2.High serum cystatin C is an independent risk factor for poor renal prognosis in IgA nephropathy.

OBJECTIVES:
To explore the value of serum cystatin C (CysC) levels in evaluating renal prognosis in IgA nephropathy (IgAN) patients.
METHODS:
We retrospectively collected the clinical data of IgAN patients diagnosed by renal biopsy at Guangdong Provincial People's Hospital from January, 2014 to December, 2018. Based on baseline serum CysC levels, the patients were divided into high serum CysC (>1.03 mg/L) group and normal serum CysC (≤1.03 mg/L) group. The composite endpoint for poor renal prognosis was defined as ≥50% decline in estimated glomerular filtration rate (eGFR) and/or progression to end-stage renal disease (ESRD). Lasso regression, multivariate Cox regression and Kaplan-Meier survival analysis were used to identify the risk factors and compare renal survival rates between the two groups. Smooth curves fitting and threshold effect analysis were used to explore the relationship between serum CysC levels and the outcomes. A nomogram model was constructed and its predictive performance was evaluated using concordance index, calibration curve, receiver operating characteristic (ROC) curve and the area under curve (AUC).
RESULTS:
A total of 356 IgAN patients were enrolled, who were followed up for 4.65±0.93 years. The composite endpoint occurred in 74 patients. High serum CysC was identified as an independent risk factor for poor renal prognosis in IgAN (HR=2.142, 95% CI 1.222 to 3.755), and the patients with high serum CysC levels had a lower renal survival rate (Log-rank χ2=47.970, P<0.001). In patients with serum CysC below 2.12 mg/L, a higher CysC level was associated with an increased risk of poor renal prognosis (β=3.487, 95% CI: 2.561-4.413, P<0.001), while above this level, the increase of the risk was not significant (β=0.676, 95% CI: -0.642-1.995, P=0.315). The nomogram model based on serum CysC and 3 other independent risk factors demonstrated good internal validity with a concordance index of 0.873 (95% CI: 0.839-0.907) and an AUC of 0.909 (95% CI: 0.873-0.945).
CONCLUSIONS
Serum CysC levels are associated with renal prognosis in IgAN patients, and high serum CysC an independent risk factor for poor renal prognosis.

3.The predictive value of radiomics for postoperative prognosis of pancreatic ductal adenocarcinoma patients

Objective To explore the predictive value of radiomics for postoperative pancreatic fistula, infection, and long-term survival in patients with pancreatic ductal adenocarcinoma (PDAC). Methods 206 patients who received radical resection of pancreatic cancer in Zhongshan Hospital, Fudan University from January 2014 to December 2020 and were pathologically confirmed as PDAC after surgery were retrospectively selected, all of whom had complete surgical data and long-term follow-up data. Pyradiomics was used to analyze the enhanced CT images of all patients and extract radiomics features. LASSO dimensionality reduction combined with logistic regression analysis was used to construct a predictive model for pancreatic fistula and abdominal infection after PDAC surgery, and evaluating the model’s effectiveness using ROC curves. A long-term survival prediction model for PDAC patients was constructed using LASSO dimensionality reduction combined with Cox regression analysis, and patient risk scores were calculated. The patients were divided into high-risk and low-risk groups based on the median, and the survival curves were compared to evaluate the effectiveness of the model. The imaging omics features with the highest weight were divided into high expression group and low expression group according to the median, and the prognostic differences and clinical features were compared. Radiomics and clinical features were combined to analyze the influencing factors of long-term prognosis and construct a clinical imaging comprehensive model. Results A total of 1 595 radiomics features were extracted. A predictive model for pancreatic fistula and infection after PDAC surgery was constructed, with AUC values of 0.81 and 0.79, respectively. The PDAC long-term survival prediction model was successfully constructed, and the prognosis of the high-risk group was worse than that of the low-risk group (P<0.001). The weight of the radiomics feature “log-sigma-5-mm-3D_glszm_ZonePercentage” was 59.557. The CA19-9 level in the high expression group is higher than that in the low expression group (P=0.017), and there is a statistically significant difference in survival curves between the two groups (P=0.021). The comprehensive clinical imaging model suggested that age, AJCC stage, lymph infiltration, CA19-9 level and imaging characteristics were risk factors for long-term prognosis of PDAC patients (HR=1.028, 4.084, 2.566, 1.232 and 2.536). Conclusions The predictive model based on radiomics has good predictive performance for pancreatic fistula, infection, and long-term prognosis after PDAC surgery. Patients with high expression of the radiomics feature “log-sigma-5-mm-3D_glszm_ZonePercentage” face poorer prognosis.

