1.Trends of Gastric Cancer Mortality and Age-Period-Cohort Among Residents in Wuxi City of Jiangsu Province from 2009 to 2023

[Purpose]To analyze the trends of mortality and disease burden of gastric cancer among residents aged 30 and above from 2009 to 2023 in Wuxi City of Jiangsu Province and to estimate the age-period-cohort effect.[Methods]The gastric cancer mortality data from 2009 to 2023 were collected from Wuxi Death Registration System,and the crude mortality rate,age-standardized mortality rate,years of life lost(YLL)and YLL rate were calculated.The average annual percen-tage change(AAPC)was calculated with Joinpoint regression to analyze the trends of mortality and YLL rate of gastric cancer.The age-period-cohort model was used to analyze the effect of age,pe-riod and birth cohort on gastric cancer mortality.[Results]From 2009 to 2023,there were 32 348 gastric cancer deaths in Wuxi,the crude mortality rate was 44.24/105,and the age-standard mor-tality rate was 25.10/105,with a total YLL of 681 618.33 person-years.The crude mortality rate,age-standardized mortality rate and YLL rate showed decreasing trends,with AAPC of-1.77%(95%CI:-2.10%~-1.43%),-4.59%(95%CI:-4.97%~-4.20%)and-2.14%(95%CI:-2.56%~-1.74%),respectively.From 2009 to 2023,crude mortality rate,age-tandardized mortality rate,YLL in men were higher than those in women,and the decline rates of all indicators in women were faster than those in men.The age effect showed that the overall risk of gastric cancer death increased with age;the period effect showed a gradual decrease in the mortality risk and death burden of gastric cancer over time;the cohort effect showed that the later born cohort had a lower risk of death and death burden.[Conclusion]From 2009 to 2023,the disease burden of gastric cancer mortality in Wuxi showed a decreasing trend,with age and sex being the primary influencing factors.In the future,emphasis should be placed on gastric cancer screening and early intervention in middle-aged and elderly men.

Furthermore, the integration of optogenetics with light-controlled stem cell technology and light-controlled gene editing technology has unveiled its immense potential in clinical translation. With the advancement of technology and the deepening of clinical practice, optogenetics holds broad prospects within ophthalmology and is poised to offer innovative therapeutic strategies for patients with visual impairments.

5.Influencing factors of exercise tolerance in patients with myocardial infarction undergoing percutaneous coronary intervention revascularization in acute phase

Objective The aim of the study was to explore the influencing factors of exercise tolerance in patients with myocardial infarction (MI) after percutaneous coronary intervention (PCI) revascularization in acute state. Methods A total of 112 patients with first MI undergoing PCI revascularization in acute state and completing cardiopulmonary exercise testing (CPET) were enrolled. Exercise capacity was evaluated by peak oxygen consumption percentage (VO2 peak%) in CPET. Patients were divided into normal exercise capacity (NEC) group (n=40) and abnormal (AEC) group (n=72) according to VO2 peak% value. Clinical manifestations, histories of hypertension and diabetes, medications, coronary arterial angiography and echocardiography findings of patients were compared. The onsets of diabetes and blood glucose levels during the period of CPET were evaluated in the MI patients with diabetes. The patients were followed up for major adverse cardiovascular events (MACE) (admission due to chest pain, re?revascularization, re?infarction and all?cause death) within 24 months after PCI. Multivariate logistic regression analyses were conducted to examine influencing factors for exercise tolerance. Results The ratio of diabetes, type C lesions in the AEC group were higher than those in the NEC group (diabetes: 37.5% vs. 17.5%; type C lesions: 69.4% vs. 42.5%, respectively, all P<0.05). The left ventricular ejection fraction (LVEF) in patients in the AEC group was lower than that in the NEC group [(60.6±10.0) % vs. (65.0±8.2) %, P=0.019]. Multivariate logistic regression analyses showed that history of diabetes and history of type C lesions were the independent risk factors for the declined exercise capacity in the MI patients after PCI revascularization ( OR=3.14, 95%CI 1.167-8.362, P=0.023; OR=3.32, 95%CI 1.444-7.621, P<0.01). Among the MI patients with diabetes, the duration of diabetes in the AEC group was significantly longer than that in the NEC group[ (7.7 ± 3.6)years vs. (5.0 ± 2.4)years] and the proportions of subjects reaching target levels of fasting plasma glucose (40.7% vs. 57.1%) and glycosylated hemoglobin A1c(HbA1c) (55.6% vs. 71.4%) in this group were significantly lower than those in the NEC group (all P<0.05). A multivariate logistic regression analysis showed that reaching HbA1c target was an independent predictor of improved exercise tolerance in MI patients with diabetes who received PCI ( OR=2.518, 95%CI 1.395-7.022, P=0.021). No significant differences were observed in incidence of admission due to chest pain, re?revascularization and re?infarction between the two groups within 24 months after PCI between the groups. Conclusions Diabetes and type C lesions are independent risk factors of declined exercise capacity in patients with first myocardial infarction who received revascularization in acute state. Reaching target HbA1c is independent factor of improved exercise capacity in patients with myocardial infarction and diabetes.

