Objective To summarize the best evidence of early rehabilitation management of hand function in patients with hand burn,and provide basis for medical personnel to carry out early rehabilitation management of hand burn.Methods According to the 5S model,all evidence on early rehabilitation management of hand function in patients with hand burns was retrieved from databases and websites including UpToDate,BMJ Best Practice,NICE,RNAO,GIN,and so on.The retrieved evidence included clinical decisions,guidelines,evidence summaries,expert consensuses,systematic reviews,Meta analysis.The search period of the summary of best evidence,expert concensus and guidelines are from the establishment of the database to December 20,2024,and the search period of the systematic reviews is from January 1,2019 to December 20,2024.Quality evaluation of literature,evidence extraction and summary were conducted.The evidence was graded according to JBI evidence pre-grading system 2014.Results Totally 17 pieces of the literature were included,including 1 clinical decision,4 guidelines,6 evidence summaries,3 expert consensus articles,and 3 systematic reviews.Totally pieces of best evidence were summarized,including 7 aspects of assessment,hand positioning,treatment of edema,dressing,use of orthotics,joint mobility training,and health education.Conclusion This study summarized the best evidence of early rehabilitation management of hand function in patients with hand burn,and provided evidence-based evidence for clinical rehabilitation management plan.According to the clinical situation,the medical staff can apply the evidence to clinical practice,so as to improve the hand function and quality of life of patients.

Objective:The evidence-based and Delphi methods were used to construct the exercise program for hospitalized patients with diabetes foot to provide guidance for clinical practice.Methods:Evidence on exercise management of diabetic foot patients was systematically searched from BMJ Best Practice, UpToDate, Registered Nurses′ Association of Ontario and other domestic and foreign databases and professional association websites. The retrieval period was from the establishment of the database to April 2021. The quality of the included literature was independently evaluated, and the evidence of the literature meeting the quality standards was extracted and summarized to form the first draft of exercise program for inpatients with diabetic foot. After two rounds of Delphi expert letter consultation, the program items were revised, and the final draft of the exercise program for inpatients with diabetic foot suitable for clinical practice was formed.Results:The effective recovery rate of the two rounds of expert correspondence questionnaire both were15/15. The expert authority coefficient was 0.865 and 0.895 respectively. And the Kendall coordination coefficient was 0.232 and 0.291 (both P<0.01). An exercise program for inpatients with diabetic foot had been formed, including 5 modules(exercise evaluation, exercise prescription, exercise monitoring, post-exercise evaluation and exercise management), 12 items and 40 operational items. Conclusions:The exercise program for inpatients with diabetic foot constructed in this study is scientific and clinically applicable, which provide scientific guidance for clinical medical staff to carry out exercise practice.

Objective:To retrieve, evaluate and summarize the best evidence of exercise management for diabetic foot patients, so as to provide an evidence-based evidence for clinical formulation of exercise programs for diabetic foot patients.Methods:Evidence on exercise management for patients with diabetic feet was retrieved by computer in the British Medical Journal (BMJ) Best Practice, UpToDate, National Institute for Health and Clinical Excellence, Registered Nurses' Association of Ontario, Scottish Intercollegiate Guidelines Network, Medlive, International Working Group on Diabetic Foot, International Diabetes Federation, American Diabetes Association, Chinese Diabetes Society, Chinese Society of Endocrinology, Joanna Briggs Institute Evidence-based Health Care Center, Cochrane Library and other domestic and foreign databases and professional association websites. Evidence included best practice information books, recommended practices, guidelines, evidence summaries, systematic reviews, and expert consensus. The retrieval time limit was from the establishment of the database to April 30, 2021. Two to four investigators independently assessed the quality of the included article, and extracted and summarized the evidence that met the quality standards.Results:A total of 13 articles were included, involving 4 guidelines, 6 evidence summaries, and 3 systematic reviews. A total of 17 best evidences were summed up in five aspects, namely exercise assessment, exercise mode, exercise intensity, exercise time and frequency, and exercise safety in diabetic foot patients.Conclusions:This article summarizes the best evidence for exercise management in patients with diabetic foot. Clinical medical and nursing staff should formulate individualized exercise programs according to the severity of systemic and local blood vessels, neuropathy and foot ulcers in patients with diabetic foot to ensure the effectiveness and safety of patients' exercise.

