Objective To analyze the efficacy, safety, and learning curve of the three-lobe holmium laser enucleation of the prostate(HoLEP)in the treatment of benign prostatic hyperplasia(BPH)in a county-level hospital.Methods A retrospective analysis was conducted on the clinical data of 65 BPH patients who underwent the three-lobe HoLEP performed by a single surgeon at the Department of Urology, Yining County People's Hospital, during Dec.2023 and Jun.2024.The enucleation efficiency was calculated by dividing the weight of the enucleated prostatic tissue by the enucleation time.A case scatter diagram of enucleation efficiency was plotted according to the chronological order of the operations, and the learning curve was analyzed after fitting.Based on the inflection points of the learning curve, the learning process was divided into the initial learning, mastery, and proficiency phases.The basic clinical data, perioperative indicators, postoperative complications, and follow-up indicators were compared among the different learning phases.Results All 65 procedures were successfully completed.Marked by the enucleation efficiency reaching a plateau, cases 1-20 were defined as the initial learning phase, cases 21-40 as the mastery phase, and cases 41 onwards as the proficiency phase.The prostate volume in the proficiency phase was significantly larger than that in the initial learning and mastery phases(both P＜0.05).There were no statistically significant differences in other baseline characteristics among the three groups(all P＞0.05).The operation time [(105.50±19.12)min vs.(85.25±26.92)min vs.(69.00±23.58)min] and enucleation time [(76.90±14.19)min vs.(63.70±22.24)min vs.(48.80±20.48)min] showed a decreasing trend across the three groups(all P＜0.05).The enucleation efficiency in both the mastery and proficiency phases was significantly higher than that in the initial learning phase [(1.16±0.44)g/min vs.(0.85 ±0.25)g/min, P＜0.05;(1.36±0.49)g/min vs.(0.85±0.25)g/min, P＜0.05].The enucleation efficiency in the proficiency phase was slightly higher than that in the mastery phase, but the difference was not statistically significant(P= 0.389).There was no significant difference in the incidence of perioperative complications among the three groups(all P＞0.05).At the 6-month follow-up, the international prostate symptom score(IPSS), post-void residual(PVR), and maximum urinary flow(Qmax)were significantly improved compared to preoperative values in all three groups(all P＜0.05);however, no significant differences were observed among the three groups(all P＞0.05).Conclusion The three-lobe HoLEP for the treatment of BPH is safe and effective in a county-level hospital setting.Surgeons with some experience in endoscopic surgery can preliminarily master this technique after a learning period of approximately 20 procedures.

To investigate the immune protective effect induced by superoxide dismutase(SOD)of the Toxocara canis(T.canis)in mice,in this experiment,seventy-five Kunming mice of six-week-age were randomly divided into 5 groups for the immune protection experiment,the mice were subcutaneously immunized with 50,75 and 125 mg/L Tc-SOD(0.1 mL)on days 0,14 and 28,re-spectively.In the blank group,mice did not receive any treatment,while the PBS group was injected with equal amounts of sterility PBS.A total of three immunizations and each immunization interval of 14 days.14 days after the third immunization,the 3 000 infected worm eggs were given orally in all groups.Before each immunization,14 days after the third immunization,and after the T.canis attack on the 7th day,the blood was sampled from the tail vein and sera were separated,while the IgG levels in serum were detected by indirect ELISA.The proliferation ability of splenic lympho-cytes and the mRNA expression levels of Th1(IFN-γ,IL-2)and Th2 cytokines(IL-4,IL-10)were analyzed with CCK-8 and qRT-PCR.The larvae reduction rates were calculated on the 7th day after the T.canis attack,and the pathological changes in the livers and lungs were observed using H&E staining.The results showed that compared with the PBS group,the serum IgG antibody levels increased with the frequency and duration of Tc-SOD immunization.It remained at a high level af-ter the T.canis attack,and the differences were very significant(P＜0.001).The mRNA expres-sion levels of IFN-γ,IL-2,IL-4,and IL-10 were dramatically increased(P＜0.01),while IL-4 andIL-10 were significantly higher than IFN-γ and IL-2 on the 14th day after the third immunization(P＜0.05),showing the Th2 type predominant immune response.And induce the proliferation of spleen lymphocytes in vitro(P＜0.01).The reduction rates of larvae of 50,75 and 125 mg/L Tc-SOD immune groups were 18.7％,24.9％and 37.6％,respectively,with significant differences in the 125 mg/L Tc-SOD group(P＜0.05),and had better immune protection effect.Moreover,the H&E staining results indicated that three Tc-SOD immune groups reduce the inflammatory cell infiltration(neutrophils,eosinophils,and macrophages)and hemorrhagic lesions in the livers and lungs of mice.The above results indicated that Tc-SOD could induce humoral and cellular immune responses in mice,mainly Th2 immune response,and provide immune protection against T.canis infection.

