Promoting the transformation of scientific and technological achievements in hospitals can significantly enhance the new productivity levels and innovation capabilities in China's biomedicine sector.However,specialized hospitals face several challenges due to their smaller scale and singular discipline layout.These include insufficient interdisciplinary collaboration,in-adequate funding and capacity to attract funds,a lack of awareness and motivation among hospital staff regarding achievement transformation,limited experience in transforming results,and difficulties in industrializing outcomes.To effectively improve the transformation of scientific and technological achievements,specialized hospitals can adopt strategies such as strengthening the in-tegration of industry,academia,research,and healthcare;increasing financial investment;enhancing theoretical training on a-chievement transformation;building a team of transformation professionals;and exploring diverse pathways for achievement trans-formation.

It is believed that qi and blood disharmony is the key pathogenesis of essential hypertension， for which mildly regulating qi and blood is recommended. According to the different pathological states and related causes of qi and blood disorders， essential hypertension can be divided into five syndrome types for differentiation and treatment. In terms of blood deficiency and liver constraint， it is recommended to nourish the blood and soften the liver， rectify qi to resolve constraint， using Xiao Yao Powder （逍遥散） or self-made Qihua Jieyu Decoction （七花解郁汤）. For qi stagnation and blood stasis， the method of regulating qi and harmonizing blood， dissolving stasis and unblocking collaterals should be used， with self-made Guitao Tongluo Decoction （桂桃通络汤）. For phlegm-dampness internal obstruction， it is recommended to move qi and promote urination， dissolve phlegm and eliminate dampness， using self-made Zhuanqi Sanzhuo Formula （转气散浊方）. For binding of phlegm and stasis， dissolving phlegm and dispelling stasis， dredging the bowels and directing the turbid downward is advised， and self-made Sanren Tiaozhi Formula （三仁调脂方） can be used. In terms of deficiency of both qi and blood， it is recommended to boost qi and nourish blood， supplement deficiency and consolidate the root， using Gui Pi Decoction （归脾汤） or self-made Shenqi Zaizao Decoction （参芪再造汤）.

Objective: To investigate the resistance to rifampicin and isoniazid and the changing trends among patients with pulmonary tuberculosis in Luohu District, Shenzhen City, Guangdong Province from 2012 to 2022, so as to provide insights into improving drug-resistant pulmonary tuberculosis control and prevention strategies. Methods: Basic information, treatment classification and drug resistance data of patients with pulmonary tuberculosis and positive pathogenic detection in Luohu District from 2012 to 2022 were collected through the Tuberculosis Surveillance System of Chinese Disease Prevention and Control Information System, and resistance rates of rifampicin and isoniazid and the changing trends were analyzed. Results: A total of 2 126 patients with pulmonary tuberculosis were collected and had a median age of 34 (interquartile range, 25) years, including 1 334 males (62.75%) and 792 females (37.25%). There were 302 patients with drug-resistance in Luohu District from 2012 to 2022, with a resistance rate of 14.21%. Among them, 60 patients were monoresistant to rifampicin (2.82%), 113 patients were monoresistant to isoniazid (5.32%), and 129 patients were multidrug resistant (6.07%). The rate of rifampicin monoresistance showed a downward trend from 2012 to 2022, while the rate of multidrug resistance showed an upward trend (both P<0.05). There was no significant tendency in the rate of isoniazid monoresistance (P>0.05). The rate of multidrug resistance among patients without Shenzhen residence was higher than that among patients with Shenzhen residence; the rates of rifampicin resistance and multidrug resistance among retreated patients were higher than those among treatment-naïve patients (all P<0.05). Conclusions The rate of rifampicin monoresistance appeared a downward trend and the rate of multidrug resistance appeared an upward trend among patients with pulmonary tuberculosis in Luohu District from 2012 to 2022. Attention should be given to non-Shenzhen residence and retreated patients.

Objective: To improve the diagnosis and therapy of childhood pulmonary thromboembolism (PTE) by analyzing the clinical features of this rare condition. Methods: A total of 8 pediatric patients (4 males, 4 females) with PTE diagnosed in the Children′s Hospital of Zhejiang University School of Medicine from March, 2014 to March, 2019 were enrolled. The clinical manifestation, laboratory results, imaging findings, diagnosis and treatment were summarized. Results: Among these 8 cases, aged from 9 hours to 14 years and 10 months. Fever was found in 4 cases, cough aggravation in 4, short of breath in 3, chest pain in 2, abdominal and back pain in one, hemoptysis in 2, cyanosis in 1, and edema of lower extremities in 2. Physical examination found decreased breath sound in 2 cases, phlegm rale in 3, and pleural friction rub in one. Pleural effusion was found in 5 cases by ultrasound. Plasma D-dimer increased in 6 cases (0.66-9.96 mg/L) and hypersensitive C-reactive protein elevated in 5 cases (10.78-78.00 mg/L). Chest enhanced CT showed pulmonary artery or venous filling defects, including pulmonary artery embolism in 7 cases and pulmonary vein embolism in one. The primary disease of these patients included Mycoplasma Pneumoniae pneumonia in 4 cases, nephritis in 2 and postoperative congenital heart disease in 2. Apart from one case who withdrew the treatment and was discharged, the other 7 patients received anticoagulant treatment had good outcome. Conclusions For children with Mycoplasma pneumoniae pneumonia, immune disorders, long-term hormone therapy, cardiovascular invasive operation or other high-risk factors, PTE should be considered when fever, cough aggravation, short of breath, chest and back pain with pleural effusion are present. Chest enhanced CT scan should be performed as soon as possible, and anticoagulation should be started once the diagnosis is confirmed.

