The transendothelial transport of low-density lipoprotein(LDL)and its subendothelial accumulation represent a central pathogenic mechanism in atherogenesis. This process involves a variety of receptors and molecular mechanisms that work together to regulate the transport of LDL in endothelial cells and its accumulation in blood vessel wall.In addition to receptor-mediated transport mechanisms, physiological and pathological factors such as hormones, inflammatory mediators and metabolites also play an important role in it.Recent studies have further elucidated the critical regulatory roles of these factors in LDL transendothelial transport, suggesting that targeting LDL transcytosis may represent an effective strategy for the prevention of early atherosclerosis and the treatment of related vascular diseases. This review summarizes recent advances in this field and discusses their potential clinical applications.

Objective To investigate the therapeutic potential of human umbilical cord-derived mesenchymal stem cells(hUCMSCs)in modulating the cGAS-STING-NF-κB signaling pathway in chronic intermittent hypoxia(CIH)mice. Methods Twenty-four C57BL/6 mice were divided into the control group, the model group, the hUCMSCs group and the hUCMSCs+STING agonist(DMXAA)group, with 6 mice in each group.Except for the control group, the other groups were exposed to hypoxic conditions for 8 hours daily for a total of 8 weeks to establish the CIH mouse model. After 8 weeks, mice were anesthetized for cardiac blood collection followed by euthanasia and lung tissue collection.Serum levels of IL-6,TNF-α,IL-1β and IL-17A were measured by ELISA.Pulmonary inflammatory infiltration and collagen deposition were assessed by HE and Masson staining.E-Cadherin and α-SMA expression levels were evaluated by immunohistochemistry.Expression levels of cGAS,STING and NF-κB mRNA were detected by RT-qPCR,while protein expression levels of E-Cadherin,N-Cadherin,α-SMA,Vimentin,cGAS,STING and NF-κB were analyzed by Western blot assay. Results Compared with the control group, levels of IL-6,IL-1β,TNF-α and IL-17A increased in the model group, inflammation and fibrosis scores increased,mRNA expression levels of cGAS,STING and NF-κB increased, and protein expression levels of N-Cadherin,α-SMA,Vimentin,cGAS,STING and NF-κB increased. In contrast,E-Cadherin protein expression was significantly decreased(P<0.05).Compared with the model group,IL-6,IL-1β,TNF-α and IL-17A decreased in the hUCMSCs group,mRNA expression levels of cGAS,STING and NF-κB were decreased, protein expression levels of N-Cadherin,α-SMA,Vimentin,cGAS,STING and NF-κB were also decreased.Meanwhile,E-Cadherin protein expression was significantly increased(P<0.05).STING activator DMXAA reversed the protective effects of hUCMSCs in CIH mice(P<0.05).Conclusion Intravenous administration of hUCMSCs alleviates pulmonary inflammatory infiltration and epithelial-mesenchymal transition in mouse model of intermittent hypoxia, which may be related to the down-regulation of the cGAS-STING-NF-κBsignaling pathway.

This study systematically collated and textual researched realgar from aspects such as name, origin, and quality by consulting ancient herbal texts, classic medical books, and modern literature. The textual research shows that current herbal works all take"realgar"as the official name, with aliases including"Huangshi shi","Shihuang","Tianyang shi","Jiguan shi","Xunhuang","Chouhuang",and"Mingxiong",etc.Ancient herbal records indicate that Xunhuang,Chouhuang, and Shuiku Realgar are all Realgar, while Orpiment is its associated mineral. In ancient times, the main production areas of Realgar were in Gansu, with outputs also seen in Shandong and Hunan.Nowadays,it is mainly produced in Hunan,Guizhou,Hubei,Gansu,Yunnan,Sichuan and other regions.Ancient herbal works mentioned characteristics like"cockscomb color","non-stinky","solid","red and bright"in their quality evaluation, while modern herbal works mostly evaluate its quality by color and texture, such as"red color","large blocks","brittle texture","glossy".The traditional efficacy of Realgar is to dry dampness, kill insects, detoxify various poisons, treat sores and activate blood.Modern studies have shown it also has anti-tumor,antibacterial and antiviral effects.Processing methods in past dynasties included water grinding, vinegar processing, refining, etc.,and currently, water grinding and acid water grinding are commonly used. This paper observed the properties of Realgar, detected the content of As?S?,and analyzed the microscopic characteristics and far-infrared spectral characteristics of qualified batches of Realgar. It was finally found that the As?S? content of qualified batches of Realgar was all more than 90%;under the scanning electron microscope,it showed massive shape, uniform distribution, obvious particles, and no agglomeration;a small amount of associated mineral Orpiment crystals were observed under the polarizing microscope;the characteristic peaks of Realgar(343-344 cm?1,224-225 cm?1,372-374 cm?1,367-369 cm?1,359 cm?1,207-208 cm?1,193-194 cm?1,168-170 cm?1)and Orpiment(390 cm?1,380 cm?1,347 cm?1,311 cm?1,300 cm?1,201 cm?1,182 cm?1,158 cm?1 and 139 cm?1)were determined."Red color and glossy"can be used as property references,"Realgar is a sulfide mineral of the Realgar family, with the main chemical component As?S?,associated with Orpiment"can be used as origin references, and"Shimen in Hunan,Wanshan in Guizhou,Yunnan,Gansu,Sichuan"can be used as production area references, which are consistent with the results of herbal textual research. This study provides a basis for the identification and analysis of Realgar, with a view to better guiding clinical medication and resource utilization.

Objective To explore the diagnostic value of a wearable flexible patch ECG instrument in arrhythmia, the alarm situation during clinical application, patient satisfaction and safety. Methods A total of 1 443 subjects wore flexible patch ECG and conventional dynamic ECG(control)for 24 h to test the validity and consistency of arrhythmia diagnosis, and counted the alarm of remote ECG and the occurrence of related adverse events during the wearing of the instrument. Results There were 987 cases of arrhythmia detected by flexible patch ECG and 992 cases by conventional dynamic ECG.The total coincidence rate of arrhythmia diagnosis was 98.7%.The mean heart rate was measured by flexible patch ECG(75.4±11.4)times/min,conventional dynamic heart rate(71.5±12.1)times/min,the intra-group correlation coefficient(ICC)of 2 instruments was 0.892(95%CI:0.537-0.956),with good repeatability. The correct alarm rate of flexible patch ECG was 100%.The incidences of skin pruritus(0.28%vs.1.32%),skin allergy, redness and swelling(0.14%vs.0.69%)and electrode strip shedding(0 vs.0.28%)during wearing the flexible patch electrocardiogram were lower than those of the conventional holter electrocardiogram(P<0.05).Conclusion The flexible patch ECG has few adverse reactions, high comfort, good safety and clinical applicability.