Objective:To evaluate left ventricular function and myocardial ischemia in patients with different degrees of left anterior descending（LAD）coronary artery stenosis by using post-stress regional myocardial work（MW）and coronary flow reserve（CFR），and to analyze the diagnostic efficacy of each parameter for different degrees of stenosis in the LAD.Methods:A retrospective analysis was conducted on 76 patients with suspected coronary artery disease who underwent coronary angiography and attended the Fourth Affliiated Hospital of Harbin Medical University from January 2023 to June 2024. According to the degree of LAD stenosis，30 cases were categorized into the mild stenosis group（stenosis < 50%），25 cases in the moderate stenosis group（50%≤stenosis < 70%），and 21 cases in the severe stenosis group（70% ≤ stenosis ≤ 90%）. Transthoracic Doppler echocardiography was applied to obtain CFR LAD. Longitudinal strain in the left anterior descending region（LS LAD），peak strain dispersion（PSD），left anterior descending region work index（WI LAD）and left anterior descending region work efficiency（WE LAD）were obtained at rest and under stress，respectively. The LAD regional MW indexes were compared between the groups in two pairs. The correlation between CFR LAD and the degree of LAD stenosis was analyzed. ROC curves were plotted to compare the diagnostic efficacy of LAD regional MW parameters and CFR LAD for moderate and severe LAD stenosis. Results:①At rest：compared with the mild stenosis group，the absolute value of LS LAD，WI LAD，and WE LAD were reduced in the severe stenosis group，and the differences were statistically significant（all P < 0.05）. Compared with the moderate stenosis group，WE LAD was reduced in the severe stenosis group，and the difference was statistically significant（ P < 0.05）. Differences in two-by-two comparisons between groups in the resting state for the remaining parameters were not statistically significant（all P > 0.05）. Under stress：compared with the mild stenosis group，the absolute values of LS LAD，WI LAD，and WE LAD were reduced in the moderate and severe stenosis groups，and the PSD was increased in the severe stenosis group，and the differences were statistically significant（all P < 0.05）. Compared with the moderate stenosis group，the absolute values of LS LAD，WI LAD，and WE LAD were reduced in the severe stenosis group，and the differences were statistically significant（all P < 0.05）. Differences between two-by-two comparisons between groups in the remaining parameters at loading state were not statistically significant（all P > 0.05）. ②CFR LAD was negatively correlated with the degree of LAD stenosis（ r = -0.809， P < 0.05）. ③The ROC curves showed that the peak WE LAD of the regional MW parameters predicated the largest area under the ROC curve（AUC）for moderate and severe stenosis in LAD，0.803 and 0.897，respectively，with cutoff values of 92.5% and 91.5%. The cutoffs of CFR LAD ≤ 2.5 and CFR LAD ≤ 2.0 predicted the greatest AUC for moderate and severe stenosis in LAD at 0.743 and 0.901，respectively. Peak WE LAD combined with CFR LAD predicted the greatest AUC for moderate and severe stenosis in LAD at 0.826 and 0.967，respectively. Conclusions:Regional MW parameters and CFR LAD can sensitively detect left ventricular hypo-function in patients with different degrees of stenosis in LAD. Peak WE LAD and CFR LAD are reliable predictors of moderate and severe stenosis in LAD. A combination of the two techniques can significantly improve diagnostic efficacy.

