1.Perioperative immune dynamics and clinical outcomes in patients undergoing on-pump cardiac surgery

Objective: To characterize perioperative dynamic changes in immune-cell phenotypes and inflammatory cytokines in patients undergoing CPB (cardiopulmonary bypass) cardiac surgery, and to explore their associations with postoperative outcomes. Methods: In this prospective cohort study, 120 adult patients who underwent elective cardiac surgery under CPB at West China Hospital from May 2022 to March 2023 were enrolled. Perioperative immune-cell phenotypes and concentrations of 40 inflammation-related cytokines were measured. The primary outcomes were the sequential organ failure assessment (SOFA) score at 24 h after surgery and ΔSOFA (the peak SOFA score within 48 h after surgery minus the preoperative SOFA score). Secondary outcomes included major adverse cardiovascular events (MACE), acute kidney injury (AKI), respiratory failure, severe liver injury, and infection. Results: The mean age of enrolled patients was 57±10 years. Of these, 52% (62/120) were male and 90% (108/120) underwent valve surgery. Plasma analysis showed that multiple pro-inflammatory cytokines increased during CPB and remained elevated up to 24 h after surgery; five chemokines and the anti-inflammatory cytokine IL-10 peaked at the end of CPB. The SOFA score increased from 1 (1, 2) preoperatively to 7 (5, 10) at 24 h after surgery, with a ΔSOFA of 6 (4, 8). Within 30 days after surgery, 48 patients (40.0%) developed AKI, 17 (14.2%) developed infection, 4 (3.3%) developed severe liver injury, 3 (2.5%) developed respiratory failure, and 3 (2.5%) experienced MACE. During the 2-year follow-up, 8 patients (6.7%) experienced MACE and 5 (4.2%) died. Conclusion: Multi-organ dysfunction is common after cardiac surgery under CPB (median ΔSOFA, 6), accompanied by perioperative activation of multiple immune-cell subsets and upregulation of pro-inflammatory, anti-inflammatory, and chemotactic mediators. This study provides data-driven evidence and research clues for further investigation of the associations between CPB-related immune perturbations and postoperative organ dysfunction and clinical outcomes.
2.Application of 3D technology-assisted teaching based on the four-in-one flipped classroom in clinical teaching of neurosurgery

Objective:To explore the application effect of 3D technology-assisted teaching based on "four-in-one" flipped classroom in clinical teaching of neurosurgery, and provide a basis for optimizing the medical education mode.Methods:A total of 50 students from the Second Clinical College were selected between June 2020 and June 2024, including eight-year program medical students and postgraduate neurosurgery students. The control group ( n=25) received the conventional teaching mode. The experimental group ( n=25) was taught using the "four-in-one" flipped classroom combined with 3D technology, including characteristic textbooks with 3D model drawings, Internet platforms (video libraries and virtual simulation modules), mobile interactive terminals (real-time question and answer), and virtual simulation technology. Effectiveness was evaluated through theoretical assessment (e.g., neuroanatomy and clinical application ability, with a total score of 100 points), operational assessment (e.g., surgical design and aseptic concept, with a total score of 100 points), and a teaching quality questionnaire. Independent samples t-test was conducted using SPSS 22.0. Results:The total score of theoretical assessment was higher in the experimental group than that in the control group [(86.52±5.21) vs. (73.56±6.32), P<0.001], with the largest difference observed in case analysis questions [(26.03±3.65) vs. (22.22±3.50), P=0.001]. In the operational assessment, the experimental group performed better in "surgical process design" [(26.30±4.14) vs. (21.44±3.45), P<0.001] and "aseptic concept" [(8.18±0.98) vs. (6.64±0.79), P<0.001]. The teaching quality questionnaire showed that the experimental group scored higher in "clinical practice skill enhancement" [(23.13±1.39) vs. (21.45±1.86), P=0.001] and "self-directed learning motivation" [(21.84±1.60) vs. (19.75±1.45), P<0.001]. Conclusions:The combination of "four-in-one" flipped classroom and 3D technology can significantly improve the teaching effectiveness of neurosurgery, especially in the cultivation of clinical thinking and practical abilities. This approach is worth promoting.
3.Effects of"Gubenpeiyuan,Jianpi hewei"Acupuncture on JAK2/STAT5α/PGE2 Signaling Pathway Mediated by Leptin in Liver Tissue of Obese Rats