The incidence of augmented renal clearance (ARC) in intensive care patients (ICU) is exceptionally high, and these patients are often co-morbid with infection. The occurrence of ARC will significantly increase the clearance rate of antibiotics, making it difficult for conventional doses to reach effective therapeutic concentrations and affect the patient's anti-infective treatment effect and prognosis. It can be seen that it is crucial to formulate a reasonable dosing regimen for ICU patients with ARC. Regrettably, few reports in China about the adjustment strategy of antibiotic dosing regimens for ARC patients. Therefore, this article reviews the domestic and foreign literature for reference to provide evidence for medical personnel to adjust the dose of antibacterial drugs for such patients.

Objective To study the effect of acupuncture on the effect of acupuncture on the learning and memory ability and inflammatory factor expression in rats with vascular dementia,to provides an experimental basis for acupuncture and treatment of vascular dementia.Methods Randomly dividing thirty-six SPF grade SD male rats into 12 rats in sham surgery(A),model(B),and acupuncture(C).Groups B and C will prepare the VD model,and Group A will only surgically isolated the bilateral common carotid arteries without ligation.In Group C,acupuncture with"Baihui","Shenshu"and"Fenglong"points were used for intervention,each course lasts for 6 days,for a total of 2 sessions;Group A and Group B were fed normally without intervention.Morris water maze experiment was used to assess spatial learning and memory,ELISA for serum IL-1β and TNF-α,Real-time PCR for TLR-4,MyD88 and NF-κB mRNA,positive expression of Iba1 and NF-κB,and Western blot for relative expression levels of IL-1β and TNF-α in hippocampus.Results Compared with group B,group C rats had a shorter escape latency period(P<0.01)and had crossed the platform more often(P<0.05);The serum concentrations of TNF-α and IL-1β were significantly decreased(P<0.01);The expression amount of mRNA of TLR-4/MyD88/NF-κB was significantly reduced in the hippocampus(P<0.01);The positive expression of Iba1 and NF-κB protein in the brain was significantly reduced(P<0.01);The relative expression level of TNF-α and IL-1β in the hippocampus was significantly reduced(P<0.01).Conclusion Needle"Baihui","Shenshu"and"Fenglong"can improve vascular dementia rats cognitive dysfunction,and the mechanism may be related to the inhibition of neuroinflammatory response by inhibiting TLR-4/MyD88/NF-κB signaling pathway activity.
6.Total electric field intensity in workplace of high-voltage direct current converter stations

Background The converter stations of high-voltage direct current (HVDC) transmission lines generate special total electric fields. At present, few investigations have been conducted on total electric fields in the workplace of converter stations from an perspective of occupational health. Objective To understand the current situation of total electric field strength in the workplace of converter stations. Methods Using purposive sampling, a calibrated HDEM-1 direct current (DC) total electric field strength measurement system was used to measure the total electric fields of 12 converter stations serving 6 DC lines in Southeast and Southwest China according to the Measurement method for total electric field strength and ion current density of the converter stations and DC transmission lines (DL/T 1089—2008). The results were evaluated according to occupational exposure limits recommended by The limits of electromagnetic environment at ±800 kV UHV DC converter station (DL/T 275—2012), the American Conference of Governmental Industrial Hygienists (ACGIH), and the International Commission on Non-Ionizing Radiation Protection (ICNIRP). Results A total of 615 check points were planned, the total electric field strength was 0.05-37.05 kV·m−1, and the median was 10.45 kV·m−1. The total electric field strength of 39 check points (6.3%) exceeded 25 kV·m−1 (the limits of ACGIH and ICNIRP), and the total electric field strength of 12 check points (2.0%) exceeded 30 kV·m−1 (the limit of DL/T 275—2012). There were statistically significant differences in the total electric field strength values and the proportions of exceeding 25 kV·m−1 between the neutral regions and the positive regions and between the neutral regions and the negative regions (P < 0.01). The proportion of total electric field strength exceeding 30 kV·m−1 in the negative regions was higher than that in the positive regions (P < 0.01). There were no significant differences in the total electric field strength of converter stations at different voltage levels and different altitudes (P > 0.05). There were no significant differences in the proportions of total electric field exceeding 25 kV·m−1 and exceeding 30 kV·m−1 in converter stations at different voltage levels and different altitudes (P > 0.05). Conclusion The total electric field in some workplace of converter stations exceeds selected limits. Converter station operators may be exposed to high-strength total electric field for a short time.
7.Predictive value of systemic immune-inflammation index (SII) for short-term prognosis in patients with sepsis complicated with acute kidney injury receiving CRRT

