1.The relationship between variant angina pectoris syncope and coronary artery spastic targeted location, arrhythmia and coronary artery stenostic lesion

Objective:Investigation of the relationship between variant angina pectoris syncope and coronary artery spastic targeted location, arrhythmias, and coronary artery stenostic lesion.Methods:This study combined retrospective and prospective registry approaches. Data were sourced from the case database of Henan province "Multicenter Clinical Observation Study of Variant Angina Pectoris". A total of 507 patients with variant angina pectoris who had complete records from June 1980 to December 2022 were consecutively enrolled. Select patients among them who experienced syncope, and analyze the target vessel sites of coronary artery spasm, arrhythmias during variant angina pectoris attacks, and the degree of stenosis in coronary artery lesions.Results:Among 507 variant angina pectoris patients, 88 experienced syncope. Age was (53.9±9.7) years and 66 patients (75.0%) were male. Forty patients (45.5%, 40/88) were aged 50-59 years. The incidence of syncope in variant angina pectoris caused by left anterior descending artery (LAD) spasm, right coronary artery (RCA) spasm, and multivessel coronary artery spasm was 7.4% (15/202), 22.7% (42/185), and 23.6% (25/106), respectively. The latter two were significantly higher than those in the LAD group ( P all<0.05). Among 77 patients with variant angina pectoris syncope, definitive electrocardiogram recordings were available during syncope episodes. All patients exhibited arrhythmias during syncope: 34 cases involved tachyarrhythmias and 43 cases involved bradyarrhythmias. The incidence of rapid arrhythmias in patients with LAD, RCA, and multi-vessel spasm syncope was 72.7% (8/11), 24.3% (9/37), and 54.2% (13/24), respectively, with P<0.05 for the first two. Bradyarrhythmias occurred in 27.3% (3/11) of LAD, 75.7% (28/37) of RCA, and 45.8% (11/24) of multivessel coronary artery spasm syncope cases, with the first two showing P<0.05. Coronary angiography analysis of 56 syncope patients revealed target vessel locations and stenosis severity: 12 patients had LAD lesions and 41 had RCA lesions, stenosis ≥50% occurred in 66.7% (8/12) and 43.9% (18/41) of these lesions, respectively ( P>0.05). Conclusions:Variant angina pectoris syncope predominantly affects middle-aged males. Bradyarrhythmias triggered by RCA spasm are a common cause, while the incidence of syncope shows no significant correlation with the degree of coronary artery stenostic lesion, whether in the LAD or the RCA.

This consensus was developed by the Orthodontic Society of the Chinese Stomatological Association to provide a systematic, scientific, and practical guideline for informed consent in orthodontic care. Orthodontic treatment is typically lengthy, highly individualized, and involves multiple factors such as growth and development, occlusal function, and facial esthetics. Rapid technological advances and diverse risk profiles make the traditional reliance on orthodontist experience or institutional templates insufficient to ensure patients′ full understanding and autonomous decision-making. To address this, the expert panel conducted extensive reviews of domestic and international guidelines, analyzed representative dispute cases, and performed multicenter patient-clinician surveys. Using a multi-round Delphi method, the group established a standardized informed consent framework covering the initial consultation, treatment, and retention phases. The consensus emphasizes that informed consent is not only a fundamental legal and ethical requirement but also a key step in building trust, improving patient compliance, and enhancing treatment satisfaction. Orthodontists should clearly and comprehensively explain treatment plans, potential risks, uncertainties, and associated costs, while respecting the autonomy of patients or guardians, and maintain continuous communication and dynamic evaluation throughout the treatment process. The release of this consensus provides unified and authoritative guidance for clinical orthodontics, helping to standardize informed consent, enhance its transparency, safeguard patient rights, reduce medical risks, and promote high-quality, sustainable development of orthodontic practice.
3.Relationships of serum soluble triggering receptor expressed on myeloid cells-2,P-selectin and length of hyperdense middle cerebral artery sign with effect of thrombolysis and short-term prognosis in patients with acute cerebral infarction

