1.Survey of post-discharge exercise behavior and analysis of factors influencing exercise intensity in patients undergoing lung surgery

Objective To investigate the post-discharge exercise behavior and factors influencing moderate to vigorous intensity physical activity (MVPA) in patients undergoing lung surgery. Methods A total of 2874 patients from the large prospective, observational perioperative lung symptom study cohort (CN-PRO-Lung 3) in the Department of Thoracic Surgery at Sichuan Cancer Hospital between April 7, 2021, and January 31, 2024, were selected as the survey subjects. A survey was conducted using the Investigation of Exercise Behavior after Lung Surgery questionnaire and the International Physical Activity Questionnaire-Short Form (IPAQ-SF) among patients who underwent lung surgery. Binary logistic regression was used to analyze the factors influencing patients’ engagement in MVPA. Results A total of 702 patients were surveyed, including 252 males and 450 females, with an average age of (52.4±10.2) years. Patients with lung cancer accounted for 85.9%. Only 36.0% of the patients had regular exercise habits, while 42.3% did not engage in any physical activity. The three main barriers for postoperative exercise were physical discomfort (pain, coughing, shortness of breath, etc, 54.7%), lack of professional guidance (41.7%), and concerns about the surgical wound (28.9%). The proportions of patients engaging in vigorous, moderate, and low-intensity physical activity were 5.7%, 28.2%, and 66.1%, respectively. Multivariate analysis showed that patients with a personal annual income ≥50000 yuan (OR=1.52, 95%CI 1.01-2.29, P=0.044), high school education or above (OR=1.92, 95%CI 1.33-2.76, P<0.001), and lobectomy (OR=1.44, 95%CI 1.02-2.03, P=0.037) engaged in more MVPA. Conclusion Patients undergoing lung surgery have inadequate physical activity after discharge, particularly lacking in MVPA. Patients with higher income, higher educational levels, and lobectomy are more frequently engaged in MVPA. Measures such as symptom control, providing exercise guidance, and enhancing education on wound care may potentially improve the inadequate physical activity in lung surgery patients after discharge.
2.Analyses of T lymphocyte subset levels and viral loads in human immunodeficiency virus antibody-confirmed positive cases in Suzhou from 2021 to 2024

ObjectiveTo investigate the T lymphocyte subset levels and viral loads in newly human immunodeficiency virus (HIV) antibody-confirmed positive cases in Suzhou (2021‒2024), and to analyze potential influencing factors by integrating their demographic characteristics, immune status, and viral replication patterns, thereby providing evidence for HIV/acquired immune deficiency syndrome (AIDS) prevention and control. MethodsPeripheral whole blood samples were collected from newly confirmed HIV-positive cases in Suzhou from 2021 to 2024. T lymphocyte subset analysis and viral load testing were performed, and influencing factors were identified in combination with demographic characteristics. Logistic regression models were employed to identify factors associated with CD4+T lymphocyte counts ≤350 cells·μL-1, and Spearman’s rank correlation test was used to analyze the correlation between logarithmic value of viral load and CD4+/CD8+ ratio. ResultsAmong the 3 022 confirmed HIV-positive samples, the median CD4+T lymphocyte count was 298.00 cells·μL-1, with 882 cases (29.19%) showing CD4+ T lymphocyte counts <200 cells·μL-1. The median CD8+T lymphocyte count was 1 011.00 cells·μL-1. The median CD4+/CD8+ ratio was 0.28, with 32.46% of cases exhibiting CD4+/CD8+ ratios <0.20, and there were statistically significant differences in CD4+/CD8+ ratio among different genders, age groups, marital status, and sample sources (all P<0.05). Multivariate logistic regression analyses indicated that individuals aged ≥20 years, those who were divorced or widowed, and cases identified through medical institutions had a significantly higher proportion of CD4+T lymphocyte counts ≤350 cells·µL⁻¹ compared to those aged <20 years, unmarried individuals, and cases sourced from voluntary counseling and testing (VCT) clinics, respectively. The mean logarithmic value of viral load was (4.29±1.15) copies·mL-1. The logarithmic value of viral load demonstrated a significantly negative correlation with both CD4+/CD8+ ratio (r=-0.43, P<0.001) and CD4+T lymphocyte count (r=-0.37, P<0.001). ConclusionA substantial proportion of newly diagnosed HIV/AIDS cases in Suzhou are late presenters with high viral load levels. Targeted interventions should prioritize high-risk populations through enhanced active surveillance and the implementation of combined T lymphocyte subsets analysis and viral load testing, which can enable earlier case-finding and timely antiretroviral therapy initiation.
3.The correlation between pathological risk score of gastric cancer and patient prognosis as well as tumor biological behavior

