1.Research on Historical Evolution and Clinical Application of Shengyutang

Shengyutang is a famous classical formula of tonic, which is made from Siwutang with Ginseng Radix et Rhizoma and Astragali Radix. It is included in the Catalogue of Ancient Famous Classical Formulas(The First Batch). Based on the Principles of Key Information Research of Ancient Famous Classical Formulas, this paper used bibliometrics to sort out and research the key information of Shengyutang in the aspects of history, composition, origin and processing, dosage, decocting method, efficacy and indications. After research, it has been found that this formula was first recorded in Lanshi Micang written by LI Dongyuan during the Jin dynasty, composed of Rehmanniae Radix, Rehmanniae Radix Praeparata, Chuanxiong Rhizoma, Ginseng Radix et Rhizoma, Astragali Radix and Angelicae Sinensis taproot. The name of the formula passed down through generations was relatively unified, with clear origins and veins. In later generations, this formula was the mainstream, and adjustments were made to the dosage and composition according to the indication. In the formula, Astragalus membranaceus var. mongholicus was selected as the origin of Astragali Radix, and the origins of other medicinal materials were consistent with the 2020 edition of Chinese Pharmacopoeia. Except for Rehmanniae Radix Praeparata, the other medicinal materials were made from raw products, and the dosage form was boiled powder. According to the measurement standard in the Jin dynasty, the recommended usage and dosage were 1.24 g of Rehmanniae Radix, Rehmanniae Radix Praeparata, Chuanxiong Rhizoma, Ginseng Radix et Rhizoma each, 2.07 g of Astragali Radix and Angelicae Sinensis taproot each, crushed into coarse particles that pass through the 4 mesh sieve but can't pass through the 10 mesh sieve, added 1 200 mL of water and boiled to 300 mL, and removed the residue. Shengyutang has the functions of tonifying Qi and blood, and blood intake, treating various sores, and restlessness and insomnia caused by excessive blood flow. In ancient times, this formula was widely used in the treatment of surgical sores, gynecological diseases, deficiency syndrome, etc. In modern clinical practice, it is mostly used to treat gynecological, neurological, musculoskeletal, hematological diseases caused by Qi and blood deficiency. In this paper, the key information of Shengyutang was researched by reviewing relevant ancient literature, in order to provide reference for the modern application and development of this formula.
2.Study on Medicinal Properties of New Foreign Introducing Chinese Materia Medica Drimia maritima(L.)Stearn Based on Literature Research and Intelligent Sense

Objective To summarize the medicinal properties of new foreign introducing Chinese materia medica Drimia maritima(L.)Stearn based on literature research;To further verify its five flavors by applying intelligent sense;To provide new ideas for the medicinal properties research of new foreign introducing Chinese materia medica.Methods Literature about Drimia maritima(L.)Stearn was retrieved from CNKI,VIP,Wanfang Data,PubMed,Web of Science,and combined with the TCM theories medicinal properties of Drimia maritima(L.)Stearn was summarized.PEN3 electronic nose and SA402B electronic tongue were used to obtain intelligent sensory information of Drimia maritima(L.)Stearn,and the principal component analysis(PCA)was used to identify its five flavors.Results Literature research summarize Drimia maritima(L.)Stearn with antioxidant,anti-tumor,strengthening cardiac functions and diuresis,reducing cough and asthma and other pharmacological effects.Combined with the theories,it explored that its property was mainly slightly warm,the tastes were mainly bitter,pungent,sweet,and slightly toxic,belonging to the lung,spleen,stomach and heart meridians.Based on intelligent sense,identification results of five tastes of Drimia maritima(L.)Stearn were bitter,slightly pungent,slightly sweet,salty,non-acid,basically consistent with the literature research.Conclusion Literature research combined with intelligent sense can better summarize and recognize the medicinal properties of Drimia maritima(L.)Stearn.This study can provide a certain reference for a more objective and adequate medicinal properties analysis of new foreign introducing Chinese materia medica.
3.Directional atherectomy combined with drug-coated balloon versus bare-mental stent for elderly femoropopliteal artery disease

