1.Genetic and clinical characteristics of children with RAS-mutated juvenile myelomonocytic leukemia.

OBJECTIVES:
To investigate the genomic characteristics and prognostic factors of juvenile myelomonocytic leukemia (JMML) with RAS mutations.
METHODS:
A retrospective analysis was conducted on the clinical data of JMML children with RAS mutations treated at the Hematology Hospital of Chinese Academy of Medical Sciences, from January 2008 to November 2022.
RESULTS:
A total of 34 children were included, with 17 cases (50%) having isolated NRAS mutations, 9 cases (27%) having isolated KRAS mutations, and 8 cases (24%) having compound mutations. Compared to children with isolated NRAS mutations, those with NRAS compound mutations showed statistically significant differences in age at onset, platelet count, and fetal hemoglobin proportion (P<0.05). Cox proportional hazards regression model analysis revealed that hematopoietic stem cell transplantation (HSCT) and hepatomegaly (≥2 cm below the costal margin) were factors affecting the survival rate of JMML children with RAS mutations (P<0.05); hepatomegaly was a factor affecting survival in the non-HSCT group (P<0.05).
CONCLUSIONS
Children with NRAS compound mutations have a later onset age compared to those with isolated NRAS mutations. At initial diagnosis, children with NRAS compound mutations have poorer peripheral platelet and fetal hemoglobin levels than those with isolated NRAS mutations. Liver size at initial diagnosis is related to the prognosis of JMML children with RAS mutations. HSCT can improve the prognosis of JMML children with RAS mutations.

2.Sequential treatment with siltuximab and tocilizumab for childhood idiopathic multicentric Castleman disease: a case report.

The patient, an 11-year-old girl, was admitted with recurrent fever for 20 days, worsening with abdominal distension for 7 days. Upon admission, she presented with recurrent fever, lymphadenopathy, hepatosplenomegaly, polyserositis, and multiple organ dysfunction. Lymph node pathology and clinical manifestations confirmed the diagnosis of idiopathic multicentric Castleman disease-TAFRO syndrome. Treatment with siltuximab combined with glucocorticoids was initiated, followed by maintenance therapy with tocilizumab. The patient is currently in complete clinical remission. Therefore, once a child is diagnosed with idiopathic multicentric Castleman disease -TAFRO syndrome, early use of siltuximab should be considered for rapid disease control, followed by tocilizumab for maintenance therapy.

Tuberculosis(TB),a chronic infectious disease caused by infection with the Mycobacterium tuberculosis complex(MTBC),has re-emerged as the leading cause of death from a single infectious agent worldwide.Because of widespread use and mis-use of anti-tuberculosis drugs,the emergence of multidrug-resistant TB(MDR-TB)and extensively drug-resistant TB(XDR-TB)is increasing,thus posing a serious threat to global health.The current problem of drug resistance is a major prevention and treatment challenge;therefore,the search for new drug targets is urgently needed.In recent years,substantial progress has been made in re-search on anti-tuberculosis drug targets and novel therapeutic strategies.Herein,we summarize recent research progress in anti-tuberculosis drug targets,primarily cell wall synthesis,nucleic acid replication and transcription,and energy metabolism.We also provide an overview of research progress regarding two novel therapeutic strategies,to provide a theoretical basis and research ideas for the development of new clinical drugs.
4.Effects of total flavonoids of Dracocephalum moldavica on apoptosis of H9c2 cells induced by OGD/R injury and endoplasmic reticulum stress.

