1.Clinical Efficacy of Gandou Fumu Granules in Intervention of Liver Fibrosis in WD Patients with Phlegm and Blood Stasis Syndrome and Effect on Cuproptosis-related Indicators

ObjectiveTo observe the clinical efficacy of Gandou Fumu Granules (GDFMG) combined with sodium dimercaptosulphonate (DMPS) on liver fibrosis in Wilson disease (WD) patients with the syndrome of phlegm and blood stasis, evaluate its effect on cuproptosis-related indicators, and explore the possible mechanisms of cuproptosis in WD-related liver fibrosis. MethodsSixty WD patients diagnosed with the syndrome of phlegm and blood stasis between January 2023 and December 2023 were randomly divided into a control group and an observation group, with 30 patients in each group. The control group received the copper chelator DMPS for the first 6 days, followed by calcium gluconate injection for the next 2 days, completing an 8-day treatment cycle. The observation group received GDFMG in addition to the treatment regimen of the control group, with both groups treated for 21 cycles. A Beckman fully automated biochemical analyzer was used to detect levels of type Ⅳ collagen (CⅣ), hyaluronic acid (HA), laminin (LN), N-terminal propeptide of type Ⅲ procollagen (PⅢ-NP), and serum copper (SCu) before and after treatment in both groups. Enzyme-linked immunosorbent assay (ELISA) was used to measure levels of ferredoxin 1 (FDX1), lipoic acid synthetase (LIAS), and dihydrolipoamide S-acetyltransferase (DLAT). Atomic absorption spectroscopy measured 24-hour urine copper levels before treatment and after the 7, 14, and 21 treatment cycles in both groups. An Fibro Touch (FT) non-invasive liver fibrosis diagnostic device was used to measure liver stiffness (LSM) in both groups before and after treatment. Traditional Chinese medicine syndrome score (TCMSS) was evaluated at the same intervals. Clinical efficacy, adverse events, and safety indicators were also compared. ResultsAfter treatment, levels of CⅣ, HA, LN, and PⅢNP significantly decreased in both groups compared to pre-treatment levels (P<0.01). The observation group showed a more pronounced reduction compared to the control group (P<0.05). There were no statistically significant differences in SCu levels in both groups before and after treatment. After treatment, levels of FDX1,LIAS and DLAT significantly increased in both groups(P<0.01). The observation group showed more notable improvements in these indicators than the control group (P<0.05). After the 7, 14, 21 treatment cycles, 24-hour urine copper levels significantly increased in both groups compared to pre-treatment levels (P<0.01). The observation group had a greater increase in 24-hour urine copper levels than the control group after treatment (P<0.05,P<0.01), and although 24-hour urine copper levels increased after 7 cycles, a gradual decline was observed in subsequent cycles. After treatment, LSM levels significantly decreased in both groups compared to pre-treatment levels (P<0.01), with the observation group showing a greater reduction than the control group (P<0.05). Clinical efficacy was significantly better in the observation group than the control group (P<0.05). No significant differences in the incidence of adverse events or safety indicators were observed between the two groups after treatment. ConclusionGDFMG combined with DMPS can reduce LSM in WD patients with liver fibrosis and the syndrome of phlegm and blood stasis, inhibit cuproptosis, and improve clinical efficacy.
2.Pathogen spectrum of diarrheal disease surveillance in Fengxian District, Shanghai, 2013‒2023

ObjectiveTo investigate the pathogenic spectrum and epidemiological characteristics of diarrheal disease in Fengxian District of Shanghai, and to provide scientific basis for the prevention and control of diarrheal diseases. MethodsBasic information of the initial adult cases visited diarrheal disease surveillance sentinel hospital in Fengxian District, Shanghai, was collected from August 2013 to 2023, and fecal samples were collected at 1∶5 sampling intervals to isolate and identify 5 kinds of diarrheagenic Escherichia coli (DEC), Salmonella (SAL), Vibrio parahaemolyticus, Campylobacter, Vibrio cholerae, Shigella and Yersinia enterocolitica (YE). Simultaneously, nucleic acid detection was performed for 3 kinds of rotavirus, 2 kinds of norovirus, intestinal adenovirus, astrovirus and sapovirus. ResultsA total of 1 861 cases of newly diagnosed diarrheal disease were reported, with the peak in July to August. Additionally, 704 surveillance samples were detected, with a total positive detection rate of 50.57%. The detection rates of bacterial, viral and mixed infection were 25.14%, 21.02% and 4.40%, respectively. Among the pathogens detected, DEC accounted for the highest (17.61%, 124/704), followed by norovirus (16.48%, 116/704), rotavirus (6.39%, 45/704), SAL (5.97%, 42/704) and Campylobacter (3.84%, 27/704). DEC detected were mainly enteroaggregative Escherichia coli and enterotoxigenic Escherichia coli, with no detection of Vibrio cholerae, Shigella and YE. The highest total pathogen detection rate was observed from June to September, and the detection peaks of norovirus were from March to June and from October to December, whereas that of DEC was from June to October. The detection rate of rotavirus peaked from January to February, but which was not detected between 2020‒2023. The SAL positive rate peak was in September, whereas that of Campylobacter was from July to September. ConclusionThe main pathogens detected in Fengxian District from 2013‒2019 are DEC, norovirus, rotavirus, SAL and Campylobacter. Different pathogens have different detection peaks, with bacteria predominating in summer and viruses in winter and spring. Prevention and control measures should be carried out according to the epidemiological characteristics of different seasons.
3.Bioinformatics Analysis and Validation of Cuproptosis-related Genes in Wilson Disease

