1.Molecular characterisation and histopathological analysis of canine distemper virus in a Malayan tiger (Panthera tigris jacksoni)

Objective:
This study aims to describe histological manifestations and characterise the CDV strain in the tiger to determine its molecular epidemiology and postulate viral pathogenicity.
Methods:
Histopathology characterisation of the infected tissues were performed through haematoxylin and eosin and immunohistochemistry staining, respectively. We conducted virus isolation and titration in Chinese hamster ovarian expressing the dog signalling lymphocytic activation molecule (CHO-SLAM) cells. Reverse transcription polymerase chain reaction was performed to confirm the presence of CDV RNA, nucleotide and amino acid sequence analyses for comprehensive characterisation of the CDV strain.
Results:
Notable histopathological changes were observed primarily in the brain, lung, liver, kidney, spleen, and stomach, with viral antigens localised in the lung, liver, kidney, and stomach tissues. CDV-induced cell cytopathic effects yielding 4.27 × 10 6 TCID 50 /mL were observed at 48 h post-inoculation in CHO-SLAM cells. Phylogenetic analysis suggested that the virus originated from the Asia-1 clade. Notably, 549H and 519I mutations in the hemagglutinin protein were observed, indicating adaptation to a non-canid wildlife species.
Conclusions
and Relevance: Overall, this study enhances our understanding of the molecular characterisation and evolutionary dynamics of the CDV strain present in the Malayan tiger and serves as a benchmark for developing effective preventative measures to protect Malayan tigers and mitigate their risk of extinction.
2.Impact of a Rapid Optimization Clinic on Glycemic Control and Insulin Deintensification in Patients With Diabetes: An Early Retrospective Audit

Introduction:
Improving glycemic control while minimizing unnecessary
insulin exposure is an important goal in diabetes
management. The Rapid Optimization Clinic (ROC) was
established as a structured multidisciplinary service to
support therapy individualization, close follow-up, and
timely insulin deintensification. This audit evaluated early
changes in glycated hemoglobin A1c (HbA1c) and insulin
treatment burden following ROC care over 3 months.
Methodology:
We conducted a retrospective audit of routine clinical
data from patients with diabetes managed in the ROC at a
district hospital. Baseline and 3-month HbA1c and insulin
data were extracted from non-electronic clinic records.
Insulin dose was standardized as total daily dose (TDD)
in units/kg/day. Insulin deintensification was evaluated
primarily by change in TDD from baseline to 3 months and
by the proportion of patients who discontinued insulin
during follow-up. Paired analyses were performed for patients with complete baseline and follow-up data for
each outcome. Continuous variables are presented as mean
± standard deviation or median with interquartile range,
as appropriate. Exploratory analyses were undertaken to
assess whether available patient factors were associated
with HbA1c improvement.
Results:
Twenty-three patients were included. Paired HbA1c data
were available for 12 patients, whereas paired TDD data
were available for 21 patients. Mean HbA1c decreased
from 10.78 ± 2.59% at baseline to 8.42 ± 2.29% at 3 months,
representing a mean reduction of 2.36 percentage points
(95% confidence interval [CI] 0.09–4.62; p = 0.043). Mean TDD
decreased from 0.434 ± 0.248 to 0.286 ± 0.313 units/kg/day,
corresponding to a mean reduction of 0.148 units/kg/day
(95% CI 0.077–0.219; p <0.001). Insulin was discontinued in
9 of 21 patients (42.9%). No clear association was observed
between HbA1c improvement and age, sex, or number of
visits. Interpretation is limited by the small sample size,
reflecting the early phase of a newly established clinic.
Conclusion
In this early audit, ROC care was associated with clinically
meaningful improvement in glycemic control and
significant insulin deintensification over 3 months. These
findings support the potential role of a structured multidisciplinary optimization clinic in delivering individualized
diabetes care and facilitating safe reduction of insulin
burden.

This report describes an approach to recover SARS-CoV-2 RNA from rapid antigen test kit (RTK-antigen) cassettes for whole-genome sequencing (WGS). RNA samples were recovered from 33 RTK-antigen cassettes for WGS, with 18 samples achieving more than 80% genome coverage. This work provides a proof-of-concept that positive RTK-antigen cassettes can be safely transported, stored and subjected to WGS, enabling swift identification of circulating variants.

