1.Associations of volatile organic compounds/semi-volatile organic compounds exposure on asthma: A review of epidemiological studies and diagnostic applications

Volatile organic compounds (VOCs) and semi-volatile organic compounds (SVOCs) are common organic compounds in indoor and outdoor air, and enter the human body primarily through the respiratory tract and directly damage the respiratory system. Previous studies have suggested that exposure to VOCs/SVOCs may associate with the prevalence, incidence, and progression of asthma, but the extent of the associations is still vague. Furthermore, biomarkers for efficient and simple asthma diagnosis, typing, and attack prediction remain unclear at this stage. From the perspective of the collection and detection methods of VOCs/SVOCs, this paper summarized the epidemiological associations and underlying biological mechanisms between VOCs/SVOCs exposure and the prevalence, incidence, and progression of asthma in children/adults. It also demonstrated the application of VOCs/SVOCs in recent years in assisting asthma diagnosis, such as distinguishing asthma patients from the healthy population, differentiating different asthma phenotypes, and predicting asthma acute exacerbations, aiming to provide a scientific basis for improving current asthma management.
2.Falsely elevated free thyroxine due to interference with anti-thyroxine autoantibodies

This article reports the clinica diagnosis and treatment of a 66-year-old male patient with elevated free thyroxine(FT 4) following coronavirus disease 2019(COVID-19) infection. The thyroid function tests showed a significant increase in FT 4, while thyroid stimulating hormone(TSH) was not suppressed. Re-testing on different platforms revealed inconsistent FT 4 elevations, indicating potential interference in FT 4 measurement. Radioimmunoprecipitation assays confirmed the presence of anti-thyroxine autoantibodies(T 4Ab) in the patient′s body, specifically the IgM subtype. This article reviews relevant literature, focusing on the identification and differential diagnosis of thyroid hormone autoantibodies interference.
3.Behavioral treatment of functional anejaculation and factors influencing the therapeutic effect

Objective:To study the effect of a modified behavioral treatment(MBT)on functional anejaculation and analyze the factors influencing the therapeutic efficacy.Methods:We enrolled in this study 59 men aged 24-45 years visiting the Andrology Clinic of Shanghai First Maternity and Infant Hospital from August 2019 to May 2021 and complaining of aejaculation in sexual inter-course but normally ejaculating during masturbation.Thirty-nine of the patients underwent conventional behavioral treatment(the CBT group)and the other 20 received MBT,namely,changing the masturbation method combined with audiovisual stimulation during sexual intercourse(the MBT group).We compared the therapeutic effects between the two groups of patients,and analyzed the correlation of the outcomes of MBT with age,abstinence duration,use of audiovisual stimulation,change of the sexual position,mean bilateral testis volume and sex hormone levels.Results:After treatment,22(37.29%)of the patients achieved successful ejaculation at least once in sexual intercourse,11(55.00%)in the MBT group,and the other 11(28.21)in the CBT group,with a significantly higher effec-tiveness rate in the former than in the latter(P<0.05).The effectiveness rate was significantly correlated to the method of standing-position masturbation plus sexual intercourse and reduction in the frequency of masturbation among various strategies of behavioral treat-ment(P<0.05).Conclusion:MBT has a certain effect on functional anejaculation,and targeting the previous events of the patient is the key to the therapeutic efficacy.Further exploration of more effective strategies of behavioral treatment will become the trend of de-velopment in the management of functional anejaculation.
4.Causal relationship between obesity and male infertility:A two-sample Mendelian randomization study

Objective:To clarify the causal relationship between obesity and male infertility through Mendelian randomization(MR)study.Methods:We assessed the causal effect of genetically predicted body mass index(BMI)on the risk of male infertility via a two-sample MR analysis,with the BMIs of 99 998 cases and 12 746 controls as the exposure factor and genetic information on male infertility obtained from a genome-wide association study of 73 479 Europeans.In the univariable MR(UVMR)analysis of the causal relationship,we mainly used inverse variance weighting(IVW),with MR-Egger regression and weighted median filtering as the supplementary methods.Sensitivity analyses including the Cochran's Q test,Egger intercept test,MR-PRESSO,leave-one-out analysis and funnel plot were performed to verify the robustness of the MR results.To evaluate the direct causal effects of BMI on MI risk,mult-ivariable MR(MVMR)was performed.Results:UVMR indicated a causal relationship between genetically predicted BMI and an in-creased risk of male infertility(OR:1.237,95%CI:1.090-1.404,P=0.001).Sensitivity analysis revealed little evidence of bias in the current study(P>0.05).With such risk factors as type 2 diabetes,alcohol consumption and smoking adjusted,MVMR confirmed a direct causal effect of genetically predicted BMI on the risk of male infertility(P<0.05).Conclusion:Genetically pre-dicted BMI may be associated with an increased risk of male infertility.Further studies are expected to explore the underlying mecha-nisms of this association and provide some new strategies for the prevention and treatment of BMI-related male infertility.
5.Practice of referral management of health and clinical services in a maternal and child health hospital