Objective: To explore the relationship between glycemic control and visceral adiposity index (VAI) among type 2 diabetes mellitus (T2DM) patients.Methods: A community-based epidemiological field study for patients with T2DM aged ≥ 40 years was conducted in China.Every participant underwent physical examinations, biochemical tests of fasting glucose, glycosylated hemoglobin (HbA1c), total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C) and so on, and a questionnaire, including anthropometric characteristics, lifestyle, disease history, family history, and medication use.Those participants with HbA1c ≥7.0% were classified as the poorly controlled in our analysis of relationship between glycemic control and VAI.Anthropometric characteristics, lifestyle, and biochemical indexes of the participants were compared among the groups of different VAI levels.Logistic models were applied in multiple analysis adjusting for possible confounders.Results: A total of 1 607 patients with T2DM were recruited in our analysis with a mean age of (59.4±8.1) years and an average T2DM duration of (7.0±6.4) years.Among them, 78.3% were on hypoglycemic therapy.The cutoff points of quartiles of VAI were calculated for the males and females, respectively.According to the ascending order of the quartiles of VAI, the participants were divided into four groups, i.e.Q1, Q2, Q3, and Q4.The poor glycemic control rate for these groups were 60.6%, 65.7%, 70.1%, and 71.0%, respectively (Trend χ2=12.20, P<0.001).After adjustment for age, gender, systolic blood pressure (SBP), diastolic blood pressure (DBP), LDL-C, smoking, cardio-cerebral vascular disease (CVD) history, hypoglycemic therapy, T2DM duration, and family history of diabetes, the Logistic regression models showed that the glycemic control rate was significantly associated with VAI levels among the patients with T2DM.Compared with the participants in group Q1, the ORs of poor glycemic control for those in groups Q2, Q3, and Q4 were 1.239 (95%CI 0.918 to 1.672), 1.513 (95%CI 1.117 to 2.050), and 1.535 (95%CI 1.128to 2.088), respectively (trend P=0.003).With each quartile increase in VAI, the OR of poor glycemic control was 1.162 (95%CI 1.054 to 1.282).Conclusion: The glycemic control among the patients with T2DM is significantly associated with VAI.High level of VAI is an indicator of poor glycemic control.

Objective To study the effects of Yoga breathing exercise on heart function and quality of life of patients with chronic pulmonary heart disease. Methods From January 2012 to May 2014 in Guangyuan Third People′s Hospital, 86 cases with chronic pulmonary heart disease were enrolled for the study and randomly divided into observation group (n=43) and control group (n=43) by adopting the method of random number table. Cases in the control group were given routine treatment and care. Based on it, patients in the observation group were given Yoga breathing exercise. The heart and lung function and the scores of WHOQOL-BREF were compared between two groups before and after 6 months treatment. Results After treatment, the pulmonary function indices included vital capacity (VC), forced expiratory volume in one second (FEV1), the ratio of FEV1 of forced vital capacity ( FEV1/FVC) , maximal mid-expiratory flow curve ( MMEF) and peak expiratory flow ( PEF) in the observation group were significantly higher than that of the control group after treatment ( t=5. 063, 6. 556, 7. 075, 2. 672, 2. 527; P<0. 05). The heart function indices included walk test in 6 min, blood brain natriuretic peptide (BNP), right ventricular ejection fraction (RVEF), pulmonary artery systolic pressure ( PASP) in the observation group were significantly better than that of the control group after treatment (t=5. 586, 17. 342, 5. 236, 6. 511; P <0. 05). After intervention, the scores of patients′ physiologic, mental, social and environmental area in the observation group were apparently higher than these of the control group (t=4. 698, 6. 477, 3. 311, 3. 407;P<0. 05). Conclusions Yoga breathing exercise can improve heart function and overall quality of life in patients with chronic pulmonary heart disease by improving alveolar ventilation and ventilation efficiency, which is worthy of clinical application.