【Objective】 To explore the value of thromboelastogram (TEG) in evaluating coagulation function of patients with liver cancer. 【Methods】 102 patients with liver cancer and 48 with hepatic hemangioma from Department of Hepatobiliary Surgery, Nanyang Central Hospital from August 2017 to September 2020 were retrospectively analyzed. TEG indicators (R, K, Angle, MA, CI, and G value) and routine coagulation indicators (Plt, PT, INR, APTT, FIB, and TT) of those patients and basic clinical data of liver cancer patients were collected, and the difference of detection parameters between the liver cancer group and liver hemangioma group was compared; The difference of TEG parameters in liver cancer patient subgroups was compared, and the correlation between TEG and routine coagulation tests in liver cancer patients was analyzed using Spearman rank correlation analysis. The sensitivity of the two detection methods in detecting the coagulation status of patients with liver cancer was compared. 【Results】 1) Compared with patients with hepatic hemangioma, Plts decreased significantly (166.6±108.824 vs 224.10±54.933, P<0.001), while PT, INR and APTT values increased significantly (13.12±2.052 vs 11.421±0.884, 1.156±0.191 vs 1.00±0.074, 29.977±5.333 vs 26.954±5.269, all P<0.05) in patients with liver cancer; MA and G values in patients with liver cancer were lower (56.991±11.574 vs 60.069±5.094, 7.667±4.682 vs 7.725±1.709, P<0.05); 2) Compared with newly diagnosed liver cancer patients, the Plt of re-diagnosed liver cancer patients decreased significantly(125.78±79.673 vs 188.86±116.437, P<0.05); the R and K value increased significantly (7.594±2.601 vs 6.058±1.739, 3.453±2.402 vs 2.438±1.990, all P<0.05), while the Angle, MA, CI and G value decreased significantly (53.897±12.288 vs 61.495±9.949, 53.556±11.407 vs 58.865±11.313, -3.494±4.253vs -0.836±3.180, 6.311±3.209 vs 8.406±5.191, all P<0.05); 3) There were significant differences in TEG parameters (R value excluded) between liver resection, transhepatic arterial chemoembolization and conservative treatment (P<0.05); 4) The R, K value of patients with liver cancer were negatively correlated with the Plt value, while the Angle, MA, CI, and G value were positively correlated with Plt value (P<0.001); the K value was negatively correlated with the Fib value, while the Angle, MA, CI, G value were positively correlated with Fib value (P<0.001); the R and K value were positively correlated with TT value, while the Angle and CI were negatively correlated with TT value (P<0.05); 5) The detection rate of hypocoagulability by TEG and routine coagulation testing was 18.63% (19/102) and 7.84%. 【Conclusion】 Compared with the newly diagnosed liver cancer patients, re-diagnosed liver cancer patients showed hypercoagulability. TEG can diagnose the coagulation abnormalties more sensitively, and help reduce the risk of bleeding.

Objective: To translate the English version of the Pressure Ulcer Risk Primary or Secondary Evaluation Tool (PURPOSE T) into Chinese and evaluate its reliability and validity. Methods: Firstly, PURPOSE T was translated and revised, cross-cultural debugging, and then 213 inpatients from Jiangsu University Affiliated Hospital from November 2017 to May 2018 were selected for investigation. Choosing the total item content validity index (S-CVI/Ave) and the content validity of the item level content validity index (I-CVI) to evaluate content validity, using the Chinese version of the Braden scale as the criterion to measure the validity-related validity of the Chinese version of PURPOSE T. The internal consistency reliability Cronbach α coefficient, the assessor reliability, and the test-retest reliability measure the reliability. Results: The Chinese version of PURPOSET S-CVI was 0.967; I-CVI was 0.6-1.0, The coefficients between PURPOSET and Braden scale in mobility, activity ability, perception status, nutritional status and humidity were 0.600, 0.661, 0.699, 0.519, and 0.783. The PURPOSET and Braden scales had a correlation coefficient of 0.407 in predicting the risk of pressure injury. The Cronbach α=0.859 of the evaluation tool, the assessor reliability were 0.87 and the correlation coefficient of the 2 measurements of the test-retest reliability were 0.91. Conclusions The Chinese version of PURPOSET has good reliability and validity and can be used as a risk assessment tool for pressure injury in hospitalized patients in China.