Objective To compare the safety and efficacy of directional atherectomy with anti-restenosis therapy(DAART,drug-coated balloon)versus conventional balloon angioplasty(bare-metal stent,BMS)in elderly patients with femoropopliteal artery disease.Methods A retrospec-tive cohort study was conducted on 116 elderly patients undergoing endovascular intervention due to femoropopliteal artery disease in our hospital between May 2016 and September 2019,divided into DAART group(57 cases)and BMS group(59 cases).Results No statistical differences were observed between the DAART and BMS groups in terms of age,risk factors,distribution of Ruth-erford classification,lesion length,lesion type,lesion location,Global Limb Anatomic Staging Sys-tem grade,infra-popliteal runoff status,or preoperative ankle-brachial index(P>0.05).However,the BMS group had significantly larger proportion of chronic limb-threatening ischemia than the other group(P<0.05).Both groups achieved a 100%success rate of surgery.The DAART group obtained obviously higher primary patency rates at 1 and 2 years than the BMS group(90.4%vs 75.0%,76.3%vs 57.3%;P<0.045).There were no significant differences in the rate of freedom from target lesion revascularization at 1 and 2 years between the two group(96.1%vs 88.8%,91.6%vs 77.7%;P>0.05).In 2 years of follow-up,the incidence of major adverse events was 10.0%(5 cases)in the DAART group and 20.8%(12 cases)in the BMS group,but no obvious difference(P>0.05).Conclusion DAART demonstrates superior mid-term efficacy than BMS in treating femoropopliteal artery disease in elderly patients.However,comprehensive preoperative assessment is essential to optimize individualized treatment strategies for this population.
4.Prediction model for extraprostatic extension of prostate based on MRI and clinical indicators

Objective To develop a Nomogram clinical prediction model for the pathological occurrence of extraprostatic extension(EPE)after radical prostatectomy in prostate cancer patients,using simplified site-specific magnetic resonance imaging(MRI)indicators and other clinical parameters.Methods A total of 181 prostate cancer patients[mean age(69.0±7.3)years]who underwent radical prostatectomy were included.These patients had received 3-Tesla multi-parametric magnetic resonance imaging(3-T mpMRI)within 6 months prior to surgery.Based on mpMRI measurements[capsular contact length(CCL)>15 mm,capsular bulging/irregularities,diameter of index lesion(dIL),and evident extraprostatic extension(eEPE)],the dIL?sEPE grading system was derived.The optimal cut-off value of dIL(denoted as dIL)was determined using the Youden J index,and categorized it into a binary variable.A Logistic regression model was established based on the dIL?sEPE grading and clinical scores.The predictive performance of clinical indicators,MRI indicators,and combined clinical and MRI indicators were compared.Finally,a clinical prediction model integrating both clinical and MRI data was developed.Results Pathological EPE was found in 46 out of 181 cases(25.4% ).A Nomogram prediction model for EPE was established with a combination of the dIL?sEPE grading and clinical indicators.Conclusion The combination of dIL?sEPE grading with clinical indicators accurately predicts extracapsular extension in prostate cancer.The Nomogram model that established,based on MRI imaging characteristics and clinical indicators has good performance and is easy to use.It is beneficial to stratifying management for prostate cancer patients,and it provides valuable guidance for patients suitable for nerve-sparing surgery.
5.Establishment of outcome indicators for the implementation of comprehensive inter-vention for multimorbidity of myopia and obesity among children and adolescents based on the RE-AIM framework