To explore the molecular mechanisms underlying acupuncture-induced weight loss,the effects of acupuncture based on the principles of consolidating the foundation and strengthening the spleen and stomach on the Leptin-mediated JAK2/STAT5α/PGE2 signaling pathway in the liver tissue of obese rats were investigated.Methods Obese rat models were established and divided into six groups:Normal control group,normal control+AG490 group,obese model group,obese model+AG490 group,obese model+acupuncture group,and obese model+acupuncture+AG490 group,with 10 rats in each group.Serum Leptin and PGE2 levels were detected using ELISA,the expression of related proteins in liver tissue was assessed by Western blot,and changes in adipocyte morphology were observed through HE staining.Results In the obese model group,serum Leptin and PGE2 levels were significantly elevated,adipocyte area was enlarged,and the expression of JAK1,JAK2,STAT5α,COX1,and COX2 proteins in liver tissue significantly increased.Acupuncture treatment significantly reduced serum Leptin and PGE2 levels,decreased adipocyte area,and inhibited the expression of related proteins in liver tissue.Conclusion Acupuncture achieves weight loss by inhibiting the JAK2/STAT5α/PGE2 signaling pathway,improving Leptin resistance,reducing inflammatory responses,and decreasing fat accumulation.
4.Visualization analysis of knowledge map and trends in glymphatic system research

BACKGROUND:The clearance pathway of metabolic waste in the brain is crucial for maintaining neural homeostasis.The accumulation of metabolic waste disrupts this equilibrium,representing a common pathological feature of many central nervous system diseases.In recent years,research focusing on the glymphatic system has emerged as a hotspot in the nervous system field.OBJECTIVE:To construct a knowledge map of glymphatic system research,visually analyze the current state of research,main hotspots,and future development trends within this area.METHODS:Utilizing CiteSpace,VOSviewer,and the Bibliometrix package in the R language environment,this study conducted an in-depth visual analysis of glymphatic system-related literature from the Web of Science Core Collection database,spanning from January 2012 to March 2024.This analysis included authors,institutions,countries,journals,keywords,and co-citation frequencies.RESULTS AND CONCLUSION:A total of 687 related articles were included in this study.The number of publications in this field increased year by year,showing an explosive growth trend in the past three years.The countries,institutions,and authors with the largest number of publications in this research field were the United States,University of Rochester,and Professor Maiken Nedergaard.The journal with the highest number of publications was JOURNAL OF CEREBRAL BLOOD FLOW AND METABOLISM.The high-frequent and high-central keywords mainly focused on the mechanism such as cerebrospinal fluid hydrodynamics,neurological diseases such as Alzheimer's disease,and imaging techniques such as diffusion tensor imaging.The literature with the highest co-citation frequency was a classic review of the glymphatic system.The above results show that the research of glymphatic system is an emerging and active field,which has attracted wide attention at home and abroad and gradually expand from theoretical research to clinical practice.
5.Update on thyroid tumors with DICER1 mutation

With the further comprehensive understanding of pathologists in the morphological characteristics of DICER1-related thyroid tumors,and the widespread application of multi-gene detection technology,the detection rate of these tumors is gradually increasing.DICER1 mutations can be detected in benign,low-risk,and malignant thyroid tumors.It is particularly noteworthy that tumors with germline mutations and in pediatric patients often exhibit more ag-gressive and prone to recurrent.Therefore,it is important to identify and diagnose these lesions.This article summarize the advancements in DICER1 mutation-related thyroid tumors through systematic interpretation.
6.Oxymatrine,a novel TLR2 agonist,promotes megakaryopoiesis and thrombopoiesis through the STING/NF-κB pathway

Radiation-induced thrombocytopenia(RIT)faces a perplexing challenge in the clinical treatment of cancer patients,and current therapeutic approaches are inadequate in the clinical settings.In this research,oxy-matrine,a new molecule capable of healing RIT was screened out,and the underlying regulatory mecha-nism associated with magakaryocyte(MK)differentiation and thrombopoiesis was demonstrated.The capacity of oxymatrine to induce MK differentiation was verified in K-562 and Meg-01 cells in vitro.The ability to induce thrombopoiesis was subsequently demonstrated in Tg(cd41:enhanced green fluorescent protein(eGFP))zebrafish and RIT model mice.In addition,we carried out network pharmacological pre-diction,drug affinity responsive target stability assay(DARTS)and cellular thermal shift assay(CETSA)analyses to explore the potential targets of oxymatrine.Moreover,the pathway underlying the effects of oxymatrine was determined by Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analyses,Western blot(WB),and immunofluorescence.Oxymatrine markedly promoted MK differentiation and maturation in vitro.Moreover,oxymatrine induced thrombopoiesis in Tg(cd41:eGFP)zebrafish and accelerated thrombopoiesis and platelet function recovery in RIT model mice.Mechanistically,oxymatrine directly binds to toll-like receptor 2(TLR2)and further regulates the downstream pathway stimulator of interferon genes(STING)/nuclear factor-kappaB(NF-κB),which can be blocked by C29 and C-176,which are specific inhibitors of TLR2 and STING,respectively.Taken together,we demonstrated that oxymatrine,a novel TLR2 agonist,plays a critical role in accelerating MK differentiation and thrombopoiesis via the STING/NF-κB axis,suggesting that oxymatrine is a promising candidate for RIT therapy.

