1.Research advances of CXCL12/CXCR4 in the rheumatoid arthritis pathogenesis

Rheumatoid arthritis(RA)is a chronic autoimmune disease of unknown etiology that can cause joint destruction and deformity.As a small molecule cytokine,the chemokine C-X-C motif chemokine ligand 12(CXCL12)regulates the pathogenesis of rheumatoid arthritis by binding to the specific receptor CXC chemokine receptor 4(CXCR4).Therefore,based on the bio-logical characteristics of CXCL12 and CXCR4,this paper intro-duces the pathogenesis of CXCL12/CXCR4 in RA and summari-zes the progress in RA-related research,with the aim of providing clinical value for understanding the pathogenesis of RA and de-veloping novel therapeutic targets.
2.Expression and diagnostic value of long noncoding RNA GUSBP3 and SAM4 in type 2 diabetes mellitus in Kazakh populations in the Xinjiang region of China

Objective To investigate the expression levels and diagnostic value of long noncoding RNA(LncRNA)GUSBP3 and LncRNA SAM4 in peripheral blood of Kazakh type 2 diabetes mellitus(T2DM)patients in Xinjiang.Methods From January to September 2019,Kazakh T2DM patients in The First Affiliated Hospital of Shihezi University were recruited through outpatient or inpatient department(T2DM group,n=30).And 30 healthy people were selected as normal control(NC)group during the same period.The general data,biochemical indicators and differentially expressed LncRNA levels were compared between the two groups.Spearman correlation analysis was used to analyze the correlation between the target genes and each index.Logistic regression was used to analyze the influencing factors for T2DM.Receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of target genes.Results The differentially expressed LncRNA were LncRNA GUSBP3 and LncRNA SAM4 in the two groups.DBP,FPG,HbA1c,fructosamine,FIns,islet β cell function index,serum creatinine(Scr),LncRNA GUSBP3 and LncRNA SAM4 were higher in the T2DM group than in the NC group(P<0.05).Spearman correlation analysis showed that the expression of LncRNA GUSBP3 was positively correlated with Scr and serum uric acid(SUA)(r=0.328,0.322,P<0.05),and negatively correlated with FIns(r=-0.295,P<0.05).The expression of LncRNA SAM4 was positively correlated with SUA(r=0.275,P<0.05)and negatively correlated with LDL-C(r=-0.212,P<0.05).Logistic regression analysis showed that DBP(OR 1.175,95%CI 1.011~1.367),FIns(OR 0.852,95%CI 0.739~0.982),LncRNA GUSBP3(OR 0.378,95%CI 0.149~0.958)was influencing factors for T2DM in Kazakh population.The area under the ROC curve of LncRNA GUSBP3 was 0.789 for T2DM diagnosis.Conclusions The expression levels of LncRNA GUSBP3 and LncRNA SAM4 were higher in Kazakh patients with T2DM than in healthy people,and their expression levels are related to some biochemical indicators.DBP,FIns and LncRNA GUSBP3 are the influencing factors for T2DM in Kazakh population.LncRNA GUSBP3 may be a biomarker for the diagnosis of T2DM in Kazakh in Xinjiang.
3.Impact of early invasive blood pressure monitoring on outcomes in out-of-hospital cardiac arrest patients undergoing extracorporeal cardiopulmonary resuscitation

Objective:To investigate the impact of early invasive arterial blood pressure (IBP) monitoring on survival and neurological outcomes in out-of-hospital cardiac arrest (OHCA) patients undergoing extracorporeal cardiopulmonary resuscitation (ECPR).Methods:This retrospective cohort study analyzed 44 OHCA patients receiving ECPR between January 2021 and January 2023. Patients were divided into: Early intervention group : IBP established within 3 min of ECMO initiation; Late intervention group : IBP established after ICU admission. Baseline characteristics, ECMO parameters, and clinical outcomes were compared. Multivariable logistic regression (adjusted for age, initial rhythm, etc.) and Spearman's correlation were used.Results:This study included a total of 44 patients treated with OHCA and ECPR, divided into an early intervention group of 23 cases and a late intervention group of 21 cases. The early intervention group showed significantly higher: Survival to discharge (43.5% vs. 9.5%, P<0.05), Good neurological recovery (CPC 1-2: 34.8% vs. 9.5%, P<0.05).Early intervention independently predicted survival (adjusted OR=18.84, 95% CI:1.97-179.98, P=0.01). Stratified analysis by pH (cutoff 7.0) demonstrated consistent benefits in both pH>7.0 ( aOR=0.392, 95% CI:0.106-0.678) and pH≤7.0 subgroups ( aOR=0.385, 95% CI: 0.075-0.695; interaction P=0.183). Early IBP positively correlated with CPC scores ( ρ=0.40, P=0.007). Conclusions:Early IBP monitoring significantly improves survival and neurological outcomes in OHCA-ECPR patients, supporting its integration into standardized protocols.
4.Exploration on the acceptability of routine biochemical test results for serum samples with varying degrees of chylous high triglyceride