Objective:To develop outcome indicators for the implementation of comprehensive inter-ventions targeting the multimorbidity of myopia and obesity in children and adolescents,providing a basis for the co-prevention of multimorbidity and the outcome measurement of implementation research in children and adolescents.Methods:Based on the RE-AIM framework,a preliminary set of indicators was constructed.The Delphi method was employed,with experts scoring and providing feedback on the proposed indicators via questionnaires.After each round of consultation,expert enthusiasm index,authority coefficient,coordination degree,and consensus level were calculated.Expert opinions were col-lected and analyzed to modify,delete,or add indicators based on consultation results and screening crite-ria.Two Delphi rounds were conducted until consensus was achieved.Results:A total of 28 experts par-ticipated actually in both rounds.The Kendall's W coefficients for the two rounds of expert consultation were0.352(x2=413.952,P<0.001)and 0.499(x2=405.044,P<0.001),both statistically sig-nificant.The final outcome indicators for implementation research on comprehensive interventions for myopia and obesity comorbidity in children and adolescents included five primary dimensions with 13 secondary and 20 tertiary indicators.The dimension of reach included the number of children and adoles-cents involved,participant representativeness,and full-course participation representativeness.The di-mension of effectiveness included multimorbidity incidence,myopia incidence,spherical equivalent,body mass index(BMI),overweight and obesity prevalence,waist-to-height ratio,comprehensive health knowledge score,and comprehensive health behavior score.The dimension of adoption covered school representativeness and representativeness of school nurses and teachers involved in implementation.The dimension of implementation included fidelity,content modification,and cost.The dimension of mainte-nance included individual health outcomes and organizational sustainment.Conclusion:This study developed implementation outcome indicators for comprehensive interventions targeting multimorbidity of myopia and obesity among the children and adolescents based on the RE-AIM framework.These indica-tors can serve as a reference for optimizing intervention research strategies related to common multimor-bidity among children and adolescents in China.
6.Effects of chronic stress on hypothalamic appetite-regulating factors in mice

Objective To explore the effects of chronic stress and stress cessation on hypothalamic appetite regulators in mice,and to explore the stress-dependent mechanism of appetite change.Methods A total of 32 male C57BL/6 mice were randomly divided into control(Ctrl)group(n=16)and chronic unpredictable mild stress(CUMS)group(n=16).The mice in the CUMS group were given CUMS to establish the stress model,and those in the Ctrl group were fed normally.The food intake and weight of mice were recorded.The CUMS model was verified through tail suspension experiments and forced swimming experiments.Eight mice in the Ctrl group and 8 mice in the CUMS group were randomly sacrificed at the 12th week.The Ctrl group was re-grouped into the cessation-control(C-Ctrl)group(n=8),the CUMS group was re-grouped into the cessation-stress(C-CUMS)group(n=8),and the mice were sacrificed at the 15th week.The mRNA and protein levels of appetite-regulating factors,including orexin 1 receptor(OX1R),leptin receptor(LEPR)and agouti-related protein(AgRP)in the hypothalamus,were detected by quantitative polymerase chain reaction and immunochemistry.Results From week 2 to week 11 of stress,the food intake of the mice in the CUMS group was significantly higher than that in the Ctrl group(all P<0.05),while there was no significant difference in body weight between the 2 groups within 11 weeks(all P>0.05).Compared with the Ctrl group,the immobility durations of forced swimming and tail suspension of the CUMS group were markedly longer after 11 weeks(both P<0.01),indicating successful modeling.AgRP and OX1R mRNA expression in the hypothalamus of the CUMS group was significantly increased(both P<0.01),while LEPR mRNA expression was significantly decreased(P<0.01);AgRP protein in the hypothalamic arcuate nucleus of the CUMS group was significantly higher than that of the Ctrl group(P<0.05),and LEPR protein was markedly lower than that of the Ctrl group(P<0.01).However,after 3 weeks of stress cessation,the C-CUMS group had less food intake and lower body weight than the C-Ctrl group(both P<0.05).The LEPR mRNA of the C-CUMS group was significantly increased(P<0.01),while AgRP and OX1R mRNA were not significantly different(both P>0.05).There was no significant difference in AgRP protein levels between the C-CUMS group and the C-Ctrl group(P>0.05),while LEPR protein level of the C-CUMS group was significantly higher than that of the C-Ctrl group(P<0.01).Conclusion CUMS can lead to increased appetite in mice,which may involve the functional regulation of LEPR and AgRP.After the stress cessation,the appetite decreases,which may involve the functional regulation of LEPR.
7.Predictive value of the percentage of Gleason pattern 4 in biopsy for adverse pathological features and biochemical recurrence after radical prostatectomy in ISUP grade group 2-3 non-metastatic prostate cancer