The rising incidence of overweight and obesity among children and adolescents has become a significant public health issue both domestically and globally.In South China,the rates of overweight and obesity among children and adolescents are relatively low compared with the national average,thanks to the unique regional climate,culture,and dietary habits,which are conducive to the prevention and control of childhood and adolescent overweight and obesity.This consensus,based on practical experiences in weight management for overweight and obese children and adolescents in South China,in combination with evidence-based medical research,systematically establishes a standardized diagnosis and treatment framework for childhood and adolescent overweight and obesity.It covers core aspects such as diagnosis and assessment,lifestyle interventions,behavioral interventions,pharmacological treatment and traditional Chinese medical therapies.This consensus incorporates regional characteristics into the management framework for childhood and adolescent overweight and obesity,aiming to provide clinicians with scientifically sound and practical guidance,and to offer a regional model for curbing the growing trend of childhood obesity.
8.The expression of the threonine and tyrosine kinase gene in eyelid basal cell carcinoma and its impact on the biological behavior of malignant tumor cells

Objective To investigate the relationship between the threonine and tyrosine kinase(TTK)gene and eyelid basal cell carcinoma(BCC).Methods Bioinformatics methods were used to screen the core gene(namely,TTK)associated with the occurrence and development of BCC from the Gene Expression Omnibus(GEO)database.Surgically removed eyelid BCC tissue specimens(BCC cells were divided into BBC Grade Ⅰ,Ⅱ and Ⅲ groups by tumor grade)and be-nign tumor tissue specimens(Control group)were collected from Ziyang Central Hospital for subsequent experiments.Cel-lular immunofluorescence assay(CIA)was used to detect the expression of the TTK gene in benign and malignant eyelid tumor cells.After knocking down TTK in BCC cells through transfection with lentiviruses(the cells transfected with LV-TTK-shRNA were taken as the TTK-shRNA group,and those transfected with LV-BBC-shRNA were taken as the BBC nega-tive control group),CIA was used to detect the expression of key proteins Bcl-2 and Bax in the apoptotic signaling pathway of each group of cells.Results The bioinformatics analysis showed that the TTK gene was the core gene associated with the occurrence and development of eyelid BCC.CIA detection results revealed that the fluorescence signal intensities in the tumor cytoplasm of Control,BCC Grade Ⅰ,Ⅱ,and Ⅲ groups were 1.03±0.07,1.28±0.11,1.58±0.13 and 1.92±0.17,respectively.The fluorescence signal intensity gradually increased,and the difference in fluorescence signal intensity among the four groups was statistically significant(all P<0.05).Compared with that in the Control group(1.02±0.05),the cell fluorescence intensity was increased in the BCC negative control group(1.74±0.12)and decreased in the TTK-shRNA group(1.31±0.09)(P<0.05).The difference in cell fluorescence intensity was significant among the Control,BCC nega-tive control and TTK-shRNA groups(all P<0.05).The fluorescence intensity of the anti-apoptotic protein Bcl-2 was 1.04±0.12 in the Control group,2.12±0.23 in the BCC negative control group,and 1.43±0.15 in the TTK-shRNA group.The fluorescence intensity of the pro-apoptotic protein Bax was 1.02±0.08 in the Control group,0.64±0.11 in the BCC negative control group,and 1.47±0.16 in the TTK-shRNA group.After TTK knockdown,the expression level of BcL-2 in BCC cells decreased,and that of Bax increased.The fluorescence intensities of BcL-2 and Bax were significantly different among the Control,BCC negative control and TTK-shRNA groups(all P<0.05).Conclusion The TTK gene plays a role in the regulation of eyelid BCC cell proliferation,and this effect is closely related to the PI3K-AKT-Bcl-2/Bax signaling path-way.
9.Clinical value of transcranial color-coded duplex sonography in elderly patients with acute ischemic stroke after endovascular therapy

