1.Salidroside exerts cytoprotective effects on bone endothelial progenitor cells via the AMPK pathway in atherosclerotic mouse model

ObjectiveTo investigate the effects of salidroside (SAL) on the impaired bioactivity of endothelial progenitor cells (EPCs) in atherosclerotic (As) mice and the potential mechanisms regarding AMP-activated protein kinase (AMPK). MethodsAtherosclerosis was induced in 8-week-old male ApoE-/- mice with high-fat diet. Intragastric administration of SAL was given to one mice group to investigate the effects of SAL on aortic plaque burden, plasma NO level, the migration and angiogenic capabilities of bone marrow-derived EPCs (BM-EPCs). The proliferation, migration and vasculogenic properties of EPCs isolated from As mice were investigated in vitro. AMPK-sh-RNA or the AMPK inhibitor Compound C was used to investigate the role of AMPK/Akt/eNOS pathway in the regulatory effects of SAL. ResultsCompared with As group, NO level was significantly elevated in SAL group. The sizes of atherosclerotic plaques at the aortic root were reduced with smaller lipid cores in SAL group compared with As group. Moreover, the migration and angiogenesis capacity of EPCs markedly decreased in As mice, while SAL treatment reversed these impairments. Incubation with SAL at concentrations of 20, 40, and 80 μmol/L for 48 hours significantly promoted the proliferation, migration, and angiogenesis of EPCs. AMPK-sh-RNA transfection abrogated the 20 μmol/L SAL improvement in EPC biological activities. Western blot analysis further demonstrated that treatment with Compound C blocked the activation of AMPK/Akt/eNOS signaling pathway induced by SAL. ConclusionSAL upregulates the biological functions of EPCs through activating the AMPK/Akt/eNOS signaling pathway, thereby ameliorating EPC dysfunction during the pathological progression of atherosclerosis.

Hearing loss is the most prevalent disabling disease. Cochlear implantation(CI) serves as the primary intervention for severe to profound hearing loss. This consensus systematically explores the value of genetic diagnosis in the pre-operative assessment and efficacy prognosis for CI. Drawing upon domestic and international research and clinical experience, it proposes an evidence-based medicine three-tiered prognostic classification system(Favorable, Marginal, Poor). The consensus focuses on common hereditary non-syndromic hearing loss(such as that caused by mutations in genes like GJB2, SLC26A4, OTOF, LOXHD1) and syndromic hereditary hearing loss(such as Jervell & Lange-Nielsen syndrome and Waardenburg syndrome), which are closely associated with congenital hearing loss, analyzing the impact of their pathological mechanisms on CI outcomes. The consensus provides recommendations based on multiple round of expert discussion and voting. It emphasizes that genetic diagnosis can optimize patient selection, predict prognosis, guide post-operative rehabilitation, offer stratified management strategies for patients with different genotypes, and advance the application of precision medicine in the field of CI.

3.Research and Performance Evaluation of Group A Rotavirus Antigen Detection Kit

Objective To prepare a group A rotavirus(RVA)antigen detection kit and evaluates its performance,providing an effec-tive means for rapid detection of rotavirus(RV).Methods In this study,colloidal gold nanoparticles(AuNPs)were used to label Rab-bit anti-G1P[8]Ab1.The Goat anti-G1P[8]Ab2 and Goat Anti-Rabbit IgG were respectively coated on the detection line(T line)and quality control line(C line)of nitrocellulose membrane(NC membrane).The absorbent paper,NC membrane,binding pad,and sample pad were assembled onto a PVC substrate in sequence and cut into 3.75mm strips.They were then installed into an adapter car-tridge to establish a RVA antigen detection kit.National reference standards for rotavirus antigen detection and reference standards pre-pared by our laboratory were used to investigate the sensitivity,specificity,inclusiveness,high-dose hook effect,stability,repeatability,and compare and evaluate similar testing kits available for sale.Results Sensitivity:when tested with national reference substance L,the lowest detection limit is diluted 1∶128 times,which was equivalent to the virus content level of the lowest detection limit concentration of enterprise reference substance 1 × 104pfu/ml or 2344copies/ml,the sensitivity was about 8 times higher than that of similar commercial products.Specificity:there was no cross-reaction between this kit and 10 common clinical pathogens causing diarrhea.Inclusiveness:this kit could simultaneously detect multiple different serotypes of RVA,including G1-G4,G8,G9,etc.Repeatability:this reagent kit was tested repeatedly,and the color of the bands was uniform with good consistency.High-dose hook effect:there was no high-dose hook effect within the range of 1 × 107pfu/ml.Thermal acceleration stability:after being stored at 37℃ and 50℃ for 14days,the test strip of the kit was clear and uniform.Conclusion This study has developed a high-performance RVA antigen detection kit,which is suit-able for rapid detection in various aspects such as clinical auxiliary diagnosis,epidemiological research,and vaccine strain screening.It has good application prospects and practical value.