Objective To explore the acceptability of routine biochemical test results for serum samples with varying degrees of chylous high triglyceride(TG).Methods Blood samples of 69 patients with different degrees of lipids were collected,including 33 patients with mild to moderate lipids(1.7 mmol/L≤TG<5.6 mmol/L)and 36 patients with severe lipids(TG≥5.6 mmol/L).Twenty-nine biochemical tests were detected before and after high-speed centrifugation.The result acceptability before high speed centrifugation of serum was compared with the results after high speed centrifugation as the gold standard[TG and total cholesterol(TC)before centrifugation].The acceptable criteria were subject to the following three conditions at the same time.Firstly,correlation coefficient(R2)was greater than or equal to 0.95.Secondly,the slope of linear re-gression equation was 1.00±0.05.Thirdly,for the same index,the number of samples whose result bias be-fore and after centrifugation was less than 1/2 total allowable error(TEa)in more than 90%of the total sam-ple numbers.Results Firstly,in the mild to moderate lipemia group,22 tests met the criteria,7 tests did not,including total protein(TP),albumin(ALB),TG,aspartate aminotransferase(AST),carbon dioxide(CO2),α-L-fucosidase(AFU),lactate dehydrogenase(LDH)(bias<10%),and the coincidence rate was 75.9%.In the severe lipemia group,12 tests met the criteria,17 tests did not,including pre-albumin(PA),AFU,γ-glu-tamyltransferase(γ-GT),LDH,AST,TC,direct bilirubin(DBIL),CO2,5'-nucleotidase(5'-NT),small and low-density lipoprotein cholesterol(sd-LDL-C),high density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),adenosine deaminase(ADA),cystatin C(CysC),glycosylated albumin(GA),total bilirubin(TBIL)(bias>10%),the coincidence rate was 41.4%,and there was a statistically sig-nificant difference in the coincidence rate between the two groups(P<0.05).Secondly,there was no statisti-cally significant difference in the acceptability of results between continuous monitoring method and endpoint method detection methods(P>0.05).Conclusion Most test results of direct determination with mild or moderate lipemia samples are acceptable,and the bias of unacceptable tests is small(<10%),so it is recom-mended to issue a test report without further sample treatment.However,due to the large number of unacceptable tests and larger bias(>10%),severe lipemia samples should be determined after high-speed centrifugation.

In recent years, with the advancement of nanotechnology, manganese-based nanomaterials have shown potential in various aspects of early breast cancer diagnosis, drug delivery, and tumor treatment. Compared to other nanomaterials, manganese-based nanomaterials exhibit excellent biocompatibility and have become a significant focus in the research of breast cancer diagnosis and treatment.

Rheumatoid arthritis(RA)is an autoimmune disease with the basic pathological manifestation of synovial inflammation.Symmetric poly-articular pain and swelling are the main symptoms in clinical practice,and even extra-articular manifestations and comorbidities such as interstitial fibrosis and coronary artery disease are triggered,which seriously affect the quality of life of patients.Traditional Chinese medicine(TCM)has achieved good clinical efficacy in the prevention and treatment of RA with the advantages of multi-pathway,multi-target,multi-component,and less toxic side effects,and plays an important role in the treatment of RA.Recently,many studies have demonstrated that Chinese medicine monomers and Chinese herbal compound can control inflammation,reduce angiogenesis,induce apoptosis of synovial fibroblasts,and inhibit their proliferation,invasion and migration by regulating the PI3K/Akt signaling pathway,so as to play a key role in the treatment of RA.For this reason,the article summarizes current knowledge regarding the PI3K/Akt signaling pathway and its role in RA,as well as summarizes the current research progress of TCM in the treatment of RA by regulating the PI3K/Akt signaling pathway.The aim of this review is to provide theoretical bases for the prevention and treatment of RA and the development of new drugs.
9.Effects of tele-rehabilitation program on ataxic dysarthria following stroke

