Objective:
To explore the differences and consistency among three objective refraction methods, namely, retinoscopy after cycloplegia, the Spot binocular vision screener, and automated computer refractometry in preschool children, so as to provide evidence for optimizing refractive assessment strategies in this population.
Methods:
A total of 116 preschool children aged 2-6 years who were referred to the Ophthalmology Department of Anhui Women and Children s Medical Center because of abnormal vision screening results between August 2025 and March 2026 were enrolled by using purposive sampling. After adequate cycloplegia was achieved with 1% atropine ophthalmic gel, objective refraction was performed by using cycloplegic retinoscopy, the Spot Vision Screener, and an autorefractor. Differences in spherical equivalent (SE) among the three methods were compared using the Friedman test, while differences in myopia detection rates were assessed using Cochran s Q test. Cycloplegic retinoscopy was used as the reference standard to evaluate the diagnostic accuracy and reliability of the Spot Vision Screener and the autorefractor for myopia detection.
Results:
The median (interquartile range) spherical equivalent (SE) values for the right eye were identical across the three methods:1.50 (0.75, 2.50) D for cycloplegic retinoscopy, the Spot Vision Screener, and the autorefractor. For the left eye, the corresponding values were 1.50 (0.50, 2.50), 1.50 (0.50, 2.50), and 1.63 (0.75, 2.50) D, respectively. There were no significant differences in SE measurements among the three methods for either eye ( χ 2=3.77, 3.28, both P >0.05). The myopia detection rates of cycloplegic retinoscopy, the Spot Vision Screener, and the autorefractor were 13.79%, 12.07%, and 13.79%, respectively, for the right eye, and 13.79%, 14.66%, and 12.93%, respectively, for the left eye, with no significant differences among the three methods (Cochran s Q =1.60, 1.14, both P >0.05). Using cycloplegic retinoscopy as the reference standard, both the Spot Vision Screener and the autorefractor demonstrated high diagnostic accuracy and reliability for detecting myopia. Compared with the Spot Vision Screener, the autorefractor showed higher sensitivity (right eye:93.75%, 81.25%; left eye:88.24%, 76.47% ), Youden index (right eye:0.93, 0.80; left eye:0.86, 0.74), overall agreement (right eye:98.28%, 96.55%; left eye:96.55%, 94.83%), and Cohen s Kappa coefficient (right eye:0.93, 0.85; left eye:0.86, 0.78).
Conclusion
Under standardized cycloplegic conditions, the Spot Vision Screener and the autorefractor can serve as alternatives to retinoscopy for assessing refractive status, particularly in large scale screening and in children with poor cooperation.

Background:
and Purpose Symptomatic intracranial hemorrhage (sICH) following endovascular thrombectomy (EVT) is a severe complication associated with adverse functional outcomes and increased mortality rates. Currently, a reliable predictive model for sICH risk after EVT is lacking.
Methods:
This study used data from patients aged ≥20 years who underwent EVT for anterior circulation stroke from the nationwide Taiwan Registry of Endovascular Thrombectomy for Acute Ischemic Stroke (TREAT-AIS). A predictive model including factors associated with an increased risk of sICH after EVT was developed to differentiate between patients with and without sICH. This model was compared existing predictive models using nationwide registry data to evaluate its relative performance.
Results:
Of the 2,507 identified patients, 158 developed sICH after EVT. Factors such as diastolic blood pressure, Alberta Stroke Program Early CT Score, platelet count, glucose level, collateral score, and successful reperfusion were associated with the risk of sICH after EVT. The TREAT-AIS score demonstrated acceptable predictive accuracy (area under the curve [AUC]=0.694), with higher scores being associated with an increased risk of sICH (odds ratio=2.01 per score increase, 95% confidence interval=1.64–2.45, P<0.001). The discriminatory capacity of the score was similar in patients with symptom onset beyond 6 hours (AUC=0.705). Compared to existing models, the TREAT-AIS score consistently exhibited superior predictive accuracy, although this difference was marginal.
Conclusions
The TREAT-AIS score outperformed existing models, and demonstrated an acceptable discriminatory capacity for distinguishing patients according to sICH risk levels. However, the differences between models were only marginal. Further research incorporating periprocedural and postprocedural factors is required to improve the predictive accuracy.

Objective:To analyze the whole genome sequence and key site mutations of hepatitis B virus (HBV) in patients with different stages of disease progression, and to understand the relationship between HBV genetic characteristics and disease progression. Methods:Serum samples and basic information of hepatitis B patients with asymptomatic HBV carrier, chronic hepatitis B patients, cirrhosis patients and primary hepatocellular carcinoma patients were collected. Nested PCR was used to amplify the samples to obtain HBV whole gene sequences. Phylogenetic trees were constructed to determine the genotype of the samples, and gene mutations of the samples were analyzed combined with reference sequences of each type. Results:A total of 256 samples were successfully amplified, including 68 asymptomatic HBV carrier patients, 118 CHB patients, 15 LC patients and 55 HCC patients, and five genotypes (B, C, D, I and C/D) were detected. The result of comparative analysis showed that the mutation rate of 56 nucleotide sites was significantly different among the four groups ( P<0.05). In addition to the discovery of C105T, A1762T/G1764A and G1899A and other previously reported key site mutations, the mutation rates of T53A, C1485T and C1628T in newly diagnosed HCC group were significantly higher than those in other groups, and the mutation rates of T2150G and T2151C in asymptomatic HBV infection group were significantly higher than those in other groups. A total of 26 sequences were deleted, mainly distributed in the pre-C and pre-S regions. The deletion mutation rate in the HCC group was significantly higher than that in the other groups. Conclusions:The data of this study indicate that some nucleotide substitution mutations and deletion mutations may be closely related to the occurrence and development of HBV-related diseases, and HCC patients are more likely to have gene mutations than non-HCC patients. These result provide a reference for understanding the relationship between viral mutation and the progression of HBV infection-related diseases.

OBJECTIVE:
To study the impacts of varicocele (VC) and varicocelectomy (VCT) on the proteomics of rat testis tissue, and to analyze the differential proteins and signaling pathways, and observe the microstructural changes of the testis tissue.
METHODS:
We selected 60 male SD rats and divided them into a sham operation (SO), a VC model control, and a VCT group. We harvested the testis tissues from the rats at 4 weeks after modeling for determination of the differential protein expressions by mass spectrometry, analysis of the changes in the protein signaling pathways by KEGG pathway repolarization, and observation of the microstructural changes in the spermatogenic cells under the transmission electron microscope (TEM).
RESULTS:
A total of 15 clinically significant proteins were effectively identified, among which RPS24, KIFAP3, HPX, RPL38, TOP2A, PRPF19, TRPM3, RPL32, CNBP and AHSG were upregulated, while RPS9, TKFC, SH3BGRL3, ACAA2 and FABP3 downregulated. The differential pathways found included the Type-I 4-aminobutyrate degradation pathway, eIF2 signaling pathway, and Type-III glutamate degradation pathway, which were all related to the pathogenesis of testicular growth arrest. Compared with the rats in the VCT group, those of the VC group rats showed ultrastructural changes in the testis tissue under the TEM, such as mitochondrial vacuolar degeneration, dense nucleoli, invagination of cell nuclear membranes, and irregularity, which were detrimental to the survival of testicular cells.
CONCLUSION
VCT affects the development and growth of the testis by altering the expressions of relevant proteins and influencing the changes of the gene pathways in the testicular cells, which may be one of the causes of VC inducing testis injury and testicular spermatogenic dysfunction. The changes in these molecular pathways can provide some theoretical evidence for an insight VC as well as potential therapeutic targets for its treatment.
