1.Differential analysis of saponins in Platycodon grandiflorus from different origins based on ultra-high performance liquid chromatography-mass spectrometry

Objective To establish a highly efficient and sensitive technical system for the identification and analysis of platycodin-type saponins, systematically compare the differences in platycodin-type saponins among Platycodon grandiflorum from different producing areas, and provide scientific references for the screening of high-quality Platycodon grandiflorum resources, authenticity evaluation, and construction of standardized quality control systems. Methods A total of 45 batches of P. grandiflorum medicinal materials from 3 producing areas (Anhui, Henan, and Jilin, with 15 batches per area) were selected as research objects. Qualitative identification and semi-quantitative analysis of saponin components were performed based on ultra-high performance liquid chromatography-quadrupole-time-of-flight tandem mass spectrometry (UHPLC-Q-TOF/MS) technology. Meanwhile, two multivariate statistical methods, principal component analysis (PCA) and partial least squares-discriminant analysis (PLS-DA), were combined to analyze the differences in platycodin-type saponins of Platycodon grandiflorus from different producing areas. Results A total of 28 saponin components were identified from Platycodon grandiflorus of the three producing areas. PCA results showed that there were minor differences in platycodin-type saponins between Henan Platycodon grandiflorus and Jilin Platycodon grandiflorus, while Anhui P. grandiflorum exhibited significant differences from both. PLS-DA further screened 15 major differential compounds. Among them, the contents of 6 components including 3''-O-acetylpolygalacin D2 and platycodin H in Anhui Platycodon grandiflorus were higher than those in Henan and Jilin Platycodon grandiflorus; platycodigenic acid A had the highest content in Jilin Platycodon grandiflorus; the contents of platycodin D3, polygalacin J, and polygalacin D were relatively higher in Henan Platycodon grandiflorus. Conclusion This study clarified the characteristic differences in core components of Platycodon grandiflorus from the three major producing areas, which provided an important theoretical basis for the screening of high-quality Platycodon grandiflorus resources, elucidation of the mechanism underlying its authenticity, and construction of a standardized quality control system.

Bisphenol A (BPA) is an environmental chemical that is widely exposed in human daily life. It has attracted much attention because of its estrogenic effect. Polycystic ovary syndrome (PCOS) is the most common endocrine metabolic disease in women of reproductive age. Its pathogenesis involves the interaction of genetic, environmental and endocrine disorders. In recent years, more and more studies have shown that BPA plays an important role in the occurrence and development of PCOS. BPA promotes the development of PCOS by interfering with insulin sensitivity, immune response and changing ovarian structure and function. In addition, BPA exposure levels are associated with hyperandrogenism, insulin resistance, obesity, dyslipidemia, and decreased ovarian reserve in PCOS patients. This review summarizes the effects of BPA on PCOS in terms of ovarian function, glucose and lipid metabolism, and inflammatory response, so as to provide theoretical basis for clinical intervention and prevention of BPA in the development of PCOS.
3.Effects of rehabilitation nursing based on knowledge-action framework on compliance behavior and coping styles in patients with chronic respiratory failure