This study investigated the effects of total flavonoids of Dracocephalum moldavica(TFDM) on apoptosis in rat H9c2 cells induced by endoplasmic reticulum stress(ERS) established by oxygen-glucose deprivation and reoxygenation(OGD/R) injury and tunicamycin(TM), and explored the potential mechanisms. After successful modeling, the following groups were set in this experiment: control group, model(OGD/R or TM) group, and TFDM low-, medium-, and high-dose groups(12.5, 25, and 50 μg·mL~(-1)). The OGD/R injury model was constructed in vitro. Cell proliferation was assessed using the cell counting kit-8(CCK-8) method. The levels of lactate dehydrogenase(LDH) and creatine kinase MB isoenzyme(CKMB) in the cell supernatant were detected. Western blot was used to assess the expression of ERS-related proteins, including glucose regulatory protein 78(GRP78), C/EBP homologous protein(CHOP), activating transcription factor 6(ATF6), and apoptotic proteins B-cell lymphoma 2(Bcl-2) and Bcl-2-associated X protein(Bax). Apoptosis was detected using the terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling(TUNEL) method. In the TM-induced ERS model, Western blot was used to measure the expression of ERS pathway-related proteins GRP78, CHOP, inositol-requiring enzyme 1(IRE1), X-box binding protein 1(XBP1), protein kinase RNA-like endoplasmic reticulum kinase(PERK), eukaryotic initiation factor 2α(eIF2α), ATF6, p-ATF6, and apoptotic proteins Bcl-2, Bax, cysteinyl aspartate specific proteinase-12(caspase-12), and cleaved caspase-12. Gene expression of GRP78, CHOP, PERK, and ATF6 was detected by real-time fluorescence quantitative PCR(RT-qPCR). Apoptosis was again detected using the TUNEL method. The results showed that in the OGD/R model, compared with the control group, the levels of LDH and CKMB in the cell supernatant were significantly increased in the OGD/R group. Compared with the OGD/R group, the levels of LDH and CKMB in the TFDM group were significantly reduced. Western blot results revealed that compared with the control group, the expression of ERS-related proteins and Bax in the OGD/R group was significantly increased, while the expression of Bcl-2 was significantly decreased. Compared with the OGD/R group, the expression of ERS-related proteins and Bax in the TFDM groups was significantly reduced, and the expression of Bcl-2 was significantly increased. TUNEL assay showed that apoptosis was significantly decreased after TFDM treatment. In the TM-induced ERS experiment, compared with the control group, the expression of ERS-related genes, ERS-related proteins, and apoptotic proteins in the TM group was significantly increased, while the expression of Bcl-2 was significantly decreased. Compared with the TM group, the expression of ERS-related genes, ERS-related proteins, and apoptotic proteins in the TFDM group was significantly reduced, and the expression of Bcl-2 was significantly increased. These results suggest that ERS exists in the OGD/R-injured H9c2 cell model, and TFDM can effectively inhibit ERS-induced apoptosis. The mechanism may be related to the downregulation of ERS pathway-related proteins and apoptotic proteins.

7.Effect of hospital-family chain rehabilitation management mode on self-management and quality of life in elderly patients with coronary heart disease

Objective:To investigate effect of hospital-family chain rehabilitation management mode on self-man-agement and quality of life in elderly patients with coronary heart disease(CHD).Methods:This randomized con-trolled study enrolled 220 elderly CHD patients admitted in Hai'an Hospital of Traditional Chinese Medicine between December 2020 and August 2022.They were divided into control group(n=110,routine nursing)and intervention group(n=110,hospital-family chain rehabilitation management mode nursing).After 6-month intervention,scores of Coronary Self-Management Scale(CSMS),China Questionnaire of Quality of life in patients with Cardio-vascular diseases(CQQC)and Perceived Social Support Scale(PSSS)were compared between two groups as well as incidence of adverse cardiovascular events.Pearson method was employed to analyze the association among total scores of CSMS,CQQC and PSSS.Results:After 6-month nursing,compared with patients in control group,those in intervention group had significant higher CSMS total score[(116.03±5.41)points vs.(89.97±4.21)points],CQQC total score[(107.30±6.99)points vs.(80.86±6.38)points],PSSS total score[(69.05±7.42)points vs.(57.05±5.19)points]and each dimensional score(P<0.001 all).Pearson correlation analysis indica-ted that total scores of CSMS,CQQC and PSSS were positively correlated with each other(r=0.437~0.562,P<0.001 all).Incidence of adverse cardiovascular events in intervention group was significantly lower comparing to that of control group(2.73%vs.13.64%,P=0.003).Conclusion:Hospital-family chain rehabilitation manage-ment mode could improve the self-management ability and quality of life,increase the patient's social support,and reduce incidence of adverse cardiovascular events in elderly patients with coronary heart disease.
8.Evaluation of right atrial function and prediction of pulmonary hypertension in patients with atrial septal defect by four-dimensional ultrasound automatic quantitative technology

