2.Recruitment of Young Cardiovascular Surgeons by the Cardiovascular Surgery Summer School

The Cardiovascular Surgery Summer School is a seminar for fourth- to sixth-year medical students, and first- to third-year residents to convey the attractiveness of cardiovascular surgery and recruit them, and is held for two days every August. The seminar was held at one location (Haneda Training Centre) with approximately 100 participants and 50 faculties this and last year. The author was in charge of planning and organising the events. The programme content and results of the participant questionnaires for both years are analysed and a programme for better recruitment of young cardiovascular surgeons is reported with some considerations.
3.The Present and Future of Hands-On Training in Cardiovascular Surgery: Innovations and Challenges

This column is a roundtable discussion featuring three U40 members who have been extensively involved in the organization and planning of hands-on training, along with Mr. Park, the CEO of EBM Corporation. The four participants discuss hands-on training in the field of cardiovascular surgery. Drawing on their experiences with programs such as the Cardiovascular Surgery Summer School, Basic Lecture Course (BLC), Online BLC, Advanced Lecture Course (ALC), and the coronary anastomosis competition “Challengers' Live Demonstrations,” they explore the essence of hands-on training, current challenges, and future prospects in this field.
4.Domestic Fellowship Training for Young Cardiovascular Surgeons in Japan: Current Trends and Observations

This article provides an overview of the current situation regarding domestic fellowships for young cardiovascular surgeons in Japan, based on the author's personal experience and the results of a nationwide survey. The author undertook a one-year domestic fellowship beginning in April 2023 at the Department of Cardiovascular Surgery, Fukuoka Children's Hospital. In addition to gaining extensive experience in pediatric cardiac surgery, the author had opportunities to engage in clinical research and present at academic conferences. Conducting research alongside hands-on clinical training offered valuable insights and deepened the author's understanding of various cardiac conditions. At the same time, practical considerations such as relocating with family and establishing childcare support systems required careful planning. Furthermore, a survey targeting U40 committee members was conducted to investigate the motivations, outcomes, and challenges associated with domestic fellowships. The results revealed that many young surgeons viewed domestic fellowships positively, citing benefits such as diverse clinical experience, acquisition of new knowledge, and the development of professional networks. However, several barriers were also identified, including limited access to information, personnel constraints due to institutional staffing systems, and disruptions to personal and family life. The findings suggest that enhanced cross-institutional support and improved information sharing are essential to facilitate the development of young surgical talent in Japan.
5.Current Status of Surgical Training by Young Cardiovascular Surgeons and Their Intention for Off the Job Training System

Surgical proficiency significantly impacts cardiovascular surgery outcomes. Regular training for surgical procedures is essential. Since 2018, cardiovascular surgery's specialty system has mandated Off the Job Training (OffJT), defined as non-clinical work training that fulfills certification criteria. A minimum of 30h of OffJT is required for specialty acquisition. Presently, a review is being conducted to examine disparities in OffJT credit acquisition across various facilities and the lack of a standardized OffJT curriculum. This report details a survey of the current OffJT landscape, focusing primarily on executives under 40 years old. We employed a questionnaire-based approach to explore the types of training being conducted and the innovative strategies being implemented, along with an analysis of the findings.
6.Successful Surgical Treatment for Pharyngeal Perforation in a Patient with Aortic Valve Infectious Endocarditis

Pharyngeal perforation is a rare but crucial complication of transesophageal echocardiography during cardiac surgery. We herein report the case of a 72-year-old man with infective endocarditis in the aortic valve, who had a poor performance status due to congestive heart failure and brain infarction. The echo probe of the transesophageal echocardiography was detected in the anterior mediastinum after median sternotomy. Pharyngeal repair after aortic valve replacement with bioprosthetic valve, following omental wrapping was performed simultaneously. The operative course was relatively good, and the patient moved to the general ward 32 days after the surgery.