Objective:To investigate the effects of rehabilitation nursing based on knowledge-action framework on compliance behavior and coping styles in patients with chronic respiratory failure (CRF), and provide scientific basis and practical guidance for clinical nursing of patients with CRF.Methods:A historical control study was conducted. A retrospective analysis was performed on the clinical data of patients with CRF in Yiwu Central Hospital between January and April 2024. According to admission time, the patients were divided into the control group (admitted from January to February 2024, received routine rehabilitation nursing) and the study group (admitted from March to April 2024, received rehabilitation nursing based on knowledge-action framework). All patients were intervened for 1 month. The differences in self-care ability, compliance behavior, coping styles, quality of life between the two groups were compared using Exercise of Self-Care Agency Scale (ESCA), Compliance Behavior Questionnaire, Trait Coping Style Questionnaire (TCSQ), St. George′s Respiratory Questionnaire (SGRQ).Results:A total of 60 patients with CRF were included, 30 patients in the study group, with 16 males and 14 females, aged (68.72 ± 4.67) years old; 30 patients in the control group, with 17 males and 13 females, aged (69.21 ± 4.18) years old. Before intervention, there were no significant differences in ESCA score, TCSQ score and SGRQ score between the 2 groups (all P>0.05). After 1 month of intervention, ESCA score in the study group was (134.79 ± 5.62) points, higher than (125.34 ± 5.51) points in the control group, the difference was statistically significant ( t=6.58, P<0.05). The compliance behavior (adherence to medical regimens, reasonable diet, work and rest, regular checking) in the study group were 30, 28, 27, 29 cases, higher than 25, 20, 20, 22 cases in the control group, the differences were statistically significant ( χ2 values were 4.81-6.68, all P<0.05). The positive coping score of TCSQ in the study group was (39.67 ± 4.54) points, higher than (32.19 ± 4.38) points in the control group, and score of negative coping score was (13.55 ± 2.28) points, lower than (18.31 ± 2.43) points in the control group, the differences were statistically significant ( t=6.49, 7.82, both P<0.05). The SGRQ score in the study group was (30.23 ± 3.29) points, lower than (35.87 ± 3.66) points in the control group, the difference was statistically significant ( t=6.28, P<0.05). Conclusions:For patients with CRF, rehabilitation nursing based on knowledge-action framework can effectively improve self-care ability and compliance behavior, form more positive coping styles, improve quality of life.
4.CT-guided dorsal root ganglion pulsed radiofrequency combined with continuous radiofrequency for thoracic postherpetic neuralgia:Efficacy and safety

Objective To observe the efficacy and safety of CT-guided pulsed radiofrequency combined with continuous radiofrequency treatment for thoracic postherpetic neuralgia(PHN).Methods A total of 110 patients with thoracic PHN were retrospectively enrolled and divided into observation group(pulsed radiofrequency combined with continuous radiofrequency treatment)and control group(pulsed radiofrequency treatment)(each n=55).The visual analogue scale(VAS)pain scores before and 1 day,7 days,1 month and 3 months after treatment,also the treatment effective rate and incidence of adverse reactions were compared between groups.Results No significant difference of VAS pain scores was found between groups before treatment(P=0.818),while 1 day,7 days,1 month and 3 months after treatment,observation group had lower VAS pain scores than control group(all P<0.001).The treatment effective rate in observation group was higher than that in control group(98.18%[54/55]vs.85.45%[47/55],P=0.025),while the incidence of adverse reactions in observation group was lower than that in control group(1.82%[1/55]vs.12.73%[7/55],P=0.028).Conclusion CT-guided pulsed radiofrequency combined with continuous radiofrequency could effectively reduce pain degree of thoracic PHN and adverse reactions.
5.Non-targeted screening and prioritization of emerging pollutants in sewage using direct injection high-resolution mass spectrometry

Objective To establish a high-throughput non-targeted screening and prioritization method for emerging pollutants(EPs)in sewage using direct injection high-resolution mass spectrometry(HRMS).Methods The sewage samples were filtered by membrane filter and directly subjected to the liquid chromatography-time-of-flight mass spectrometer based on a method modified from our previous study.A C18 chromatographic column was applied for a gradient elution separation,and accurate mass and mass spectral fragment information were obtained through the MS full scan mode and MS/MS DIA data collection mode.After peak detection and alignment,the features from the raw data through open source software MZmine 3,and then high-throughput screening strategies such as MassBank and PubChem databases were used for compound annotation.Finally,the candidate features were confirmed with chemical standards by compared their retention time and mass spectrum fragmentation ion peaks.Results 13 EPs were identified,including 7 industrial chemicals,4 pharmaceuticals,1 pesticide and 1 metabolite.High detection rates were observed for metformin(86.2%),2-hydroxybenzothiazole(79.3%),1,2-benzisothiazole-3-one(72.4%),and 1,2-benzisothiazole-3-one(72.4%).The quantitative concentration range of EPs was 1.37~19.05 ng/mL,with the high concentrations observed for melamine(19.05 ng/mL)and furosemide(18.49 ng/mL).Ecological risk assessment identified 1,2-benzisothiazol-3-one,4-aminoacetophenone,creatinine,2-hydroxybenzothiazole,and furosemide as key pollutants.Conclusion This direct injection coupled with HRMS workflow enables efficient non-targeted screening and prioritization of emerging EPs in sewage samples,highlighting five ecotoxicologically critical EPs.The methodology enhances environmental monitoring capabilities and provide critical technical support for interdisciplinary research such as environmental forensics and health risk assessment.

