1.Fibroblast growth factor 21 attenuates oxidative stress injury in retinal pigment epithelial cells under high glucose via FGFR1/PI3K/Akt signal pathway

AIM:To investigate the effect of fibroblast growth factor 21(FGF21)on high glucose-induced oxidative stress in retinal pigment epithelial(RPE)cells and to clarify the underlying molecular mechanisms.METHODS:Single-cell sequencing data from the GEO database were analyzed to determine the expression profile of the FGF21 receptor FGFR1 in RPE cells. Human ARPE-19 cells were cultured and randomly assigned to control, high glucose(30 mmol/L), and high glucose+FGF21 analog treatment groups, with additional siFGFR1 and PI3K inhibitor groups. Cell viability in different treatment groups was assessed using CCK-8 assay, intracellular reactive oxygen species(ROS)levels were quantified using DCFH-DA fluorescent probing combined with immunofluorescence staining and flow cytometry. Transcriptome sequencing was performed on cells from the high glucose group and high glucose+FGF21 group to analyze the enrichment level of the PI3K/Akt signaling pathway. Western blotting was performed to detect phosphorylation levels of PI3K/Akt pathway components.RESULTS:Single-cell sequencing revealed specific expression of FGFR1 in RPE cells of retinal tissues from diabetic model mice. Under In vitro experiments, high glucose(30 mmol/L)exposure reduced ARPE-19 cell viability by 49.7% and increased ROS levels by approximately 2-fold. Whereas treatment with the FGF21 analog(60 ng/mL)restored cell viability and attenuated high glucose-induced ROS accumulation. Mechanistic studies demonstrated that FGFR1 knockdown inhibited the antioxidative stress of FGF21. Further validation of the molecular mechanism revealed that high glucose significantly suppressed the PI3K/Akt pathway activation(the levels of p-Akt and p-PI3K were decreased by 33.9% and 36.6%, respectively), while FGF21 effectively reversed this inhibitory effect and restored the expression of p-Akt and p-PI3K. Treatment with the PI3K inhibitor LY294002 inhibited the cytoprotective effect of FGF21 and significantly increased the ROS-positive cells, these findings confirm that PI3K/Akt signaling is indispensable downstream mechanism for FGF21 to exert its effects.CONCLUSION:FGF21 alleviates high glucose-induced oxidative stress and cellular injury in RPE cells by activating the PI3K/Akt signaling pathway through its receptor FGFR1.

As the core vehicle for preserving and transmitting traditional Chinese medicine(TCM) academic thought and clinical experience, the establishment of a robust quality evaluation system for TCM clinical case reports is a crucial component in the current standardization and modernization of TCM. Based on the practical experience of constructing the China Clinical Cases Library of Traditional Chinese Medicine by the China Association of Chinese Medicine, this study conducted a comprehensive analysis of critical challenges, including insufficient authenticity and unfocused evaluation criteria. It proposed a three-dimensional evaluation framework grounded in the structure-process-outcome logic, encompassing three dimensions of authenticity and standardization, characteristics and advantages, application and translational impact. This framework integrated 12 key evaluation indicators in a systematic manner. The model preserved the academic characteristics of TCM syndrome differentiation and treatment, while aligning with modern scientific research standards, achieving a balance between individualized TCM experience and standardized evaluation. Concurrently, this study provided theoretical foundations and methodological guidance for evaluating the quality of TCM clinical cases, contributing significantly to the inheritance of TCM knowledge, evidence-based practice, and the reform of talent evaluation mechanisms.
3.Investigation on the microclimate of primary and secondary school classrooms in five provinces and municipalities of China in winter

