1.Constitutional Chromosome 21 Abnormality in B-ALL with iAMP21 in a Patient Developing Treatment-Related Myelodysplastic Syndrome

The initial molecular cytogenetic characteristics of blasts plays a significant role in determining the treatment course of B-cell acute lymphoblastic leukemia (B-ALL).B-ALL with intrachromosomal amplification of chromosome 21 (iAMP21) has been well known to have unfavorable prognosis. Also, there are previously recognized germline mutations that increase the risk of ALL, such as trisomy 21, Down syndrome. This case report is about a 16-year-old girl who presented with lymphadenitis, purpura, and fever followed by initial lab of elevated white blood cell with blasts.She had some notable facial features, but no typical Down syndrome related one.Bone marrow biopsy and fluorescence in situ hybridization finalized the diagnosis as B-ALL with iAMP21, high-risk group. The minimal residual disease-negative complete remission was achieved after the induction chemotherapy with Korean multicenter high-risk protocol. However, abnormal karyotype was sustained in bone marrow. Microarrays with her buccal swab raised the possibility that the abnormal karyotype was not from the leukemic blasts but rather from the germline. Although she underwent scheduled chemotherapy uneventfully as slow early responder type, thrombocytopenia and abnormal karyotype persisted, leading to the diagnosis of acute myeloid leukemia. Additional chemotherapy and peripheral blood stem cell transplantation was performed which resulted in engraftment. This case highlights the discovery of a constitutional genetic aberration, which played like a silent yet critical background factor for B-ALL with iAMP21. As the number of reported cases are limited, the role of germline chromosome 21 mutation as the indicator for prognosis of B-ALL should be studied further.

We analyzed the Korean Longitudinal Study of Aging data collected from 2010 to 2018. A data of total of 1,359 participants, aged 65 or older were used to estimate the change in pain. A latent growth model and growth mixture modeling were performed to estimate the overall change in pain and to categorize the types of pain changes.
Results:
The pain changes of older adults were classified into two categories: low-stable and high increasing. The depressive symptom showed a stronger relationship among the high-increasing type of pain than the low-stable type. The high-increasing type had a higher percentage of females, lower income, relatively low educational attainment, and a higher percentage of rural residents than the low-stable type.
Conclusion
The significance of this study is that it reiterated the importance of early pain diagnosis and intervention by identifying the types of pain changes in older adults and analyzing their effects on depressive symptoms. Therefore, it is especially important to pay attention to interventions that are designed to help vulnerable groups with a high risk of pain obtain effective pain management.

10.Clinical evaluation of GenBody hMPV Ag Rapid Test: a diagnostic accuracy study

Background:
Human metapneumovirus (hMPV) is a major cause of acute respiratory infections in children and adults worldwide; however, no antiviral therapies or vaccines are currently available. Therefore, rapid and reliable diagnostic tools are required to support timely patient management and control outbreaks.
Methods:
We retrospectively evaluated GenBody hMPV Ag Rapid Test results using 165 consecutive clinical samples collected from patients with suspected respiratory infections between May and August 2024. Specimens were pre-characterized using a confirmatory reverse transcription-quantitative polymerase chain reaction (RT-qPCR) assay. Diagnostic performance was summarized using percent positive agreement (PPA), percent negative agreement (NPA), and overall percent agreement (OPA), with subgroup analyses by age and cycle threshold (Ct) groups.
Results:
The GenBody assay achieved a PPA of 92.7% (51/55), NPA of 100% (110/110), and OPA of 97.6% (161/165), with no false-positive results. Age-stratified analysis showed high PPA in infants (93.3%) and children (97%), whereas estimates in hMPV-positive adolescents (n = 2) and adults (n = 5) were less precise owing to the small sample size. In the Ct-stratified analysis, PPA was 100% for specimens with Ct < 28 (39/39), whereas all false-negative results occurred in the Ct > 28 group (12/16).
Conclusion
In this retrospective, single-center evaluation, the GenBody hMPV Ag Rapid Test showed high agreement with RT-qPCR, with reduced detection in specimens with higher Ct values. Further prospective studies and long-term stability assessments are warranted to confirm the performance across a broader range of clinical settings and storage conditions.