ObjectiveTo explore the role of cuproptosis and identify cuproptosis-related genes in Wilson disease (WD) through bioinformatics analysis and clinical validation,providing implications and directions for the diagnosis and treatment of WD. Methods(1) Screening of target genes: The differentially expressed genes (DEGs) between WD and healthy control were obtained from GeneCards,and the cuproptosis-related genes were obtained from Gene Expression Omnibus (GEO) and published literature.The cuproptosis-related genes in WD were obtained by intersection.Through gene ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses,the specific biological process,functions or metabolic pathways of cuproptosis-related genes in WD were predicted.Molecular docking and PyMOL visualization were then performed to analyze and verify the potential regulatory mechanism of Gandou Fumu Decoction for cuproptosis.(2)Validation of target genes: The blood samples of 15 WD patients treated in the department of encephalopathy and 15 healthy volunteers undergoing physical examinations in the health management center were randomly collected from the First Affiliated Hospital of Anhui University of Chinese Medicine.The expression levels of target genes were determined by Western blot and real-time PCR. Results(1) A total of 3 607 DEGs in WD were obtained from GSE107323 in GEO,and 68 cuproptosis-related genes were obtained from GeneCards and published literature.Twelve common target genes were obtained by intersection,including three up-related genes(SQSTM1,MIF1,and TAX1BP1) and nine down-regulated genes(CP,SERPINE1,AOC3,GPX4,SLC27A5,VEGF-A,PDHB,PDK1,and ATP7B).The common target genes were mainly enriched in monocarboxylic acid metabolism,oxidoreductase activity,negative regulation of molecular functions,which mainly involved HIF-1,ferroptosis and other signaling pathways.Molecular docking and PyMOL visualization results showed Gandou Fumu Decoction had good binding ability with the cuproptosis-related genes PDK1,SERPINE1,VEGFA,and AOC3 in WD.(2)A total of 30 blood samples were collected,including 15 WD patients and 15 health volunteers.Western blot results showed that expression levels of target genes were consistent with the results obtained by bioinformatics analysis.RT-qPCR results showed that compared with healthy volunteers,WD patients had down-regulated mRNA levels of SERPINE1,GPX4,SLC27A5,and VEGF-A and up-regulated mRNA levels of SQSTM1 and MIF1(P<0.05). ConclusionThe expression levels of cuproptosis-related genes in WD patients are consistent with the results predicted by bioinformatics analysis.The characteristic preparation Gandou Fumu Decoction of Xin'an Medicine showed good binding abilities with the cuproptosis-related genes in WD.Cuproptosis may play a key role in the pathophysiological mechanism of WD,which can provide a new target for the diagnosis and treatment of WD.
4.Measurement method and current application of medication adherence in patients with chronic respiratory diseases

Medication adherence is an important indicator for assessing whether patients follow medical advice during treatment, and its level directly affects disease control and the quality of life of patients. Therefore, accurate and effective assessment is essential for chronic disease management and intervention. This paper takes chronic obstructive pulmonary disease (COPD) and asthma, two types of chronic respiratory diseases, as representative to review the existing measurement methods and current application status of medication adherence. It is found that the existing assessment methods for medication adherence can be categorized into objective measurement methods and subjective measurement methods. Objective measures include drug concentration monitoring, pill counting, and electronic medication devices, which generally offer high accuracy. Subjective measures include physician assessments, inhalation technique evaluations, and questionnaires. While these methods are straightforward and easy to implement, their accuracy is often influenced by the subjective factors of assessors of patients, which may lead to biased results. Currently, there is still a lack of a universally accepted “gold standard” for evaluating medication adherence. Selecting the appropriate measurement method requires a comprehensive consideration of factors such as research objectives, disease type, patient characteristics, and data availability to ensure the validity and reliability of the assessment results.
5.Improvement of quality standards for Zhuang medicine Yingbupu (Aralia armata)