Effective referral management of health and clinical services in maternal and child health hospitals plays an important role in enhancing patients′ medical experience, improving the efficiency and quality of maternal and child health services. A tertiary grade A maternal and child health hospital has carried out a practice of health and clinical service referral management based on information technology construction. A referral information module embedded in the hospital information system has been designed and constructed, and started to be applied in outpatient clinics in July 2021. At the same time, corresponding system and process construction, as well as quality control management and continuous improvement, have been carried out. The outpatient referral rate from July to December 2021 was 2.8% (11 466/412 808), from January to June 2022 it was 5.6% (22 705/402 586), from July to December 2022 it was 5.5% (22 233/402 959), and from January to June 2023 it was 6.7% (23 373/347 898). The referral rate has continued to improve and can provide reference for the referral management of other maternal and child health institutions.
6.Effects of dapagliflozin on left ventricular diastolic function in elderly patients with HFpEF during a-cute decompensation

Objective:To study the effects of dapagliflozin on left ventricular diastolic function in elderly patients with heart failure with preserved ejection fraction(HFpEF)during acute decompensation.Methods:A total of 160 elderly patients with HFpEF during acute decompensation treated in Department of Geriatrics,Fujian Provincial Geriatric Hospital were retrospectively enrolled between January 2021 and January 2024.According to different therapeutic regimens,they were divided into routine treatment group(n=80,routine anti-heart failure drug thera-py)and combined dapagliflozin treatment group(n=80,dapagliflozin on basis of routine anti-heart failure drug therapy).All patients were treated for 3 months.The clinical curative effect,changes of left ventricular diastolic function indexes[early diastolic mitral annular peak velocity(e'),ratio of left ventricular early diastolic filling peak velocity(E)to e'(E/e'),tricuspid regurgitation velocity(TRV),left atrial diameter(LAD)],inflammatory factors[serum amyloid A(SAA),high sensitive C-reactive protein(hsCRP),interleukin-6(IL-6)],myocardial enzymes[lactate dehydrogenase(LDH),creatine kinase(CK),creatine kinase isoenzyme(CK-MB),α-hydroxybutyrate dehy-drogenase(α-HBDH)]and N-terminal pro brain natriuretic peptide(NT-proBNP),and the occurrence of adverse re-actions were compared between the two groups.Results:Compared with routine treatment group,total effective rate was significantly higher in combined dapagliflozin treatment group(85.00%vs.95.00%,P=0.035).After treatment,E/e',TRV,LAD,levels of SAA,hsCRP,IL-6,LDH,CK,CK-MB,α-HBDH and NT-proBNP in combined dapaglifloz-in treatment group were significantly lower than those in routine treatment group(P<0.01 all).There was no significant difference in the incidence rate of adverse reactions between the two groups(P=0.718).Conclusion:Dapagliflozin can significantly improve left ventricular diastolic function and effectively alleviate the progression of heart failure in elderly pa-tients with HFpEF during acute decompensation,which has certain clinical significance.
7.A comparative study on iodine nutritional status of the populations consuming iodized salt and non-iodized salt in iodine adequate areas