Objective:To assess the predictive value of the percentage of Gleason pattern 4 (G4%) in prostate biopsy for adverse pathology and biochemical recurrence.Methods:We retrospectively analyzed consecutive patients who underwent radical prostatectomy in our institution between January 2019 and December 2023, and included those who were diagnosed with ISUP 2-3 cancer at biopsy. A total of 109 patients were included in this study. The average age of patients was (67.40±6.11) years, and the average BMI of patients was (25.36±2.97) kg/m 2. 49 Cases (45.0%) had a PI-RADS score of 5, and the median prostate volume was 32.60 (24.57, 45.63) ml. The median of most recent tPSA before biopsy was 9.76 (6.89, 12.95) ng/ml, the median PSAD was 0.28 (0.17, 0.44) ng/ml 2, and the median f/tPSA was 0.11 (0.08, 0.16). Clinical T 2b or higher stage was found in 84 cases (77.1%). The total biopsy core length was (22.91±5.18) cm, with a median of 24 (20, 24) biopsy cores and a median of 6 (4, 9) positive cores. Gleason score 3+ 4 was found in 52 cases (47.7%), and Gleason score 4+ 3 in 57 cases (52.3%). Cribriform was present in 30 cases (27.5%). G4% was calculated based on the proportion of Gleason grade 4 tumor relative to total tumor, tumor proportion relative to total tissue, and tissue length. Patients were divided into high-G4% (≥2.45%) and low-G4% (<2.45%) groups based on the median G4% value, with 55 and 54 cases, respectively. No significant differences were observed in baseline characteristics between the two groups ( P>0.05). The main risk factor of adverse pathology was analyzed by logistic regression, and receiver operating characteristic (ROC) curve and area under curve (AUC) were performed. Patients were further stratified by the G4% cutoff value from ROC, and Kaplan-Meier survival curves were plotted to compare biochemical recurrence free survival (BCRFS) between groups. The main risk factor affecting BCRFS was analyzed by Cox regression. Adverse pathology was defined as postoperative Gleason score ≥4+ 3 or pathological stage ≥T 3a. Results:Adverse pathology occurred in 44 (80.0%) high-G4% and 16 (29.6%) low-G4% patients ( P<0.01). Multivariate analysis identified G4% as an independent risk factor for adverse pathology ( OR=1.23, 95% CI 1.02-1.50, P=0.033). The highest ROC AUC value was seen for G4% (0.799), significantly outperforming Gleason score (0.799 vs. 0.641, P=0.003), tPSA (0.799 vs. 0.615, P=0.003), PSAD (0.799 vs. 0.679, P=0.038), positive cores (0.799 vs. 0.677, P=0.009), clinical T stage (0.799 vs. 0.607, P=0.001) and cribriform (0.799 vs. 0.639, P=0.001). The G4% cutoff value for predicting biochemical recurrence was 10.97%. The median BCRFS was significantly higher in the low G4% (<10.97%) group than that in the high G4% (≥10.97%) group (55 vs. 28 months, P=0.002). Cumulative recurrence free survival rates at 1 and 3 years were 94.6% vs. 74.1% and 78.0% vs. 47.6%, respectively. Multivariate Cox regression analysis indicates that G4% was an independent risk factor affecting BCRFS ( HR=1.11, 95% CI 1.00-1.23, P=0.041). Conclusions:For patients with ISUP 2-3 nmPCa, a higher G4% in biopsy specimens demonstrates strong predictive ability for adverse pathology and biochemical recurrence, outperforming traditional clinical indicators such as Gleason score and PSA.
8.Epidemiological characteristics of hospital-associated infection in elderly patients in a three-A hospital from 2020 to 2024