Objective To investigate the influence factors of Cornell product and its clinical value in pa-tients with left ventricular high voltage.Methods 104 patients with left ventricular high voltage were studied and divided into two groups according to Cornell product:Cornell product normal group(n = 78)and Cornell product abnormal group(n=26). Clinical manifestations ,complicaions and ultracardiographic data in these patients were collected and analyzed. Results Chronic kidney disease(CKD)(OR=4.266,95%CI 1.276-14.257,P=0.018) and left ventricular mass index(LVMI)(OR = 1.023,95%CI 1.004-1.043,P = 0.016)were related to Cornell product. Cornell product was positively correlated LVMI appeared(r=0.497,P<0.001). The correlation of Cor-nell product with left ventricularend-diastolic dimension ,interventricular septal thickness and left ventricular poste-rior wall thickness was weak.ROC-AUC of Cornell product for CKD was 0.634,95%CI=0.506-0.780,P=0.039, sensibility 47.8%,and specificity 81.5%. Conclusions LVMI and CKD are independent influence factors of Cor-nell product. Cornell product has a certain diagnositc value for left ventricular hypertrophy ,but it has no associa-tion with the location of ventricular hypertrophy.
8.The role of immunoglobulin G isotype rheumatoid factors on the diagnosis and prognosis of rheumatoid arthritis

Objective To investigate the diagnostic and prognostic value of immunoglobulin (Ig)G isotype rheumatoid factors (IgG-RF) in rheumatoid arthritis (RA).Methods Five hundred patients with RA were enrolled randomly.IgG-RF antibody was detected by enzyme-linked immunosorbent assay (ELISA).The correlations between serum IgG-RF antibody and clinical features,disease activities,laboratory of RA patients were evaluated.The comparison of continuous variables was performed by using the Student t-test or Mann-Whitney U test in accordance with normality testing.Chi-square test was performed for categorical variables.A value of P less than 0.05 was considered statistically significant.Results ① IgG-RF was positive in 41.0% (205/500) of RA patients.In patients with anti-citrullinated protein/peptide autoanti-bodies (ACPA) negative,RF negative or the seronegative patients (both ACPA and RF were negative),the positive rate of IgG-RF was 22.4%(24/107),13.2%(17/129) and 9.1%(5/55),respectively.② Compared with patients with negative IgG-RF,patients with positive IgG-RF had higher rates of joint deformity [(58.5%(120/205) vs 39.3%(116/295),x2=17.918] and bone erosion [(75.6%(118/156) vs 60.3%(140/232),x2=9.796] (P<0.01,respectively).③ The patients with positive IgG-RF had higher rates of elevated ESR(86.3% vs 67.8%,x2=22.426),IgG(29.9% vs 20.0%,x2=6.310),compared to patients with negative IgG-RF (P<0.05,respectively),and levels of ESR [(59±35) mm/1 h vs (47±32) mm/1 h,t=3.989] and CRP [(390±450) mg/L vs (290±340) mg/L,t=3.004] was higher in IgG-RF positive group than the negative (P<0.01,respectivelys).④ Compared with the IgG-RF negative patients,the positive group had higher smoking rates (22.9% vs 12.5%,x2=9.227),higher current smoking rates (16.6% vs 7.1%,x2=11.119) and higher smoking index [(107±238) vs (49±161),t=3.199](P<0.05,respectively).Conclusion IgG-RF had its clinical values in RA diagnosis.IgG-RF is significantly associated with joint deformity,bone erosions and smoking.
9.Anxiety and depression state and their influencing factors in housebound elderly subjects