Madam S, who diagnosed to have stage IV lung adenocarcinoma with exon 21 L858R point mutation (T3N2M1a)
was admitted for massive pericardial effusion in April 2016. She was ECOG 4 on admission. Her ECOG improved
to 1 after pericardial tapping and initiation of free sample erlotinib 100 mg daily. Repeated CT thorax post treatment
showed the disease was partial responded. Due to financial constraints, she had never bought any EGFR-TKI. She
was given a free sample of erlotinib intermittently for total of 12 months followed by intermittent afatinib supply for
2 years. Due to this limited supply, she took half doses of afatinib by cutting a 40 mg tablet once every few days to
sustain the continuation of cancer treatment. No major side effects were observed and she remained ECOG 0 with
good weight gain. Up to her last clinic visit in September 2021, her PFS was more than 5 years. Intermittent doses of
EGFR-TKI may prolong PFS in patients with advanced EGFRm+ NSCLC who has limited treatment options.
5.Linguistic and Content Validation of the Stroke Impact Scale (SIS) 3.0 in Three Languages

Introduction: A stroke can have devastating consequences and may reduce the Quality of Life (QoL) of stroke survivors. Specific QoL measurement is therefore needed to appreciate and quantify the impact of this condition. The
objective of the study is to culturally adapt, translate and validate the Bahasa Melayu, Mandarin and Tamil versions
of the Stroke Impact Scale (SIS) 3.0 for its application in clinical practice and stroke research in Malaysia. Methods:
A total of nine translators translated the SIS 3.0 and five professional experts carried out the linguistic validation of
the SIS 3.0 in accordance with the Mapi Research Institute’s linguistic validation guideline. The steps for linguistic
validation included forward translation, synthesis, backward translation, clinicians’ review, and cognitive debriefing.
Ten to thirteen healthcare practitioners rated the translated versions of SIS 3.0 using the Content Validity Index (CVI)
and Content Validity Ratio (CVR). Results: During the translation processes, concerns were expressed about various
grammatical and semantic issues, such as the appropriateness of some phrases used for the Malaysian demographic.
The Scale-CVI average was 0.91 and 0.95 for relevance and clarity respectively which indicated excellent content
validity. The CVR values ranged from 0.40 to 1.00. Conclusion: The Bahasa Melayu, Mandarin and Tamil versions
of SIS 3.0 was well developed. The translated versions of SIS 3.0 could be adopted in clinical, community and educational settings. Nevertheless, further in-depth psychometric testing including construct validity and reliability on a
larger sample among the multi-ethic Malaysians stroke survivors is advised.
6.Knowledge of the Malaysian National Immunisation Programme and its associated factors among parents in Dungun, Terengganu: A cross-sectional study

Introduction:
The number of unvaccinated children and the rate of vaccination refusal among parents have recently increased despite the robust vaccination programme in Malaysia. The purpose of this study was to determine the level of knowledge of the Malaysian National Immunisation Programme among Malaysian parents as well as the variables that contribute to poor knowledge.
Methods:
This study used systematic random sampling to recruit 375 parents from seven health clinics in the Dungun District of Malaysia. The participants were asked to complete a self-administered validated questionnaire that included sociodemographic and knowledge questions. A multiple logistic regression analysis was applied to determine the associated factors of poor knowledge of immunisation. P-values of <0.05 were considered statistically significant.
Results:
A total of 189 (50.4%) respondents had poor knowledge of immunisation. The multiple logistic regression analyses revealed that a low educational level (adjusted odds ratio (OR): 1.92; 95% CI 1.25–2.95) was associated with poor knowledge while the age-group of 30-40 years old (adjusted OR: 0.56; 95% CI 0.34–0.93) was protected against it as compared to the age-group of <30 years old.
Conclusion
The prevalence of poor knowledge of immunisation among parents in Dungun, Terengganu, is relatively high. The focus of health promotion and education must be switched to increasing immunisation knowledge among the public, particularly among high-risk groups.

