV infection,the level of DNA replication and the titer of VZV
increased by 5. 7 folds and 4 folds respectively. Conclusion The successful establishment of MRC⁃5IFNAR1⁃ cell line may be a
potential scheme to increase the yield of vaccines based on human diploid cells,and provided a reference for expanding
production of VZV vaccine.

Pudilan Xiaoyan Oral Liquid has effects in clearing away heat and detoxifying,and is used to treat pharynx and throat swelling caused by the syndrome of excessive heat and toxin accumulation. Its efficacy is to relieve swelling and pain( redness,swelling and hot pain). It is included in the Chinese Pharmacopoeia of 2015 Edition,and has been listed in provincial health insurance directories of Shaanxi,Jiangsu,Liaoning,Hunan,Tianjin,Xinjiang and Hebei. It has been recommended by health departments of Beijing,Chongqing and other provinces as a preferred drug for the prevention and treatment of H1 N1 and HFMD,and listed in the diagnosis and Treatment Guide of HFMD by the Ministry of Health,the Clinical Application Guide of Chinese Patent Medicine edited by the Lung Department Disease Branch of China Association of Chinese Medicine,and the Clinical Practice Guide of Single Administration/Combined Administration of Antibiotics in Treatment of Common Infectious Diseases by China Association of Chinese Medicine. To further improve the clinician's understanding of drugs and better guide the rational clinical application,we invited front-line clinical experts from respiratory department,infectious department and dermatology of traditional Chinese and Western medicine to develop and compile the expert consensus. The consensus fully considered the clinical evidence and the expert clinical experience to give recommendations for clinical problems with evidence support and consensus suggestions for clinical problems without evidence support by the nominal group method.This consensus is based on clinical research evidence and expert experience in a simple and clear format,which provides a preliminary reference for the clinical use of the drug.

Danshen (Salvia miltiorrhiza) is a perennial herbal plant as a bulk commodity of traditional Chinese medicinal material. With the growing market demand, the planting scale of Danshen has been fast enlarged. The occurance of diseases on Danshen has become as a major threaten of its production. Root diseases are most devastated on Danshen. Rootrot is the most important disease, and Fusarium wilt comes second. Leaf spot occurs most on aerial part of Danshen. In the Danshen industry, the chemical control is most commonly used, with biological agents as supplements. This review summarized the symptom, pathogen, epidemiology, and control method of 7 diseases on Danshen (rootrot, Fusarium wilt, southern blight, root knot nematode, leaf spot, leaf red and virus diseases), providing guidelines for disease diagnostics, pathogen isolation and identification, proper pesticide application and integrated control.

10.Effects of Potassium Supplementation in Patients With Left Ventricular Dysfunction After Coronary Artery Bypass Grafting

Objectives: To investigate the effects of potassium supplementation after coronary artery bypass grafting (CABG) in patients with reduced left ventricular ejection fraction (EF < 50%). Methods: Present study retrospectively analyzed data from 200 CABG patients with cardiopulmonary bypass from January 2015 to June 2017. Patients were divided as heart failure with high ideal potassium supplement group (low EF<50%, potassium supplement to 5.0 mmol/L, n=41); heart failure normal potassium supplement group (low EF<50%, potassium supplement to 4.5 mmol/L, n=46); and normal potassium supplement group (normal EF>50%, potassium supplement to 4.5 mmol/L, n=113). The differences on related parameters during intraoperative and postoperative period were compared among the 3 groups. The primary endpoint was the incidence of atrial and ventricular arrhythmias requiring the use of lidocaine and amiodarone hydrochloride. Results :Atrial and ventricular arrhythmias requiring lidocaine and amiodarone hydrochloride were found in 13 patients (31.7%) in the ideal hyperkalemia group, while 28 (62.2%) in the normal hyperkalemia group needed both drugs (P=0.005). The blood pressure of ideal high potassium supplementation group was significantly higher than that of normal potassium supplementation group ([135.87±11.32] mmHg vs [111.21±31.31] mmHg, P<0.001). In the ideal hyperkalemia group, the time in the ICU was shorter ([3.92±0.98]days vs [5.12±1.12]days, P<0.001) and the hospitalization time was shorter ([10.54±2.14] days vs [13.54±2.01]days, P<0.001). There was no significant difference between the two groups in postoperative 24-hour urine volume, oxygen partial pressure, postoperative 24-hour bleeding volume and postoperative complications (P>0.05). Conclusions:The ideal potassium target of 5.0 mmol/L for patients with LVEF<50% CABG can reduce the incidence of arrhythmias that require the intervention of lidocaine and amiodarone hydrochloride during the perioperative period and shorten the stay time in the ICU.