OBJECTIVE To investigate the age-group differences and seasonal distribution patterns of epidemiologi-cal characteristics of hospital-associated infection in elderly inpatients in a three-A hospital.METHODS Based on the Xinglin Real-time Monitoring System,a retrospective analysis was conducted on clinical data of 67 157 patients hospitalized in the Air Force Hospital of Eastern Theater Command from Jan.2020 to Dec.2024.Elderly patients were divided into three age groups:young-age group(60-69 years),middle-age group(70-79 years)and ol-dest-age(≥80 years).The epidemiological characteristics and seasonal distribution patterns of the three groups were analyzed.RESULTS From 2020 to 2024,there were 1 524 cases of hospital-associated infection,with an inci-dence rate of 2.27%.The infection rate was higher in the oldest-age group(6.08%)than those in the young-age group(1.77%)and the middle-age group(2.52%)(P<0.001).The top three departments with the highest in-cidence rates of hospital-associated infection were the geriatric department(10.97%),intensive care unit(9.65%)and respiratory department(4.96%).The lower respiratory tract was the main site of infection,and the infection rate was higher in winter than in other seasons.The pathogens were predominantly gram-negative bacteria(60.35%),with Pseudomonas aeruginosa being the most frequently detected.CONCLUSIONS Advanced age,male gender and hospitalization in winter are key risk factors for hospital-associated infection.Seasonal pre-vention and control strategies should be implemented in departments with high incidence of respiratory infections,and monitoring of multidrug-resistant bacteria should be enhanced.
9.Research progress on bladder and urethral dysfunction in Parkinson′s disease

Parkinson′s disease (PD) is a neurodegenerative disorder prevalent among the elderly, characterized by the degeneration of dopaminergic neurons in the substantia nigra. The motor symptoms of PD include resting tremor, bradykinesia, rigidity, and postural gait disturbances. Additionally, non-motor symptoms such as depression, pain, cognitive impairment, sleep disorders, and autonomic dysfunction are gaining increasing attention. Among these, bladder and urethral dysfunction is one of the most common manifestations of autonomic dysfunction and significantly impacts patients′ quality of life in the later stages of the disease. This article provides a review of recent research on the epidemiology, pathogenesis, and treatment of bladder and urethral dysfunction in PD, aiming to enhance the understanding and management of urinary symptoms associated with PD.
10.Study on left atrial remodeling in patients with non-obstructive hypertrophic cardiomyopathy and atrial fibrillation using four-dimensional automatic left atrial quantification technology

Objective:To evaluate the left atrial structure and function in patients with non-obstructive hypertrophic cardiomyopathy(NOHCM)and atrial fibrillation(AF)using four-dimensional automatic left atrial quantification(4D Auto LAQ)technology,and to explore the correlation between left atrial remodeling and AF.Methods:Ninety patients with NOHCM treated in the First Affiliated Hospital of Zhengzhou University from May 2023 to September 2024 were prospectively selected and divided into AF group( n=27)and non-AF group( n=63)according to whether they had AF. Clinical data of all enrolled patients were collected,and two-dimensional,color Doppler,and tissue Doppler ultrasound parameters were measured. 4D Auto LAQ technology was used to obtain left atrial volume and strain parameters. Inter-group comparison,univariate and multivariate Logistic regression analysis,and repeatability test were used to analyze the data. Results:Compared with non-AF group,left atrial maximum volume index(LAVImax),left atrial minimum volume index(LAVImin)and left atrial pre-systolic volume index(LAVIpreA)were increased in the AF group,but left atrial total ejection fraction(LAEF),left atrial passive ejection fraction(LApEF),left atrial active ejection fraction(LAaEF)and the absolute values of left atrial reservoir longitudinal and circumferential strains(LASr,LASr-c),left atrial conduit longitudinal and circumferential strains(LAScd,LAScd-c)and left atrial contraction longitudinal and circumferential strains(LASct,LASct-c)were decreased(all P<0.05). Multivariate Logistic regression analysis showed that LAVImin was independently associated with AF( OR=0.837, P<0.001). Conclusions:4D Auto LAQ can quantitatively evaluate the left atrial structure and function in patients with NOHCM and AF,and LAVImin is independently associated with the presence of AF.