OBJECTIVE To improve the quality standard of Zhuang medicine Yingbupu (Aralia armata). METHODS A total of 23 batches of Yingbupu (A. armata) were studied. Their macroscopic characteristics and powder microscopic features were observed. TLC was employed for the qualitative identification of oleanolic acid and araloside A. Items such as water content, total ash, acid-insoluble ash, and ethanol-soluble extract were determined according to the methods specified in the 2020 edition of the Chinese Pharmacopoeia (part Ⅳ). UPLC fingerprint was established for 23 batches of samples by using Similarity Evaluation System for Chromatographic Fingerprints of Traditional Chinese Medicine (2012 edition), and the contents of oleanolic acid and araloside A were determined. RESULTS The powder microscopic characteristics of the medicinal material were distinctive. Oleanolic acid and araloside A were detected by TLC in all 23 batches. Among the 23 batches of samples, the content ranges of moisture, total ash, acid-insoluble ash, and ethanol-soluble extract were 6.9% to 10.4%, 1.8% to 6.8%, 0.1% to 1.9%, and 2.8% to 8.4%, respectively. Based on the UPLC fingerprint, a total of 15 common peaks were obtained, and 9 of these common peaks were identified. The content ranges of oleanolic acid and araloside A in the 23 batches of samples were 0.86% to 2.69% and 0.16% to 1.10%, respectively. CONCLUSIONS This study has added items such as moisture and total ash content fingerprint, TLC identification. A preliminary quality standard has been established for the medicinal material of Yingbupu (A. armata), stipulating that the moisture content should not exceed 11.0%, the total ash content should not exceed 5.0%, the acid-insoluble ash content should not exceed 2.5%, the ethanol-soluble extract(No. content should not be less than 4.0%, and the contents of zyyzdxk-2023165) oleanolic acid and araloside A should not be less than 1.00% and 0.45%( calculated by a dried basis), respectively.

Colorectal tumorigenesis generally progresses from adenoma to adenocarcinoma, accompanied by dynamic changes in the tumor microenvironment (TME). A randomized controlled trial has confirmed the efficacy and safety of Shen-Bai-Jie-Du decoction (SBJDD) in preventing colorectal tumorigenesis. However, the mechanism remains unclear. In this study, we employed single-cell RNA sequencing (scRNA-seq) to investigate the dynamic evolution of the TME and validated cell infiltration with multiplex immunohistochemistry and flow cytometry. Bulk RNA sequencing was utilized to assess the underlying mechanisms. Our results constructed the mutually verifiable single-cell transcriptomic atlases in Apc Min/+ mice and clinical patients. There was a marked accumulation of CCL22+ dendritic cells (DCs) and an enhanced immunosuppressive action, which SBJDD and berberine reversed. Combined treatment with cholesterol and lipopolysaccharide induced characteristic gene expression of CCL22+ DCs, which may represent "exhausted DCs". Intraperitoneal injection of these DCs after SBJDD treatment eliminated its therapeutic effects. TMEM131 derived CCL22+ DCs generation by TNF signaling pathway and may be a potential target of berberine in retarding colorectal tumorigenesis. These findings emphasize the role of exhausted DCs and the regulatory mechanisms of SBJDD and berberine in colorectal cancer (CRC), suggesting that the multi-component properties of SBJDD may help restore TME homeostasis and offer novel cancer therapy.

OBJECTIVES:
To explore the efficacy of DSA-guided intrathecal drug delivery system combined with Zi Wu Liu Zhu Acupoint Therapy for management of cancer pain and provide reference for its standardized clinical application. Methods and.
RESULTS:
Recommendations were formulated based on literature review and expert group discussion, and consensus was reached following expert consultation. The consensus recommendations are comprehensive, covering the entire treatment procedures from preoperative assessment and preparation, surgical operation process, postoperative management and traditional Chinese medicine treatment to individualized treatment planning. The study results showed that the treatment plans combining traditional Chinese with Western medicine effectively alleviated cancer pain, reduced the use of opioid drugs, and significantly improved the quality of life and enhanced immune function of the patients. Postoperative follow-up suggested good treatment tolerance among the patients without serious complications.
CONCLUSIONS
The formulated consensus is comprehensive and can provide reference for clinicians to use DSA-guided intrathecal drug delivery system combined with Zi Wu Liu Zhu Acupoint Therapy. The combined treatment has a high clinical value with a good safety profile for management of cancer pain.

