1.Pregnancy complicated with antithrombin Ⅲ deficiency: a case report and literature review

Objective:To report the diagnosis and treatment of a pregnant woman with antithrombin (AT) Ⅲ deficiency in pregnancy, and combine the diagnosis and treatment ideas with the literature.Methods:The clinical diagnosis and treatment process of one pregnant woman with AT-Ⅲ deficiency admitted to the Second Affiliated Hospital of Zhengzhou University from October 2023 to June 2024 were retrospectively analyzed. Combined with relevant literature, the effect of pregnancy with AT-Ⅲ deficiency on pregnancy outcome was summarized, and reference was provided for the formulation of anticoagulant regimen.Results:The patient had 3 adverse pregnancy histories and a deep venous thrombosis of the lower limbs. The AT activity fluctuated between 34% and 50% during this pregnancy. The patient received 3 transfusions of fresh frozen plasma during pregnancy and was administered low molecular weight heparin from pre-pregnancy until 24 hours before delivery. Warfarin, hydroxychloroquine, cyclosporine and methylprednisolone were added in the second trimester. The pregnancy was terminated at 35 weeks of gestation. The infant was born with a weight of 2,330 g and was transferred to the pediatric department for treatment. After 14 days, the infant was discharged. Follow-up to date has not revealed any abnormalities.Conclusion:Precise treatment of pregnancy with AT-Ⅲ deficiency can effectively improve pregnancy outcomes.

3.Analysis on reliability and validity of the Pittsburgh sleep quality index

Objective To assess the reliability and validity of the Pittsburgh sleep quality index (PSQI) .Methods The cross-sectional survey was conducted in the outpatient department of this hospital .544 insomnia and non-insomnia voluntary participants (18-65 years old) from July to October 2011 were assessed with PSQI and the insomnia severity index (ISI) ,which were randomly delivered to them .Results The test-retest reliability of PSQI was 0 .994 .The split-half reliability coefficient of PSQI was 0 .824 . The overall Cronbach′s alpha coefficient was 0 .845 .The confirmatory factor analysis showed that the fitting indicators for the χ2/df ,root mean square error of approximation(RMSEA) ,non-normed fit index(NNFI) ,comparative fit index(CFI) and goodness-of-fit index(GFI) were 4 .83 ,0 .09 ,0 .96 ,0 .98 and 0 .97 respectively .The correlation coefficient between the PSQI and ISI was 0 .842 . The correlations coefficients between PSQI with the questionnaire type ,interview date and interview department were -0 .046 , 0 .276 and -0 .331 .Grouping by individuals with or without insomnia ,the area under the receiver operator characteristic curve (ROC) for PSQI was 0 .944(95% CI:0 .925-0 .963) .Conclusion PSQI has good reliability and validity ,and could be used for the sleep quality comprehensive evaluation in the patients with insomnia .
4.Simultaneous determination of tetrandrine,fangchinoline and berberine hydrochloride in Fengtongan Capsule by RP-HPLC

AIM:To develop a method for simultaneously determining tetrandrine,fangchinoline and berberine hydrochloride in Fengtongan Capsule(Radix Stephaniae tetrandrae,Medulla Tetrapanacis,Fructus Chaenomelis,Cortex Phellodendri Chinensis,etc).METHODS:RP-HPLC was used with Phenomenex Gemini C_ 18 column.The mobile phase was a mixture of methanol-acetonitrile-phosphate buffer solution(0.01 mol/L potassium dihydrogen phosphate and 0.01 mol/L sodium heptanesulfonate were mixed equally,which contained 0.2% triethylamine,then was adjusted to pH 3.0 by phosphoric acid)(25∶20∶55).The detection wavelength was at 230 nm.RESULTS:There was a good linear relationship in tetrandrine content of 0.051 52-0.824 3 ?g(r= 0.999 9),fangchinoline content of 0.035 20-0.563 2 ?g(r= 0.999 9),and berberine hydrochloride content of 0.031 36-0.501 8 ?g(r= 0.999 7).The average recovery of tetrandrine was 97.23%(RSD=1.3%,n=9),99.56%(RSD= 1.6%,n=9)for fangchinoline and 98.22%(RSD=1.5%,n=9)for berberine hydrochloride.CONCLUSION:The method is simple,selective and reproducible.It can be applied to the quality control of Fengtongan Capsule.