5.Advances on sleep electroencephalogram in the subtyping and treatment of insomnia disorder

Insomnia disorder is a common clinical mental disorder.Currently, clinical subtyping of insomnia disorder relies primarily on symptomatic descriptions, lacking objective measures and subtyping-based treatment approaches. In recent years, increasing attention has been drawn to sleep electroencephalography (EEG) as a valuable tool for observing abnormal sleep architecture and continuity of insomnia disorder. Sleep EEG analysis holds the potential to elucidate the underlying biological mechanisms of insomnia disorder, facilitating data-driven subtyping and enhancing personalized therapeutic strategies.Five types of sleep EEG subtypes of insomnia disorder were systematically searched and summarized: classifications derived from objective sleep duration, power spectral characteristics, cyclic alternating pattern, spindle and microarousal.EEG characteristics of each subtype and clinical outcomes are discussed.This review aims to provide evidence-based insights for clinical subtyping and personalized treatment of insomnia disorder.
6.Clinical effect of type Ⅰ tympanoplasty for chronic suppurative otitis media with external ear canal fungal infection

OBJECTIVE To observe the clinical efficacy of endoscopic type Ⅰ tympanoplasty in patients with chronic suppurative otitis media accompanied by fungal otitis media.METHODS Based on the symptom characteristics of the external auditory canal and tympanic mucosa,80 patients were divided into four groups:dry ear without fungi group,dry ear with fungi group,wet ear without fungi group,and wet ear with fungi group.Record its clinical characteristics.Patients with fungal otitis externa should receive medication treatment during the perioperative period.Failure to heal the tympanic membrane repaired 6 months after surgery,recurrent perforation of the graft,and/or persistent inability to dry the ear are considered repair failures.RESULTS A total of 80 patients were included in this study,and the overall healing rate of the tympanic membrane in the four groups of patients was 98.8%.There was no significant difference in the postoperative tympanic membrane healing rate among the four groups of patients.The postoperative air bone conduction difference in all four groups of patients decreased compared to preoperative levels,but the difference was not statistically significant.CONCLUSION Patients with chronic suppurative otitis media accompanied by fungal otitis media undergoing type Ⅰ tympanoplasty showed no decrease in tympanic membrane healing rate and no significant improvement in efficacy.

Although social cognitive impairment is commonly observed in patients with schizophrenia, how to effectively assess it and the clinical value of such evaluations warrant further discussion. In 2022, the release of the "European Psychiatric Association guidance on assessment of cognitive impairment in schizophrenia" provided guidance for research and clinical practices in this field. This article aims to analyze the necessity, effectiveness, and feasibility of conducting assessments in clinical settings based on the recommended approaches outlined in the guidance, considering the current clinical landscape in China.

Although cognitive impairment is considered a core symptom of schizophrenia and significantly impacts functional outcomes, it is often overlooked in clinical practice. The European Psychiatric Association released “Guideline on Assessment of Cognitive Impairment in Schizophrenia” in 2022, providing recommendations for research and clinical practice. This paper interprets the neurocognitive aspects of schizophrenia covered in the guidelines, introduces neurocognitive assessment tools used in China and their application settings, examines the relationship between subjective and objective assessments of cognitive impairment, and briefly discusses the connections between neurocognition and positive and negative symptoms. The paper also considers the future development of neurocognitive assessment tools in China.

Cognitive impairment in schizophrenia emerges early and persists, is a core symptom that severely affects social functioning. In 2022, the European Psychiatric Association published the " European Psychiatric Association guidance on assessment of cognitive impairment in schizophrenia", presenting the latest evidence and systematic review results. The guidance recommends comprehensive and detailed cognitive assessments for all individuals with schizophrenia, emphasizing the phase of early intervention. In conjunction with the recommendations of the guidance and the current state of research both domestically and internationally, this article provides an overview of cognitive impairment in schizophrenia, including definition, impact, selection of assessment tools, and limitations of cognitive assessments. It also connects with clinical practice in China, outlining the development and shortcomings of cognitive assessments for schizophrenia. It is suggested that a clinically simple questionnaire suitable for the Chinese population is needed to help physicians quickly identify cognitive impairment.

Cognitive impairment is one of the core features of schizophrenia and severely affects the prognosis of the disorder. Improving cognitive impairment is of great significance for alleviating symptoms, preventing relapses, enhancing the quality of life, and reducing the social burden in patients with schizophrenia. Based on the "Guidelines for the Assessment of Cognitive Impairment in Schizophrenia" published by the European Psychiatric Association in 2022, the article focuses on the intervention and management of cognitive impairment in schizophrenia patients following a systematic assessment, while also considering the specific context of practice in China. This paper reviews recent progress in cognitive impairment interventions for schizophrenia, providing clinical guidance across three key aspects: pharmacotherapy, physical therapy, and psychotherapy. Other interventions are also discussed. Additionally, it addresses the existing challenges and unresolved issues, aiming to provide a basis for decision-making in the current treatment of cognitive impairment in schizophrenia.
