1.Based on 16S rDNA Technology and TLRs/MyD88/NF-κB Signaling Pathway, Molecular Mechanism of Shenling Baizhusan Resistance to Diarrhea Irritable Bowel Syndrome Rats Was Investigated

ObjectiveBased on 16S rDNA technology and molecular biology methods, the molecular mechanism of Shenling Baizhusan in the treatment of diarrhea-predominant irritable bowel syndrome (IBS-D) was investigated. MethodsThe 42 SD rats with SPF were randomly divided into no control group, SLBZS-H, medium (SLBZS-M), low (SLBZS-L) dose group, positive control group and model group, with 7 rats in each group. The rat model of IBS-D was prepared by ice-cold senna (0.45 g∙mL-1) gavage (10 mL∙kg-1) combined with restraint stress for 14 consecutive days. After successful modeling, the corresponding drugs were given to each group with a gavage volume of 10 mL∙kg-1: The positive group was administered with 2.36 , 1.18, 0.59 g∙mL-1 of Shenling Baizhusan in the Positive group and the Model group with the same volume of normal saline for 14 d. The general condition of the rats: Weight, feces, mental state and death were observed and recorded. The body weight, abdominal wall retraction reflex score (AWR) and loose stool rate of rats in each group were measured before (the first day), after the model (day 14) and after treatment (day 28). Hematoxylin-eosin staining was used to observe the morphological characteristics of colon tissues of experimental animals. Enzyme-linked immunosorbent assay was used to quantitatively analyze the concentration of inflammatory mediators in the peripheral blood of experimental animals. Western blotting was used to detect the expression levels of key proteins of Toll-like receptor 4 (TLR4), Toll-like receptor 2 (TLR2), myeloid differentiation factor 88 (MyD88) and nuclear factor-κB (NF-κB) signaling pathway in rat colon tissue. 16S rDNA technology was used to detect the structural changes of intestinal microbiota in rats. ResultsCompared with Control, the colon of the Model group showed partial mucosal epithelial shedding and inflammatory cell infiltration. The contents of TNF-α, IL-1β, IL-6 and 5-HT in serum increased (P<0.05), the protein expressions of TLR2, TLR4, MyD88 and NF-κB in colon tissue increased (P<0.05), the diversity indices of Richness, Chao1, abundance-based coverage estimator(ACE) and Shannon decreased (P<0.05), and the phylum Firmicutes, Actinobacteria, The relative richness of Bacteroides-H, Lactobacillus and Ligilactobacillus decreased (P<0.05), while the relative richness of Bacteroidetes, Proteobacteria and Prevotella increased (P<0.05). Compared with the model group, the colonic structure and organization of the SLBZS-H group, SLBZS-M group, SLBZS-L group and Positive group were clearer, and only a small number of inflammatory cells were present in some areas, and the serum contents of TNF-α, IL-1β, IL-6 and 5-HT were decreased (P<0.05), TLR2, TLR4, The protein expressions of MyD88 and NF-κB decreased (P<0.05), and compared with the model group, the diversity indices of Richness, Chao1, ACE and Shannon in the SLBZS-H, SLBZS-M and SLBZS-L groups increased (P<0.05), and the richness of Firmicutes and Actinobacteria increased (P<0.05). The richness of Proteobacteria and Prevotella decreased (P<0.05), and the abundance of Prevotella decreased (P<0.05), Bacteroides-H, Muribaculum, Lactobacillus and salivarius in the Positive group salivarius (P<0.05). ConclusionShenling Baizhusan can effectively treat IBS-D, and its molecular mechanism may be to play a therapeutic role by improving intestinal flora and inhibiting the TLRS/MyD88/NF-κB signaling pathway to reduce inflammatory response.
2.Analysis of influencing factors for prolonged postoperative ileus of pancreaticoduodenectomy during hospitalization

Objective:To investigate the influencing factors of prolonged postoperative ileus (PPOI) in patients undergoing pancreaticoduodenectomy (PD) during hospitalization.Methods:The data of 339 patients underwent PD admitted to the Department of Hepatobiliary Surgery of the Second Hospital of Hebei Medical University from January 2018 to September 2023 were retrospectively analyzed, including 204 males and 135 females, aged (60.6±11.2) years. Among the 339 patients, 112 (33.0%) had pancreatic tumors, 94 (27.7%) had Vater ampullary tumors, 82 (24.2%) had common bile duct tumors, and 51 (15.0%) had duodenal tumors. A total of 339 patients with PPOI were included in the PPOI group ( n=43) and those without PPOI were included in the control group ( n=296). The two groups were compared in terms of age, PD operation (open or laparoscopic), gastrojejunostomy (retrocolic or antecolic gastrojejunostomy), grade B or C pancreatic fistula, hypokalemia, and postoperative use of patient-controlled intravenous analgesia (PCIA). The index comparing P<0.05 between the two groups was further included in the multivariate logistic regression analysis to analyze the influencing factors of PPOI in PD patients. Results:There were statistically significant differences in age >70 years, PD operation, gastrojejunostomy, grade B or C pancreatic fistula, hypokalemia, and postoperative use of PCIA between the two groups (all P<0.05). Multivariate logistic regression analysis showed grade B or C pancreatic fistula ( OR=3.17, 95% CI: 1.48-6.82), open surgery ( OR=2.90, 95% CI: 1.35-6.24), retrocolic gastrojejunostomy ( OR=2.47, 95% CI: 1.23-4.95), postoperative usage of PCIA ( OR=2.61, 95% CI: 1.21-5.62), age >70 years ( OR=2.47, 95% CI: 1.71-5.19) had a high risk of PPOI during postoperative hospitalization (all P<0.05). Conclusion:Postoperative grade B or C pancreatic fistula, open surgery, retrocolic gastrojejunostomy (compares with antecolic gastrojejunostomy), postoperative using PCIA, and age >70 years are independent risk factors for PPOI in patients undergoing PD during postoperative hospitalization.
3.Clinical analysis of intraabdominal unicentric Castleman disease:report of 8 cases