Objective:To investigate the effect of tele-rehabilitation program on ataxic dysarthria following stroke.Methods:A total of 101 patients with stroke and dysarthria who received treatment at The First People's Hospital of Huzhou between January 2021 and October 2023 were included in this study. They were divided into an observation group ( n = 51) and a control group ( n = 50) using the random number table method. Patients in the control group received conventional family rehabilitation guidance, while those in the observation group participated in a tele-rehabilitation program. All patients were treated for 12 weeks. Before and 12 weeks after the intervention, the Frenchay Dysarthria Assessment (FDA) and speech clarity tests were used to assess speech function. Changes in FDA scores and speech clarity scores post-intervention were compared with pre-intervention scores. The observation group was further divided into mild, moderate, and severe groups based on FDA scores, and the differences in effect of tele-rehabilitation program on ataxic dysarthria among the three groups after the intervention were compared. Results:At 12 weeks after intervention, both the observation and control groups showed improvements in FDA scores and speech clarity [observation group: (43.88 ± 16.13) points, (68.65 ± 16.99)%; control group: (49.68 ± 12.09) points, (62.36 ± 14.20)%] compared with their baseline measurements [observation group: (55.12 ± 12.63) points, (57.96 ± 13.48)%; control group: (54.16 ± 10.69) points, (57.84 ± 12.78)%] ( t = 12.23, -14.01, 12.42, -13.86, all P < 0.001). Moreover, the observation group showed significant improvements compared with the control group ( t = 2.04, -2.01, both P < 0.05). At 12 weeks after intervention, both the mild and moderate groups showed improvements in FDA scores and speech clarity score [mild group: (31.41 ± 3.08) points, (82.94 ± 6.97)%, moderate group: (40.08 ± 4.67) points, (70.92 ± 6.26)%] compared with their baseline measurements [mild group: (42.24 ± 6.78) points, (70.53 ± 7.98)%, moderate group: (56.17 ± 4.65) points, (57.58 ± 6.54)%, t = 10.06, -20.25, 45.21, -21.34, all P < 0.001]. There were no significant improvements in the severe group for FDA scores and speech clarity score [(74.20 ± 4.29) points, (38.90 ± 5.80)%] compared with their baseline measurements [(74.50 ± 4.01) points, (37.50 ± 4.74)%, t = 1.96, -1.44, both P > 0.05]. Conclusion:The tele-rehabilitation program can improve post-stroke ataxic dysarthria, particularly showing significant effectiveness for patients with mild to moderate ataxic dysarthria.
10.Genetic variations in four geographical isolates of Gohieria fusca based on cytochrome b and internal transcribed spacer genes

Objective To investigate the genetic diversity and genetic differentiation of different geographical isolates of Gohieria fusca.. Methods G. fusca isolates were sampled from Wuhu (WH), Bengbu (BB) and Bozhou cities (BZ) of Anhui Province and Jiaxing City of Zhejiang Province (JX). Mitochondrial cytochrome b (Cytb) and ribosomal internal transcribed spacer (ITS) genes were amplified in WH, BB, BZ and JX isolates of G. fusca using PCR assay. The gene sequences were edited and aligned using the software Chromas 2 and DNASTAR 1.00, and the haplotype, haplotype diversity (Hd) and nucleotide polymorphism (Pi) of each isolate were calculated using the software DnaSP 5.10.00. The genetic differentiation among isolates (Fst) and gene flow value (Nm) were estimated using the software MEGA 10.2, and a phylogenetic tree was built. Tests of neutrality and analysis of molecular variance (AMOVA) were performed using the software Arlequin 3.1 and a haplotype network was built based on the Median-Joining network using the software Network 10.2. Results PCR assay showed that the sizes of the Cytb and ITS genes were 372 bp and 1 301 to 1 320 bp, respectively. All four isolates of G. fusca presented high genetic diversity based on mitochondrial Cytb and ITS genes (Hd = 0.804, Pi = 0.006 91). AMOVA showed genetic differentiation among geographical isolates of G. fusca (Fst = 0.202 40, P < 0.05), and the genetic variation was mainly caused by intra-population variations (79.76%). Gene flow analysis showed a high level of gene flow among G. fusca isolates (Nm > 1). Tests of neutrality based on Cytb gene measured a Tajima’s D value of −1.796 31 (P < 0.05) and a Fu’s FS value of −3.293 98 (P < 0.05) in WH isolate of G. fusca, indicating population expansion in WH isolate of G. fusca. Haplotype network analysis and phylogenetic analysis revealed no remarkable geographical distribution pattern among different geographical isolates of G. fusca. All four isolates of G. fusca presented high genetic diversity (Hd = 0.985, Pi = 0.011 97). AMOVA showed moderate level of genetic differentiation between four isolates (Fst = 0.104 62, P < 0.05). The tests of neutrality based on ITS genes measured a Tajima’s D value of −6.088 20 and a Fu’s FS value of −1.935 99 (both P > 0.05) in the whole isolate of G. fusca, indicating no obviously population expansion. Conclusions The four geographical isolates of G. fusca have high genetic diversity and remarkable genetic differentiation. Since a high level of gene flow is detected among different geographical isolates of G. fusca, no obvious geographical distribution pattern of G. fusca is found.