Objective:To evaluate the predictive value of systemic immune inflammation index (SII) for short-term prognosis in patients with sepsis complicated with acute kidney injury who received continuous renal replacement therapy (CRRT) .Methods:From Feb. 2018 to Jan. 2022, 90 patients with sepsis complicated with acute kidney injury who received CRRT in Hangzhou Cancer Hospital were included as the research objects. According to the survival of patients within 90 days, they were grouped into a survival group of 60 cases and a death group of 30 cases. The clinical baseline data such as age, sequential organ failure assessment (SOFA) score, acute physiological function and chronic health status scoring system II (APACHE II), albumin (ALB) were recorded. Automatic blood cell analyzer was applied to detect lymphocyte count, platelet count and neutrophil count in peripheral venous blood of patients with sepsis complicated with acute kidney injury who received CRRT, and the SII was calculated; Pearson correlation analysis was performed to analyze the correlation between SII and clinical baseline data in dead patients with sepsis complicated with acute kidney injury receiving CRRT; ROC curve was drawn to analyze and compare the predictive value of SII, SOFA score and APACHEII for death in patients with sepsis complicated with acute kidney injury receiving CRRT; Kaplan-Meier curve was performed to describe the changes in patient survival; multivariate Cox regression analysis was performed to analyze the factors affecting 90-day death in patients with sepsis complicated with acute kidney injury receiving CRRT.Results:SII (2636.74), C-reactive protein (CRP) [ (92.45±29.74) mg/L], SOFA score (12.83±3.24) and APACHEII (28.30±7.51) in the death group were higher than those in the survival group [341.92, (50.24±16.13) mg/L, 10.00±3.12, 25.13±6.87], and ALB [ (2.50±0.53) g/dl] was lower than that in the survival group [ (2.79±0.61) g/dl] ( P<0.05). SII was significantly positively correlated with CRP, SOFA score and APACHEII, and significantly negatively correlated with ALB in patients with sepsis complicated with acute kidney injury who received CRRT ( P<0.05). The areas under the curve (AUC) of SII, SOFA score, and APACHEII for predicting death in patients with sepsis complicated with acute kidney injury who received CRRT were 0.936, 0.827, and 0.736, respectively, and AUC of SII prediction was greater than that of SOFA score and APACHEII ( P<0.05). The 90-day survival rate of patients with sepsis complicated with acute kidney injury who received CRRT in the high SII group (23/46, 50.00%) was lower than that in the low SII group (37/44, 84.09%) ( P<0.05). SII was an independent risk factor for death within 90 days in patients with sepsis complicated with acute kidney injury who received CRRT ( P<0.05) . Conclusion:SII can better predict the 90-day mortality risk of patients with sepsis complicated with acute kidney injury receiving CRRT, and assist clinical assessment of short-term prognosis.
8. Current status of occupational exposure to power frequency electromagnetic field in converter stations