Objective:To summarize the clinical characteristics of patients with unicentric Castleman disease(UCD).Method:The clinical data of 8 abdominal UCD patients who received surgical resection at the Second Hospital of Hebei Medical University from Oct 2019 to Oct 2022 were analyzed, and the imaging characteristics, pathological types and prognosis were summarized.Result:There were 2 males and 6 females. The average age of patients was (33.0±13.7) years old, and their BMI was (23.2±4.5) kg/m 2. The median maximum diameter of the tumor was 4.5 (3.0-4.9) cm. The average postoperative hospital stay was 6.5 (3.3-12.0) days. One was lost during follow up, there was no recurrence or other postoperative complications in the remaining 7 patients. Conclusion:The incidence of unicentric Castleman disease is rare. Complete resection of the tumor is the main treatment for UCD patients, and the prognosis of UCD is good.
4.Diffusional kurtosis imaging value for assessment of liver cancer and tumoral cell invasion of peritumoral zone

Objective Study the apply of diffusional kurtosis imaging(DKI) value to assess liver cancer and tumoral cell invasion of peritumoral liver zone. Methods This research belonging to prospective study which included 24 patients with liver cancer and confirmed by clinical history and imaging features(liver cancer group), 10 healthy volunteers as control group. The liver cancer group underwent MRI plain and contrast enhanced scan, and DKI examination, while control group underwent MRI plain scan and DKI scan. The signal features of liver parenchyma and liver cancer lesion could be observed from the routine MRI and DKI. Fractional anisotropy (FA), mean diffusion (MD), axial diffusivity (Da), radial diffusivity (Dr), fractional anisotropy kurtosis (Fak), mean kurtosis (MK), kurtosis anisotropy (Ka) and radial kurtosis (Kr) value of four groups, the distant liver parenchyma(far away from the tumor>2 cm), peritumoral liver parenchyma(the distance≤2 cm around the tumor) and liver cancer were recorded. The differences of DKI parameters were evaluated using one-way analysis of variance (ANOVA). Results The signal of liver cancer in MR plain scan showed mild long T1 and mild long T2 signal, fast in and fast out enhanced feature of the neoplasms could be observed from the enhanced MRI and signal of liver cancer would not lower in DKI with b value up to top. The difference of DKI parameters including FA, MD, Da, Dr and Ka value had statistical significance in these four groups excepted for MK and Kr value. MD, Da and Dr value of normal parenchyma were higher than that of peritumoral parenchyma and liver cancer,while the Ka value was reverse. The differences of MD, Da, Dr and Ka value only had no statistical significance between the distant liver parenchyma and peritumoral liver parenchyma(P>0.05),and the differences of them had statistical significance among the rest group(P<0.05). Conclusion The DKI quantitative parameters can reflect the differences of different tissue, meaning that they can provide molecular imaging information for evaluating liver cancer and peritumoral zone.
5.Assessment of risk factors for stroke through Relief F algorithm based on stroke registry database

Objective To assess the factors associated with occurrence of stroke in Xuzhou region on the basis of stroke registry database using data mining methods Relief F algorithm.Methods Five hundred and forty-six patients with acute cerebral infarction from June 2013 to June 2014 in Stroke Registry database and 546 healthy people at the same period in the region were chosen;their clinical data were collected and retrospectively analyzed.And the related data were collected,arranged and put into epidata database.Firstly,the data were normalized and converted to the number between 1 and 10.Relief F algorithm was used to analyze the weight of past histories and hematology index between healthy control group and stroke group.Matlab software was used to program and calculate,and the main program runs for 20 times.And then,the obtained results for each weighted average were summarized.Finally,the weight of past medical history and blood parameters in healthy control group and stroke group were obtained.Results Relief F algorithm was applied for data analysis:as compared with healthy controls,stoke group had higher weight of infarction (0.0353125) and history of drinking (0.01175),while not higher weight of history of hypertension,diabetes mellitus or transient ischemic attack;as compared with healthy controls,stoke group had higher weight of uric acid nitrogen level (0.0072),blood uric acid level (0.0071),cholesterol level (0.0067) and homocysteine level (0.0064),followed by high-density lipoprotein (0.0062),low density lipoprotein (0.0041) and triglyceride (0.0039).Conclusion Application of Relief F algorithm can excavate the closely related potential risk factors for stroke in stroke registration database.