Objective:
To understand the microclimate in primary and secondary school classrooms for the study period during the winter heating season, so as to provide a reference for the revision and improvement of relevant health standards.
Methods:
In December 2024, stratified random sampling was used to select 30 primary and secondary schools and 180 classrooms from the northern regions with centralized heating (Liaoning Province, Tianjin City) and the southern regions without centralized heating (Shanghai City, Anhui Province, and Jiangxi Province). Indoor temperature, relative humidity, wind speed, CO 2 and other indicators were measured on site. Variance analysis, t-test, Mann-Whitney U test and Kruskal-Wallis H test were used to analyze the differences in the microclimate of classrooms among regions and urban and rural differences.
Results:
The average temperature in the middle of the classrooms tested on site was (16.47±4.72)℃, and the variance analysis showed that the difference between the regions was statistically significant ( F=27.80, P <0.01). Among them, Tianjin had the highest average temperature of (20.43± 2.12 )℃, followed by Liaoning (19.03±2.23)℃, Shanghai (15.33±5.32)℃, Anhui (12.79±1.74)℃, and Jiangxi (11.69± 1.68 )℃. Horizontal temperature difference was 0.90 (0.50, 1.60)℃, the vertical temperature difference was 0.20 (0.10,0.60)℃, the average relative humidity was (44.39±16.16)%, the wind speed was 0.03(0.01,0.11)m/s, and the differences among different provinces and cities were statistically significant ( H/F =40.62, 82.69, 95.06, 55.28, all P <0.01). The average CO 2 volume concentration in urban areas of Tianjin, Liaoning, and Shanghai was 0.21(0.16,0.30)%, and there was no statistically significant difference ( H=4.65, P =0.10). There were grade differences in relative humidity ( F =3.71, 6.21) and CO 2 ( H =14.72, 12.92) in the north and the south (all P <0.05). In addition, the temperature, relative humidity, wind speed and CO 2 in the middle of the classroom were 42.8%, 67.8%, 100.0% and 22.2% respectively.
Conclusions
The temperature in the middle of the classroom in the non centralized heating area is lower than the standard, the relative humidity of classroom in the centralized heating area is lower than the standard,and the CO 2 in the classroom in winter is lower than the standard. It is recommended to install heating facilities in schools with low temperatures to increase the temperature and increase the frequency of ventilation in classrooms or adopt mechanical ventilation strategies to reduce CO 2 volume concentration.
4.Mechanism of Yishen Huoxue Tongqiao Formula in Improving Unilateral Vestibular Labyrinth Destruction by Regulating Metabolism-neuroplasticity

ObjectiveThis study aims to explore the mechanism by which Yishen Huoxue Tongqiao Formula improves metabolism-neuroplasticity and treats unilateral vestibular labyrinth destruction by regulating the metabolic balance of glutamate (Glu)/γ-aminobutyric acid (GABA). Methods48 Sprague-Dawley (SD) adult rats were randomly divided into the sham operation group, model group, Yishen Huoxue Tongqiao Formula groups with low, medium, and high doses (9.20, 18.39, 36.78 g·kg-1), and betahistine group (1.62 mg·kg-1). A unilateral vestibular labyrinth destruction (vestibular dysfunction) model was established by intratympanic injection of chloroform into the right ear, while the control group received intratympanic injection of normal saline. Drugs were administered once daily for seven consecutive days. During the period, behavioral tests were performed to evaluate the behaviors of rats after unilateral vestibular labyrinth destruction. Hematoxylin-eosin (HE) staining and Nissl staining were used to observe the neuronal morphology in the medial vestibular nucleus. Golgi staining was employed to assess the number of dendritic spines of neurons in the medial vestibular nucleus. Ultra-performance liquid chromatography-tandem mass spectrometry (LC-ESI-MS/MS) was utilized to detect Glu/GABA. Immunofluorescence and immunohistochemistry were used to detect the expressions of neuronal nuclei (NeuN), growth-associated protein 43 (GAP-43), and glial fibrillary acidic protein (GFAP). Western blot and real-time fluorescent quantitative polymerase chain reaction (Real-time PCR) were applied to determine the expressions of glutamate-immunoreactive (Glu-IR), GABA, GFAP, postsynaptic density protein 95 (PSD-95), and GAP-43. ResultsCompared with the sham operation group, the model group presented with head deviation, balance disorder, increased tail suspension score, nuclear consolidation of medial vestibular nerve neurons, and decreased Nissl bodies (P<0.01). The number of dendritic spines in neurons and NeuN-positive cells decreased. The content of Glu decreased. The content of GABA increased (Glu/GABA decreased). The expression of GAP-43 was down-regulated, and GFAP was up-regulated (P<0.05, P<0.01). The expressions of Glu-IR, PSD-95, and GAP-43 proteins, as well as Glu-IR mRNA decreased, while the expressions of GABA and GFAP proteins and mRNA increased (P<0.05, P<0.01). Compared with those in the model group, the head deviation, imbalanced behavior, and tail suspension scores in each treatment group decreased, with alleviated neuronal injury and recovered Nissl bodies (P<0.01). The number of dendritic spines of neurons increased, and the number of NeuN-positive cells rebounded. The content of Glu increased, and the content of GABA decreased (Glu/GABA increased). GFAP was down-regulated, and GAP-43 was up-regulated (P<0.05, P<0.01). The expressions of Glu-IR, PMD-95, and GAP-43 proteins, as well as Glu-IR mRNA increased, while the expressions of GABA and GFAP proteins and mRNA decreased. The effect was more significant in the high-dose group (P<0.01). ConclusionThe Yishen Huoxue Tongqiao Formula can alleviate vestibular dysfunction, and its mechanism may be associated with regulating the metabolic balance of Glu/GABA, mitigating neural damage, improving synaptic plasticity (promoting GAP-43 expression and inhibiting GFAP expression), and facilitating vestibular compensation.
5.Construction of a renal rehabilitation, diagnosis and quality control information platform