8.A cross lagged analysis of the co-occurrence of health risk behaviors and school connectedness among junior high school students

Objective:
To explore the causal relationship between adolescent school connectedness and the co-occurrence with health risk behaviors among junior high school students, so as to provide the reference for reducing the occurrence of adolescents health risk behaviors among junior high school students.
Methods:
A total of 924 students from two junior high schools in Jishou City were selected by the convenience sampling and cluster sampling methods, and two follow up surveys were conducted at 6 month intervals in April (T1) and October (T2) of 2023 using the Adolescent Health Risk Behavior Questionnaire and the School Connection Scale. The scores of the co-occurrence of school connectedness and health risk behaviors among junior high school students were compared by different survey periods and genders using the t-test, and cross lagged analyses were performed using Mplus 8.3 software.
Results:
School connectedness scores among junior high school students in T1 and T2 surveys were (38.86±7.46) (37.87± 7.71 ) and co-occurrence of health risk behaviors scores were (1.64±0.68) (1.83±0.53), respectively, and the differences between the scores of the 2 surveys were statistically significant ( t=4.24, -4.14, P <0.05). The correlation between school connectedness and co-occurrence of health risk behaviors were statistically significant in both surveys ( r =-0.46 to -0.33, P <0.05). Cross lagged analyses showed that school connectedness in T1 negatively predicted the co-occurrence of health risk behaviors in T2 ( β =-0.08), and the co-occurrence of health risk behaviors in T1 negatively predicted the school connectedness in T2 ( β =-0.15) ( P <0.05).
Conclusions
There is a longitudinal causal relationship between school connectedness and co-occurrence of health risk behaviors among junior high school students. School connectedness should be improved through various ways to reduce the co-occurrence of health risk behaviors.

Objective To comprehensively excavate and analyze the research status,research hotspots and future trends of the literature related to the field of acupoint catgut embedding therapy for pain treatment in the CNKI database.Methods We searched the CNKI database from its establishment to June 2022,and scientifically analyzed the authors,keywords,and institutions of the included literature of acupoint catgut embedding therapy for pain treatment through specific algorithms of Citespace to generate a visual knowledge map.Results A total of 319 documents were included for statistical analysis,the number of publications in the field of acupoint catgut embedding therapy for the treatment of pain was generally on the rise,the number of publications by various authors was on the low side,and there was a lack of co-operation between the research teams,with the main institutions being the Guang'anmen Hospital,Chinese Academy of Chinese Medical Sciences,Affiliated Hospital of Youjiang Medical Universities of Nationalities and the Guangzhou University of Chinese Medicine,forming a 10-keyword clustering,and the hotspots of diseases under study were mainly mixed haemorrhoids,postoperative pain,low back and leg pain and dysmenorrhoea,etc..The main interventions were pure acupoint catgut embedding therapy and the combination of acupoint catgut embedding therapy and other acupuncture therapies,and the main research method was clinical research.Conclusion Acupoint catgut embedding therapy for the treatment of pain has a good development prospect,the future needs to deepen the clinical research,strengthen the mechanism research,pay attention to the joint use of acupoint catgut embedding therapy and other traditional Chinese medicine methods,and pay attention to the research of different thread materials.
10.The effect of Xiaoaiping combined with camrelizumab on serum VEGF and tumor markers in advanced esophageal cancer

Objective To explore the effects of Xiaoaiping and camrelizumab on vascular endothelial growth factor(VEGF)and tumor markers in patients with advanced esophageal cancer.Methods A total of 43 patients with advanced esophageal cancer who were treated in the First Affiliated Hospital of Guangxi Medical University and Guangxi International Zhuang Medicine Hospital Affiliated to Guangxi University of Chinese Medicine from March 2020 to March 2022 were selected and divided into control group and combined treatment group according to the random number table method.Both groups were treated with camrelizumab immunotherapy,and the patients in combined treatment group were also treated with Xiaoaiping injection until disease progression.The levels of VEGF and tumor markers in two groups were detected after the first cycle,the third cycle,the sixth cycle,the ninth cycle and the seventeenth cycle of treatment.Results The levels of VEGF,carbohydrate antigen 125(CA125),carbohydrate antigen 199(CA199)and carcinoembryonic antigen(CEA)in the first cycle and the third cycle in two groups had no significant difference before and after treatment(P>0.05);There was no significant difference in CA125,CA199 and VEGF levels between two groups in the 6th cycle(P>0.05);There was a significant difference in CEA level between two groups(P<0.05);The levels of VEGF,CA125,CA199 and CEA in combined treatment group were significantly lower than those in control group at the 9th cycle and the 17th cycle(P<0.05).Conclusion Xiaoaiping combined with camrelizumab has a good clinical effect on patients with advanced esophageal cancer,which can reduce the levels of VEGF and tumor markers in patients.