Purpose To explore the clinical value of real-time three-dimensional automatic left atrial quantification technology in evaluating left atrial volume and function in heart failure with preserved ejection fraction(HFpEF).Materials and Methods A total of 65 patients diagnosed as HFpEF at the Fourth Affiliated Hospital of Harbin Medical University from December 2021 to October 2023 were prospectively enrolled.The control group included 65 healthy subjects who underwent ultrasound examination during the same period and were matched with the HFpEF group in terms of age and gender.According to the New York Heart Association(NYHA)cardiac function classification,patients with NYHA grade Ⅰ+Ⅱ were classified into the HFpEF group A,and those with grade Ⅲ+Ⅳ into the HFpEF group B.Relevant clinical data,conventional ultrasound parameters and three-dimensional ultrasound parameters were recorded in both the HFpEF group and the control group.Left atrial volume parameters,longitudinal strain parameters and circumferential strain parameters were analyzed.The area under the receiver operating characteristic curve was used to compare the diagnostic efficacy of left atrial functional parameters for HFpEF.Results Compared with the control group,the HFpEF group exhibited significant abnormalities in cardiac structure and function.Specifically,left ventricular posterior wall thickness,interventricular septal thickness at end-diastole,and mean E/e′ were significantly increased(t=-5.127,-5.886,-16.670,all P<0.05),while the absolute value of left ventricular global longitudinal strain(LVGLS)and septal and lateral mitral annular e′ were significantly decreased(t=-17.092,40.279,45.412,all P<0.05).All left atrial volume parameters were significantly increased,whereas left atrial functional and strain parameters were significantly decreased(t=-13.632-6.912,all P<0.05).Compared with HFpEF group A,HFpEF group B showed lower left atrial total emptying fraction,left atrial expansion index,left atrial contraction strain and absolute value of LVGLS(t=2.062,3.545,-2.189,-2.586),as well as a higher left atrial minimum volume(t=-2.187),respectively(all P<0.05).Left atrial reservoir strain was negatively correlated with N-terminal pro-B-type natriuretic peptide and mean E/e′(r=-0.395,-0.626,both P<0.05),and positively correlated with the absolute value of LVGLS(r=0.602,P<0.05).The LASr and LAEI had high predictive value for HFpEF,with area under the curve of 0.898 and 0.817,cut-off values of 20.5%and 112%,sensitivities of 96.9%and 83.1%,specificities of 75.4%and 78.5%,and Youden indices of 0.723 and 0.616,respectively.Conclusion Real-time three-dimensional automatic left atrial quantification technology enables early and sensitive detection of left atrial dysfunction in HFpEF.Among the parameters derived,LASr(a strain parameter)and LAEI(a functional parameter)exhibit high diagnostic efficacy for HFpEF.

Objective To investigate the clinical efficacy of Kirschner wire,reconstruction plate and locking compression plate internal fixation in the treatment of Robinson type 2A and 2B midshaft clavicle fracture.Methods One hundred and fifty patients with midshaft clavicle fracture in the hospital from August 2006 to August 2015 were selected and divided into the group A,B and C.The group A adopted Kirschner wire,group B adopted the reconstruction plate and group C adopted the locking compression plate internal fixation.Then the incision length,operative time,intraoperative blood loss,fracture healing time,postoperative Constant-Murley scores and DASH scores and postoperative complications incidence rate were compared among the three groups.Results The incision length,operative time and intraoperative blood loss in the group A and C were significantly better than those in the group B(P＜0.05).The fracture healing time,postoperative activity,muscle strength and daily life score,postoperative complications incidence rate,postoperative DASH score in the group C were significantly better than those in the group A and B(P＜0.05).Conclusion The locking compression plate in the treatment of Robinson type 2A and 2B midshaft clavicle fracture can effectively reduce the surgical trauma,shortens the fracture healing time and decreases the postoperative complications.