Objective:To explore the correlation between glycemic control of type 2 diabetes mellitus (T2DM) patients and brachial-ankle pulse velocity (baPWV). Methods:A community-based cross-sec-tional study was conducted in Beijing, China. Every subject underwent physical examinations, glycated hemoglobin ( HbA1 c ) , blood lipid and baPWV measurements and completed a standardized question-naire. T2DM patients were divided into well controlled and poorly controlled groups according to HbA1c levels. The correlation between glycemic control of T2DM patients and baPWV was analyzed. Results:In this study, 1 341 subjects were recruited, including 733 T2DM patients and 608 non-diabetes sub-jects. Compared with non-diabetes subjects, abnormal baPWV ( baPWV≥1 700 cm/s) rate for T2DM patients was higher (40. 8% vs. 26. 8%, P<0. 001). With HbA1c<6. 5% or <7. 0% as the aim of glycemic control in T2DM patients, the abnormal baPWV rates for non-diabetes subjects, well controlled and poorly controlled T2DM patients were significantly different (non-diabetes vs. HbA1c<6. 5% T2DM vs. HbA1c≥6. 5% T2DM: 26. 8% vs. 32. 8% vs. 42. 6%, P <0. 001; non-diabetes vs. HbA1c <7. 0% T2DM vs. HbA1c≥7. 0% T2DM:26. 8% vs. 36. 1% vs. 43. 4%, P<0. 001). After being ad-justed for gender, age, smoking status, diabetes mellitus family history, T2DM duration, cardiovascular diseases ( CVD ) , waist hip ratio ( WHR ) , systolic blood pressure ( SBP ) , diastolic blood pressure ( DBP) , total triglycerides ( TG) , high density lipoprotein cholesterol ( HDL-C) , and low density lipo-protein cholesterol ( LDL-C ) , the Logistic regression models suggested that glycemic control status of T2DM patients was associated with abnormal baPWV. Compared with non-diabetes subjects, the ORs for abnormal baPWV in HbA1 c <6 . 5% T2 DM patients and HbA1 c≥6 . 5% T2 DM patients were 0 . 927 (95%CI 0. 560-1. 537) and 1. 826 (95%CI 1. 287-2. 591). Compared with non-diabetes subjects, the ORs for abnormal baPWV in HbA1c<7. 0% T2DM patients and HbA1c≥7. 0% T2DM patients were 1. 210 (95%CI 0. 808-1. 811) and 1. 898 (95%CI 1. 313-2. 745). Conclusion:The glycemic con-trol status of T2DM patients from communities is significantly associated with baPWV. Poor glycemic con-trol is a risk factor for abnormal baPWV. Keeping HbA1c under control might lower the risk of cardiovas-cular diseases in T2DM patients.

Objective To examine the association between the polymorphisms of apolipoprotein E gene (APOE) and geriatric depression (GD),and then conduct an exploratory investigation to analyse whether the APOE polymorphisms would relate to the depressive syndrome severity,the cognitive function,or the level of serum lipid in patients.Methods Participants,including 120 GD patients and 120 normal controls were enrolled to detect the two single nucleotide polymorphisms of APOE,rs7412 and rs429358 using the technology of SNP site testing.The frequency differences of genotype and allele were compared between the two groups.Then the association between APOE polymorphisms and clinical or demographic data of patients were clarified.The relationship between clinical or demographic data,and the cognitive function of GD patients were investigated using the Logistic multiple regression analysis.Results The frequency of APOE genotype and allele showed no significant difference between the two groups(P＞0.05).Patients carrying e4 allen had significantly lower total scores of Man-Machine System Engineering((22.38±2.22) vs (25.28±2.28),t=3.091,P＜0.01) and higher levels of TC (t=2.225,P＜ 0.05) and LDL(t=2.728,P＜0.01) compared with those without ε4 allen.The specific symptoms of patients carrying e4 allen were cognitive impairment(load 0.902) and retardant factors(load 0.695),while patients without ε4 allen had characteristic symptoms of anxiety(load 0.990) and weight factors(load 0.864).Ranked by the effect power,the risk factors of cognitive impairment of GD patients are ε4(b'=1.097) and then TCTC (b'=0.401).Conclusions APOE may not modulate the susceptibility to GD.Patients carrying ε4 allen have severer cognitive impairment and higher levels of serum lipid.The different genotypes may lead to different clinical symptoms.

OBJECTIVETo investigate the role of histone acetylation in regulating influenza virus replicative intermediate double-stranded RNA (dsRNA)-induced interleukin-6 (IL-6) expression in A549 cells.

METHODSA549 cells were treated with influenza virus replicative intermediate dsRNA, histone deacetylase (HDAC) inhibitor trichostatin A (TSA), or HADC small interfering RNA (siRNA). The changes in the cellular IL-6 promoter activities were detected by dual-luciferase assay, and IL-6 mRNA and protein expressions in the cells were determined using real-time RT-PCR and ELISA, respectively.

RESULTSInfluenza virus replicative intermediate dsRNA obviously up-regulated IL-6 expression in the cells. HDAC inhibitor TSA significantly enhanced the activity of IL-6 promoter and increased IL-6 mRNA expression in A549 cells, and HDAC3 may play an important role in this process. HDAC inhibitor TSA and DNMT inhibitor DAC showed no synergic effect in regulating IL-6 expression.

CONCLUSIONSInfluenza virus replicative intermediate dsRNA-induced IL-6 expression in A549 cells is regulated by histone acetylation.