Objective:To compare iodine nutritional status of different populations consuming iodized salt and non-iodized salt in iodine adequate areas, and to provide a basis for formulating iodine supplementation strategies.Methods:In October 2021, Luyi County in Henan Province was selected as an iodine adequate area consuming iodized salt, while Ningling County was selected as an iodine adequate area consuming non-iodized salt. Stratified by water iodine (50 - 59, 60 - 69, 70 - 79, 80 - 89, 90 - 100 μg/L), one village was selected from each layer. One hundred children aged 8 - 10, one hundred adults, and 20 pregnant women were selected from each village to collect their urine and salt samples for testing salt and urinary iodine, and their thyroid gland was measured by ultrasound.Results:A total of 600 salt samples in Luyi County were collected, with the coverage rate of iodized salt (99.8%, 599/600) and the consuming rate of qualified iodized salt (95.5%, 573/600). A total of 1 008 salt samples in Ningling County were collected, with the rate of non-iodized salt (93.8%, 946/1 008). The median urinary iodine of children in Luyi County ( n = 240) was higher than that in Ningling County ( n = 468, 305.0 vs 232.0 μg/L, Z = - 8.10, P < 0.001). There was no statistically significant difference in median urinary iodine between pregnant women in Luyi County ( n = 120) and Ningling County ( n = 53, 240.0 vs 236.0 μg/L, Z = - 1.02, P = 0.306). The median urinary iodine of adults in Luyi County ( n = 238) was higher than that in Ningling County ( n = 486, 289.0 vs 178.5 μg/L, Z = - 11.14, P < 0.001). Children's urinary iodine ( r s = 0.21, P = 0.001) in Luyi County and adults' urinary iodine ( r s = 0.17, P < 0001) in Ningling County were positively correlated with water iodine. There was no statistically significant difference in the incidence of thyroid enlargement in children between Luyi County (0.8%, 2/240) and Ningling County (0.4%, 2/468, χ 2 = 0.80, P = 0.586), but the incidence of thyroid nodules in children in Luyi County (11.2%, 27/240) was higher than that in Ningling County (1.7%, 8/468, χ 2 = 27.36, P < 0.001). The incidence of thyroid nodules in pregnant women in Luyi County (23.3%, 28/120) was lower than that in Ningling County (46.5%, 33/71, χ 2 = 10.99, P = 0.001). There was no statistically significant difference in the incidence of adult thyroid nodules between Luyi County and Ningling County (χ 2 = 0.86, P = 0.354), with a ratio of 29.6% (71/240) to 32.9% (160/486). Conclusions:Providing population with non-iodized salt in iodine adequate areas, the overall iodine nutrition is at an appropriate level. However, children consuming iodized salt in iodine adequate areas have high level of iodine nutrition, and it is necessary to consider supplying non-iodized salt or reducing the concentration of iodized salt. Pregnant women in iodine adequate area should maintain the current policy of supplying iodized salt unchanged.
8.Analysis of iodine nutritional status of key populations in iodine adequate areas of Henan Province in 2022

Objective:To investigate the iodine nutritional status of key populations in iodine adequate areas in Henan Province, and provide a basis for timely adoption of targeted prevention and control measures and scientific formulation of iodine supplementation strategies in iodine adequate areas.Methods:From March to September 2022, a cross-sectional survey was conducted in 60 iodine adequate counties (cities, districts) in 13 provincial-level cities in Henan Province. Administrative villages with a median water iodine level of 40 - 100 μg/L within each county (city, district) were stratified by water iodine value (40 - 49, 50 - 59, 60 - 69, 70 - 79, 80 - 89, 90 - 100 μg/L). One administrative village was selected from each water iodine layer, and 40 non boarding students aged 8 - 10 years old (age balanced, half male and half female) and 20 pregnant women were selected from each administrative village to collect their household edible salt samples and once random urine sample for test salt iodine and urinary iodine contents; and the thyroid volume of children was measured.Results:A total of 12 203 samples of household edible salt were collected from children aged 8 - 10 years old, with a median salt iodine of 24.5 mg/kg, the iodized salt coverage rate was 85.3% (10 414/12 203), and the qualified iodized salt consumption rate was 77.1% (9 406/12 203). A total of 3 999 samples of household edible salt were collected from pregnant women, with a median salt iodine of 24.0 mg/kg, the iodized salt coverage rate was 84.2% (3 366/3 999), and the qualified iodized salt consumption rate was 74.5% (2 981/3 999). A total of 12 241 urine samples from children aged 8 - 10 years old were collected, with a median urinary iodine of 290.4 μg/L. A total of 4 084 urine samples from pregnant women were collectedd, with a median urinary iodine of 233.0 μg/L. The thyroid volume of 11 971 children was examined, and the rate of goiter was 2.1% (257/11 971). Stratified by water iodine content, the median urinary iodine levels of children in 40 - 59, 60 - 79, and 80 - 100 μg/L water iodine groups were 269.7, 298.0 and 308.0 μg/L, respectively, with a statistically significant difference between the groups ( H = 67.32, P < 0.001). The goiter rates of children were 2.2% (100/4 603), 2.1% (80/3 733), 2.1% (77/3 635), respectively, with no statistically significant difference between the groups (χ 2 = 0.03, P = 0.986). The median urinary iodine levels of pregnant women were 225.4, 243.1, 234.4 μg/L, respectively, with a statistically significant difference between the groups ( H = 10.96, P = 0.004). Conclusions:The iodine nutrition level of children in iodine adequate areas in Henan Province is at an excessively suitable level, and pregnant women's iodine nutrition is at an appropriate level. It is recommended to strengthen surveillance on the basis of maintaining current prevention and control measures, and adjust prevention and control measures in a timely manner according to changes in iodine nutrition in the population.
9.Risk factors of central line-associated bloodstream infection in the inten-sive care unit based on propensity score matching