Objective To explore the predictive role of pathological risk score of gastric cancer (PRSGC) in the prognosis of gastric cancer patients, and to further investigate the correlation between PRSGC and tumor biological behavior. Methods Clinicopathological data were retrospectively collected from 1 120 patients with gastric cancer who underwent surgical resection at Zhongshan Hospital, Fudan University, between April 2006 and November 2019. PRSGC was calculated using the previously established DeepRisk model, and patients were divided into high and low PRSGC groups (560 patients per group) based on the median PRSGC value. The proportions and spatial distances of immune cell subsets in gastric cancer tissues were assessed between the two groups. Data from stomach adenocarcinoma samples in The Cancer Genome Atlas was integrated to analyze PRSGC-associated genetic mutations and signaling pathways. Results The survival rate of the high PRSGC group was lower than that of the low PRSGC group, and the recurrence rate was higher than that of the low PRSGC group (P<0.000 1). Compared with the low-PRSGC group, the high-PRSGC group exhibited increased infiltration of regulatory T cells (Tregs) and neutrophils within the tumor region (P<0.05), while the infiltration levels of natural killer T cells and cytotoxic T cells were markedly decreased (P<0.05). Spatial analysis revealed that a shortened spatial distance between CD4+ T cells and Tregs was closely associated with poor prognosis in the high-PRSGC group (P=0.03). Furthermore, in the high-PRSGC group, the number of TIM3+ cells showed a positive correlation with the infiltration levels of Tregs (r=0.69, P=0.01), CD8+ T cells (r=0.61, P=0.04), and CD4+ T cells (r=0.67, P=0.02). Gene set enrichment analysis indicated that a high PRSGC was associated with the activation of pathways related to actin cytoskeleton regulation, the cGMP-PKG pathway, the Hippo pathway, and tumor proteoglycans. Conclusion A high PRSGC is associated with an immunosuppressive tumor microenvironment, as well as tumor invasion and metastasis. It effectively predicts the prognosis of patients with gastric cancer and, by doing so, provides a potential theoretical basis for developing individualized strategies that combine targeted therapy with immunotherapy.

With the continuous deepening of the concept of precision diagnosis and treatment for lung cancer, how to achieve higher efficiency and accuracy in the screening, diagnosis, and treatment pathways in clinical practice has become an important issue that urgently needs to be overcome. The current clinical difficulty lies in the fact that despite continuous advancements in imaging and molecular diagnostic technologies, there are still limitations in manual efficiency and subjective experience when it comes to massive data analysis and multi-scale feature extraction. Artificial intelligence (AI), especially algorithm systems based on deep learning, is an innovative technology capable of deeply empowering medical big data. This method utilizes algorithms such as convolutional neural networks, combined with radiomics, pathomics, and multi-modal data fusion analysis, demonstrating immense potential in early precise detection and benign-malignant differentiation of pulmonary nodules, digital pathological subtype recognition and non-invasive prediction of driver genes, precise 3D surgical planning and automatic delineation of radiotherapy target volumes, as well as dynamic risk warning during follow-up. This innovative technology provides a brand-new solution for realizing intelligent and individualized lung cancer diagnosis and treatment models. This consensus, based on the latest evidence from evidence-based medicine and combined with the development trends in the AI field and real-world clinical needs, was ultimately formed by gathering the consensus opinions of multidisciplinary experts in radiology, pathology, thoracic surgery, and other fields. The main content covers the application specifications of AI in the three core scenarios of lung cancer screening, diagnosis, and treatment, the technical standards for data collection and algorithm validation, as well as the ethical and regulatory challenges faced at the current stage. It aims to clarify the applicable boundaries of AI as a clinical auxiliary decision support tool, providing scientific guidance and standardized exploration directions for peers currently engaged in or planning to carry out AI-assisted clinical diagnosis, treatment, and translation of lung cancer.
5.Investigation of chemical hazards in the production line of a lithium battery manufacturing plant