ObjectiveTo develop a full-process data platform of renal rehabilitation, diagnosis and quality control information. MethodsA hierarchical architectural design was proposed, adhering to clinical pathway models and standardized data protocols. The platform comprehensively covered assessment, intervention, follow-up and quality control for maintenance hemodialysis (MHD) patients. By integrating multidisciplinary resources and standardizing rehabilitation workflows, it delivered standardized and intelligent rehabilitation services. ResultsThe platform achieved standardized and intelligent management of rehabilitation services, effectively improved the physiological function, psychological state and quality of life convenience for MHD patients, while significantly reduced the economic and care burden on patients' families and society. ConclusionThe rehabilitation service model based on a full-process data platform may provide scientific and systematic support for MHD patients.
6.Escherichia coli Exopolysaccharides/Vancomycin Combination for Combating Methicillin-resistant Staphylococcus aureus Biofilm

Biofilm formation is one of the key reasons that bacterial infections are difficult to treat.So it is of great significance to develop effective strategies to resist bacterial biofilms.Although antibiotics are important clinical tools for the treatment of bacterial infections,their therapeutic efficacy is often unsatisfactory when targeting bacterial biofilm-associated diseases.In this study,exopolysaccharides(EPS)from Escherichia coli(E.coli)were extracted and purified.It was demonstrated that the obtained E.coli EPS had the ability to inhibit methicillin-resistant Staphylococcus aureus(MRSA)biofilm formation and disperse a mature biofilm.To improve the anti-biofilm effect of vancomycin(VAN),E.coli EPS was used in combination with VAN.The combination increased the inhibition rate of MRSA biofilm and increased the dispersion rate of mature biofilm from 10%to 80%.When combined with E.coli EPS,the number of bacterial colonies within the MRSA biofilm remarkably decreased by 88%,resulting in a significant improvement over the use of VAN alone at an equivalent concentration.Meanwhile,E.coli EPS could down-regulate the expression of MRSA biofilm-related genes.E.coli EPS showed good anti-biofilm effect,and E.coli EPS/VAN combination could provided a potential strategy for treatment of MRSA biofilm infections.
7.Simultaneous Determination of Four Off-flavors in Freshwater Fish by Gas Chromatography-Mass Spectrometry Combined with Silica Solid Phase Extraction