The female reproductive system is essential for sustaining reproductive endocrine homeostasis,however,its vulnerability to various endogenous and exogenous insults,including pathological conditions,pharmacological agents,genetic predispositions,and environmental factors,often results in compromised fertility.The existing protective approaches(including surgical interventions,hormonal replacement therapies,and assisted reproductive techniques)are constrained by several limitations,such as adverse therapeutic effects,technical complexities,and their incapacity to reverse ovarian senescence.Artemisinin and its derivatives(ARTs),characterized by their unique endoperoxide bridge configuration,have exhibited outstanding therapeutic performance across multiple domains including malaria treatment,anticancer therapy,inflammation modulation,and parasitic infection control.Emerging research has identified their novel protective capabilities against various reproductive system pathologies.This comprehensive review systematically elucidates the molecular mechanisms underlying artemisinin-based interventions in reproductive pathologies and evaluates their clinical translation prospects,thereby proposing innovative strategies for the development of next-generation fertility-protective agents with enhanced safety and efficacy profiles.

Epidemiology research has found that ABO blood group and the gene coding ABO glycosyltransferases are associated with many human diseases. The activity of ABO glycosyltransferases varies with different blood types, mediating different glycosylation modifications. The variation in glycosylation level might be the risk factor of specific disease. Based on the literature retrieval and analysis, glycosylation levels regulated by ABO glycosyltransferases mainly affect the ABH blood group antigens and von Willebrand factor (vWF). By modulating key glycosylation components, ABO glycosyltransferases partly determine the activity or expression levels of the ABH antigens and vWF, thereby affecting the development and progression of diseases. Exploring the pathogenic mechanisms of ABO glycosyltransferases can improve the understanding of the molecular pathology of related diseases and provide reference for clinical research and application.
10.Association of anti-rituximab antibodies with relapse after therapy in children with frequently relapsing or steroid-dependent nephrotic syndrome

Objective:To investigate the association between anti-rituximab antibodies (ARA) and relapse after rituximab (RTX) therapy in children with frequently relapsing or steroid-dependent nephrotic syndrome (FRNS or SDNS).Methods:A retrospective cohort study was conducted. Clinical and laboratory data were collected from 48 FRNS or SDNS children treated with RTX in the Department of Pediatrics, General Hospital of Eastern Theater Command, between April 2024 and October 2024. Data included RTX dosing frequency, relapse events, peripheral CD20? B-cell counts, and ARA levels. With a 6-month observation period after the last RTX therapy, the children were divided into an ARA-positive group and an ARA-negative group based on ARA test results. Chi-square test, independent sample t-test, or Mann-Whitney U test were used to compare relapse rates and laboratory indicators between the two groups. The predictive value of ARA levels for relapse was evaluated using univariate receiver operating characteristic (ROC) curve analysis. Results:Among the 48 children (36 males, 12 females), the age of disease onset was 3.5 (2.0, 6.0) years, the ages at the first and last RTX treatments were 7.0 (5.0, 12.0) years and 9.5 (7.0, 13.0) years, respectively. The overall ARA positive rate was 29% (14/48). The relapse rate in the ARA-positive group was significantly higher than that in the negative group ( P<0.05). The ARA level was 0.01 (0.01, 5.88) μg/L, and all 12 children with ARA levels >5.88 μg/L relapsed. ROC curve analysis showed that ARA levels predicted relapse after RTX treatment in FRNS or SDNS children with an area under the curve (AUC) of 0.73, sensitivity of 0.50, specificity of 1.00, and an optimal cut-off value of 5.02 μg/L. All children received single-dose RTX therapy, with no significant difference in treatment frequency between the two groups ( P>0.05). At 3 months after the last rituximab therapy, CD20? B cell counts were significantly higher in the ARA-positive group ( P<0.05). During follow-up, 15% (7/48) of the children experienced infusion-related adverse reactions, with no significant difference in incidence between the two groups ( P>0.05). Conclusion:ARA is significantly associated with relapse in FRNS or SDNS children after RTX therapy.