ObjectiveTo investigate the chemical hazards in the production line of lithium batteries, so as to provide a scientific basis for the management of occupational-health risk and to promote the healthy and sustainable development of the lithium battery industry. MethodsAn on-site survey on the process flow of the production of lithium battery was conducted in an enterprise. Volatile organic compounds (VOCs) in the occupational environment were collected by Summa canisters, carbonates and N-methyl pyrrolidone (NMP) were collected using activated carbon tubes, and airborne metals were collected using filter membranes. VOCs, carbonates and NMP were detected by gas chromatography-mass spectrometry (GC-MS), and airborne metal elements in the dust samples were analyzed by inductively coupled plasma mass spectrometry (ICP-MS). ResultsNon-targeted environmental monitoring results indicated that NMP was detected in the negative /positive electrode coating, assembly and drying filling workstations, dimethyl carbonate (DMC) was detected in the assembly, drying and electrolyte injection workstations, and ethyl methyl carbonate (EMC) was detected solely in the electrolyte injection workstation. Semi-quantitative analyses of VOCs identified 136 pollutants, including acrylonitrile and halohydrocarbons. Quantitative targeted environmental monitoring results revealed the highest geometric mean (GM) concentration of EMC (31.450 mg·m-3) was found in the assembly and drying workstations, diethyl carbonate (DEC) was detected in all workstations. While vinylene carbonate (VC) and ethylene carbonate (EC) were detected only in electrolyte injection, assembly and drying workstations. NMP was detected in all positive electrode coating samples, with a GM concentration of 5.68 mg·m-3 (concentration range: 4.0‒ 7.4 mg·m-³). Lithium was exclusively detected in dust samples from the liquid injection workstation (GM: 0.014 μg·m-³). ConclusionNMP, EMC, DEC, and other chemicals are identified at the key workstations such as the positive electrode coating, electrolyte injection, assembly and drying in the lithium production line. Furthermore, semi-quantitative VOCs analyses identified 136 pollutants, demonstrating a characteristic of multicomponent chemical exposure.
6.Effect and mechanism of dapagliflozin on gut microbiota in a mouse model of metabolic associated fatty liver disease

ObjectiveTo investigate the effect of dapagliflozin on liver lipid metabolism and gut microecology in mice with metabolic associated fatty liver disease (MAFLD), and to clarify its potential mechanism. MethodsA total of 50 male C57 mice were randomly divided into Control group, type 2 diabetes+MAFLD group (MAFLD group), dapagliflozin group (DAPA group), meldonium group (THP group), and dapagliflozin+meldonium group (DAPA+THP group), with 10 mice in each group. High-fat diet combined with streptozotocin was used to establish a mouse model of MAFLD. Treatment outcomes were assessed based on histopathology and biochemical parameters such as blood glucose and blood lipid levels, and the transcriptomic and metagenomic analyses were used to identify differentially expressed genes and the changes in gut microbiota. A one-way analysis of variance was used for comparison of normally distributed continuous data between multiple groups, and the least significant difference t-test was used for comparison between two groups; the Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between multiple groups, and the Nemenyi test was used for comparison between two groups. ResultsHistopathological examination showed that the mice in the MAFLD group had excessive lipid deposition and hepatocyte steatosis; compared with the MAFLD group, the DAPA group had a significant improvement in hepatocyte steatosis, while the THP group and the DAPA+THP group had a less significant improvement compared with the DAPA group. Compared with the Control group, the MAFLD group had a significant increase in fasting blood glucose (P<0.05), significant increases in the serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), malondialdehyde, total cholesterol, triglyceride, and low-density lipoprotein cholesterol (P<0.05), and a significant reduction in high-density lipoprotein cholesterol (P<0.05). Compared with the MAFLD group, the DAPA group, the THP group, and the DAPA+THP group had significant reductions in the serum levels of ALT and AST (P<0.05). The results of 16S rRNA sequencing showed that compared with the Control group, the MAFLD group had significant changes in gut microbiota, with an increase in Firmicutes and a reduction in Bacteroidetes, as well as reductions in S24-7 and Erysipelotrichaceae and an increase in Lactobacillaceae. The levels of the above flora were upregulated to normal levels in the DAPA group, the THP group, and the DAPA+THP group. The liver transcriptomic analysis showed that the enriched metabolic pathways included steroid hormone biosynthesis, bile secretion, inflammatory mediator regulation of TRP, fatty acid elongation, and lipid biodegradation processes, and the related genes mainly involved the key targets of lipid metabolism such as Acot2, Angptl4, Scd2, and Npc1l1. ConclusionDapagliflozin can alleviate MAFLD through the pathways such as steroid hormone biosynthesis, bile secretion, inflammatory mediator regulation of TRP, and fatty acid elongation, as well as by regulating gut microbiota homeostasis.

Through reviewing literature on thyroid cancer management strategies, this article discusses the problems of active monitoring strategies, the real implementation status, and the hidden dangers of thermal ablation as a downgrade treatment. It points out that further research is needed to accurately identify low-risk PTMCs and implement active monitoring methods. The improvement in the 5-year survival rate of thyroid cancer is due to standardized diagnosis and treatment. At present, it is still too early to downgrade the diagnosis and treatment of thyroid cancer in China. The World Health Organization recommends the three early principles of early detection, early diagnosis, and early treatment for the treatment of malignant tumors as the fundamental principles of thyroid cancer diagnosis and treatment.