An effective method for simultaneously detecting four semivolatile earthy-musty odors in freshwater fish by gas chromatography-mass spectrometry(GC-MS)was developed.The concurrent extraction of geosmin(GSM),2-methylisoborneol(MIB),2-isopropyl-3-methoxypyrazine(IPMP),and 2-isobutyl-3-methoxypyrazine(IBMP)in fish tissue was conducted with n-hexane.The optimized QuEChERS material was implemented,and it was found that C18,primary secondary amine(PSA)and MgSO4 could adsorb the target analytes in n-hexane.So only the graphitized carbon black(GCB)could be used to purify the extraction.The adsorption rates of different materials for the four kinds of odors materials were explored in n-hexane and ethyl acetate.The experimental results revealed that the adsorption rates of silica for the four targets were 99.5%-100%in n-hexane and 0.7%-5.0%in ethyl acetate respectively.Then the silica solid phase extraction(SPE)method was utilized to eluent the compounds using 1.0 mL n-hexane/ethyl acetate in different proportions.The results of the comparative analysis demonstrated that n-hexane/ethyl acetate(4∶1,V/V)was the optimized eluent.Based on the obtained results,n-hexane extraction and GCB purification combined with silica SPE were used to isolate GSM,MIB,IPMP and IBMP from fish and the method was validated.It was found that the method showed good linearity in the range of 0.5-200 ng/mL,and with detection limits of 0.6 μg/kg for GSM and MIB,0.2 μg/kg for IPMP and IBMP.The limits of quantitation(LOQ)were 1.0 μg/kg for GSM and MIB,0.6 μg/kg for IPMP and IBMP.Good recoveries(77.5%-112.0%)and relative standard deviations(1.56%-9.42%)were also obtained.The use of silica SPE greatly mitigated the issue that the off-flavor compounds were easily lost in the gas blowing concentration process.There was no cross contamination in this method because the sample pretreatments were conducted separately,which was different with the most commonly used HS-SPME method for detecting semi-volatile substances.The sensitivity of this method was high enough to produce good quantitative results below the odor thresholds of the examined off-flavor compounds.
8.Predictive value of changes in serum VIP and 5-HT levels for the outcome of spinal cord electrical stimulation in patients with postherpetic neuralgia

Objective To investigate the predictive value of serum vasoactive intestinal peptide(VIP)and 5-hydroxytryptamine(5-HT)levels on the outcome of spinal cord electrical stimulation(SCS)in patients with postherpetic neuralgia(PHN).Methods A total of 96 PHN patients who received SCS treatment in Ziy-ang Central Hospital from January 2022 to December 2023 were selected.According to the disease outcomes of all PHN patients after 6 months of treatment,a good group(n=71)and a poor group(n=25)were set up.The clinical data of the two groups were collected and the serum VIP and 5-HT levels were detected in all pa-tients before treatment.The predictive value of serum VIP and 5-HT on disease outcome after SCS treatment in PHN patients was evaluated by receiver operating characteristic(ROC)curve,and the influencing factors of disease outcome after SCS treatment in PHN patients was explored by multivariate Logistic steppe gression a-nalysis.Results The levels of serum VIP and 5-HT in poor group were higher than those in good group(P<0.05).The area under the curve(AUC)of serum VIP and 5-HT for predicting the disease outcome of PHN patients after SCS treatment were 0.829(95%CI:0.779-0.874)and 0.743(95%CI:0.693-0.793),respec-tively,and the AUC of combined prediction was 0.941(0.891-0.986).There were no significant differences in age,gender,body moss index,education,location of onset,hypertension and drinking history between the two groups(P>0.05).The time of initial hospital admission in the poor group was longer than that in the good group,skin rash area in the poor group was larger than that in the good group,and diabetes mellitus and smoking history in the poor group were higher than those in the good group(P<0.05).The time of admis-sion for initial treatment>3 d(OR=2.188,95%CI:1.383-3.461),skin rash area>10 cm2(OR=2.018,95%CI:1.283-3.173),diabetes mellitus(OR=2.264,95%CI:1.379-3.717),serum VIP level ≥41.78 ng/L(OR=3.022,95%CI:1.685-5.420),serum 5-HT level ≥99.27 ng/mL(OR=3.579,95%CI:1.885-6.793)were the influencing factors of disease outcome after SCS treatment in PHN patients(P<0.05).Con-clusion The elevated levels of serum VIP and 5-HT before treatment are associated with poor outcomes after SCS in patients with PHN,and could be used as potential markers to predict the outcomes of SCS in patients with PHN.
9.Effects of isosorbide mononitrate tablets combined with CRRT on renal,cardiac function and serum levels of ST2 and NT-proBNP in uremia patients complicated with chronic heart failure

