1.Fracture resistance of premolar screw-retained implant-supported hybrid abutment crowns with monolithic restorations after thermomechanical aging

PURPOSE:
. This study evaluated the fracture resistance of premolar screwretained implant-supported hybrid abutment crowns (HACs) fabricated from different monolithic restorative materials following artificial aging.
MATERIALS AND METHODS:
. Forty-four titanium implant analogs were restored with HACs fabricated from four materials: 5 mol% yttria-partially stabilized monolithic zirconia (ST), strength-gradient multilayered zirconia (YM), lithium disilicate ceramic (LD), and resin composite containing dispersed nanoparticles (CM) (n = 11 per group). All specimens were subjected to thermocycling (10,000 cycles) and mechanical loading (1.2 million cycles) to simulate 5 years of clinical service.Fracture resistance was evaluated under compressive loading, and failure modes were analyzed using optical microscopy and scanning electron microscopy.Statistical analysis was performed using the Kruskal-Wallis and Steel-Dwass tests (α = 0.05).
RESULTS:
. Significant differences were found among all groups (P = 0.00033-0.0022). The YM group exhibited the highest median fracture resistance (2.06 kN, IQR: 2.00 – 2.60 kN), followed by ST (1.74 kN, IQR: 1.69 – 1.80 kN), LD (1.29 kN, IQR: 1.14 – 1.41 kN), and CM (0.87 kN, IQR: 0.79 – 0.92 kN). All specimens survived the artificial aging protocols. Distinct fracture patterns were observed depending on the restorative material, with zirconia-based groups showing more favorable resistance than resin composite.
CONCLUSION
. All tested HACs exhibited fracture resistance values exceeding maximum bite forces, supporting their clinical applicability. Strength-gradient multilayered zirconia demonstrated superior reliability after simulated 5-year aging, emphasizing its structural advantage and broader safety margin compared with other restorative materials.
2.Inhibition of TNFα-interacting protein α (Tipα)-associated gastric carcinogenesis by BTG2(/TIS21) via downregulating cytoplasmic nucleolin expression

To understand the regulation of Helicobacter pylori (H. pylori)-associated gastric carcinogenesis, we examined the effect of B-cell translocation gene 2 (BTG2) expression on the biological activity of Tipα, an oncoprotein secreted from H. pylori. BTG2, the human ortholog of mouse TIS21 (BTG2(/TIS21)), has been reported to be a primary response gene that is transiently expressed in response to various stimulations. Here, we report that BTG2 is constitutively expressed in the mucous epithelium and parietal cells of the gastric gland in the stomach. Expression was increased in the mucous epithelium following H. pylori infection in contrast to its loss in human gastric adenocarcinoma. Indeed, adenoviral transduction of BTG2(/TIS21) significantly inhibited Tipα activity in MKN-1 and MGT-40, human and mouse gastric cancer cells, respectively, thereby downregulating tumor necrosis factor-α (TNFα) expression and Erk1/2 phosphorylation by reducing expression of nucleolin, a Tipα receptor. Chromatin immunoprecipitation proved that BTG2(/TIS21) inhibited Sp1 expression and its binding to the promoter of the nucleolin gene. In addition, BTG2(/TIS21) expression significantly reduced membrane-localized nucleolin expression in cancer cells, and the loss of BTG2(/TIS21) expression induced cytoplasmic nucleolin availability in gastric cancer tissues, as evidenced by immunoblotting and immunohistochemistry. Higher expression of BTG2 and lower expression of nucleolin were accompanied with better overall survival of poorly differentiated gastric cancer patients. This is the first report showing that BTG2(/TIS21) inhibits nucleolin expression via Sp1 binding, which might be associated with the inhibition of H. pylori-induced carcinogenesis. We suggest that BTG2(/TIS21) is a potential inhibitor of nucleolin in the cytoplasm, leading to inhibition of carcinogenesis after H. pylori infection.
3.Anterior mediastinal abscess diagnosed in a young sumo wrestler after closed blunt chest trauma.

Most mediastinal abscesses result from infections after thoracotomy, esophageal perforation or pene- trating chest trauma. This disease is rarely caused by closed blunt chest trauma. All previously reported such cases after closed blunt chest trauma presented with hematoma and sternal osteomyelitis resulting from sternal fracture. Here we report a 15-year-old sumo wrestler who presented with an anterior mediastinal abscess without any mediastinal fracture. The mediastinal abscess resulted from the hematogenous spread of Staphylococcus aureus to a hematoma that might have been caused by a closed blunt chest trauma incurred during sumo wrestling exercises.