Objective:To evaluate the changes of right atrial volume and function in patients with atrial septal defect(ASD)by four-dimensional ultrasound automatic quantitative technology,and to explore the predictive value and clinical significance for ASD with pulmonary arterial hypertension(PAH).Methods:Sixty-one patients with ASD and 32 healthy volunteers(control group)who attended Central China Fuwai Hospital of Zhengzhou University,Fuwai Central China Cardiovascular Hospital from March 2023 to April 2024 were prospectively collected,and classified ASD patients into ASD without PAH group(non-PAH group, n=30)and ASD with PAH group(PAH group, n=31)according to whether or not they had PAH,and obtained right atrial reserve,ductal,systolic longitudinal and circumferential strains(RASr,RAScd,RASct,RASr-c,RAScd-c,RASct-c),right atrial minimal,maximal,and presystolic volumes(RAVmin,RAVmax,and RAVpreA),and calculated right atrial total,passive,and active ejection fraction(RAEF,RAPEF,RAAEF). The ultrasound parameters and clinical data were compared between groups. Pearson's linear correlation was applied to analyse the correlation between the parameters(RASr,RAA,PASP,NT-proBNP)and mean pulmonary artery pressure(mPAP). ROC curves were plotted and the area under the curve(AUC)was calculated to analyse the value of the four-dimensional strain parameters of the right atrium in independently and jointly predicting the combination of ASD with PAH. Results:①Compared with the control group,RAVmin,RAVmax,and RAVpreA were elevated in the non-PAH and PAH groups,and the absolute values of RAPEF and RAScd were decreased,whereas the absolute values of RAEF,RAAEF,RASr,RASct,RASr-c,and RASct-c were elevated in the non-PAH group,and decreased in the PAH group(all P<0.05);compared with the non-PAH group,only RAVmin,RAVmax,and RAVpreA were elevated in the PAH group,and the rest of the above parameters were reduced,with statistically significant differences(all P<0.05). ②The right atrial four-dimensional strain parameter RASr had the highest predictive value for ASD combined with PAH,with an AUC of 0.876,a specificity of 83.3%,and a sensitivity of 77.4%,respectively. ③RASr was negatively correlated with mPAP( r=-0.591, P<0.001)and RAA,PASP and NT-proBNP were positively correlated with mPAP( r=0.539,0.697,0.616;all P<0.001). ④The predictive value of RASr combined with RAA,PASP,and NT-proBNP was superior for ASD combined with PAH(AUC=0.933). Conclusions:Four-dimensional ultrasound automatic quantitative technology can effectively evaluate the changes of right atrial volume and function in patients with ASD. ASD patients without PAH have increased right atrial reserve and pump function,and decreased right atrial pipeline function. The right atrial function of ASD patients with PAH is significantly reduced,and the impairment is more severe. Among the right atrial four-dimensional strain parameters,RASr has the highest efficacy in predicting ASD combined with PAH. Moreover,the combination of RASr with RAA,PASP,and NT-proBNP has the best predictive efficacy for ASD combined with PAH,which holds significant clinical value.
9.Effect of Qishen Yixin Granules on microcirculatory endothelial dysfunction induced by Ang Ⅱ and high-fat diet in mice and its mechanism

Aim To clarify the mechanism by which Qishen Yixin Granules improved microcirculation vas-cular endothelial dysfunction(VED)in mice,through activating the Nrf2/HO-1 signaling pathway to regulate oxidative stress.Methods C57 mice were randomly divided into six groups:blank group,model group,pos-itive drug group,and low-,medium-,and high-dose groups of Qishen Yixin Granules.The VED model was established by long-term infusion of Ang Ⅱ combined with a high-fat diet.Each treatment group received the corresponding drug intervention.After four weeks of drug intervention,cardiac function was assessed by echocardiography.Carstairs staining was used to ob-serve the formation of microthrombi in myocardial tis-sue.The micro vascular ischemia was evaluated by Hei-denhain staining.The ultrastructure of endothelial cells was observed by electron microscopy.The levels of EMPs,ROS,NO,ET-1,TF,TM,VWF,and TXA2 in serum were measured by ELISA.The expression levels of MDA,SOD,and GSH-Px in mouse heart tissue were determined by chemical methods.Cardiac microvascu-lar density and the expression of Nrf2,Keap1,and HO-1 proteins were detected by Immunohistochemical stai-ning.The protein expressions of Keap1,cytoplasmic Nrf2,nuclear Nrf2,and HO-1 in myocardial tissue were detected by Western blot.Results Qishen Yixin Granules could effectively improve the cardiac function of mice,alleviate the damage of endothelial cells and endothelial function.They could up-regulate serum NO levels and the activities of antioxidant enzymes SOD and GSH-Px,while down-regulating the expression of ROS and vascular inflammatory injury factors such as ET-1,VWF,TXA2,TF,TM,and EMPs.Qishen Yixin Granules also increased the positive counts of CD34,Nrf2,and HO-1,as well as microvessel density.Fur-thermore,they inhibited the expression of MDA,Keap1,and cytoplasmic Nrf2 protein in myocardial tis-sue,while increasing the expression of nuclear proteins HO-1 and Nrf2.Conclusions Qishen Yixin Granules may inhibit oxidative stress and inflammatory response by regulating the Nrf2/HO-1 signaling pathway,thereby improving vascular endothelial damage and cardiac function in VED mice.