9.Amphotericin B liposome-induced acute cardiac dilatation and heart failure in a pediatric patient

An 8-year-old male patient with chronic granulomatous disease received amphotericin B liposome (unknown dose) in addition to anti-infection treatments with meropenem, compound sulfamethoxazole, and voriconazole due to recurrence of secondary pulmonary infection. After 3 days, the patient developed tachycardia, edema, and worsening dyspnea. Echocardiography revealed severe right heart enlarge- ment and pulmonary hypertension. Cardiotonic, diuretic, and pulmonary antihypertensive therapies were given. After over half a month, his pulmonary infection was improved, pulmonary arterial pressure decreased, but the right heart enlargement persisted. Suspending treatment about half a month later, amphotericin B liposome was reinitiated at a gradually increased dose from 2 mg once daily, in combination with piperacillin sodium and tazobactam sodium and compound sulfamethoxazole due to aggravated cyanosis and cough. After the administration of amphotericin B liposome (50 mg once daily) on day 5, the patient experienced wheezing and facial edema. Laboratory tests showed B-type natriuretic peptide (BNP) 4 679 ng/L; echocardiography demonstrated right heart enlargement and pulmonary hypertension. Suspecting that the cardiac dilatation and heart failure were associated with amphotericin B liposome, the drug was discontinued. The anti-infection regimen was switched to biapenem, linezolid and voriconazole, along with continued cardiotonic and diuretic managements. The patient′s symptoms were improved after 10 days, the treatment regimen was changed to compound sulfamethoxazole and voriconazole. However, after 3 days, the patient′s abdominal distension and dyspnea worsened. Endotracheal intubation and mechanical ventilation were initiated along with cardiotonic and diuretic therapy; anti-infection therapy with cefoperazone sodium and sulbactam sodium combined with voriconazole was given based on bronchoalveolar lavage fluid and sputum culture results. One month later, the patient′s condition was improved, showing no right ventricular dilation and reduced pulmonary arterial pressure on echocardiography and BNP 800 ng/L.
10.Clinical manifestations of medically refractory hereditary movement disorders in children and efficacy of deep brain stimulation

Objective:To investigate the clinical manifestations of medically refractory hereditary movement disorders in children and the efficacy of deep brain stimulation (DBS).Methods:A case series study.The clinical and follow-up data of 20 children with medically refractory hereditary movement disorders who underwent DBS treatment at the Neurology and Functional Neurosurgery Departments of Beijing Children′s Hospital, Capital Medical University, from July 2018 to April 2024, were retrospectively analyzed.The severity of movement disorder symptoms and surgical effects were evaluated using the Burke-Fahn-Marsden Dystonia Rating Scale Movement(BFMDRS-M) or the Unified Parkinson′s Disease Rating Scale Ⅲ(UPDRS Ⅲ).Results:There were 12 males and 8 females among the 20 children, with an onset age ranging from 4 months to 12 years and 5 months.Fourteen patients had hereditary dystonia, which is related to KMT2B in 11 patients, TOR1A in 2 patients and SGCE in 1 patient.Two patients had choreoathetosis, which is related to ADCY5-related familial movement disorders.Two patients had early-onset Parkinson′s disease, which is related to ATP6AP2 in 1 patient and VPS13C in 1 patient.Two patients had neurodevelopmental disorders with involuntary movements, which is related to GNAO1 in 1 patient, and the other patient was idiopathic.All the children were given oral Levodopa, Benzhexol, Baclofen, Tiapride Hydrochloride, Clonazepam alone or in combination.Three children showed obvious dyskinesia after Levodopa treatment.The symptoms of movement disorders in all children exhibited little to no improvement.Levetiracetam and Zonisamide had unstable effects in the treatment of myoclonia.DBS surgery was performed on all the patients aged from 3 to 16 years.Electrodes were successfully inserted into bilateral globus pallidus internus in 14 cases and bilateral subthalamic nuclei in 4 cases.The target was unknown in 2 cases.No surgery-related complications were observed.The patients were followed up for 3 months to 6 years, and the last follow-up age of the patients ranged from 5 years and 7 months to 22 years and 1 month.The rate of improvement in BFMDRS-M score was 37%-100% in 16 patients and >70% in 7 patients with hereditary dystonia.The rate of improvement in UPDRS Ⅲ score was 23% in 1 patient with VPS13C-related early-onset Parkinson′s disease. Conclusions:Childhood medically refractory hereditary movement disorders are a case series that exhibits significant phenotypic and genotypic heterogeneity.DBS surgery demonstrates significant efficacy for KMT2B-, TOR1A-, and SGCE-related hereditary movement disorders.