7.ACTINOMYCES ODONTOLYTICUS: A RARE CAUSE OF PRIMARY CHRONIC LACRIMAL CANALICULITIS

Actinomyces odontolyticus is a predominant Actinomyces species in the oral cavity, pharynx, and distal esophagus. We report a rare case of primary chronic lacrimal canaliculitis caused by A. odontolyticus. A 63-year-old woman presented with left eye epiphora for three years associated with medial upper lid swelling, redness, and chronic yellowish discharge. After clinical examination and investigations, she was successfully treated with topical cefuroxime 5%, topical fluorometholone 0.1%, and intracanalicular syringing with cefuroxime 5%. Spontaneous extrusion of canaliculiths led to complete resolution.
8.Development, validity and reliability of instrument to evaluate implementation fidelity of the Family Doctor Concept (FDC) programme in public primary care clinics in Malaysia

Introduction:
The Family Doctor Concept (FDC) programme was introduced to public primary care clinics in late 2013 as part of the reform agenda in the primary healthcare delivery system. The study aimed to develop a validated and reliable instrument to evaluate the FDC implementation fidelity in primary care clinics.
Methods:
The instrument, which adapted the concept of patient-centred care (PCC), resulted from a series of expert discussions, a literature search, an FDC guideline, and a review of meeting minutes. A 2-step process was conducted with experts to achieve content and face validity. Consequently, the instrument was piloted in 5 public primary care clinics in Selangor involving 8 trained raters. Inter-rater reliability was estimated using intraclass correlation (ICC), while internal consistency was measured using Kuder–Richardson Formula 20 (KR-20).
Results:
The final instrument comprises 3 sections (instructions, clinic’s characteristics and assessment items), with Section 3 containing 15 items divided into four components – population registration, formation of a primary care team, integrated treatment, and monitoring and evaluation. The ICC for total score was excellent, 0.981, while the ICCs of the individual component scores were good to excellent (population registration: 0.937, formation of primary care team: 0.742, integrated treatment: 0.996, and monitoring and evaluation: 0.996). The value of KR-20 was 0.615, which was considered adequate.
Conclusion
The instrument developed was found to be valid in terms of face and content validity and reliable in measuring the fidelity of FDC implementation with excellent inter-rater reliability.

9.A Two Years Retrospective Study on Epidemiology Characteristics of Sexually Transmitted Infections From Malaysian Private Healthcare Perspective

Introduction: Sexually transmitted Infections (STIs) are major public health concerns reaching an all-time high, globally. In Malaysia data on the prevalence of STIs remains scarce which limits the understanding of STI transmission
dynamics and the role of interventions in the control of STIs. The aim of this study is to determine the epidemiology
characteristics of STIs mainly from Malaysian private healthcare institutions. Method: A two years (2016 and 2017)
retrospective review was conducted on 160 multiplex RT-PCR STI reports from KPJ hospitals, Malaysia. Results:
There were 65 (40.6 %) patients positive STIs [male: 21/65 (32.3 %); female: 44/65 (67.7 %)]. The STIs was prevalent among young adults (56/65; 86.2 %) from the central region (46/65; 70.8 %). Females had 1.7 times greater risk
to develop STI (20 per 100) and two times higher chance to have multiple STIs (10 per 100) in comparison to male.
The single STIs was caused mainly by U. parvum (N=17). In males, U. urealyticum (N=3) and C. trachomatis (N=3)
were prevalent, while U. parvum (N=15) was prevalent in females. There were 19 dual infections of STIs which were
commonly caused by U. parvum and M. hominis (N=5). There were seven STIs cases caused by three pathogens
concurrently including U. urealyticum, U. parvum and C. trachomatis (N=2) and U. urealyticum, M. genitalium
and C. trachomatis (N=2). Females from urban communities have higher risk in comparison to males for developing
multiple STIs. Conclusion: This study provides an imperative platform for temporal trends of STIs in Malaysia which
reflects the health status of certain populations that warrant immediate public health interventions.

COVID-19 pandemic results in unprecendeted impacts globally. New discoveries and knowledge on this pandemic is evolving. A study was conducted to determine factors that are associated with knowledge, attitude and practice among medical students at one of the public universities in Malaysia towards COVID-19 pandemic. A cross-sectional study was conducted through online surveys by using a set of questionnare. The questionnaire was adapted and adopted from validated sources. It was found that, the scores for knowledge, practice and attitude were high among medical students. Male students had higher level of knowledge whereas females had significantly higher level of attitude and practice. Level of knowledge differed between years of study. Clinical year students had higher level of knowledge and practice compared to pre-clinical year students. Findings from this study might prove as the baseline for planning awareness campaigns for students for better containment of COVID-19 and its further spread