Objective To explore the independent risk factors for central line-associated bloodstream infection(CLABSI),provide basis for developing intervention measures for infection prevention and control as well as con-ducting targeted treatment.Methods Patients who were diagnosed with CLABSI in a hospital from January 2019 to December 2023 were recruited retrospectively and defined as the infection group.According to 1:4 propensity score matching method,patients who received central venous catheter(CVC)without infection were taken as the control group.With whether CLABSI occurred as the dependent variable,the possible risk factors of the matched two groups as the independent variables,logistic regression analysis was conducted,and independent risk factors for pa-tients developing CLABSI were explored.Results A total of 42 patients in the CLABSI group and 168 patients in the non-CLABSI group were matched.Multivariate logistic regression analysis showed that high score of acute physiology and chronic health evaluation(APACHE)Ⅱ(OR=1.217,95%CI[1.094-1.357],P<0.001),long duration of central venous catheterization(OR=1.273,95%CI[1.157-1.400],P<0.001),and femoral venous catheterization(OR=6.846,95%CI[1.511-31.014],P=0.013)were independent risk factors for CLABSI.A total of 118 strains of pathogens were isolated from 42 CLABSI patients,with Gram-negative bacteria being the ma-jority(n=56).Conclusion High score of APACHE Ⅱ,long duration of central venous catheterization,and femo-ral venous catheterization are independent risk factors for CLABSI.The main pathogens are Gram-negative bacilli.Strict prevention and control measures for CLABSI should be implemented to reduce the risk of infection.
10.Clinical Study on the Treatment of Knee Osteoarthritis with Xiaoyusan Simplified Plaster

Objective To observe the clinical efficacy of Xiaoyusan simplified plaster in the treatment of knee osteoarthritis and the significance of intervention in cartilage metabolism.Methods Totally 105 patients with knee osteoarthritis were divided into Xiaoyusan simplified prescription group,Xiaoyusan group and diclofenac diethylamine group according to random number table method.Xiaoyusan simplified prescription group:Apply it to the swollen and painful area,covering the entire lesion area,and change the dressing every 24 hours;Xiaoyusan group:Apply it to the swollen and painful area,covering the entire lesion area,and change the dressing every 24 hours;diclofenac diethylamine group:Apply evenly to the swollen and painful area,3 times a day;all the groups were treated for 2 courses(4 weeks).The changes of VAS score,WOMAC score,knee joint activity and the content of cartilage metabolism index HA,type Ⅱ collagen,COMP and CTX-Ⅱ before and after treatment were compared.Safety indicators of the three groups were monitored.Results The total effective rates of Xiaoyusan simplified plaster group and Xiaoyusan group were all over 97.1%(33/34),better than the diclofenac diethylamine group(81.8%,27/33),with statistical significance in the three groups(P<0.05).Xiaoyusan simplified plaster group and Xiaoyusan group were superior to diclofenac diethylamine group in VAS score,WOMAC score,sitting-standing 5 times time(P<0.05);Xiaoyusan simplified plaster group and Xiaoyusan group were superior to diclofenac diethylamine group in delaying the decrease of HA content and the increase of COMP content,with statistical significance(P<0.05).After treatment,there was no significant difference in VAS score,WOMAC score,knee joint activity and bone metabolism index content between Xiaoyusan simplified plaster group and Xiaoyusan group(P>0.05).The blood routine,urine routine,liver function,kidney function and electrocardiogram examinations before and after treatment in the three groups were all normal.Conclusion Xiaoyusan simplified plaster has the same efficacy as Xiaoyusan,which can reduce the pain of knee osteoarthritis patients,increase the knee joint activity and daily living ability of patients,and delay the cartilage injury metabolism.