Objective To analyze the relationships of serum soluble triggering receptor ex-pressed on myeloid cells-2(sTREM2),P-selectin and the length of hyperdense middle cerebral arter-y sign(HMCAS)with the effect of thrombolysis and short-term prognosis in patients with acute cere-bral infarction(ACI).Methods A total of 109 ACI patients with thrombolytic therapy were divided into ineffective group(n=27)and effective group(n=82)based on thrombolysis outcomes.Ninety days after treatment,the 109 patients were further divided into good prognosis group(n=78)and poor prognosis group(n=31)according to the modified Rankin Scale(mRS).General information,the National Institutes of Health Stroke Scale(NIHSS)score,serum sTREM2 level,P-selectin levels,and HMCAS length were compared between the two groups.Binary Logistic regression model was used to analyze the influencing factors of thrombolysis outcomes in ACI patients.Pearson correlation coef-ficient was calculated to analyze the correlation between laboratory indicators and mRS in ACI pa-tients.Receiver operating characteristic(ROC)curve was plotted to analyze the predictive values of serum sTREM2,P-selectin and HMCAS length for prognosis of patients.Results The NIHSS score at admission in the effective group was significantly lower than that in the ineffective group(P<0.01).The serum sTREM2 and P-selectin levels as well as the HMCAS length in the effective group were significantly lower or shorter than those in the ineffective group(P<0.01).The results of bina-ry Logistic regression analysis showed that NIHSS score>15(OR=2.649,95%CI,1.357 to 5.167),serum sTREM2 above the mean level(OR=2.686,95%CI,1.860 to 8.786),P-selectin a-bove the mean level(OR=4.043,95%CI,1.860 to 8.786),and HMCAS length>1 mm(OR=3.827,95%CI,1.740 to 8.414)were all risk factors for poor thrombolysis outcomes in ACI pa-tients(P<0.01).The serum sTREM2 and P-selectin levels as well as the HMCAS length in the good prognosis group were significantly lower or shorter than those in the poor prognosis group(P<0.01).Serum sTREM2,P-selectin and HMCAS length were all positively correlated with mRS in ACI patients after thrombolytic therapy(r=0.686,0.597,0.662,P<0.001).Areas under the curve(AUCs)for predicting the prognosis of ACI patients after thrombolytic therapy by serum sTREM2,P-selectin,HMCAS length,and the combination of the three indicators were 0.833,0.862,0.889 and 0.914 respectively,with sensitivities of 0.806,0.742,0.806 and 0.742 re-spectively,and specificities of 0.923,0.897,0.936 and 0.949,respectively.Conclusion The effect of thrombolysis in ACI patients is related to NIHSS score at admission,serum sTREM2 level,P-selectin level,and HMCAS length.High levels of serum sTREM2 and P-selectin and a long HMCAS may increase the risk of poor thrombolysis outcomes in ACI patients.

6.Customized Design and Three-Dimensional Molding of Bracketless Orthodontic Appliance

Objective A finite element simulation analysis of a bracketless orthodontic appliance was carried out to determine the correction amount under different working conditions.A design scheme for the bracketless orthodontic appliance was also formulated to create an orthodontic appliance formed by the fused deposition process with a personalized orthodontic effect.Methods Combined with the computed tomography data of the patient,a tooth model was reversely reconstructed.The correction amount of the tooth under tilting,twisting,and translation working conditions was established using the finite element method.Materials with different elastic moduli were chosen to make three-dimensional(3D)shaped bracketless orthodonitc appliance,and the orthodontic force was measured.Results Different correction amounts could be designed according to different working conditions so that the orthodontic appliance had a personalized treatment effect.The orthodontic force of the bracketless orthodontic appliance increased with elastic modulus and thickness.Under the given working conditions,the orthodontic force was minimal(90 mN)when the orthodontic material was at the minimum(90 mN).The orthodontic force reached its maximum value when the orthodontic material was at the maximum(61.66 N).Conclusions According to the patient's tooth condition and the size of the orthodontic force required for each step of correction in the whole correction process,the design should consider bracketless orthodontics with different elastic moduli and thicknesses to treat deformed teeth to realize the effect of staged orthodontic correction.
7.Expression and clinical value of miR-21 in septic acute respiratory distress syndrome

Objective:To investigate the expression level of miR-21 in patients with septic acute respiratory distress syndrome, and analyze its correlation with inflammatory factors and its value in evaluating clinical prognosis.Methods:Patients with sepsis admitted to the intensive care unit (ICU) of the First Affiliated Hospital of Hebei North University from October 2020 to March 2022 were prospectively recruited. The patients were divided into an ARDS group and a non-ARDS group according to whether ARDS occurred within 3 days after admission. The ARDS group was divided into a low-risk group and a high-risk group according to the oxygenation index, and the ARDS group was divided into a survival group and a death group according to their 28-day survival status. In addition, 50 healthy volunteers who underwent physical examination during the same period were included in the control group. The expression levels of serum miR-21 and inflammatory factors [tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6)] were compared in each group at admission. To analyze the relationship between miR-21 expression and inflammatory factors and explore the value of miR-21 in predicting the prognosis of sepsis patients with ARDS.Results:A total of 106 patients with sepsis were included. Compared with the healthy control group, the levels of serum miR-21, TNF-α and IL-6 in the ARDS group and non-ARDS group were significantly increased (all P<0.05). Compared with the low-risk group, the levels of serum miR-21, TNF-α and IL-6 in the high-risk group were significantly increased (all P<0.05). Compared with the survival group, the serum miR-21 expression level and TNF-α and IL-6 levels in the death group were significantly increased (all P<0.05). Pearson correlation analysis showed that the serum miR-21 expression level in the ARDS group was positively correlated with IL-6 and TNF-α levels ( r=0.842, 0.697, P<0.001). ROC curve analysis showed that the AUC of serum miR-21 in predicting death from sepsis complicated with ARDS was 0.876 (95% CI: 0.761-0.942), the best cut-off value was 2.25, and its sensitivity and specificity were 92.9% and 71.7%, respectively. Conclusions:The expression level of serum miR-21 is significantly increased in patients with sepsis ARDS, is positively correlated with the level of inflammatory factors, is closely correlated with the severity and prognosis of the disease, and has great value in evaluating the prognosis of patients with sepsis ARDS.
8.Study on the Effects of Biyuanshu Oral Solution on mRNA Expression of IFN-γ and Immune Checkpoint B7-H1/ PD-1 of Nasal Sinus Mucosa in CRS Model Mice

