Background: Rural areas in Korea are experiencing both super-aging and depopulation, creating critical gaps in health and social care. Using a citizen participatory approach, we sought to address the care gaps for older adults in rural areas. This study examined the changes in frailty and depressive symptoms observed during a citizen-led intervention. Methods: This study is a single- arm pre–post quasi-experimental design. A 12-week intervention was implemented using local citizens as care providers. Intervention components included identifying and planning individual care needs, providing health education, organizing tailored community activities, and conducting AI-assisted weekly check-up calls to monitor health status. Results: Changes appeared more pronounced among vulnerable subgroups. Older adults with frailty showed an observed decrease in depressive symptoms compared to those with prefrailty or robust status. Conversely, among those with depressive symptoms, frailty levels appeared to increase more slowly than those without depressive symptoms. These patterns are consistent with the previously reported bidirectional associations between frailty and depression and may reflect the tendency for changes in one domain to coincide with changes in the other, rather than indicating a causal influence. Conclusion This citizen-led care intervention showed more noticeable short-term changes among older adults with higher vulnerability, particularly those with frailty or depressive symptoms. These findings indicate potential roles for citizen participation in enhancing social support and supporting ongoing monitoring in rural depopulation areas. The results suggest that citizen participation is a potentially feasible and sustainable approach to care systems in aging, resource-limited communities.

This study developed a hemp seed soup designed for the elderly, using the functional benefits of hemp, a nutrient-dense ingredient rich in dietary proteins, essential fatty acids, polyphenols, and antioxidants. Soups were formulated with varying levels of hemp seed powder (0% (CS), 25% (HS25S), 50% (HS50S), and 75% (HS75S)), to address the dietary needs and physiological characteristics of the elderly, including mastication and swallowing difficulties. Increasing the hemp seed powder levels significantly enhanced the crude protein, total polyphenol, and flavonoid contents while improving the antioxidant activities, as assessed by 2,2-diphenyl-1-picrylhydrazyl (DPPH) and 2,2’-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid) (ABTS) radical scavenging assays. HS75S exhibited the highest nutritional values and antioxidant activity. By contrast, food texture analysis showed that the viscosity decreased at higher hemp seed concentrations, with HS25S and HS50S demonstrating favorable textures for elderly consumption.Consumer acceptance testing identified HS50S as the most preferred sample, scoring highest in flavor, texture, and overall acceptability. This study highlights the potential of hemp seeds as a functional ingredient in developing nutritious and elderly-friendly food products. The hemp seed soup, with its enhanced protein, antioxidant properties, and tailored texture, offers a promising solution to improve the nutritional status and health of the elderly. Future studies should explore the long-term health benefits of hemp seed soup and its integration with other functional ingredients to optimize the sensory and nutritional properties.

Background: The diagnostic accuracy of preoperative radiologic findings in predicting the tumor characteristics and clinical outcomes of papillary thyroid microcarcinoma (PTMC) was evaluated across all risk groups. Methods: In total, 939 PTMC patients, comprising both low-risk and non-low-risk groups, who underwent surgery were enrolled. The preoperative tumor size and lymph node metastasis (LNM) were evaluated by ultrasonography within 6 months before surgery and compared with the postoperative pathologic findings. Discrepancies between the preoperative and postoperative tumor sizes were analyzed, and clinical outcomes were assessed. Results: The agreement rate between radiological and pathological tumor size was approximately 60%. Significant discrepancies were noted, including an increase in tumor size in 24.3% of cases. Notably, in 10.8% of patients, the postoperative tumor size exceeded 1 cm, despite being initially classified as 0.5 to 1.0 cm based on preoperative imaging. A postoperative tumor size >1 cm was associated with aggressive pathologic factors such as multiplicity, microscopic extrathyroidal extension, and LNM, as well as a higher risk of distant metastasis. In 30.1% of patients, LNM was diagnosed after surgery despite not being suspected before the procedure. This group was characterized by smaller metastatic foci and lower risks of distant metastasis or recurrence than patients with LNM detected both before and after surgery. Conclusion Among all risk groups of PTMCs, a subset showed an increase in tumor size, reaching 1 cm after surgery. These cases require special consideration due to their association with adverse clinical outcomes, including an elevated risk of distant metastasis.

Background: Turner syndrome (TS) is a common chromosomal abnormality, which is caused by loss of all or part of one X chromosome. Hormone replacement therapy in TS is important in terms of puberty, growth and prevention of osteoporosis however, such a study has never been conducted in Korea. Therefore, the purpose of our study was to determine relationship between the starting age, duration of estrogen replacement therapy (ERT) in TS and develop a hormone replacement protocol suitable for the situation in Korea. Methods: This is retrospective study analyzed the medical records in TS patients treated at the Severance hospital, Yonsei University College of Medicine, Seoul, Korea from 1997 to 2019. Total of 188 subjects who had received a bone density test at least once were included in the study. Korean National Health and Nutrition Examination Survey (KNHANES) was used for achieving bone mineral density (BMD) of normal control group. Student’s t-test, MannWhitney U test, ANOVA and correlation analysis were performed using SPSS 18.0. Results: Each BMD measurement was significantly lower in women with TS than in healthy Korean women. Early start and longer duration of ERT is associated with higher lumbar spine BMD but not femur neck BMD. Femur neck BMD, but not lumbar spine BMD was significantly higher in women with mosaicism than 45XO group. Conclusion Early onset and appropriate duration of hormone replacement therapy is important for increasing bone mineral density in patients with Turner syndrome. Also, ERT affects differently to TS patients according to mosaicism.