Objective To investigate the intracranial hemodynamic parameters with transcranial color-coded duplex sonography(TCCS)in elderly patients with acute ischemic stroke(AIS)after endovascular therapy,and analyze their association with functional outcomes.Methods A total of 360 elderly AIS patients undergoing endovascular therapy in our hospital from March 1,2020 to January 31,2024 were prospectively included,and according to the score of modified Rankin scale(mRS)at 90 d,they were divided into a poor function group(mRS score 3-6,145 cases)and good function group(mRS score≤2,215 cases).TCCS was conducted on all the patients in 12 h after endovascular therapy to evaluate intracranial blood flow status.Peak systolic velocity(PSV),end-diastolic velocity(EDV),mean blood flow velocity(MFV),pulsatility index(PI),MFV ratio and PSV ratio,adjusted PSV ratio and adjusted MFV ratio were recorded in all pa-tients.Receiver operator characteristic(ROC)curve analysis was employed to analyze the value of TCCS parameters in predicting poor functional outcomes,and the AUC values and cut-off values were also analyzed.Results The PSV ratio,MFV ratio,adjusted PSV ratio and adjusted MFV ra-tio were significantly higher in the poor function group than the good function group(P<0.01).The predictive performance of PSV ratio and MFV ratio were not good enough for poor functional outcome in the AIS patients(AUC<0.750),while the adjusted PSV ratio and adjusted MFV ratio showed better predictive performance(P<0.01).Multivariate logistic regression analysis showed that NIHSS score at admission ≥14.00,ASPECT score at admission≥8.00,adjusted PSV ratio>1.39 and adjusted MFV ratio>1.40 were independent risk factors for poor functional outcomes in AIS patients(P<0.01).ROC curve analysis indicated that combined NIHSS score at admission+ASPECT score at admission showed poor value in predicting poor functional outcome in the AIS(AUC=0.780,95%CI:0.734-0.822),the predictive performance of combination of above two scores+adjusted PSV ratio+adjusted MFV ratio was significantly improved(AUC=0.976,95%CI:0.955-0.989),with higher sensitivity and specificity(Z=8.261,P<0.01).Conclusion Early TCCS detection in elderly AIS patients after endovascular treatment is an effective method for identifying poor functional outcomes at 90 d,with the advantages of non invasiveness and easy op-eration.And,better predictive performance is obtained when adjusted PSV ratio and adjusted MFV ratio are combined with conventional predictors.
10.Remimazolam general anesthesia on the core body temperature of patients undergoing gynecological laparoscopic surgery and the effects of thermoregulatory vasoconstriction

Objective To investigate the effect of remimazolam on core body temperature(CBT)and thermoregulatory vasoconstriction in patients undergoing gynecological laparoscopic surgery.Methods 90 gynaecology patients with ovarian or cervical cancer from Jan 2024 to Jun 2024 were randomly divided into experimental group(group R)and control group(group C).45 patients in each group were induced and maintained with remimazolam or propofol.After induction of anesthesia,the CBT and mean skin temperature(MST)were continuously monitored using a nasopharyngeal temperature probe and a skin temperature probe respectively.Record CBT and MST of two groups of patients before surgery(T0),30 min(T1),60 min(T2),90 min(T3),120 min(T4),150 min(T5),180 min(T6)after induction of anesthesia and at extubation(T7),as well as forearm-fingertip temperature gradient(TFOR-FIN).Record vasoconstriction threshold and time to onset of vasoconstriction by TFOR-TIN.Record the changes in mean arterial pressure(MAP),heart rate(HR),and cardiac index(CI)at each time point(T0,T1,T2,T3,T4,T5,T6 and T7 time point);Record the incidence of hypothermia,hypotension and bradycardia and the use of vasoactive drugs.Results Compared with T0 time point,the CBT and TFOR-TIN at T1 to T7 time point decreased significantly,but the MST increased(P<0.05);In group C,the CBT at T1 to T5 decreased significantly than in group R(P<0.05);Compared with group C,MST in group R increased significantly at time T1 to T7(P<0.05).The vasoconstriction threshold in group R was significantly higher than that in group C,and the time to reach the vasoconstriction threshold was significantly less than that in group C(P<0.05).Compared with T0 time point,MAP and CI at T1,T2,T3 time point decreased significantly in two groups,MAP and CI at T1,T2,T3 time point in group R were higher than those in group C(P<0.05);Compared with group R,the incidence of perioperative hypothermia(PH)and hypotension and the utilization rate of ephedirine in group C were increased(P<0.05).Conclusion Remimazolam and propofol for gynecological laparoscopic general anesthesia can cause intraoperative temperature drop in patients,compared with propofol,remimazolam general anesthesia has less influence on CBT,more stable hemodynamics,less influence on thermoregulatory vasoconstriction,less degree of diastolic blood vessels,has better body temperature protection.