OBJECTIVE:To investigate the effects of Biyuanshu (BYS)oral solution on IFN-γ of chronic rhinosinusitis (CRS)model mice ,and to investigate its potential mechanism on the basis of B 7-H1/PD-1 immune checkpoint. METHODS :Male C57 mice were randomly divided into normal group ,sham operation group ,chemical medicine control group (clarithromycin,103 mg/kg),BYS low-dose ,medium-dose and high-dose groups (BYS oral solution ,3.1,6.2,12.4 mL/kg),with 20 mice in each group. Except for normal group without any treatment ,other mice were all open maxillary sinus ,sham operation group was not filled with sponge with bacteria ,while model group and administration groups were filled with sponge with bacteria to induce CRS model. Since 8th week after modeling ,normal group ,sham operation group and model group were given normal saline 0.2 mL intragastrically,administration groups were given relevant medicine intragastrically ,once a day ,for consecutive 14 d. The nasal symptoms and general condition of mice were observed ,and the pathological changes of mice ’s nasal sinus mucosa were observed by HE staining ;qRT-PCR was used to measure the mRNA expression of IFN-γ,B7-H1 and PD- 1 in nasal sinus mucosa of mice. RESULTS:The normal group and sham operation group had no abnormal in nose ,and the epithelium and cilia of the nasal sinus mucosa were intact ;there was no significant difference f8y3j0127@163.com in the relative mRNA expression of IFN-γ,B7-H1 and PD- 1 between 2 groups(P>0.05). In model group ,the mice were found to have runny n ose,frequent scratching and sneezing ,a small amount of yellow secretion in the nasal cavity ,and serious depilation ;the nasal sinus mucosa was seriously damaged ,cilia was exfoliated ,and the gland in the submucosa was hyperplasia ,lymphocyte infiltration was also found ;the relative mRNA expression of IFN-γ,B7-H1 and PD- 1 were significantly increased compared with normal group (P<0.01). Compared with model group,the nasal symptoms ,general condition and pathological changes of the nasal sinuses in each administration group were improved in varying degrees ;the relative mRNA expression of IFN-γ and B7-H1 in chemical medicine control group ,BYS medium-dose and high-dose groups ,as well as the relative mRNA expression of PD- 1 in administration groups were decreased significantly;above indexes of BYS medium-dose and high-dose groups were significantly lower than BYS low-dose group ,while relative mRNA expression of IFN-γ in BYS high-dose group were significantly higher than BYS medium-dose group. The relative mRNA expression of IFN-γ in BYS low-dose and medium-dose groups,the relative mRNA expression of B 7-H1 in BYS low-dose group,the relative mRNA expression of PD- 1 in BYS groups were significantly higher than chemical medicine control group ; mRNA expression of IFN-γ in BYS high-dose group was significantly higher than chemical medicine control group(P<0.05 or P< 0.01). Above indexes of BYS medium-dose group were similar to those of chemical medicine control group (P>0.05). CONCLUSIONS:BYS oral solution can improve chronic inflammation in nasal sinus mucosa of mice ,the mechanism of which may be associated with intervening mRNA overexpression of B 7-H1/PD-1 by inhibiting mRNA expression of IFN-γ.
9. Diagnostic value of serum chitinase-3-like protein 1 in chronic liver disease of significant fibrosis and cirrhosis

Objective:
To study the correlation between the level of serum Chitinase-3-like protein 1 (CHI3L1) and the significant liver fibrosis and liver cirrhosis in patients with chronic liver disease, and to evaluate its diagnostic value.
Methods:
165 patients with chronic liver disease were selected, liver histopathological examination was performed to detect serum CHI3L1 concentration. Four indexes of hepatic fibrosis (type III procollagen, collagen IV, laminin, hyaluronic acid), aspartate aminotransferase/platelet ratio index (APRI) and FIB-4 (fibrosis- 4) scores were based on the pathological findings of liver biopsy and compared the advantages and disadvantages of serum CHI3L1 with other methods for the diagnosis of hepatic fibrosis and liver cirrhosis. A multivariate regression analysis model was created, and receiver operating characteristic curve was analyzed.
Results:
The level of serum CHI3L1 increased with increase of fibrosis stage and was highest in liver cirrhosis stage. In the period of S0 to 1, the levels of S2 to 3 and S4 were 62.82 (41.40 ~ 87.20), 70.94 (48.47 to 122.60) and 141.06 (78.18 ~ 197.40), and there were statistically significant differences between the groups (