OBJECTIVE： To prepare Jinlei capsule and evaluate its quality. METHODS： Wet granulation was adopted. The molding technology of Jinlei capsule content granule was optimized with drug-excipient ratio， excipient ratio and ethanol volume fraction as factors， using comprehensive score of particle forming rate， moisture content and fluidity as the evaluation index. According to 2015 edition of Chinese Pharmacopoeia （part Ⅳ）， the characters， moisture， volume difference， disintegration time limit were checked. Qualitative identification of Gentianopsis paludosa and Lysimachia christinale were analysed by TLC， and quantitative analysis of luteolin and kaempferol were analysed by HPLC. RESULTS： The optimal molding technology of Jinlei capsule content granule was that the maltodextrin and the micro-silica gel are mixed as a mixed auxiliary material according to 10 ∶ 1 and then mixed with the drug powder by 1 ∶ 0.5， using the wet granulation method with 90％ ethanol as wetting agent. The characters， moisture， volume difference and disintegration time limit of Jinlei capsule were in line with Chinese Pharmacopoeia. TLC showed the same color spots on the corresponding positions of the reference chromatogram. The linear range of luteolin and kaempferol were 4.4-88.0 μg/mL and 9.6-96.0 μg/mL （all r＝0.999 9）. RSD of precision （n＝6）， reproducibility （n＝6） and stability （18 h， n＝7） tests were all lower than 2.5％. The average recoveries were 95.74％ and 99.77％ （RSD＝1.50％， 2.72％， n＝6）； the content of them were 2.52， 0.34 mg/g. CONCLUSIONS： The optimal molding technology of Jinlei capsule is stable and feasible； prepared Jinlei capsule in controllable in quality.

Objective To identify and revise the major nursing outcomes in patients with depressive disorders,in order to provide valid tools for evaluating the common problems of nursing care in these patients. Methods The nursing diagnosis and outcomes associated with the depressive disorders were initially selected by literature review.The frequency of the diagnosis and the significance of the outcomes were evaluated by 43 professional clinical nurses,in order to establish the major nursing outcomes.After the establishment,the major outcomes were consulted and revised by 39 experts.Results Eight major nursing outcomes were identified according to the criterion that “the outcome should be resulted from top5 common diagnosis and its weighting coefficient should be more than 0.055”.The active coefficients for the two rounds of experts′consultation were 1 00.00% and 94.87%,and the authority coefficients were 0.71 and 0.84.Moreover,the name,definition, expression and indicator number for the 8 outcomes were revised.Conclusions The 8 major nursing outcomes have good sensitivity and effectiveness for evaluating and measuring the common problems of nursing care in patients with depressive disorders.These can also provide good evidence to identify nursing outcomes for other diseases.

Objective To compare the differences between electronic lesion of pyramidal tract and internal capsule hemorrhage in establishing spastic rat models.Methods Experimental animals were randomly assigned into three groups:electronic lesion group (EL),internal capsule hemorrhage group (ICH) and control group (CON,n=28).Seven,14,21 and 28 d after each treatment,the general conditions of the rats and behavioral tests on them were recorded; and the gastrocnemius Hoffman reflex (H reflex) was detected; and the percentage of type Ⅰ muscle fiber in quadriceps femoris were examined by skeletal muscle myosin ATP staining.Results Rats in EL group and ICH group showed significant differences in behavioral tests as compared with CON group (P＜0.05); the duration of symptoms of neurological deficit was significantly longer in EL group than the other two groups (P＜0.05).In contrast to CON group,the amplitude of gastrocnemius H reflex in EL group and ICH group was increased and the latency of H reflex was decreased with significant difference (P＜0.05),which could be observed till 28 d and 14 d,respectively,after each treatment.The percentage of type Ⅰ muscle fiber in quadriceps femoris EL and ICH groups was obviously increased as compared with that in CON group (P＜0.05),which persisted to 21 and 14 d,respectively,after each treatment.Meanwhile,rats in EL group showed significant differences in behavioral test,gastrocnemius H reflex and percentage of type] muscle fiber in quadriceps femoris as compared with ICH rats (P＜0.05).Conclusion The muscle tone of EL rats is higher,and lasts longer than that ofICH rats.

Objective To research the relationship between high temperature and population death in cardio-cerebrovascular diseases in Shijiazhuang.Methods The time series of maximum daily temperature and daily death number of cardio-cerebrovascular diseases were described,and the relationship between daily maximum temperature and daily mortality of cardio-cerebrovascular diseases in summer was analyzed.The impact of daily highest temperature in summer from 2009 to 2011 on daily death of cardio-cerebrovascular diseases was analyzed.Results In summer,the highest temperature which could reach to 41.7℃ emerged mostly in July and August,and when the highest temperaiure was higher than 34℃,cardio-cerebrovascular diseases death increased obviously as the temperature increasing.The relative humidity was more than 70％ in most time of the year round.The excessive death in hot days accounted for 10.9％ in which the old people occupied the majority.Conclusion The extreme high temperature may obviously increase the cardio-cerebrovascular death in Shijiazhuang.