Objective To analyze the anxiety and depression state in housebound elderly subjects and their influencing factors so as to provide theoretical basis for improving the psychological status of the elderly.Methods Three hundred and fifty elderly patients aged > 60 years suffering from anxiety and depression in two communities of Beijing were clusterly sampled and analyzed.The basic information of housebound elderly subjects with anxiety and depression was recorded during questionnaire survey.The influencing factors for anxiety and depression were explored by using the anxiety self-assessment scale and depression self-rating scale.Results Based on univariable analysis,the factors influencing anxiety and depression of the housebound elderly included ageing,lower cultural level,low income,infrequency of offspring visit and friends meeting,lost spouse,chronic diseases,many medications,and the unsatisfactory living environment(all P < 0.05).However,the participating of entertainment activity and religious beliefs were not influencing factors(all P > 0.05).Logistic multivariate regression showed that women (OR =1.432,95%CI:1.236 1.684,P<0.05),low income(OR =2.078,95% CI:1.675 2.412,P<0.05),the widowed(OR =2.342,95 % CI:1.857-2.769,P < 0.05),chronic diseases (OR =2.078,95 % CI:1.674-2.531,P<0.05),medication (OR =3.056,95 % CI:2.678 3.756,P < 0.05),less visit by children (OR =2.898,95%CI:2.475 3.425,P<0.05),less friends visiting(OR=1.965,95%CI:1.685-2.446,P<0.05) were risk factors for anxiety in the elderly.Meanwhile,women (OR =3.221,95 % CI:2.674-3.853,P < 0.05),low income (OR =2.342,95 % CI:1.768-2.754,P < 0.05),chronic disease (OR =2.078,95 % CI:1.564 2.675,P<0.05),the widowed(OR=2.223,95%CI:1.789 2.746,P<0.05),less visit by children (OR=2.987,95%CI:2.463-3.524,P<0.05),less friends visiting(OR=2.658,95%CI:2.242 3.168,P<0.05) and poor satisfaction with surrounding environment(OR =1.765,95 % CI:1.375 2.371,P<0.05) were risk factors for depression among the elderly.Conclusions The elderly,especially those with advanced age,low cultural level,low economic income,living alone and poor living environment,are more likely to have anxiety and depression.Therefore,some effective corresponding measures should be taken in order to improve the psychological condition and life quality of old people.
10.Study on Effect of Qing-Chang Wen-Zhong Decoction on Interferon Gamma Induced Protein 10 in Ulcerative Colitis Rats

This paper was aimed to study the effect of Qing-Chang Wen-Zhong (QCWZ) decoction on interferon gamma induced protein 10 (IP10) in colon tissues of rats with ulcerative colitis (UC).The UC model was induced using 4.5% DSS added to distilled water for 7 days.At the same time,low-,medium-and high-dose of QCWZ decoction and mesalazine was given by gavage route daily.Then,the rats were killed and the colon tissues were taken.Expression level of interleukin-1 alpha (IL-1α),IL-1β,IL-6,tumor necrosis factor alpha (TNF-α) and interferon gamma (INF-γ) in colon were detected by Elisa assay.The expression and distribution of IP10 protein were detected by immunohistochemistry (IHC).The results showed that compared with the normal group,inflammatory factors (IL-1α,IL-1β,IL-6,TNF-α,INF-γ) and IP10 expression level in DSS-induced UC rats were significantly increased.After 7 days of intervention,inflammatory factors (IL-1α,IL-1β,IL-6,TNF-α,INF-γ) and IP10 decreased significantly (p<0.01,p<0.05).It was concluded that QCWZ decoction may down-regulate the expression of IP 10 and inflammatory factors (IL-1α,IL-1β,IL-6,TNF-α,INF-γ),and then inhibit intestinal inflammation and repair intestinal mucosal damage,so as to achieve the purpose of UC treatment.