Objective To investigate the effect of isosorbide mononitrate tablets in conjunction with contin-uous renal replacement therapy(CRRT)on renal and cardiac function in uremia patients complicated with chronic heart failure(CHF).Methods A total of 120 in uremia patients complicated with CHF admitted to this hospital from May 2020 and July 2022 were selected as the research subjects.According to the treatment plan,they were divided into the observation group(62 cases)and the control group(58 cases).The control group was treated with CRRT,while the observation group was treated with isosorbide mononitrate tablets on the basis of the control group.The treatment course for both groups was 4 weeks.The clinical efficacy of the two groups of patients after treatment,the changes in renal function,cardiac function before and after treat-ment,as well as the levels of serum creatinine(Scr),and N-terminal pro-brain natriuretic peptide(NT-proB-NP)were observed and compared.The adverse reactions during the treatment period was recorded.Results The total clinical effective rate of the observation group was higher than that of the control group(P<0.05).The levels of blood urea nitrogen(BUN),left ventricular end-diastolic diameter(LVED),heart rate(HR),mean arterial pressure(MAP),sST2,and NT-proBNP after treatment in both groups decreased compared with those before treatment,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05).The hemoglobin(Hb),albumin(ALB),left ventricular e-jection fraction(LVEF),and six-minute walk distance(6MWD)in both groups NT-proBNP increased and the levels in the observation group were higher than those in the control group,and the differences were statisti-cally significant(P<0.05).The total incidence of adverse reactions in the observation group was slightly low-er than that in the control group,but the difference was not statistically significant(P>0.05).Conclusion The ther-apeutic regimen of isosorbide mononitrate tablets combined with CRRT can significantly improve the renal and cardiac functions of patients with uremia complicated with CHF,reduce the levels of serum sST2 and NT-proBNP,and it has few adverse reactions and good safety.
10.Relationship between CT renal imaging parameters and glomerular filtration rate measured by 99mTc-DTPA renal dynamic imaging

Objective:To explore the relationship between CT renal imaging parameters measured by 3D-slicer software and glomerular filtration rate (GFR) measured by 99mTc-DTPA renal dynamic imaging. Methods:A retrospective analysis was conducted on the clinical data of 177 patients (65 renal transplant donors, 60 patients with obstructive nephropathy, and 52 patients with renal tumors) admitted to Beijing Friendship Hospital, Capital Medical University from January 2015 to August 2021. GFR was measured for all patients. After three-dimensional imaging reconstruction of the urinary system enhanced CT using the 3D-slicer software platform, renal cortical volume, parenchymal volume, and average CT values were measured. A total of 189 kidneys (65 healthy kidneys, 72 hydronephrotic kidneys, and 52 tumor kidneys) were analyzed for the above parameters. The statistical analysis methods used independent sample t-test, one-way analysis of variance (ANOVA), Pearson correlation analysis, and receiver operating characteristics (ROC) curve analysis. Results:Compared with healthy kidneys, renal cortex volume and parenchyma volume in hydronephrotic and tumor kidneys were significantly reduced ( P<0.001), and GFR was significantly lower ( P<0.001). Among the 189 renal parameters, except for renal medulla volume ( r=0.146, P=0.531), renal cortex volume ( r=0.784, P<0.001) and renal parenchyma volume ( r=0.698, P<0.001) were significantly correlated with GFR. The results of one-way ANOVA showed significant differences between the groups in terms of renal cortex volume ( F=142.62, P<0.001), renal parenchyma volume ( F=92.92, P<0.001), average CT value of the renal cortex ( F=12.68, P<0.001), average CT value of the renal parenchyma ( F=19.68, P<0.001), and renal medulla volume ( F=3.26, P=0.041). Significant differences were observed between the subgroups for both renal cortex and parenchyma volumes ( P<0.001). Compared with other parameters, renal cortex volume showed higher diagnostic performance in distinguishing different levels of renal function. When the renal cortex volume was set to 77.91 mL and 45.46 mL, respectively, the diagnostic performance for distinguishing normal renal function from mild renal impairment ( AUC=0.830, 95% CI: 0.761-0.889, P<0.001) and mild renal impairment from severe renal impairment ( AUC=0.894, 95% CI: 0.787-0.952, P<0.001) showed high sensitivity and specificity. Conclusions:The measurement of renal imaging parameters using 3D-slicer software has clinical value in evaluating renal function in patients. Renal cortex volume demonstrates good diagnostic performance in distinguishing different levels of renal function, and is worthy of clinical promotion and application.