OBJECTIVE: To analyze the current status of occupational exposure to power frequency electromagnetic field in converter stations. METHODS: Eight converter stations with voltage levels of ±500 kV and ±800 kV within normal operation were selected as the research subjects using the typical sampling method. Power frequency electric field and power frequency magnetic field strengths were measured and calculated according to the GBZ/T 189.3-2018 Measurement of Physical Agents in Workplace--Part 3: Electric Field and Magnetic Field between 1 Hz and 100 kHz. The GBZ 2.2-2007 Occupational Exposure Limits for Hazardous Factors in the Workplace--Part 2: Physical Factors were used to evaluate whether the power frequency electric field strength exceeds the regulatory limit(the occupational exposure limit of power frequency electric field in 8 hours workplace is 5.000 kV/m). Meanwhile, the test results were evaluated according to the short-term occupational exposure limit of 50 Hz electric field and magnetic field recommended by the International Committee on Nonionizing Radiation Protection in 2010 that are 10.000 kV/m and 1 000.00 μT. RESULTS: The power frequency electric field and magnetic field strengths of 582 working environment detection points were measured. The median and 0-100 th percentile of power frequency electric field and power frequency magnetic field strength were 4.342(0.001-12.003) kV/m and 5.51(0.10-186.90) μT, respectively.The exceeding standard rate of power frequency electric field strength in converter station workplaces was 37.8%(220/582), which concentrated in 500 kV alternating current filter area and 500 kV alternating current field area. Among them, 5 detection points had power frequency electric field strength exceeding 10.000 kV/m. The magnetic flux density of all the detection points did not exceed 1 000.00 μT. The power frequency electric field strength in ultra-high voltage region was higher than that in high voltage region(P<0.01). There was no significant difference in power frequency magnetic field strength(P>0.05). There was no significant difference in power frequency electric field and magnetic field between rectifier stations and inverter stations(P>0.05). The 8 hours time weighted average(TWA) value of power frequency electric field strength of 8 converter station operators was 1.044-2.335 kV/m, which did not exceed the occupational exposure limit. CONCLUSION: The converter station operators might be exposed to excessive power frequency electric fields for a short time, but the 8 hours TWA value of the power frequency electric field meets the requirements of standards, and the power frequency magnetic field exposure strength also meets the requirements of the relevant standards.
9.Bilateral facetectomy combined with pedicle screw fixation and interbody fusion in the treatment of bilateral lumbar foraminal stenosis in 41 cases

Objective To explore the clinical efficacy in bilateral lumbar foraminal stenosis (LFS) after treatment with bilateral facetectomy combined with pedicle screw fixation and interbody fusion.Methods A total of 41 cases of patients with bilateral LFS underwent bilateral facetectomy combined with pedicle screw fixation and interbody fusion from February 2010 to August 2013 in Department of Spine Surgery,the People's Hospital of Tianjin City,were retrospectively analysed.The clinical efficacy was assessed by Oswestry disability index (ODD questionnaire and visual analogue scale (VAS) before and after operation,anterior and posterior disc height and L1-S1 angle were measured as well.Then the ODI and VAS scores,and changes in anterior and posterior disc height and L1-S1 angle were calculated at the time of the last patient follow-up visit.Results All 41 patients were followed up for 12 to 36 months,with an average of (26.2±2.4) months.Compared with preoperation,at the time of the last follow-up visit the back pain VAS score,leg pain VAS score and ODI were decreased,while the anterior and posterior disc height were increased,there were statistically significant differences (P<0.05).No statistically significant difference was found in L1-S1 angle between preoperation and postoperation (P>0.05).Conclusion The short-term clinical curative effect of posterior bilateral facetectomy combined with pedicle screw fixation and interbody fusion in the treatment of bilateral LFS is satisfactory.
10.Research progress in lncRNAs and the diagnosis, prognosis, and drug resistance of gastric cancer

Long non-coding RNAs (lncRNAs) are noncoding RNAs with lengths exceeding 200 bp. These lncRNAs cannot code complete proteins but control gene expression. Recent studies have shown that lncRNAs are closely related to the diagnosis, prognosis, and drug resistance of gastric cancer. This article summarizes the recent progress of studies in China and abroad on lncRNAs relative to the diagnosis, prognosis, and drug resistance of gastric cancer.
